Evaluation of the clicky hip as a screening tool in the diagnosis of developmental dysplasia of the hip.

AIMS: The early diagnosis of developmental dysplasia of the hip (DDH) is important because treatment in early infancy is usually nonoperative. A consensus statement from the British Society for Children's Orthopaedic Surgery identified ongoing uncertainty regarding the predictive value of a documented clicky hip. The aim of this study was to evaluate the association between a clicky hip at the newborn or six- to eight-week examination and pathological DDH requiring treatment in a selective screening programme in the UK. METHODS: This was a retrospective analysis of prospectively collected reg

Editorial Commentary: Hip Microinstability: Stability Is the Goal, Plication Is a Tool.

Hip microinstability remains one of the most challenging conditions we encounter in hip preservation surgery. The difficulty is not performing a capsular plication but determining when instability is truly present and clinically relevant. In our experience, no single clinical, radiographic, or intraoperative finding should define the diagnosis. Rather, the entire clinical picture must be considered when evaluating the potentially unstable hip. Capsular plication represents an important component of this strategy but should not be considered in isolation. Careful diagnosis and thoughtful patien

Total Hip Arthroplasty for Neglected Pathological Femoral Neck Fracture in a Young Patient with Autoimmune Polyglandular Syndrome.

INTRODUCTION: Neglected pathological femoral neck fractures in young patients are rare and present significant surgical challenges, particularly in the presence of systemic metabolic disorders such as autoimmune polyglandular syndrome (APS). Poor bone quality, impaired healing potential, and endocrine instability complicate management, often limiting the success of head-preserving procedures. PURPOSE: The purpose of the study was to describe the surgical decision-making, technical considerations, and early outcomes of total hip arthroplasty (THA) in a young patient with a neglected pathologic

Femoral Nerve Palsy as a Complication of Pavlik Harness Treatment for Developmental Dysplasia of the Hip: Ten-Year Prospective Study.

BACKGROUND: Femoral nerve palsy (FNP) is a recognized complication of Pavlik harness (PH) treatment for developmental dysplasia of the hip (DDH), but its true incidence and clinical significance may be underestimated. This prospective study aims to determine the incidence, risk factors, and clinical course of FNP in infants undergoing PH treatment, and to evaluate whether FNP predicts treatment failure. METHODS: A 10-year prospective study (2015 to 2025) was conducted on all infants treated with a PH for Graf II to IV DDH at a specialist center. Patients were reviewed weekly with clinical and

A method for early ultrasound classification of developmental dysplasia of the hip in infants: combining YOLOv8-based keypoint detection with radiomics.

This study introduces an innovative method for early ultrasound classification of developmental dysplasia of the hip (DDH) in infants, integrating key point detection based on the YOLOv8 model with radiomics. Leveraging YOLOv8's advanced capabilities, the method accurately identifies and locates key anatomical landmarks in pediatric hip ultrasound images. Constructing angular information through these key points serves as a crucial clinical feature for DDH classification. By fusing these clinical features with radiomic features, the method further characterizes hip joint morphology and functio

Evaluation of Salter and Pemberton osteotomies in developmental dysplasia of the hip: the role of the congruity index as a prognostic factor.

BACKGROUND: The Salter osteotomy (SO) and the Pemberton osteotomy (PO) are common surgical methods for treating paediatric developmental dysplasia of the hip (DDH). Although they each have their own advantages, complications resulting from open surgeries are still relatively common. This study aimed to evaluate the clinical outcomes of SO and PO in the treatment of DDH and to identify prognostic factors associated with postoperative outcomes. METHODS: Between April 2011 and May 2018, we retrospectively reviewed 72 DDH patients who underwent open reduction combined with pelvic osteotomy (SO/PO

Quality Improvement Report: Decreasing Incorrectly Scheduled Screening US for Hip Dysplasia in Premature Infants.

Screening US for developmental dysplasia of the hip in infants with risk factors should optimally be performed after 6 weeks corrected gestational age (CGA). Scanning before this age leads to higher false-positive rates and the potential need for repeat examination, which creates waste and undue stress on the child and family. Pediatric radiologists at the authors' institution observed that many premature infants were being scheduled too early for their screening US. This quality improvement project aimed to decrease incorrectly scheduled screening examinations from a baseline rate of 50% to l

Correlation between acetabular index at 3 and 12 months of age: a longitudinal radiographic study of 228 neonates treated for 6 or 12 weeks with the von Rosen splint for developmental dysplasia of the hip.

BACKGROUND AND PURPOSE: Developmental dysplasia of the hip (DDH) affects around 1.5% of newborns in Sweden with few late detected cases (0.12 per 10,000). The most common treatment for DDH in Sweden is with the von Rosen splint, with radiographs at 3 and 12 months of age. Little is known about the remodeling of acetabular dysplasia following treatment initiated in the neonatal period. We aimed to examine the correlation between the acetabular index (AI) at 3 and 12 months. METHODS: We included 228 patients with early detected DDH with dislocatable hips (Barlow) and dislocated hips (Ortolani),

Is the Hip Located? What is the Value of Advanced Imaging at the Time of Spica Exchange for Infantile Hip Dysplasia?

BACKGROUND: Axial imaging is typically obtained following operative reduction of developmental dysplasia of the hip (DDH) to confirm a concentric reduction. Although the utility of axial imaging has been studied following the index reduction, its value following spica cast exchange remains unclear. METHODS: This single-center study included all children 2 years or younger with idiopathic DDH who underwent closed or open reduction between 2012 and 2023. Patients with concurrent osteotomies or dysplasia secondary to syndromic/neuromuscular causes were excluded. Institutional protocol during thi

Spinal Anesthesia: A Safe and Effective Alternative to General Anesthesia for Infantile DDH Surgery.

BACKGROUND: Operative hip reductions for infantile DDH are traditionally performed under general anesthesia (GA). This introduces potential safety concerns, as risks related to GA are highest in children below 1 year. While surgery can be temporarily delayed to reduce these risks, this decreases the chances of a successful reduction via closed methods alone and also compromises valuable remodeling potential. This pilot study sought to evaluate the safety and efficacy of spinal anesthesia (SA) in infants undergoing operative hip reduction. METHODS: This is a multicenter, retrospective cohort s

Late Dislocation Following Normal Multiplanar Dynamic Ultrasonography for Developmental Dysplasia of the Hip: A Case Report.

CASE: We describe a case of late dislocation of the hip in a male infant at the age of 5 months after a normal screening ultrasound study using multiplanar static and dynamic images at the age of 6 weeks. The patient was treated with adductor tenotomy, closed reduction, and spica casting, and followed to the age of 7 years with normal hip stability. CONCLUSION: This case demonstrates that despite evidence of both static and dynamic stability on ultrasonography earlier in life, late developmental hip dislocation can occur.

To Wean or Not to Wean: A Randomized Controlled Trial of Pavlik Harness Weaning in Infantile Developmental Dysplasia of the Hip.

BACKGROUND: After successful treatment of developmental dysplasia of the hip (DDH) with a Pavlik harness, controversy exists about whether it's best to continue harness use for an additional "weaning" period or simply terminate treatment. The purpose of this randomized controlled trial (RCT) was to compare radiographic outcomes between patients with stable hip dysplasia who were weaned (W) or not weaned (NW) after Pavlik harness treatment. METHODS: This was a single-center RCT of infants with stable ultrasonographic dysplasia in whom a Pavlik harness was initiated at <3 months of age. After 2

Super-resolution ultrasound imaging reveals microvascular alterations in joint capsule of dysplastic hip: a rabbit model study.

BACKGROUND: While pronounced angiogenesis has been observed in the synovial tissues of adult patients with developmental dysplasia of the hip (DDH), the vascular supply to the infantile hip joint capsule remains unclear. This study aimed to investigate early microvascular alterations in DDH using super-resolution ultrasound imaging (SRUS). METHODS: A neonatal rabbit model of DDH was established by maintaining extreme hip flexion with medical tape for 10 days. Serial ultrasound evaluations, including conventional and SRUS, were performed at 0, 1, 2, and 4 weeks after constraint removal. Morphom

[Developmental dysplasia of the hip : Do's and Don'ts].

Congenital hip dislocation represents the most severe form of developmental dysplasia of the hip (DDH) and, if left untreated, it leads to permanent functional impairment. Since the nationwide implementation of ultrasound screening in Germany in 1996, the condition can be detected and managed early on. The primary goal of treatment is a stable concentric reduction of the femoral head to allow normal acetabular development. In irreducible hips, operative reduction and additional corrective osteotomies may be required. Early diagnosis, standardized treatment protocols, and close follow-up are es

Clinical Long-term Outcomes after Modified Pemberton Acetabuloplasty for Developmental Dysplasia of the Hip: a 25-Year Follow-up.

BACKGROUND: Developmental dysplasia of the hip (DDH) is among the most common congenital musculoskeletal disorders and may lead to early hip osteoarthritis if left untreated. Modified Pemberton acetabuloplasty is an established, growth-preserving osteotomy technique for surgical correction in childhood. However, robust clinical long-term outcomes with follow-up periods exceeding two decades have not yet been published. The aim of this study was to retrospectively evaluate functional and patient-reported outcomes after an average follow-up of 25.6 years. METHODS: In this monocentric retrospect
