[Molecular diagnosis of non-syndromic craniosynostosis: new challenges for real clinical practice].

Craniosynostoses (CS) are a congenital malformation caused by absence or premature closure of skull sutures. This leads to deformation and impaired growth of skull. Traditionally, syndromic and non-syndromic forms are distinguished. For a long time, it was believed that molecular genetic mechanisms are significant exclusively for syndromic CS. However, clear data on involvement of somatic and germinal mutations in pathogenesis of non-syndromic forms were accumulated in recent years. Currently, there are no unified clinical guidelines defining the strategy for molecular genetic diagnosis for no

Outcomes and Complications of Monobloc Versus Le Fort III Procedures for Treating Craniosynostosis: A Literature Review.

BACKGROUND: Craniosynostosis is a congenital disorder resulting in the early fusion of several bones in the skull, causing a deformity of the head and facial structures. The 2 most commonly performed surgical procedures to treat craniosynostosis include the monobloc and Le Fort III procedures. This literature review compares the functional and anatomical outcomes and complications between the monobloc and Le Fort III procedures for treating craniosynostosis. METHODS: A PubMed search was conducted to identify studies assessing the outcomes following monobloc and Le Fort procedures. Data on ana

Addressing unilateral lambdoid craniosynostosis using endoscopic suturectomy and barrel-stave osteotomies: surgical technique and literature review.

PURPOSE: Unilateral lambdoid craniosynostosis (ULC) is a rare cranial deformity requiring intervention to correct asymmetry of the posterior skull. Conventional open remodeling and distraction osteogenesis offer robust expansion but carry risks of higher blood loss, hardware complications, or reoperation. We report a novel, minimally invasive technique combining endoscopic lambdoid suturectomy with barrel-stave osteotomies, supplemented by a literature review. CASE DESCRIPTION: A 2-month-old healthy male presented with classic left ULC features, including occipitoparietal flattening, ipsilate

Fraser Syndrome From Prenatal Diagnosis to Multidisciplinary Management and Long-Term Care: A Narrative Review.

Fraser syndrome (FS) is a rare autosomal recessive congenital malformation disorder characterised by cryptophthalmos, syndactyly, and variable respiratory, renal, genitourinary, craniofacial, and other systemic abnormalities. Considerable phenotypic heterogeneity creates challenges in prenatal recognition, prognostic counselling, neonatal management, and long-term care. Prenatal diagnosis may be particularly difficult because characteristic external abnormalities are not always detectable on ultrasonography, while oligohydramnios secondary to renal abnormalities may further limit fetal visuali

Giant Pleomorphic Adenoma of the Parotid Gland: Surgical Management of a Long-Standing Tumor With Facial Deformity and Preservation of Facial Nerve Function.

BACKGROUND: Pleomorphic adenoma (PA), the most prevalent benign neoplasm of the parotid gland, typically manifests as a slow-growing, painless mass. In cases where treatment is substantially delayed, the tumor may undergo progressive enlargement, potentially resulting in significant facial deformity. This, in turn, can complicate surgical management. CASE PRESENTATION: A 60-year-old female patient exhibited a long-standing right parotid mass that had been gradually increasing in size since 2007, with a notable escalation in dimensions during the year prior to the preliminary assessment. Longitudinal Development of Facial Symmetry after Primary Cleft Lip Repair: A Comparison of Subperiosteal and Supraperiosteal Techniques.

BACKGROUND: Primary cleft lip repair aims to restore both function and aesthetics of the nasolabial complex. Although several surgical techniques provide comparable functional outcomes, their long-term effects on facial symmetry remain unclear. This study compared postoperative facial symmetry following supraperiosteal and subperiosteal primary cleft lip repair using standardized photographic and digital anthropometric analysis. METHODS: This retrospective monocentric study included 46 patients with non-syndromic unilateral cleft lip and palate. Patients underwent either supraperiosteal repai

Functional stabilization and neuromuscular adaptation in active condylar hyperplasia: a comparative cohort study.

BACKGROUND: Active unilateral condylar hyperplasia (UCH) is a progressive mandibular growth disorder characterized by facial asymmetry, occlusal alteration, and mandibular deviation. Although single-photon emission computed tomography/computed tomography (SPECT/CT) is widely used to assess condylar hyperactivity, little is known about the clinical and neuromuscular adaptations occurring during the observation period before definitive treatment. This study evaluated whether conservative functional stabilization is associated with adaptive clinical and neuromuscular changes during this period.

Nutritional strategies to manage postural orthostatic tachycardia syndrome (POTS) and its comorbidities: A narrative review.

BackgroundPostural orthostatic tachycardia syndrome (POTS) is a heterogeneous disorder of the autonomic nervous system characterized by orthostatic intolerance and involvement of multiple organ systems. It commonly coexists with conditions such as hypermobile Ehlers-Danlos syndrome (hEDS), mast cell activation syndrome (MCAS), and a spectrum of gastrointestinal (GI) disorders. These overlapping conditions frequently manifest with shared GI symptoms such as nausea, bloating, abdominal pain, early satiety, and altered bowel habits that can substantially compromise nutritional status and quality

When Appearances Deceive: A Silent Somatotroph Macroadenoma Presenting With Acromegaloid Features and Paradoxically Low Insulin-Like Growth Factor-1.

Silent somatotroph adenomas are uncommon pituitary neuroendocrine tumours (PitNETs) of the PIT1 lineage that express growth hormone (GH) immunohistochemically but without biochemical evidence of acromegaly. We report the case of a 32-year-old male patient referred with recurrent epistaxis, headache, right-sided diplopia, and third cranial nerve palsy. Clinical examination revealed acromegaloid features including frontal bossing, spade-like hands, and coarse facies, which were strongly suggestive of longstanding GH excess. However, preoperative insulin-like growth factor-1 (IGF-1) was paradoxic

A combined classification system for preaxial foot polydactyly and its derived surgical strategies.

BACKGROUND: Pediatric polydactyly is a relatively common congenital condition of hand and foot, affecting physical and mental development of children. Hand polydactyly has been brought into focus recently. By contrast, the studies on foot polydactyly are limited. Among the types, there is little international research on preaxial foot polydactyly since it is a relatively rare type, and the domestic research on it is fewer. The purpose of our study is to propose a new classification for guiding operation. METHODS: We collected all patients with preaxial foot polydactyly who had preoperative ra

Medial patellofemoral ligament reconstruction in osteogenesis imperfecta using quadriceps tendon graft: a rare case report.

Osteogenesis imperfecta (OI) is a rare connective tissue disorder associated with bone fragility and ligamentous laxity, predisposing patients to recurrent patellar instability. Surgical treatment of recurrent patellar instability is challenging because standard medial patellofemoral ligament (MPFL) reconstruction techniques involving patellar tunneling may increase the risk of patellar fracture. We present a 15-year-old male patient with OI type V and recurrent patellar dislocation treated with a minimally invasive MPFL reconstruction using a pedicled quadriceps tendon graft. This patella-spa

Effectiveness of prenatal predictors for congenital diaphragmatic hernia on survival to discharge and extracorporeal membrane oxygenation requirement: a retrospective cohort study.

INTRODUCTION: Congenital diaphragmatic hernia (CDH) is associated with high mortality and morbidity. Antenatal counselling enables parents to prepare for the postnatal outcomes and relies on effective prognostic tools. Ultrasound (USS) predictors including observed/expected lung head ratio (o/e LHR) and liver position, are used independently, or combined into severity classifications. The aim was to evaluate the association between USS predictors, and severity classifications with survival to discharge and extracorporeal membrane oxygenation (ECMO) requirement in prenatally diagnosed CDH at Bi
