New approaches to preventing venous thromboembolism, prognostication in stroke and myocardial infarction, and antiplatelet therapy after spontaneous coronary artery dissection.

Immobility following orthopedic surgery is a risk factor for venous thromboembolism (VTE), but the optimal duration of anticoagulant thromboprophylaxis remains controversial. The protocol of the "Enhanced Recovery and Abbreviated Length of Anticoagulation for Thromboprophylaxis After Primary Hip Arthroplasty" (ENABLE-Hip) is presented, an investigator-initiated randomized double-blind trial, comparing shorter anticoagulant duration with standard-of-care. To improve outcomes after complex thrombotic events, a novel multidisciplinary approach is presented, based loosely on the tumor board model

Knowledge, attitudes, and perceptions of developmental dysplasia of the hip among pregnant women in Jordan.

OBJECTIVES: To assess the awareness of developmental dysplasia of the hip (DDH) among pregnant women in Jordan. METHODS: A descriptive cross-sectional study was carried out among pregnant females, aged 18 years and above, who had visited 3 hospitals in Jordan. Data were collected through face-to-face interviews, and responses were recorded using a Google form by the researchers. RESULTS: A total of 605 women completed the questionnaire. Most participants correctly reported DDH as the cause of ambulation problems (n=531, 87.6%), and approximately 60% reported it as painful. The majority agree

Radiographic criteria in developmental dysplasia of the hip in late infancy, inter and intrareader agreement.

INTRODUCTION: Developmental Dysplasia of the hip (DDH) is a common pediatric disorder screened for by antero-posterior (AP) pelvic radiographs in infants aged between 4-9 months. We chose from the radiographic indicators commonly used in the diagnosis the acetabular index, the Shenton line and the ossification and symmetry of the femoral head to assess for their reliability and variability among readers. In addition, this study aimed to obtain the mean age of appearance of the ossification center of the femoral head in the Lebanese population. METHODS: 149 pelvic AP radiographs of children be

A Rare Case of Congenital Knee Dislocation.

Congenital knee dislocation (CKD) is a rare condition that can present as an isolated finding or in association with genetic syndromes. It is most commonly diagnosed postnatally and classified into three grades based on severity. While generally having a favorable prognosis, particularly for milder cases, prompt treatment is crucial. We present a case of CKD successfully treated with conservative measures, with serial casting and Pavlik harness, in an otherwise healthy infant. This report aims to increase awareness of the clinical presentation and management of CKD in general practitioners giv

Congenital Knee Dislocation: A Case Report.

Congenital knee dislocation (CKD) is a rare musculoskeletal condition that may occur in isolation or be associated with other congenital abnormalities. Prognosis depends on the severity of the deformity and the timing of diagnosis and treatment. Comprehensive assessment and close follow‑up are essential to optimize recovery and to exclude associated anomalies. We report the case of a female newborn with CKD, diagnosed postnatally, associated with developmental dysplasia of the hip (DDH), generalized ligamentous laxity, congenital heart disease, and a chromosomal abnormality. Early conservative

Quantification of the changes in acetabular index and Center-edge angle of pelvic radiograph induced by photography angles.

BACKGROUND: In regions lacking professional digital imaging equipment, parents sometimes use smartphones to take photos of pelvic X-ray films and transmit them to clinicians via a messaging application for consultation. However, the accuracy of measuring parameters through photos is unknown. Certain factors that affect photograph quality may lead to deviations in pelvic radiographic measurements. This study investigates the impact of photography angles on measurements of the Acetabular Index (AI) and Center-edge Angle (CEA). METHOD: We collected pelvic radiographs from 50 patients who were su

The epidemiology of hip dysplasia in the nationwide Norwegian Mother, Father, and Child Cohort Study.

AIMS: Developmental dysplasia of the hip (DDH) is a congenital disorder with several assumed risk factors, including breech presentation, female sex, and familial predisposition. Although several of these risk factors are included in national screening programmes, delayed diagnoses of DDH still occur. The aim of this study was to examine the prevalence of these and other risk factors in order to improve the current screening programmes. METHODS: This study used data from the Norwegian Mother, Father, and Child Cohort Study (MoBa) and the Medical Birth Registry of Norway (MBRN). We used descri

Universal Clinical DDH Screening Complemented with Targeted Ultrasound Is Effective in Finland.

BACKGROUND: The late diagnosis rate of developmental dysplasia of the hip (DDH) with universal ultrasound screening is 0.2 per 1,000 children according to a recent meta-analysis, which is the same as in Japan where selective ultrasound screening is used. We hypothesized that Finland's current program of universal clinical screening complemented with targeted ultrasound is noninferior to universal and selective ultrasound screening programs. METHODS: For this retrospective cohort study, we collected the number of children <15 years of age who were diagnosed with DDH (International Classificati

Bilateral Chronic Anterior Cruciate Ligament Tear or Congenital Absence of Anterior Cruciate Ligament? - A Case Report.

INTRODUCTION: Congenital absence of the anterior cruciate ligament (ACL), first reported by Giorgi in 1956 is an extremely rare condition. The prevalence of this condition is reported to be around 0.017/1,000 live births. This congenital anomaly can present in isolation or be associated with other skeletal malformations, including fibular hemimelia, congenital femoral deficiencies, and hip dysplasia. Congenital ACL deficiency may often go undiagnosed in early life. Others will eventually experience symptoms due to the long-term effects of knee instability that can lead to chronic knee pain, pa

Simulation-based training reduces unnecessary ultrasound referrals for developmental dysplasia of the hip in newborns.

BACKGROUND: Developmental dysplasia of the hip is a common newborn condition characterized by malformation or instability of the hip joint. Identifying dysplasia of the hip early is imperative for avoiding long-term consequences. In the United States, dysplasia of the hip screening of newborns uses the Ortolani and Barlow maneuvers. However, evidence suggests that inadequate training in these techniques may lead to unnecessary imaging referrals for further assessment. We investigated the impact of a dysplasia of the hip simulator (MiHip) and its associated training program on ultrasound referr

Morphological differences between residual childhood hip dysplasia with previous osteotomy and adolescent-onset hip dysplasia.

BACKGROUND: Hip dysplasia (HD) at skeletal maturity can result from residual developmental dysplasia of the hip (DDH) treated in childhood or from primary adolescent-onset HD (AOHD). This study aims to compare the pathomorphology of these two HD subtypes with that of a normal control group. METHODS: This retrospective study reviewed patients who underwent periacetabular osteotomy for symptomatic HD between 2013 and 2020. The study included 27 residual HD patients (32 hips) following a previous pelvic osteotomy and 39 AOHD patients (68 hips), compared to 29 age- and sex-matched healthy individ

Genetic Analysis of Familial Developmental Dysplasia of the Hip Associated With a Heterozygous Variant in the COMP Gene: A Case Report.

BACKGROUND: Developmental dysplasia of the hip (DDH) is a prevalent congenital musculoskeletal disorder characterized by structural abnormalities of the hip joint. While its etiology involves genetic and environmental factors, specific genetic mechanisms remain poorly understood. Mutations in the COMP gene (COMP; OMIM: 600310), classically associated with skeletal dysplasias such as pseudoachondroplasia (PSACH) and multiple epiphyseal dysplasia (MED), are rarely linked to isolated DDH. METHODS: A 29-year-old male proband with familial DDH underwent clinical evaluation and radiographic imaging

Network Analysis of Legg-Calve-Perthes Disease and Its Comorbidities.

Background/Objectives: Legg-Calvé-Perthes disease (LCPD) is characterized by idiopathic avascular necrosis of the femoral head in children. There are several hypotheses regarding the cause of LCPD; however, the exact cause remains unclear. Studies on comorbidities can provide better insight into the disease. We aimed to perform a network analysis to identify the associations between LCPD and comorbidities. Methods: We analyzed patients aged ≤ 12 years with LCPD as defined by ICD-10 codes M91.1, M91.8, and M91.9 registered in the National Health Insurance Service cohort database from 2002 to 20

Clinical and Genetic Characterization of Hereditary Sensory and Autonomic Neuropathy Type IV in a Consanguineous Population: Identification of Novel NTRK1 Variants and Expansion of Phenotypic Spectrum.

Hereditary sensory and autonomic neuropathy type IV (HSAN4) is a rare neurological disorder characterized by anhidrosis and congenital insensitivity to pain caused by biallelic pathogenic variants in NTRK1. The condition develops because of dorsal root and autonomic ganglion neurodegeneration, which ultimately results in reduced sensation and autonomic neurological dysfunction. We ascertained several neuropathic patients and performed genetic testing using gene panels and exome sequencing (ES). Genetic variants were confirmed by Sanger sequencing. Thirteen families, each with a single affected

Epidemiology and Screening of Developmental Dysplasia of the Hip in Europe: A Scoping Review.

Developmental hip dysplasia or developmental dysplasia of the hip (DDH) includes a wide range of deformities of the hip, such as congenital dysplasia, subluxation, and dislocation. It is usually identified through neonatal screening during the first 6-8 weeks of life. The incidence of DDH ranges from 1-7% in neonates among some populations, but this may vary among different ethnicities and countries. A consensus about the ideal age for screening has not been reached to date. The aim of this study is to summarize the existing data regarding the incidence of congenital hip dysplasia and screenin

Open reduction after failed closed reduction following failed Pavlik harness treatment in developmental dysplasia of the hip: One- or two-stage?

INTRODUCTION: The current published evidence for the treatment of developmental dysplasia of the hip (DDH) with failed closed reduction (CR) following failed Pavlik harness (PH) treatment is still limited. This study aimed to determine whether an one-stage open reduction (OR) would lead to a similar outcome to a two-stage OR in these patients. Meanwhile, the occurrence of femoral head avascular necrosis (AVN) and further surgery (pelvic osteotomy, PO) within the follow-up period was investigated. MATERIALS AND METHODS: A consecutive series of DDH patients who failed CR following failed PH tre

Who performs neonatal hip assessment: is there a cause for concern?

OBJECTIVE: The UK falls behind other European countries in the early detection of developmental dysplasia of the hip (DDH) and screening strategies differ for early detection. Clinical detection of DDH is challenging and recognised to be dependent on examiner experience. No studies exist assessing the number of personnel currently involved in such assessments. Our objective was to review the current screening procedure by studying a cohort of newborn babies in one teaching hospital and assess the number of health professionals involved in neonatal hip assessment and the number of examinations u
