Chronic abdominal pain: Budd-Chiari syndrome with converging pathologies.

INTRODUCTION: Chronic abdominal pain can be diagnostically challenging, especially when vascular causes like Budd-Chiari Syndrome (BCS) are overlooked. BCS, often linked to prothrombotic conditions such as protein S deficiency, may be further complicated by anatomical anomalies like May-Thurner Syndrome (MTS), leading to significant hepatic congestion and morbidity. This case highlights the need to consider vascular etiologies in patients with hypercoagulable risk factors to ensure timely diagnosis and intervention. CASE STUDY: A 21-year-old Caucasian female with a complex history including t

X-Linked Hypophosphataemia and Burosumab: A Systemic Disease With a New Treatment.

X linked hypophosphataemia (XLH) is a systemic, chronic condition that significantly impairs quality of life. In XLH, a phosphate regulating endopeptidase homologue X-linked (PHEX) gene mutation leads to excess fibroblast growth factor 23 (FGF23), causing hypophosphataemia and subsequent rickets, lower limb deformity, pain and other sequelae, however there are likely other non-FGF23 mediated mechanisms contributing to disease. Burosumab is an FGF23 inhibiting monoclonal antibody that has been shown to be significantly more effective in treating X linked hypophosphataemia than previously availa

Diagnosis and Treatment of Hypophosphatasia.

Hypophosphatasia (HPP) is a rare inherited metabolic disorder characterized by deficient activity of tissue-nonspecific alkaline phosphatase (TNAP) caused by variants in the ALPL gene. Disease manifestations encompass skeletal hypomineralization with rickets and lung hypoplasia, vitamin B6-dependent seizures, craniosynostosis, and premature loss of deciduous teeth. The clinical presentation can comprise failure to thrive with muscular hypotonia, delayed motor development, and gait disturbances later in childhood. In adults, pseudofractures are a characteristic indicator of severely compromised

Factors associated with impaired physical functionality in X-linked hypophosphatemia.

X-linked hypophosphatemia (XLH) is a rare inherited disorder characterized by elevated levels of FGF23, chronic hypophosphatemia, impaired bone mineralization, and chronic long-term manifestations. Treatment for XLH has been mainly focused on normalizing its biochemical abnormalities. Despite treatment, patients with XLH often present impaired physical function and decreased quality of life (QoL). We hypothesize that physical functionality and QoL are more strongly associated with chronic pain and decreased muscle mass than persistent biochemical abnormalities or exposure to conventional treat

Vein of Galen Malformation with Pial Arteriovenous Fistula in a 12-month-old Child.

Pial arteriovenous fistulas are extremely rare vascular malformations of the brain (accounting for approximately 1.6% of vascular malformations of the brain) and are classified as Galenic and non-Galenic pial fistulas. We present a rare association of a mural-type vein of Galen malformation with a pial fistula involving the precentral branch of the right middle cerebral artery (MCA) in a 12-month-old infant. A 12-month-old preterm male infant, born vaginally with a delayed cry (birth weight 1.2 kg), presented with progressive macrocephaly, recurrent upper respiratory tract infections, developm

Methodology for the international working group clinical practice guidelines on X-linked hypophosphatemia in children and adults.

The guideline panel, comprising international experts in X-linked hypophosphatemia (XLH), patient partners from the XLH patient population, and guideline methodologists, held 18 teleconferences between January 2023 and July 2024 to develop comprehensive guidelines for the diagnosis and management of XLH in children and adults. For a subset of our questions, we utilized the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) methodology, assessed the certainty of evidence and formulated GRADEd recommendations. For these questions, the panelists and methodologists collabo

An overview of obesity-related complications: The epidemiological evidence linking body weight and other markers of obesity to adverse health outcomes.

Obesity is a highly prevalent chronic multisystem disease associated with shortened life expectancy due to a number of adverse health outcomes. Epidemiological data link body weight and parameters of central fat distribution to an increasing risk for type 2 diabetes, hypertension, fatty liver diseases, cardiovascular diseases including myocardial infarction, heart failure, atrial fibrillation, stroke, obstructive sleep apnoea, osteoarthritis, mental disorders and some types of cancer. However, the individual risk to develop cardiometabolic and other obesity-related diseases cannot entirely be

Markedly discordant hypophosphatasia in a young girl.

Hypophosphatasia (HPP) is the inborn-error-of-metabolism from deactivating mutation(s) of ALPL, the gene that encodes the cell surface "tissue-nonspecific" isoenzyme of alkaline phosphatase (TNSALP). HPP's "biochemical signature" comprises low serum alkaline phosphatase activity together with elevated plasma levels of the TNSALP natural substrates phosphoethanolamine (PEA), pyridoxal 5'-phosphate (PLP), and inorganic pyrophosphate (PPi). Excess extracellular PPi (ePPi) inhibits mineralization and affected children prematurely shed deciduous teeth and often suffer weakness and rickets. Yet, HPP

Exploring the size limits of Bionano optical genome mapping to resolve alternative structures of linked interspersed chromosomal duplications.

BACKGROUND: Determining the correct structure of large, interspersed duplications and related complex genomic rearrangements in genetic disease is critical when establishing causal roles and requires a technology able to span the entire duplicated segment(s) on single molecules. We assessed the use of Bionano optical genome mapping (OGM) for this purpose. METHODS: We combined OGM, Illumina short-read sequencing and fluorescence in situ hybridisation (FISH) to characterise three large interspersed duplications/triplications, and used the deepC algorithm to predict impact on local topologically-

Prenatal Diagnosis of 6q Terminal Deletion Associated with Coffin-Siris Syndrome: Phenotypic Delineation and Review.

Chromosome 6q deletion syndrome is a rare entity that has a highly variable clinical presentation and size of deletions. The most frequent manifestations of 6q terminal deletion are intellectual disability, facial dysmorphism, brain structural anomalies, and congenital heart defects. The phenotype is not clinically recognizable, except in those who harbor a terminal 6q deletion that includes the ARID1B gene, in whom features similar to Coffin-Siris syndrome (CSS) can be observed. We report the case of a female newborn with a prenatal diagnosis of a terminal deletion on 6q25.1q27, which encompa

Expanding the literature on Cantú syndrome: recognising early clinical and phenotypic clues.

A late preterm (ex 35 weeks' gestation) male infant was referred at 5 weeks of age (term corrected) with respiratory distress and feeding difficulties which had been present since birth. Examination revealed hypertrichosis, coarse features, a harsh continuous murmur and 2 cm hepatomegaly. His echocardiogram identified a patent ductus arteriosus which failed initial medical treatment and required early device closure. Structural heart disease alongside characteristic phenotypic features, including hypertrichosis and coarse facial features, prompted targeted genetic evaluation which confirmed a

Cantu syndrome-associated SUR2[H60Y] mutation confers selective gain of function on Kir6.1 ATP-sensitive potassium channels.

Gain-of-function (GOF) mutations in either Kir6.1 (encoded by KCNJ8) or SUR2 (encoded by ABCC9) are causally associated with Cantu syndrome (CS), characterized by coarse facial appearance, hypertrichosis, and multiple cardiovascular abnormalities. To date, all SUR2 mutations identified in association with CS have demonstrated GOF because of reduced ATP sensitivity using patch-clamp analysis, with the notable exception of SUR2[H60Y], which showed WT behavior in Kir6.2-SUR2A channels. We readdressed the effect of SUR2[H60Y] on channel function of the relevant Kir6.1-SUR2B channels, in intact cel

Pathophysiology of Femoral Fractures in Hypophosphatasia.

PURPOSE OF REVIEW: In this review, we will examine the pathophysiology, anatomy, biochemistry, and genotype-phenotype correlation of femoral fractures in adult hypophosphatasia. RECENT FINDINGS: Hypophosphatasia (HPP) is a rare genetic disease characterized by low activity of tissue-nonspecific alkaline phosphatase (TNAP). The disease presents a broad spectrum of clinical manifestations primarily determined by the degree of residual TNAP activity. Adults with HPP of moderate clinical severity may present with spontaneous femoral fractures that are like the atypical femoral fractures (AFF) of

Clinical Characteristics and Management of Rare Metabolic Bone Diseases: An Audit of the Rare Metabolic Bone Disease Registry of India.

Rare diseases, defined by the 2002 Rare Disease Act, affect fewer than 5 in 10,000 individuals. Rare metabolic bone diseases (MBDs), such as osteogenesis imperfecta, hypophosphatasia, osteopetrosis, and other unclassified disorders, can disrupt bone development and remodeling, posing diagnostic and management challenges. This study analyzed data from the rarembd.in registry (2010-2024), a 15-year database documenting only rare MBDs. Clinical presentation and demographic data of patients with rare MBDs were collated. Common MBDs (osteoporosis, primary hyperparathyroidism) were excluded. Genetic

Deficient Cardiolipin Remodeling Alters Muscle Fiber Composition and Neuromuscular Connectivity in Barth Syndrome.

BACKGROUND: Barth syndrome (BTHS) is a rare X-linked mitochondrial disorder caused by mutations in the TAFAZZIN gene, which disrupts cardiolipin (CL) remodeling and mitochondrial function. While cardiac manifestations of BTHS are well characterized, the mechanisms underlying skeletal muscle weakness and fatigability are poorly understood. METHODS: We investigated neuromuscular and mitochondrial alterations in a novel murine model (TazPM) carrying a patient-derived D75H point mutation in Tafazzin. This mutation preserves protein abundance but abolishes enzymatic activity. Skeletal muscle funct

Horizontal Palpebral Fissure Best Predicts Subjective Facial Asymmetry in Unilateral Anophthalmia/Microphthalmia.

PURPOSE: Congenital anophthalmia/microphthalmia are developmental eye disorders with variable severity. The absence of a normal-sized eye can cause facial asymmetry. Outcome is often reported as relative horizontal palpebral fissure (rHPF) and/or orbital dimensions. Yet, the predictive value of these measurements is unknown. This study aims to test the relation between facial and ultrasound measurements and subjective outcomes. METHODS: In this retrospective study, 3D facial scans of 31 patients with unilateral an/microphthalmia were analyzed. The scans were obtained using the Vectra scanner,
