PURPOSE: Periacetabular osteotomy (PAO) is an effective treatment to correct developmental dysplasia of the hip (DDH). Traditionally, the goal of correction during PAO is based on parameters measured on 2-dimensional images. The aim of the study is to introduce an optimized workflow of PAO in DDH patients by means of personalized correction goal and accuracy of execution.
METHODS: Five patients with DDH were prospectively enrolled. Preoperative computed tomography was performed. Surgical planning was done by the treating surgeon and engineers. The planned correction involved reorienting the o

BACKGROUND: Polymelia, or accessory limb, is a rare congenital abnormality with unknown etiology. Here we report the case of an infant with an accessory limb, review the diagnostic and therapeutic consideration of such cases, and highlight the potential challenges in management of an accessory limb.
CASE PRESENTATION: We report the case of a 2-month-old male with an accessory limb arising from the left buttock. The limb was sensate and mobile joints. Our diagnostic workup did not reveal associated spinal dysraphism, but a left dislocated hip was found. The limb was surgically removed, and the

In patients with Crowe III developmental dysplasia of the hip (DDH), surgery presents challenges such as severe bone defects and inadequate acetabular cup coverage. This study compares the clinical efficacy of 3D-printed personalized Augments prostheses with conventional femoral head reshaping and structural bone grafting in total hip arthroplasty (THA) for patients with Crowe III DDH. A retrospective analysis was conducted on 52 Crowe III patients. The 3D group (26 cases) used 3D printing technology combined with computer simulation to design personalized Augments prostheses. Preoperative mod

In hip arthroplasties performed on patients with Crowe type 4 dysplasia, femoral shortening is often required to position the hip into the true acetabulum. Nonunion at the osteotomy site is one of the significant comorbidities in osteotomies that lower the hip into the true acetabulum. This study aimed to evaluate the clinical and radiological outcomes of hip arthroplasty with femoral shortening in patients with Crowe type 4 dysplasia using the technique we developed. This study retrospectively evaluated the outcomes of 96 (117 hips) patients who underwent subtrochanteric transverse shortening

BACKGROUND: S-ROM prosthesis, one well-used femoral prosthesis in the patients with developmental dysplasia of the hip (DDH), has a skipping size of the distal stem diameter. The purpose of this study was to investigate whether its 2-mm incremental diameter could meet clinical needs for high-riding DDH patients.
METHODS: Between July 2018 and December 2022, the Hartofilakidis type C DDH patients with S-ROM stem (9 or 11 mm) were retrospectively enrolled according to the inclusion criteria and exclusion criteria in our institute. The intraoperative femur fractures, the diameter of the femoral

» Persistent abnormal acetabular index values (>25-30° at 4-5 years or > 35° at 2 years of postreduction) may predict poor outcomes and indicate the need for pelvic osteotomy.» Assessment of the acetabular cartilaginous morphology and cartilaginous coverage of the femoral head by magnetic resonance imaging may assist in guiding surgical decisions.» Earlier pelvic osteotomy leverages remodeling potential, but observation until 4 to 5 years may be preferred to minimize risks and allow spontaneous correction.» Routine monitoring is crucial for stable, dysplastic hips, focusing on acetabular index

BACKGROUND: In the treatment of DDH, stable but dysplastic hips are safe to observe, and these children do not usually need abduction treatment. It has been reported, that also clinically unstable hips have good spontaneous recovery potential, but only a few studies have investigated the observation strategy in clinically mildly unstable (Barlow positive) hips. A conclusion on the safe treatment strategy for these children has not been made.
MATERIALS AND METHODS: All early diagnosed mildly unstable (Ortolani negative) hips treated in Tampere University Hospital in 1998-2018 were found, and d

BACKGROUND: The change of designation from congenital dysplasia of the hip to developmental dysplasia of the hip (DDH) indicates that the disease spectrum of hip dysplasia has changed. However, the wide-used Crowe classification existed before the DDH definition was proposed. Thus, the Crowe classification does not effectively identify DDH patients who have concurrent femoral neck deformities, resulting in these cases being overlooked.
METHODS: A total of 473 DDH patients (574 hips) who had undergone total hip arthroplasty from January 2016 to September 2023 were recruited into this study. Th

BACKGROUND: Patients with secondary hip osteoarthritis due to developmental dysplasia of the hip (DDH) often have abnormal femoral morphology, making stem design critical for long-term outcomes. The FMS-anatomic stem previously demonstrated favourable mid-term results. Its successor, the Anatomic Fit stem, was developed with a reduced hydroxyapatite-coated area to enhance proximal load transfer and a narrower lateral flare to facilitate insertion. This study aimed to analyze the long-term clinical and radiographic outcomes of these stems.
METHODS: The one-hundred seventy-eight hips underwent

LONP1 encodes a mitochondrial protease essential for protein quality control and metabolism. Variants in LONP1 are associated with a diverse and expanding spectrum of disorders, including Cerebral, Ocular, Dental, Auricular, and Skeletal anomalies syndrome (CODAS), congenital diaphragmatic hernia (CDH), and neurodevelopmental disorders (NDD), with some individuals exhibiting features of mitochondrial encephalopathy. We report 16 novel LONP1 variants identified in 16 individuals (11 with NDD, 5 with CDH), further expanding the clinical spectrum. Structural mapping of disease-associated missense

BACKGROUND: Developmental dysplasia of the hip (DDH) is the most common developmental musculoskeletal disorder. Late presentation of DDH is defined by a diagnosis made when a child is older than 3 months. Few studies evaluate the effectiveness of conservative management with bracing in this cohort.
AIMS: This study aimed to evaluate the effectiveness of nonoperative treatment, using the Pavlik harness and Boston brace, in patients presenting with developmental dysplasia of the hip (DDH) who commenced treatment following initial ultrasound scans done between 91 and 182 days (3 to 6 months). Th

INTRODUCTION: Total Hip Arthroplasty (THA) with leg lengthening can offer great outcome in patients with severe hip deformities but comes with higher risk of intra-operative nerve injury. The aim was to investigate the relationship between surgical steps and multimodal intraoperative neuromonitoring (IONM) events during THA in severe hip deformities. METHODS: This cohort study of 27 cases with symptomatic severe hip deformities, such as Crowe 3 and 4 developmental dysplasia of the hip (DDH). All cases received THA with IONM between September 2019 and December 2023. The goal of surgery in all c
