OBJECTIVE: To assess the early and late outcome of endourethral injection with bulking agents in children with urinary incontinence (a neuropathic bladder or exstrophy-epispadias complex), by reviewing our experience over a 5-year period.

BACKGROUND: With today's improved surgical techniques, complications in hypospadias surgery are seen less often, especially in distal form (1-5%). However, in proximal or complex hypospadias complications rate varied between 15 and 57%. Corrective surgery is mandatory in maior complications (urethrocutaneous fistulas, persistent chorde, urethral strictures, superficial skin separation). Urethral duplication is a rare congenital anomaly, resulting from a wide range of malformations of the urogenital sinus. Generally, the duplication develops on the sagittal plane; the accessory urethra may run dorsally or ventrally to the orthotopic one. For hypospadias repair tubularized incised plate urethroplasty provides excellent cosmetic and functional results. PURPOSE: Complete repair of bladder exstrophy in the newborn includes bladder, and posterior and penile urethral closure, along with epispadias repair and abdominal wall closure without bladder neck reconstruction. PURPOSE: Whereas the literature on bladder exstrophy is replete with outcomes of specific continence surgical procedures in highly select patients, there are no data on the outcomes related to continence for a complete exstrophy population, which is more comprehensive with respect to a variety of surgical procedures performed to achieve continence. PURPOSE: We report the association of complete urethral duplication and bladder exstrophy in 5 males, and describe the main characteristics to achieve diagnosis.
MATERIALS AND METHODS: From 1983 to 2000, 5 males with single bladder exstrophy and complete urethral duplication were seen at 3 institutions. Although bladder exstrophy was obvious at birth, only 1 patient had the preoperative diagnosis of associated urethral duplication. All patients were evaluated with abdominal ultrasound, voiding cystourethrogram after bladder closure and cystoscopy.

PURPOSE: Penoscrotal transposition is a rare congenital abnormality of the external genitalia. We determine whether there is a genetic basis for this disorder, define the incidence of coexisting organ system anomalies, and compare the results of surgical techniques to correct transposition and hypospadias.

BACKGROUND: Although bladder catheterization causes contractions, accidental removal and urinary retention are risks of stents in hypospadias repair.
METHODS: An 8F feeding tube was used as a stent in 22 patients (group I). In the other 22 patients (group II), the same size catheter was passed into the bladder, and oxybutynin chloride was administered.
