We report the case of a 24-year-old woman who received immunosuppressive treatment for systemic lupus erythematosus. In her past medical history, she had been treated for disseminated toxoplasmosis (ocular and cardiac). Four months later, she was hospitalized and treated for a presentation suggestive of cerebral toxoplasmosis. The diagnosis of primary central nervous system lymphoma (PCNSL) was finally made using stereotactic brain biopsy; however, the evolution was rapidly fatal within 6 days after admission. We discuss the challenge of differential diagnosis between cerebral toxoplasmosis an

IMPORTANCE: Uveitis is characterized by inflammation of the uvea-the middle portion of the eye composed of the iris, ciliary body, and choroid-causing eye redness, pain, photophobia, floaters, and blurred vision. Untreated uveitis may cause cataracts, glaucoma, macular edema, retinal detachment, optic nerve damage, and vision loss.
OBSERVATIONS: Uveitis predominantly affects individuals aged 20 to 50 years. Anterior uveitis affects the iris and ciliary body (41%-60% of cases); intermediate uveitis affects the pars plana (attachment point of vitreous humor) and peripheral retina (9%-15%); post

PURPOSE: The in vivo role of RPE cells in ocular toxoplasmosis is poorly understood. Toxoplasma gondii activates Epidermal growth factor receptor (EGFR) to avoid autophagic killing. We examined the in vivo role of RPE using mice with dominant negative (DN) EGFR in RPE. We examined whether EGFR blockade in RPE increased resistance to ocular toxoplasmosis, induced autophagy-dependent killing of T. gondii, and modulated retinal invasion by T. gondii.
METHODS: We bred mice expressing tetracycline-repressible transactivator under the control of MART-1 (RPE promotor) with mice expressing the tetrac

Purpose: The purpose of the study is to report a case of ocular toxoplasmosis with branch retinal artery occlusion (BRAO). Case: The patient was a 36-year-old man from Okinawa, Japan, who was generally healthy and had no medical history. He was referred to our hospital with a complaint of sudden loss of vision in the left eye. Best corrected visual acuity was 0.1 in the left eye at the initial examination, and intraocular pressure was 21 mmHg. Anterior segment slit-lamp examination showed a few cells of the anterior segment and anterior vitreous. Fundus examination of the left eye showed retin

Toxoplasmosis, affecting one-third of the global human population, urgently requires new therapeutic strategies due to current treatment limitations and drug resistance. Toxoplasma gondii calcium-dependent protein kinase 1 (TgCDPK1) is a promising drug target due to its essential role in parasite survival and its presumed unique glycine gatekeeper residue. Aiming at identifying new and selective inhibitors of TgCDPK1, we performed computational structural analyses and unexpectedly found that a human kinase (BUB1), despite having only 14% sequence identity, shares this glycine gatekeeper with T

INTRODUCTION: The apicomplexan parasite Toxoplasma gondii establishes chronic infection in the central nervous system, including the retina, causing ocular toxoplasmosis (OT). This persistence relies on a fine balance between inflammatory and immunomodulatory mechanisms, especially in the immune-privileged ocular environment. We previously described the immunologic interactions between retinal cells, and particularly the roles of type I and III interferons. In this study, we investigated the regulatory dynamics of PD-L1, a central immunomodulatory receptor on immune cells.
METHODS: We first i

BACKGROUND: Toxoplasmosis promotes acute and chronic symptoms ranging from ocular to severe congenital or neurotoxoplasmosis. A proper host immune response and a healthy gut microbiota control the pathophysiology of toxoplasmosis, presenting an opportunity for pro-resolving mediators.
OBJECTIVE: Here, we evaluated the role of the anti-inflammatory/pro-resolving protein annexin (Anx)A1 in Toxoplasma gondii (Tg) infection.
RESULTS: AnxA1 levels increase in the brain during Tg infection, and AnxA1 knockout (KO) mice display higher susceptibility to disease, an increased number of brain cysts, a

Toxoplasma gondii, one of the most prevalent zoonotic parasites globally, may be transmitted to the fetus if a primary infection occurs during pregnancy. The earlier the gestational period, the lower the probability of transmission, but if it occurs, the higher the probability of pregnancy loss or severe fetal neurological or ocular damage. Primary prevention measures to pregnant women include avoiding eating or handling raw or undercooked meat, washing hands thoroughly after gardening, avoid handling contaminated soil or water or come in contact with cat feces. The infected fetus may exhibit

Toxoplasma gondii causes ocular toxoplasmosis (OT) with potential retinal complications like rhegmatogenous retinal detachment (RRD), and retinal breaks (RB). While various management strategies exist, there is no universally accepted standard of care, and the most common combination procedure is scleral buckle (SB), pars plana vitrectomy (PPV), and silicone oil. The purpose of this case series is to evaluate and discuss the risk factors, phenotypes, management, and outcomes of rhegmatogenous RRD in six eyes (five patients) with OT from retrospective chart review. The age range was broad (27-7

Purinergic signaling, mediated by extracellular ATP (eATP) and P2 receptors, plays a vital role in physiological and pathological processes. The P2X7 receptor (P2X7R), a ligand-gated cation channel, is crucial in inflammation, cell death, and immune responses. Widely expressed in retinal cells, P2X7R contributes to visual function regulation and retinal degeneration. This review explores P2X7R involvement in retinal diseases, including age-related macular degeneration (AMD), Behçet's disease (BD), diabetic retinopathy (DR), glaucoma, retinitis pigmentosa (RP), uveitis, Stargardt's disease (STG

INTRODUCTION AND IMPORTANCE: Dandy-Walker syndrome (DWS) is a rare congenital posterior fossa malformation, affecting 1 in 25 000-30 000 live births. Prenatal diagnosis is possible via ultrasound and MRI. This case highlights a rare Dandy-Walker variant with bilateral optic atrophy and status epilepticus, emphasizing the need for awareness of progressive neurological and visual impairment in Dandy-Walker spectrum disorders. The case adds to the evolving knowledge on the diverse phenotypic presentations of the Dandy-Walker variant, particularly in resource-limited settings where diagnostic and

UNLABELLED: Toxoplasmosis is caused by infection with Toxoplasma gondii and is one of the most prevalent food-borne parasitic disease worldwide. T. gondii disseminates through the host organism and forms a latency-specific structure called bradyzoite cysts, found primarily in muscle and neuronal cells. In mice, Toxoplasma leads to sustained brain microvascular abnormalities, including capillary rarefaction, microglial activation, and blood-brain barrier (BBB) breakdown, resulting in synaptic and neuronal loss, behavioral and cognitive damages. We hypothesized that cyst-bearing neurons could si

Toxoplasma gondii is an apicomplexan parasite with an enormous global reach, infecting over a billion people worldwide. An opportunist in humans, T. gondii causes severe disease only in a select few scenarios but is otherwise relatively benign. Through mechanisms that are unclear T. gondii has a propensity to persist in neuronal tissues including the brain and retina, and it is in these sites that it can cause the most severe disease. Disease occurs in those with suppressed immune function, including HIV/AIDS and organ transplant patients. However, infection can also lead to recurrent ocular d

Toxoplasma gondii is responsible for the disease toxoplasmosis and has the broadest host range among apicomplexan parasites, as it infects virtually all warm-blooded vertebrates. Toxoplasmosis is a zoonotic and emerging public health concern with considerable morbidity and mortality, especially in the developing world, affecting approximately one-third of the world's human population. Clinical presentation varies among species, and the infection establishes lifelong chronicity in hosts. Most of the host species (including healthy humans) are asymptomatic on the one hand, it is fatal to marsupi

BACKGROUNDToxoplasma gondii is a zoonotic protozoan capable of infecting warm-blooded animal species and humans. Although toxoplasmosis presents mostly as mild or asymptomatic infection in immunocompetent individuals, in unborn children and people with weakened immune systems, the disease can be severe with ocular, neurological or multi-systemic manifestations and even death.AIMWe aimed to collate and analyse data on T. gondii seroprevalence in humans to model and compare age-dependent prevalence in geographic regions in Europe.METHODSA systematic review identified 1,822 scientific publication

This international consensus effort, convened by The Infectious Uveitis Treatment Algorithm Network (TITAN) group, brought together 103 uveitis experts to address long-standing controversies in the management of ocular toxoplasmosis (OT), a leading cause of infectious posterior uveitis worldwide. Despite the availability of advanced diagnostic tools, variability persists in clinical decision-making. Controversies were identified from previous surveys on OT management, which served as the foundation for developing the survey questions in this study. Using a two-round modified Delphi method, exp
