Anal and ano-urogenital malformations: a histopathological study of "imperforate anus" with a reconstruction of the pathogenesis.

Histopathological information about "anorectal malformations" is scarce and the pathogenesis still controversial. Autopsy specimens of 20 human fetuses and newborns with "main" types of the disorder were studied histologically. Supplemented with surgical-anatomical data from the literature and with information from our own and earlier embryological research in animal models as well as from recent observations on the normal development of the human perineum, the study allowed for a new reconstruction of the pathogenesis of the disorder. The histological analysis of the malformations in human fe

Triplication of colon with diphallus and complete duplication of bladder and urethra.

Gastrointestinal duplications are a common type of congenital anomaly usually restricted to one part of the gut. Triplication of colon is a rare variation of duplication of the hindgut. Hindgut duplications are commonly associated with anomalies of other derivatives of the cloaca, namely, the lower genitourinary system. Here, we describe a child with triplication of colon with associated duplication of bladder and urethra and diphallus. The child described here is the only reported survivor with triplication.

Unusual case of bladder duplication: complete duplication in coronal plane with single urethra and no associated anomalies.

Duplication of the bladder is an unusual congenital anomaly. Complete and incomplete forms are well defined in published reports. We present an unusual case of a 3-year-old boy with two separate-walled bladders, one lying in front of the other, communicating through a tiny septum, at the neck of the primary bladder that drained to a normal single urethra. This type of isolated bladder anomaly was not defined in the classification of the duplication of the bladder. In patients with a lower abdominal cystic mass, complete bladder duplication must be included in the differential diagnosis of pelv

A novel approach to skin coverage for epispadias repair.

PURPOSE: Current techniques for epispadias repair have resulted in significant improvement in the reconstruction of the urethra, corpora and glans. The final challenging step is to enhance the cosmetic result by accomplishing skin coverage for the penis and subpubic area, creating a penopubic and penoscrotal angle without dorsal suture lines, and avoiding future dorsal tethering of the penis as a result of scar contraction. We report a novel technique for penile skin coverage in the patient with epispadias that results in a superior cosmetic outcome. MATERIALS AND METHODS: The technique invol

Urodynamic testing in the diagnosis of small animal micturition disorders.

Urodynamic testing provides a quantitative assessment of the function of the small animal lower urinary tract. Most commonly these techniques are utilized to assess urethral tone (urethral pressure profile or UPP) and bladder detrusor muscle function (cystometrogram or CMG). A UPP may be indicated in cases of canine and feline micturition disorders. Examples include suspected primary sphincter mechanism incontinence (PSMI), ureteral ectopia, other congenital abnormalities, suspected neurological disorders, and mechanical or functional urethral obstruction. A UPP can be performed effectively ut

Diphallus with ectopic bowel segment: a case report.

A 1-year-old boy presented with complete duplication of the penis, with an ectopic bowel segment as an associated anomaly. The boy had duplicated epispadiac urethras. The right one ended blindly whereas the left one was patent, leading into the bladder. There was separation of the symphysis pubis and bifid scrotum. Investigations revealed no other anomalies. A review of the literature concerning diphallus showed a preponderance of associated anomalies such as exstrophy, bladder duplication, ureteral or renal defects, and imperforate anus, but no cases with an ectopic bowel segment. We report t

Urethral polyp in a 1-month-old child.

Urethral polyps are a rare finding in children, particularly in the very young. They are suspected by the presence of various clinical signs such as obstruction, voiding dysfunction and haematuria. There is an association with other urinary tract congenital anomalies. They are usually benign fibro-epithelial lesions with no tendency to recur and are treated by surgical ablation, fulguration or laser therapy. We report a 1-month-old boy with an antenatally diagnosed left ectopic pelvic kidney, postnatal urinary tract infection and no clinical signs of obstruction. Voiding cystourethrography to

[Penile- and urethral anomalies].

Penile and urethral anomalies are common congenital disorders with some rare variations. For example, the floppy penis is dealt with in only a few publications with a small numbers of cases. Other anomalies have not been published at all, i.e., the ectopic penis. Some anomalies become apparent immediately at birth, others remain undiscovered until adolescence. To date there is no established classification system dealing with urethral and penile anomalies. Also, no established guidelines exist for diagnosing and treating these anomalies. Surgical corrections of penile and urethral anomalies sh

Terminal deletion of the long arm of chromosome 10.

The case of two female patients with de novo terminal deletion of the long arm of chromosome 10, one with del(10)(q26.2) and the other with del(10)(q26.3), is reported. Both presented with megabladder associated with urinary tract abnormalities. The case of four similar patients has been previously reported with bladder dilatation secondary to urinary obstruction. These new cases highlight the possible involvement of the bladder and the urethra in the syndrome of chromosome 10q terminal deletion, suggesting a careful renal and urinary tract evaluation in such situations. Moreover, the possibil

Complete primary repair of bladder exstrophy: initial experience with 33 cases.

PURPOSE: We evaluated our initial experience with complete primary repair of bladder exstrophy in 33 children. MATERIALS AND METHODS: Between 1998 and 2001, 33 children with classic bladder exstrophy were treated with 1-stage primary repair for the first time in all except 4, who had undergone previous failed initial bladder closure. Our series included 26 boys and 7 girls with a mean age of 2 months (range 3 weeks to 14 months). The bladder was closed in continuity with the urethra and complete penile disassembly was used for epispadias repair. Anterior transverse innominate osteotomy was pe

Laparoscopic vaginal reconstruction using a sigmoid colon segment: a preliminary report.

PURPOSE: Sigmoid vaginoplasty is an alternative technique for vaginal replacement. We describe the successful use of laparoscopy for reconstruction of a vagina in patients with the Mayer-Rokitansky-Kuster-Hauser syndrome. MATERIALS AND METHODS: Laparoscopic surgery was performed in 5 girls with a mean age of 17 years and 5 months. We used 4 ports, the first of which was inserted supraumbilically by the Hasson technique. The next 10 mm port was introduced through the right lower quadrant under visual control. Then 5 and 15 mm ports were inserted at the left lower quadrant. The scope was introd

Transvaginal sling suspension of bladder neck in female patients with neurogenic sphincter incontinence.

PURPOSE: Many surgical options exist to enhance bladder neck closing pressure in women. Most procedures are relatively large with a success rate of between 70% and 90%. Sling procedures with the sling placed between the anterior vaginal wall and bladder neck cause a risk of traumatic lesions of the bladder neck at operation and of postoperative erosion of the sling into the urethra. We evaluated the results of surgical treatment for neurogenic pelvic floor paralysis in girls with spina bifida by transvaginal rectus abdominis sling suspension. MATERIALS AND METHODS: Between 1991 and 2001 we tr

Urinary bladder perforation in a premature infant with Down syndrome.

Urinary bladder perforation due to bladder catheterization in neonates is a rare iatrogenic complication. It has been reported secondary to various causes and a variety of surgical settings in neonates. A case of urinary bladder perforation due to catheterization in a premature baby with Down syndrome, who presented with progressive renal failure and mild-to-moderate ascites, is reported. Urinary bladder perforation should be considered in a case of neonatal ascites with renal failure, which is unexplainable by other causes. We recommend that bladder catheterization in a baby with Down syndrom

Bladder neck cinch for pediatric neurogenic outlet deficiency.

PURPOSE: The fascial bladder neck sling achieves continence in 50% to 90% of children with neurogenic outlet deficiency. Most slings apply only partial pressure around the bladder neck. We evaluated the effectiveness of a rectus fascia bladder neck cinch which applies circumferential pressure around the bladder neck and elevation as a means of increasing outlet resistance. MATERIALS AND METHODS: Fifteen children with spina bifida underwent a fascial bladder neck cinch procedure at the time of augmentation cystoplasty. A 1 to 1.5 cm width of variable length rectus fascia was harvested and a ve

Reconstruction of hypospadias and epispadias with buccal mucosa free graft as primary surgery: more than 10 years of experience.

PURPOSE: The most common techniques to reconstruct the urethra for severe hypospadias or epispadias involve use of the genital skin. Buccal mucosa free graft is usually used in a second operation due to a paucity of usable genital tissue. We report our experience using buccal mucosa free graft as primary surgery for hypospadias and epispadias. MATERIALS AND METHODS: Between 1989 and 2001 the urethra was reconstructed using buccal mucosa free graft as a primary operation in 22 patients 3 to 13 years old with middle or proximal hypospadias (20) and exstrophy/epispadias (2). Vaginal and penile reconstruction.

PURPOSE OF REVIEW: Reconstructive surgery for patients with genital abnormalities or for patients who require reconstructive efforts is challenging. This review highlights those articles, which are outstanding among all those important papers, which have been published during the last year (2002-2003). RECENT FINDINGS: A greater understanding of embryonal development improves the success of reconstructive surgery. Other factors, such as the patient's sex, influence the surgical technique used and the degree of invasiveness or complexity. In the adult the pressure to shorten hospital stays has

Imaging of mild traumatic brain injury using 57Co and 99mTc HMPAO SPECT as compared to other diagnostic procedures.

Invasive pulmonary aspergillosis (IPA) is a leading cause of morbidity and mortality in haematological malignancy patients and hematopoietic stem cell transplant recipients(1). Detection of IPA represents a formidable diagnostic challenge and, in the absence of a 'gold standard', relies on a combination of clinical data and microbiology and histopathology where feasible. Diagnosis of IPA must conform to the European Organization for Research and Treatment of Cancer and the National Institute of Allergy and Infectious Diseases Mycology Study Group (EORTC/MSG) consensus defining "proven", "proba

[Lithiasis in complete ureteral duplication and prostate cancer: combined surgery treatment].

The diagnosis of congenital anomalies of the urogenital tract in the adult is uncommon. Complete ureteral duplication represents a congenital anomaly of the ureters, usually draining a duplex kidney, with a second orifice entering the bladder, urethra or other structures. In the majority of cases, this pathology is completely asymptomatic; sometimes it's diagnosed because of complications such as infections, hydronephrosis, lithiasis, or in the evaluation of the urinary tract. Early diagnosis of localized prostatic adenocarcinoma is increased due to worldwide use of PSA screening and the optim
