A boy with a closed partial cloacal septation defect with a patent urachus is reported. He had an intact abdominal wall, a patent urachus, a colovesical fistula, intact genitalia and urethra, imperforate anus, and a lipomyelocystocoele. Patients with similar constellation of findings have been reported as cloacal exstrophy variants. What distinguishes this case from the other reported variants is the intact abdominal wall with the patent urachus, the small and normally formed phallus and urethra, and the presence of a lipomyelocystocoele. Since 1988 twelve children underwent urethral reconstruction by using an autologous bladder mucosal graft. In 10 of these previous surgery for hypospadias or other penile anomalies had failed. Bladder mucosal graft was used in 2 cases for primary urethral reconstruction. Our results in using this method, mostly in hypospadias cripples, are satisfying, so that we are encouraged to use the autologous bladder mucosal graft for primary urethral reconstruction in selected cases with severe forms of hypospadias as well.

In an attempt to create continence in myelomeningocele children we performed urethral lengthening/submucosal reimplantation, a form of bladder neck reconstruction, to create a valve allowing catheterizable access to the bladder. Of 15 patients with cloacal exstrophy currently under management 11 have undergone procedures for the establishment of urinary continence. Augmentation of the urinary bladder is a popular surgical procedure for increasing the capacity and lowering the intraluminal pressure in patients whose bladder is small, noncompliant, or has high pressure. Among 250 augmentations performed at our hospital, nine patients have had 16 episodes of postoperative extravasation of urine from their augmented bladder. Five male patients with epispadias and total urinary incontinence were managed in the present work. Urethral mucosal tube was performed by folding one lip of the bladder neck and urethral wall to encircle the mucosal tube snugly, and the other lip was used to overlap the first one at a distance of 3 cm length. This technique provided an effective functioning segment that maintained continence. Our results since we have adopted this technique were excellent in four cases.

Nine boys whose age ranged from 5 days to 3 months with urinary ascites were managed in Beijing Children's Hospital. All of them presented with gradual abdominal distension from birth, 5 had dripping of urine, the 5-day old new born had anuria since birth. The clinical course and the pathologic features of 29 patients with the prune belly syndrome (PBS) are reviewed. There were 26 males and 3 females. In addition to the classical triad of deficient abdominal musculature, urinary tract abnormalities, and cryptorchidism, a broader spectrum of other defects was found including musculoskeletal (58%) and gastrointestinal (31%) abnormalities. We reviewed our experience with endoscopic sub-ureteral injection of polytetrafluoroethylene (PTFE) in 337 children (84% females, 16% males). To determine the frequency of vesicoureteral reflux (VUR) in boys, the authors retrospectively studied 724 boys who underwent voiding cystourethrography for the first time. VUR was identified in 196 of these patients (27.0%). Urinary tract infection was the indication for cystourethrography in 188 patients (25.9%), 80 of whom (42.5%) had VUR. Agenesis of the bladder is a rare congenital anomaly, recorded most often in stillborn infants. Renal agenesis and other severe malformations are frequently associated. An apparent failure of the infraureteral tissue of the mesonephric duct to develop into trigone and proximal urethra seems to be the cause of this anomaly. We report a case of bladder agenesis with anal atresia, left renal dysplasia, and bicornuate uterus.

Surgical correction of major hypospadias defects is a challenging problem. Bladder mucosa has been reported as an excellent tissue for construction of a neourethra. We used bladder mucosa in 13 boys to create 15 neourethras when there was inadequate preputial skin to perform a vascularized pedicle flap or a free preputial graft. In 3 cases a bladder mucosa graft was used for initial repair of the hypospadias. All other boys had undergone prior hypospadias surgery. In 4 boys the initial result was satisfactory. Seven minor complications occurred in 5 boys, while 4 had more major complications.

Intermittent clean catheterization (CIC) has been proposed in 1975 for treatment of neurogenic bladder in children and tean-agers. We report herein the results of our experience from 1976 to 1986 with this method in a large number of patients with bladder dysfunction. 165 patients were included in this study (111 girls and 54 boys). 9 patients were lost for follow-up. We report a rare case of complete duplication of the bladder, urethra and external genitalia associated with other multiple congenital anomalies, such as ventriculoseptal defect, malrotation of the gut, ectopic anal opening, malascended left kidney, rachischisis of the lumbar spine and sacrum, and an umbilical hernia. To our knowledge the occurrence of these associated anomalies in a single case of complete duplication of the bladder, urethra and external genitalia has not been reported previously.

Ureaplasma urealyticum cultures from 124 patients with urinary tract disease were serotyped by indirect immunofluorescence, using antisera to serotypes I to VIII. A similar range of serotypes was recovered from first-voided, midstream, and bladder-aspiration (SPA) urine, upper urinary tract samples, and vaginal swabs. Serotype VI was predominant (44/124) among the samples, whereas serotypes V (1/124 samples) and VII (0/124 samples) were uncommon. Since 1976, 23 children with bladder or cloacal exstrophy, meningomyelocele, sacral agenesis, the prune belly syndrome and noncompliant bladders associated with urethral valves or prior diversion underwent augmentation cystoplasty.
