Current Understanding of Crouzon Syndrome Pathophysiology and New Therapeutic Approaches.

Crouzon syndrome (CS) is a rare genetic disorder characterized by the premature fusion of cranial sutures, leading to craniofacial abnormalities and potential neurological complications. CS is caused primarily by gain-of-function mutations in the FGFR2 gene and, less commonly, by mutations in the FGFR3 gene (specifically associated with CS with acanthosis nigricans). Managing CS requires a multidisciplinary approach, combining early and later surgical interventions to prevent intracranial hypertension and correct craniofacial deformities, along with ongoing care to address associated complicat

Spectrum of Otological Manifestations in Treacher Collins Syndrome: A Case Series of 9 Patients.

Treacher Collins syndrome (TCS) is an uncommon congenital disorder predominantly involving craniofacial, orbital, and otological structures. The various ear malformations seen in 9 patients with TCS are described. TCS predominantly affects the external and middle ear structures, with inner ear structures being relatively spared, not unexpected given the dual embryological origin of the human ear. The external and middle ear malformations were categorized and graded as those involving the ear pinna, external auditory canal, tympanic cavity, ossicles, facial nerve canal, oval window, and bony la

Bone deformities with hereditary multiple osteochondromas.

INTRODUCTION: Hereditary multiple osteochondromas (HMO) is a genetic skeletal disorder caused by defects in exostosin glycosyltransferase 1 (EXT1) or 2 (EXT2) genes. It develops mainly in the growth period and causes multiple osteochondromas (OC) in the physis of the long bones, leading to discomfort and deformities. This study aimed to investigate the anatomical distribution of OC, the frequency of deformities of the lower limbs, scoliosis and surgeries performed in a cohort of patients with HMO at the time of their enrolment in a regional surveillance programme. METHODS: The study populatio

French experience in the management of fibular hemimelia: Radiographic and functional outcomes.

INTRODUCTION: Fibular hemimelia is a rare congenital condition characterized by hypoplasia of a bone segment associated with malformations. Its clinical presentation is highly variable, not only in terms of the types of malformations but also their severity. If untreated, it can lead to significant functional impairments. Managing patients with this condition often spans several decades. There is no standardized treatment protocol, and to our knowledge, no recent French study has analyzed the surgical strategies implemented and the long-term functional outcomes in these patients. The last Fren

Genealogical Rabson-Mendenhall syndrome caused by INSR gene mutation.

Rabson-Mendenhall syndrome (RMS) is a rare autosomal recessive disorder caused by mutations in the insulin receptor gene (INSR), leading to severe insulin resistance. Clinical manifestations of RMS include hypertrichosis and acanthosis nigricans. A 3-yr-old male patient presented with darkened skin on the neck, without any apparent precipitating factors, and did not exhibit symptoms of polyuria or polydipsia. Both the patient and his older sister displayed signs of hypertrichosis and acanthosis nigricans. Laboratory investigations revealed significantly elevated levels of insulin and C-peptide

[Visceral perforations in a child with Ehlers-Danlos syndrome type IV].

The vascular type IV of Ehlers–Danlos syndrome is an autosomal dominant connective tissue disorder, the diagnosis of which is often delayed. Severe complications: arterial rupture or colon perforation may rarely occur already in childhood. We present a five-year-old child who developed bowel and bladder ruptures. He was admitted with severe abdominal pain and muscle defense. He had a history of chronic constipation, recurrent complaints of dysuria and suffusions. Based on clinical signs and imaging, acute surgery was indicated. A 3 mm perforation was found at the rectosigmoid part of the bowel

Bardet-Biedl Syndrome: Report of a Classical Case from North India.

Bardet-Biedl syndrome (BBS) is a genetic disorder affecting cilia and its functions and manifests floridly in late childhood as a multisystem disorder with the classical triad being polydactyly, atypical retinal dystrophy, and central obesity. So far, very few case reports have been reported from India and even fewer from North India. We are reporting a classic case of a 14-year-old boy presenting to the medicine outpatient department with features of lower respiratory tract infection, who, on work-up, was diagnosed as a case of BBS on clinical grounds. He underwent conservative treatment and,

Short-term outcomes of gradual ulnar lengthening in multiple hereditary osteochondromas of the forearm with monolateral external fixator.

Hereditary multiple exostoses is an autosomal dominant genetic condition primarily affecting long bones. Forearm deformities, including wrist ulnar deviation, ulnar shortening, radial or ulnar bowing, and radial head dislocation, are common manifestations. Gradual ulnar lengthening is suggested as a viable treatment option for managing these deformities. This retrospective single-center study included 15 forearms treated with gradual ulnar lengthening using a monolateral external fixator by a single surgeon. Clinical and radiological characteristics were recorded preoperatively and during foll

Keloid formation after congenital toe syndactyly release is associated with preoperative toe hypertrophy: A retrospective observational clinical study.

BACKGROUND: Keloids that form after the division of congenital toe syndactyly are rare and difficult to treat, and their etiology is unknown. In this retrospective study, we investigated the correlation between postoperative keloid development and preoperative hypertrophy of the toe syndactyly and made preliminary recommendations for perioperative therapy. METHODS: Twenty-six patients with hypertrophic toe syndactyly (hypertrophy group) and 26 non-hypertrophic patients (control group) were enrolled between 2014 and 2022. All patients were treated by the same surgeon for syndactyly division an

Determining the cause of optic nerve atrophy in syndromic craniosynostosis using logistic regression.

The restricted intracranial volume in syndromic craniosynostosis is due to limited skull expansion caused by early fusion of multiple cranial sutures. This led to progressive increased in intracranial pressure which has long been established as the cause of optic nerve damage. Optic nerve damage secondary to the narrowing of optic canal in syndromic craniosynostosis has been reported but not comprehensively explored. The objective of this study is to predict using logistic regression the cause of optic nerve atrophy is caused by increased intracranial pressure or structural narrowing of optic

Tissue-Selective Effects of PLEC Isoform Deficiency: Insights From A Muscle Only Phenotype.

INTRODUCTION/AIMS: The functional diversity of the PLEC gene is largely attributed to its multiple tissue-specific transcript isoforms. This study investigated the tissue-specific differential expression of plectin transcript isoforms in a patient with plectin 1f deficiency who presented solely with muscular dystrophy (LGMDR17) and lacked epidermolysis bullosa simplex (EBS) symptoms. We aimed to characterize the isoform-specific expression patterns that may underlie this tissue-restricted pathology. METHODS: RNA was isolated from skeletal muscle tissue and primary fibroblast cells of both the

Crouzonodermoskeletal syndrome requires individualized surgical management: Scoping review of a rare complex craniofacial syndrome.

Crouzonodermoskeletal syndrome (CDSS) is a rare condition characterized by premature fusion of the coronal sutures, midface hypoplasia, beaked nasal deformity, mandibular prognathism, and proptosis. A specific mutation in the Fibroblast Growth Factor 3 (FGFR3) gene causes CDSS (Ala391Glu) and thus provides a diagnostic marker for early identification. Beyond the Crouzon syndrome-like craniofacial appearance, the CDSS phenotype includes acanthosis nigricans, choanal stenosis/atresia, hydrocephalus, Chiari malformation, and achondroplasia, although presentations vary substantially throughout the

Small and porous ossicles, with flat stapes footplate and incudal fractures in the oim mouse model of osteogenesis imperfecta.

Hearing loss affects approximately 70% of individuals with osteogenesis imperfecta (OI), a genetic connective tissue disorder characterized by bone fragility and deformities. No effective treatments exist for OI hearing loss, and its etiology is unknown limiting the development of new targeted therapies. This work investigates the impact of OI type I collagen mutations on the ossicle bone properties in the homozygous oim mouse model of severe OI, which is known to exhibit hearing loss. The morphology and porosity of the ossicles of 14-week-old oim and wild-type mice were analyzed using high-re

A Challenging Case of Closing Gastroschisis With Gangrenous Bowel: A Case Report.

Gastroschisis is a rare congenital anomaly characterized by herniation of abdominal viscera through a right-sided abdominal wall defect without a protective membrane, typically identified during prenatal imaging. One of its most serious forms, known as closing gastroschisis, may result in bowel strangulation and significant clinical challenges. We report the case of a Yemeni male neonate delivered at 34 weeks of gestation via emergency cesarean section following premature rupture of membranes. At birth, the infant presented with a small abdominal wall defect and herniated bowel that appeared d

Buffalo chest after minimally invasive repair of pectus excavatum for pectus excavatum: results of the Chest Wall International Group (CWIG) survey.

BACKGROUND: The term "Buffalo chest" (BC) refers to a communication between the pleural cavities, a rare but potentially severe condition reported after minimally invasive repair of pectus excavatum (MIRPE) for pectus excavatum (PE). Literature highlights cases of life-threatening bilateral spontaneous pneumothorax (SP) in patients undergoing this procedure. Subpleural blebs are a frequent condition in adolescent populations and preventing related consequences should be advisable. This study aims to assess, through a survey, whether experts in pectus surgery consider Nuss repair a risk factor
