Vascularised iliac graft as a method of acetabular augmentation in subluxed hip in adolescents.

This article presents the clinical experience of patients with a dysplasic subluxated hip treated with the same procedure involving a vascularised graft of iliac crest by sartorius muscle. This study was performed in three different centres, in two different countries, and in 31 patients with 37 hips (9 bilateral). All patients were clinically evaluated according to the Merle d Aubigne scale and radiological measuring of acetabular coverage, Sharp angle, and CE angle. Walking ability preoperatively was 4.87 and postoperatively 5.12, which was not statistically significant. Preoperative inclina

Bilateral symptomatic snapping biceps femoris tendon due to fibular exostosis.

We present an unusual case of an athletic 17-year-old male cyclist with bilateral chronic dislocating biceps femoris tendons. On flexion of the knee, the biceps tendon subluxed over a large exostosis, creating a snapping sound. Snapping of tendons is common around the hip, ankles, shoulder, and elbow, but rare at the knee. When it does occur, snapping about the knee can be due to discoid meniscus, rheumatoid nodules, synovial plicae, iliotibial band syndrome, congenital snapping knee, and snapping tendons. Research revealed only 5 previous cases due to subluxation of the biceps femoris tendon.

Bilateral incidence and severity of acetabular dysplasia of the hip.

BACKGROUND: Most Japanese patients have secondary osteoarthritis, mainly due to developmental dislocation of the hip (DDH) or acetabular dysplasia (AD). However, the precise pathomechanism of AD remains unknown. The purpose of this study was to investigate the frequency of bilateral AD and determine the correlation of the severity of AD between the right and left hips. METHODS: A total of 206 patients with prearthritis or early-stage osteoarthritis caused by AD were examined radiographically, and their history of treatment for DDH during infancy was reviewed. There were 187 women and 19 men i

New clinical findings in the Richieri-Costa/Pereira type of acrofacial dysostosis.

The Richieri-Costa/Pereira form of acrofacial dysostosis is an autosomal-recessive condition characterized by short stature, Pierre-Robin sequence, preaxial and postaxial abnormalities in hands, congenital talipes, cleft mandible and malformations of the larynx. We report female infant presenting with severe micrognathia, a hypoplasic clavicle, median mandible cleft, bilateral hand abnormalities and talipes, laryngeal malformations, hip subluxation with acetabular dysplasia and mesomelic shortening of limbs. A few reported patients have clavicular hyploplasia but hip subluxation with acetabula

Undetected central core disease myopathy in an infant presenting for clubfoot surgery.

A 1-year-old child was scheduled for two stage bilateral clubfoot surgery. Preoperative evaluation was normal and total intravenous anesthesia with a continuous sciatic nerve block was performed. Two months later, before the second clubfoot correction, a hip subluxation was evident suggesting a provisional diagnosis of neuromuscular disease. Anesthesia was identical, except that a femoral nerve block, necessary to permit a diagnostic muscle biopsy was performed. The perioperative course was uneventful but result of the muscular biopsy was surprising in that central core disease was diagnosed.

[Reconstructions of the teratogenic hip dislocation in childrens with AMC].

UNLABELLED: Hip diclocation is the cause of the mobility impairment in children with arthrogryposis. Hip joint reconstruction is a commonly accepted treatment method in unilateral dislocations, but opinions are divided in bilateral. AIM OF THE STUDY: Hip joint function assessment after reconstruction due to teratogenic dislocation in arthrogryposis. MATERIAL AND METHODS: In years 1988 to 2005 we performed 9 reconstructions of the teratogenic hip dislocation in 8 children with AMC (Arthrogryposis multiplex congenita). Mean age of children at the time of procedure was 3.5 years. Mean follow-up

Long-term follow-up after one-stage reconstruction of dislocated hips in patients with cerebral palsy.

Twelve consecutive patients (average age 10.6 years) with 14 dislocated hips underwent one-stage hip reconstruction between 1973 and 1981. The procedure consisted of (1) adductor myotomy and anterior obturator neurectomy, (2) circumferential capsulotomy, iliopsoas and external rotator tenotomies, and ligamentum teres and pulvinar excision, (3) shortening femoral varus derotational osteotomy, (4) acetabuloplasty, and (5) spica immobilization for 4 weeks. All patients were followed clinically and radiographically at an average of 16.7 (range 12.4-19.5) years. No patients were lost to follow-up.

[Pseudodominant inheritance of spinal muscular atrophy--father and son suffering from SMA].

BACKGROUND: cancer registries (CRs) are essential tools for oncological surveillance. The accurate coding of topography and morphology (through ICD-O-3 coding), traditionally performed manually, is complex and time-consuming. Artificial Intelligence (AI) offers new opportunities to automate this process, overcoming the limitations of existing algorithms, which often focus only on topography. OBJECTIVES: to develop an AI-based algorithm capable of automatically assigning the combined topography-morphology (topo-morpho) code from a synthetic clinical pathway expressed in natural language (LN-PD

Combined pelvic osteotomy in the treatment of both deformed and dysplastic acetabulum three years--prospective study.

Kuwait has a very high incidence of all types of developmental dysplasia of the hip. One of the most complicated forms is the deformed dysplastic acetabulum, which has not only abnormal orientation but it also has lost its shape by the pressure of the subluxed femoral head. The outer part of the socket involves the upper part of the original acetabulum, thus giving it a bipartite appearance. Clear border like edge separates this outer part from the inner part, which represents the lower part of the original acetabulum and it has no direct contact with the femoral head. Combined pelvic osteotom

Re-dislocation following open reduction for developmental dysplasia of the hip.

We have reviewed 17 patients (18 hips) who required repeat open reduction for recurrent or persistent dislocation after a previous attempt at open reduction for developmental dysplasia of the hip (DDH). The mean follow-up was 5.5 (3-15) years. At the review, five patients were limping and had limited hip motion but no pain. Six hips were classified as Severin class III or more, and avascular necrosis was evident in 11. We suggest that technical failure is usually the cause for re-dislocation in unilateral cases, as we discovered at repeat open reduction in five of six patients with unilateral

The surgical treatment of teratologic dislocation of the hip.

Since 1974, our approach to treatment of teratologic dislocation of the hip in children has been surgical. We retrospectively reviewed 20 teratologic hip dislocations in 13 children with a minimum follow-up of 2 years. Closed treatment failed in most of the hips. At a mean age of 13 months, 11 patients (18 hips) had an open reduction and a one-and-a-half hip spica. It was followed in most of these hips by a femoral varus derotational osteotomy 6 weeks later. Only three hips had an open reduction followed by 3 months of casting. One of these three hips had salmonella infection and a redislocati

Baby S began oral feedings on day 8 at 20 ml/kg/day. The infant reached full enteral feedings on day 11 and was subsequently advanced to an ad lib demand schedule. The broviac catheter was removed on day 13. An orthopedic surgeon was consulted to evaluate abnormalities of the upper extremities and hip laxity noted on admission examination. The orthopedic specialist found decreased abduction of the hips bilaterally with no instability and normal range of motion on exam. An ultrasound showed bilateral posterior subluxation with no frank dislocation. The infant was evaluated by a physical therapi

Congenital dislocation of the knee in Ibadan, Nigeria.

Between January 1996 and December 2001, 41 congenital dislocations of the knee joints (30 patients) were reduced with closed methods by immediate reduction without anaesthesia and serial casting in plaster of Paris immobilization for a period of six to eight weeks. The patients' age ranged from the age of one week to four weeks with a male to female ratio of 1.5:1. The right knee was involved in 46.65% the left in 16.6% and bilateral involvement in 36.65% of patients. Routine check of the hip did not reveal any patient with hip instability. All the patients followed up after 2 years showed exc

Acetabular dysplasia associated with hereditary multiple exostoses. A case report.

Hereditary multiple exostoses is an autosomal dominant disorder characterised by multiple osteochondromata, most commonly affecting the forearm, knee and ankle. Osteochondromata of the proximal femur have been reported to occur in 30% to 90% of affected patients with coxa valga in 25%. Acetabular dysplasia is rare but has been described. This is the first report of a patient requiring surgical intervention. A girl was seen at the age of nine with hereditary multiple exostoses and when 12 developed bilateral pain in the groin. Radiographs showed severely dysplastic acetabula with less than 50%
