What Is the Proportion of Late-developing DDH After a Normal Ultrasound in Newborns With Breech Presentation?

BACKGROUND: Ultrasound at 2 to 6 weeks of age is routine clinical practice in neonates at high risk; however, developmental dysplasia of the hip (DDH) may develop after a normal screening examination. There is a current lack of consensus regarding the use and most appropriate timing of subsequent imaging. QUESTIONS/PURPOSES: (1) What proportion of neonates with a breech presentation who had a normal screening ultrasound result for DDH developed DDH at 6-month follow-up? (2) Are patient factors (sex and concomitant congenital muscular torticollis) associated with an increased odds of late-deve

Preventing Missed Developmental Dysplasia of the Hip: Universal vs. Selective Ultrasound Screening.

BACKGROUND: Developmental dysplasia of the hip (DDH) encompasses a spectrum of hip abnormalities in infants. Early identification enables nonoperative management and improved outcomes. Selective screening uses clinical examination to determine referral for ultrasound (US), whereas universal screening performs routine US in all newborns after birth. Despite widespread use of both strategies, there is no consensus on the optimal approach. METHODS: A systematic review was conducted to evaluate the outcomes of selective vs. universal screening. MEDLINE, Embase, and the Cochrane Library were searc

Late Recognition of Suspected Ehlers-Danlos Syndrome After Recurrent Small-Bowel Perforations: A Case Report.

BACKGROUND: Ehlers-Danlos syndrome (EDS) may be overlooked when gastrointestinal and musculoskeletal findings are assessed separately. CASE PRESENTATION: An older woman with two previous small-bowel perforations had bilateral congenital hip dislocation, recurrent shoulder dislocations, skin hyperextensibility, and muscularis propria thinning in nonperforated jejunum. Systematic reassessment using the 2017 classification met minimal clinical criteria suggestive of arthrochalasia EDS, while recurrent bowel perforation raised concern for vascular EDS. Genetic testing was declined. CONCLUSION: T

Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. A 36-year old obese caucasian woman underwent a laparoscopic cholecystecto

Gut microbiota in patients receiving dialysis: Mechanisms, complications and the road to personalized microbiome‑based care (Review).

UNLABELLED: In vivo evaluation of the immunotoxicity of xenobiotics is mainly carried out in rodents. Recent trends following the 3R (reduction, refinement, replacement) principle have driven development of in vitro methods for screening immunotoxic chemicals. The recently reported IMMUNOTOX-T immunotoxicity assay uses the human monocytic leukemia cell line THP-1, and evaluates potential immunotoxicity by profiling production of various cytokines by cells treated with test chemicals. Development of the assay involved small-scale evaluation of transferability, within-laboratory repeatability (W

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Was the ultrasound examination for developmental dysplasia of the hip performed correctly? Introduction of a rapid training tool for nonexpert practitioners.

Pediatricians and general practitioners are involved in the newborn screening for developmental dysplasia of the hip. They often rely on the quality of the ultrasound (US) examination to make diagnostic and therapeutic decisions. Therefore, the professional must be able to assess its quality. The aim of our work is to present a new system to evaluate the quality of a neonatal hip US and to assess the effectiveness and reproducibility of this tool among professionals involved in the evaluation of the neonatal hip but not experts in the radiological examination of hip US. In a 15-min training se

Spontaneous recovery in the majority of stable dysplastic hips treated with active surveillance.

AIMS: Worldwide controversy exists on the optimal treatment of stable dysplastic hips. The most common treatment options are abduction brace treatment and active surveillance. The primary aim of this study was to assess the effect of active surveillance in stable hip dysplasia, by investigating the percentage of Graf IIb stable dysplastic hips that recover spontaneously without abduction brace treatment. The second aim was to identify prognostic factors for spontaneous recovery of stable dysplastic hips. METHODS: A single-centre, prospective cohort study was conducted at the Máxima Medical Ce
