Lower urinary tract abnormalities in children with multicystic dysplastic kidney.

PURPOSE: To determine the nature and frequency of associated structural abnormalities of the lower urinary tract in children with multicystic dysplastic kidney. MATERIALS AND METHODS: The authors reviewed imaging findings from studies in 59 children with unilateral multicystic dysplastic kidney. All patients had undergone renal ultrasonography (US), renal scintigraphy, and voiding cystourethrography. The diagnosis was based on characteristic imaging findings (echogenic, cystic kidney at US that did not function at scintigraphy). RESULTS: Nineteen (32%) patients had one or more abnormalities

Mutation of HOXA13 in hand-foot-genital syndrome.

There are several human syndromes which involve defects of the limbs and the Müllerian ducts or its derivatives. The hand-foot-genital (HFG) syndrome is an autosomal dominant, fully penetrant disorder that was originally described by Stern et al. Additional reports describing other affected families have also been published. Limb anomalies include short first metacarpals of normal thickness, small distal phalanges of the thumbs, short middle phalanges of the fifth fingers, and fusion or delayed ossification of wrist bones. In the feet, the great toe is shorter due to a short first metatarsal a

Kidney transplantation to patients with congenital malformations of the distal urinary tract.

Fourteen of 1000 consecutive kidney transplant patients had congenital malformations affecting the bladder or urethra: six had congenital valvulus of the urethra, two congenital sclerosis of the bladder outlet, and six a neurogenic bladder. Pretransplant surgery had been performed in all patients: reimplantation of ureter (n = 11), resection of congenital valvulus (n = 7), and nephrectomy (n = 6). Four patients had an intestinal bladder. Age was 0-17 (median 1) years at diagnosis. Follow-up time was 3-10 (median 5) years. Special transplant surgery techniques were required in five patients. [Congenital prepubic sinus].

Presentation of one case of Urethral Duplicity of the Blind Epispadias variant, also known as Congenital Prepubic Sinus. This form of accessory urethra is a congenital malformation which consists in a pseudofistular passage which starts at the proximity of the bladder's anterior side-though uncommunicated with it-and spreads to the skin of the pubic area where it ends. No other malformative condition was found in the comprehensive study conducted, and only the existence of repeat episodes of mucopurulent drainage material advised its removal. This manoeuvre was followed by a complete cure and,

Inherited and congenital diseases of the feline lower urinary tract.

Congenital urinary tract disorders of young cats may result from heritable (genetic) or acquired disease processes that affect differentiation and growth of the developing urinary tract, or from similar disease processes that eventually affect the structure or function of the mature urinary system. Although congenital diseases of the feline lower urinary tract are uncommon, clinical signs associated with these anomalies may be indistinguishable from those of other acquired causes of lower urinary tract disease. Early detection and proper management of congenital disorders may result in restora

Congenital megalourethra.

The authors report on two patients who had fusiform megalourethra with unusual features. The first patient had left hydronephrosis with duplication of the ureter, oblique sagittal septum in the urinary bladder, and Y duplication of urethra with megalourethra of both channels. The second had features of the abdominal muscle deficiency syndrome, with anterior ectopic anus, rectourethral fistula, and fusiform megalourethra. Both patients were treated by preliminary urinary diversion. The first patient subsequently had staged reconstruction. The other patient died of urinary sepsis. The authors be

[Usefulness of the urodynamic study of the lower urinary tract in renal transplantation].

OBJECTIVES: To analyze the indications, diagnostic cost-effectiveness and therapeutic implications of urodynamic assessment of the lower urinary tract in two groups of patients: 1) those awaiting renal transplantation and those with a functioning renal graft and voiding disturbances. METHODS: The first group comprised 22 patients who were evaluated before renal transplantation for a total of 32 urodynamic studies. The second group comprised 10 patients with a functioning renal graft for a total of 14 post-transplant urodynamic studies. The etiology of the chronic renal failure, indications fo

Posterior urethral valves in patients with Down syndrome.

Renal and urological anomalies in Down syndrome (DS) have received little attention compared with the nephrourological findings described in other chromosomal abnormalities. Renal hypoplasia, hydroureteronephrosis, ureterovesical and ureteropelvic junction obstruction, and vesicoureteral reflux, but not posterior urethral valves, have been associated with DS. We report the occurrence of posterior urethral valves in three male infants with DS at a single institution. All had multiple urological procedures for correction or palliation of obstruction. Children with DS may have an increased risk f

Ureterocele eversion with vesicoureteral reflux in duplex kidneys: findings at voiding cystourethrography.

OBJECTIVE: Ureterocele eversion refers to the sudden appearance of a bladder diverticulum at the site of ureterocele compression during voiding cystourethrography (VCUG). The radiologic appearance closely resembles a congenital bladder (paraureteral) diverticulum. Distinguishing ureterocele eversion with vesicoureteral reflux in duplex kidneys from congenital bladder diverticula with reflux is important for preoperative planning. This study describes the findings of ureterocele eversion and lower pole vesicoureteral reflux in duplex kidneys on VCUG and demonstrates how its appearance can be mi

The 3-loop technique: a reliable technique for anterior pubic fixation in bladder exstrophy.

PURPOSE: In exstrophic anomalies the ultimate urological outcome largely depends on successful initial closure of the lower urinary tract and soft tissues. We believe that secure anterior pubic fixation is crucial for ensuring successful closure. After being dissatisfied with other methods of anterior pubic fixation we introduced the 3-loop method. The 3-loop technique and our experience with it are described. MATERIALS AND METHODS: In 2 years 7 consecutive cases of bladder exstrophy were closed using the 3-loop technique. Patient age at closure ranged from newborn to 9 years. RESULTS: In al

Labial mucosa and combined labial/bladder mucosa free graft for urethral reconstruction.

One-stage urethral reconstruction was performed using a free graft of labial mucosa and combined labial/bladder mucosa. We present the results of this technique in 12 cases that had a minimum follow-up period of 3 years. Eleven patients had medium penile or posterior hypospadias and one had chordee penis without hypospadias. Urethroplasty with labial mucosa was performed by two techniques: labial mucosa used alone or combined with bladder mucosa. The labial mucosa was harvested from the inner surface of the upper and/or lower lip, depending on which method was used. Seven patients, six with me

Pubovaginal slings for the management of urinary incontinence in female adolescents.

A pubovaginal sling is an effective treatment for type III incontinence secondary to poor proximal urethral sphincter function. We used a pubovaginal sling to treat incontinence in 15 female adolescents. The etiology of incontinence was spinal dysraphism in 10 patients and prior trauma in 3. Simultaneous bladder augmentation was performed in the remaining 2 patients for poor bladder compliance. Three patients required additional procedures including repeat slings in 2 and repeat augmentation in 1. Of 13 patients followed for more than 6 months 11 remain dry, 1 leaks small amounts and wears 1 p

[Urethral opening pressure as a parameter for introduction of the proper voiding modality in myelodysplastic patients].

To introduce the proper voiding modality to patients with myelodysplasty, urethral opening pressure (UOP), an intravesical pressure just at the beginning urine flows out beyond the external urethral sphincter, was measured in 63 myelodysplastic patients. Among 45 renal units with any morphological or functional changes at the first UOP measurement, 37 units (82.2%) were included in the high UOP group (> or = 35 cmH2O). And among 41 ureters with VUR of more than grade 2, 32 (78.0%) were in the high UOP group. In addition, deformity of the urinary bladder was observed in 36 patients, and 26 (72.

Congenital renal damage associated with primary vesicoureteral reflux detected prenatally in male infants.

To assess the course of vesicoureteral reflux, we performed cystography, renal scintigraphy, and urography in all neonates with the prenatal diagnosis of renal pelvic dilation and revealed the presence of primary reflux (grades I to V) in 27 cases. Higher grades of reflux were associated with congenital renal damage, as shown by reduced tracer uptake during scintigraphy. Reflux was diagnosed more frequently in male infants (male/female ratio, 6:1), in many of whom bladder abnormalities were found by cystography. In another group of seven infants, in whom the reflux was associated with other ur

Young-Dees-Leadbetter bladder neck reconstruction for sphincteric urinary incontinence: the value of augmentation cystoplasty.

Young-Dees-Leadbetter bladder neck reconstruction was performed in five females and two males with bladder exstrophy, two females and one male with epispadias, and two females with congenital short urethra. Eight patients had simultaneous augmentation colocystoplasty (7 exstrophy, 1 female epispadias), seven of whom are continent for three hours or more. Two patients void spontaneously and five are managed by clean intermittent catheterization (CIC) with which there were considerable problems in the beginning. Three of the four patients in whom simultaneous bladder augmentation was not underta

[Congenital prepubic sinus: blind epispadial duplication of the urethra].

Presentation of one case of congenital epispadial urethral duplication in a 1-year old child. The malformation consisted on one normal urethra and the other one a prepubic epispadial accessory urethra, with the proximal end finishing blind into the anterior side of the bladder. The mictional cystography performed showed normal bladder and urethra, while the retrograde urethrography through the dorsal sinus showed a 3 cm long tract with no communication to the urethra or the bladder. Treatment consisted in total exeresis of the accessory urethra by way of suprapubic approach, with no symphysiot

Comprehensive planning of operative strategy for separation of ischiopagus tripus twins with particular reference to quality of life.

A 27-year-old mother was diagnosed by prenatal ultrasonography as having triplets at gestational age 32 weeks. Following cesarean section at 37 weeks, a pair of female babies were noted for the first time to be joined by a common pelvis with three lower limbs. They had separate upper gastrointestinal tracts, which joined in the distal ileum, leading to a common colon, rectum, and a single anus. Each twin had a functioning kidney, with a single ureter leading to a common bladder. A common urethra originating from the bladder neck ran into the urogenital sinus of one baby. Prior to the surgical

Urethral atresia and the prune belly syndrome. Report of 6 cases.

An association between urethral atresia and the prune belly syndrome (PBS) has been recognised, but few reports discuss the outcome of treatment for these gravely ill patients. Of the 34 patients with prune belly syndrome evaluated at our institutions, 6 had urethral atresia (3 males and 3 females). Two of these patients died as neonates as a result of pulmonary insufficiency and 1 was stillborn. The common feature of the 3 surviving patients (2 males and 1 female) was the presence of a vesicocutaneous fistula. As neonates, 2 suffered from pulmonary insufficiency due to oligohydramnios. The me

Unrecognized congenital posterior urethral minivalves in men.

Removal of small congenital posterior urethral valves, unrecognized in prior urologic investigations of 10 men, relieved "prostatic" symptoms in 9. In follow-ups (5-20 years, average 12.5 years), 3 have experienced a recurrence attributed to bladder neck obstruction in 2 and to unknown causes in 1. Bladder neck incision carried out in 2, cured 1. The other was lost to follow-up.

Is the preliminary film necessary prior to the micturating cystourethrogram in children?

The aim of the study was to determine the value of the preliminary film in children undergoing a micturating cystourethrogram (MCU). The coded computer reports of 806 children undergoing MCUs in a 12-month period were retrospectively reviewed for abnormalities of the lumbar spine, hips, or for calcifications. Vesicoureteric reflux was present in 185 patients (23%). Four patients had renal calculi (0.5%), and in all cases the abnormality was evident on preceding imaging of the upper urinary tract. Spinal anomalies (other than known meningomyelocele) were present in four patients. The clinically
