Natural history of congenital hip dislocation (CDH) and hip dysplasia.

For normal development of the hip joint to occur, there must be a delicate, genetically determined balance between growth of the acetabular and triradiate cartilages and a well-centered femoral head. This balance may be profoundly affected by the intrauterine environment. Without treatment, it is uncertain how many dysplastic, unstable hips will retain their dysplastic features throughout life. The natural history of untreated complete dislocation varies considerably and is affected by societal considerations. There may be little, if any, functional disability in many cases. Hip pain in adolescence.

No single pelvic or femoral osteotomy can be expected to correct the range of deformity encountered in the adolescent with a subluxed hip or the sequelae of treatment at a younger age. A range of pelvic and femoral osteotomies on both sides of the joint are required when there is marked deformity. The principles of management of the anatomic derangement of the painful adolescent hip are different from those in the younger child. Anatomic and biomechanical restoration at skeletal maturity must be perfect for the hip to function well in the long term. The plasticity and capacity for remodeling i

The hip in arthrogryposis multiplex congenita.

The hip is involved in up to 80% of individuals with a diagnosis of arthrogryposis multiplex congenita. The hip deformity consists of contracture with or without dislocation. Isolated contracture can usually be treated conservatively by manipulation and splinting, only occasionally requiring operative intervention. Dislocation is as frequently bilateral as unilateral. Bilateral hip dislocations are best left unreduced; only accompanying contractures should be treated. The unilateral dislocation should be treated aggressively, because persistent dislocation will give rise to pelvic obliquity an

A plan of management and treatment results in the arthrogrypotic hip.

In a retrospective review of 52 hips in 26 patients with arthrogryposis, 65% of all hips studied were affected by the disorder; 46.6% of the patients were independent ambulators, 33.3% required some type of bracing for ambulation, and 20% were confined to a wheelchair. Based on the results of the authors' treatment as determined by a functional hip score, the authors recommend a one-stage open reduction, varus shortening femoral derotational osteotomy for all unilateral hip dislocations and those bilateral hip dislocations that have a less severe generalized involvement. They also recommend an

Brief clinical report and review: the Marden-Walker syndrome.

We have studied a sibship with one confirmed and three probable cases of the Marden-Walker syndrome (MWS). Our patient had the major manifestations of blepharophimosis and squint; narrowly arched palate with micrognathia; small mouth and mouth-breathing; facial deformities and distortions; congenital muscle weakness with resulting scoliosis; mild pectus excavatum; camptodactylies and hip and finger joints subluxation. In addition, he had small, apparently low-set and slightly malformed auricles with a unilateral preauricular tag. However, he had no apparent renal or cardiovascular involvement.

A comparative study of bilateral versus unilateral congenital dislocation of the hip.

The presentations, treatment requirements, results, and complications of 50 patients with bilateral congenital hip dislocation were evaluated and compared with a similar group of unilateral dislocations. Diagnosis was significantly delayed in the bilateral dislocations. The only consistent sign on initial clinical examination of the bilateral group was the symmetrical restriction of hip abduction. Treatment requirements were similar for the unilateral and bilateral dislocations, and in fact, the unilateral dislocations were more likely to undergo reconstructive procedure after successful close

[Treatment of congenital dislocation of the hip in older children (author's transl)].

In children who are older than three years the obstacles of reduction can be overcome without danger for the blood supply of the femoral head only by operative measures. It stands the test to combine open reduction with the nearly always necessary correction of the dysplastic acetabulum, the shortening of the femur and derotating osteotomy. The results definitely depend on the individual indication and the operating technique. In cases of severe dysplasia of the acetabulum the acetabuloplasty shows better results in correction and reduction of the hip joint than Salter's osteotomy. Chiari's os

Late congenital dislocation of the hip.

Congenital dislocation of the hip in a child over the age of 4 may be discovered as a late untreated displacement, presenting with subluxation or dislocation, or as a late treated displacement presenting as a subluxating or subluxated hip. Conservative treatment is inadequate in untreated dislocations. Surgical reduction is the only possible procedure, associated with shortening femoral. The procedure may be used up to the age of 7 in bilateral cases, and up to the age of 12 in unilateral cases. Subluxating hips are recognised on the anteroposterior radiograph by a break in Shenton's line. A d

Pelvic obliquity as a factor in dysplastic hip in infancy.

Screening of the newborn to detect hip dysplasia has led to the increasing recognition of fixed pelvic obliquity, in which one hip has restriction of abduction and the other, restriction of adduction. The hip with limited abduction is apparently short, so that the infant presents signs clinically indistinguishable from those of congenital dislocation. During the years 1974-78, 192 consecutive cases of unilateral dysplastic hips were treated by us; 44 (23%) were diagnosed as having pelvic obliquity. Nine were males and 35, females; 39 were firstborn. A characteristic postural pattern was found

Skeletal traction vs. femoral shortening in treatment of older children with congenital hip dislocation.

A review of skeletal traction compared with femoral shortening in treatment of older patients with congenital dislocation of the hip clearly favors femoral shortening. During the period 1952-74, 30 hips in 22 patients (average age, 6 6/12 years) were treated with skeletal traction, and during 1971-77, 17 hips in 12 patients (average age, 6 8/12 years) were treated with femoral shortening. In those treated with skeletal traction, avascular necrosis developed in 57%; redislocation or subluxation in 30%; an X-ray rating (Severin) of poor in 43%; and the incidence of pin and skin complications was

Arthrography of the hip in children. Technique, normal anatomy and findings in unstable hip joints.

A method by which bilateral hip arthrography was performed under general anaesthesia in 130 infants and children 1 month to 7 years of age is described. Significant contractures of the adductor muscles had been corrected before the examination. Most of the children were girls with unilateral congenital idiopathic hip instability detected before the age of one year. From films exposed in standardized projections valuable information was obtained about the configuration of the joint cavity and the size and shape of the femoral head. Criteria for normal appearance in infants of ages 2 to 12 month

The natural history of congenital disease of the hip.

Fifty-four adults with eighty hips affected by congenital disease which had not been treated have been reviewed. Fifty-nine per cent of forty-two dislocated hips had fair or poor grading scores. The incidence of osteoarthritis was markedly increased in the presence of a well-developed false acetabulum. Unilateral dislocation led to valgus deformity and degenerative changes in the ipsilateral knee in seven of twenty-two patients. Dislocation did not increase the incidence of symptomatic lumbar spondylosis. The height of the dislocated head on the ilium was not found to be related to the prognos

[Aetiopathogenesis of scoliosis, hip dysplasia and torticollis in infancy (author's transl)].

The special risk of the newborn to acquire, skeletal deformities is based on the increased plasticity, intensity of growth and extrapyramidal activity of reflexes which influence each other. These 3 factors may lead to structural growth- and weight bearing deformities in the presence of shrinking contractures and a unilateral oblique habitual position of the trunk. Considering the "Seventh Syndrome" the dual pathogenesis of scoliosis in infancy is discussed from our present point of view (including scoliosis due to oblique body position - moulded baby syndrome - and its deterioration as agains

Congenital hip disease in a Cree-Ojibwa population: a retrospective study.

Retrospective study of data from annual surveys and hospital records over 23 years confirmed the 1950 report of a high prevalence of congenital hip disease (CHD) in the Cree-Ojibwa population of Island Lake, Man. Annual ascertainment rates ranged from 35 to 600 cases per 1000 live births; 5-year rates for dislocation or subluxation were the highest reported for any population. The minimally declining rates of CHD may reflect upgrading in criteria for hip abnormality as well as decreasing isolation and increasing outbreeding of the population. The preponderance of females (female:male ratio, 1.

Combined procedure of open reduction and shortening of the femur in treatment of congenital dislocation of the hips in older children.

A one-stage, combined operative procedure for reduction of congenitally dislocated hips in older children consists of shortening of the femur; open reduction by an inferior approach to the joint; reconstruction of the acetabular roof; correction of anteversion of both the femoral neck and the neck-shaft angle; anterior transposition of the iliopsoas muscle. On 60 hips operated in children, ages 5 to 15 with a follow-up period, ranging from 5 to 9 years, the results were found to be excellent in 3 per cent, good in 60 per cent, fair in 30 per cent, and poor in 7 per cent. The procedure is indic
