[Futher means for reducing radiation dose during exposure of the pelvis in infants (author's transl)].

A simple dose-saving procedure for the diagnosis of congenital hip dysplasia employing a diaphragm in nearfocus position is presented. Doses to the skin and the gonads were measured with a phantom for the new and other techniques. With optimal techniques the skin dose is smaller than 2 mR and the gonadal doses lie around 0.1 mR for both male and female patients. An estimate of the dose burden from undesired radiation is appended.

Juvenile fibromatosis and other diseases of connective tissue.

There are a number of features which are common to such variable conditions as juvenile fibromatosis on the one hand and torticollis and congenital dislocation of the hip, and possibly scoliosis, on the other. Various pathological syndromes are presented, all of them characterized by disturbance and contracture of connective tissue. Surgery is carried out in severe cases when it is a practicable proposition. The administration of an enzyme inhibitor at cellular level has introduced a new dimension in the management of localized fibromatosis.

[Aetiopathogenesis of scoliosis, hip dysplasia and torticollis in infancy (author's transl)].

The special risk of the newborn to acquire, skeletal deformities is based on the increased plasticity, intensity of growth and extrapyramidal activity of reflexes which influence each other. These 3 factors may lead to structural growth- and weight bearing deformities in the presence of shrinking contractures and a unilateral oblique habitual position of the trunk. Considering the "Seventh Syndrome" the dual pathogenesis of scoliosis in infancy is discussed from our present point of view (including scoliosis due to oblique body position - moulded baby syndrome - and its deterioration as agains

[Frequency of necroses of the head of the femur in the treatment of the so-called congenital dislocation of the hip joint according to Hoffmann-Daimler (author's transl)].

The article reports on the results which can be obtained with the method described by Hoffmann-Daimler regarding the functional treatment of the so-called congenital dislocation of the hip joint. The report is based on a patient material of 110 children with 159 dislocated hips treated at the Orthopaedic University Clinic at Erlangen between 1971 and 1976. Observation of the course of the disease revealed a much lower rate of incidence of necrosis of the head of the femur than stated in literature, using the Hoffmann-Daimler method. The incidence of necrosis on pre-treatment of most children w

[The inguinal incision for the open reduction of congenital hip dislocation (author's transl)].

Operative reductions of congenital hip dislocation are sometimes difficult when lateral incisions are used since the medial part of the acetabulum and the lower limbus are difficult to visualize. In the Ludloff technique this part of the acetabulum is seen excellent, but not the lateral part. High dislocations with inverted limbus are difficult to reduce and only with partial excision of the limbus. An inguinal incision beginning lateral at the spina ilica ventralis offers the best view and can be used for all different degrees of dislocation and age groups. The technique is described, the adv

Family studies of relation between Perthes disease and congenital dislocation of the hip.

A family study of Perthes disease and congenital dislocation of the hip was made in the Faroe Islands, with a population of 40 000. The examination included 1123 sibs and first cousins of 43 probands with Perthes disease, 1942 sibs and first cousins of 59 dislocation probands, and 5205 sibs and first cousins of 172 unaffected matched controls. Both conditions occur with exceptionally high incidences in this population. Thus the incidence of Perthes disease was found to be 41: 10 000 males and 7: 10 000 females, of congenital dislocation of the hip 7: 10 000 males and 59: 10 000 females. These

Difficulties in diagnosing and managing congenital dislocation of the hip.

In a 10-year retrospective study of 25 921 consecutive deliveries in a neonatal unit in Newcastle upon Tyne 271 cases of congenital dislocation of the hips were identified. Of these, the outcome was unsatisfactory in 12: four diagnoses were missed at birth and eight children required further surgical treatment. Radiological abnormalities were detected in a further five children at long-term follow-up examination. From the results of this study and other published series it was concluded that lack of attention to detail was the main cause of inadequate diagnosis and management of congenital dis

[A new technique for socket implantation into dysplastic acetabula (author's transl)].

This report presents a new technique of socket implantation into the dysplastic acetabulum. A major part of the resected femoral head serves as the so-called intra acetabular femoral head graft and is screwed into the lateral part of the original acetabulum. By this technique it is possible to achieve a complete bony reconstruction of the dysplastic acetabulum into which a square shaped small ceramic socket can be easily implanted using few especially designed instruments. After the description of the operative technique this report presents the clinical results of this procedure based on 11 p

Hip dysplasia in beef cattle.

A bilateral hip lameness has been found in young beef bulls of the Hereford, Aberdeen Angus, Galloway and Charolais breeds in the United Kingdom, North America and Australia. The incidence is usually small and sporadic, but up to half of the male animals on one farm may be affected clinically by the time they are one year old. Clinical lameness begins at three months to two years old, and very occasionally up to three years of age; some calves are obviously affected at birth. Aberdeen Angus cattle tend to be affected earlier than other breeds. Clinical evidence of the condition is almost entir

Proximal focal femoral deficiency.

The term proximal focal femoral deficiency (PFFD) is applied to a spectrum of conditions characterized by partial absence and shortening of the proximal femora and thought to result from an early disturbance of growing mesenchyme. The mildest cases, Classes A and B, exhibit a relatively normal acetabulum and capital femoral epiphysis, despite the dysplastic shaft. There may be a subtrochanteric varus deformity or a pseudoarthrosis accompanying the shortening of the femoral shaft. At the opposite end of the spectrum, the most severely deformed cases, Classes C and D, have involvement of the ace

Proximal femoral focal deficiency.

Proximal femoral focal deficiency, a distinct clinical and radiographic entity that results in leg length discrepancy, has received little attention in the radiographic literature. Thirteen patients were studied and the degree of deficiency established using radiographic criteria. The full extent of hip instability often could not be ascertained on radiographs obtained during the first year of life. Hip arthrograms combined with cine radiography can establish the presence of an unossified femoral head and aid in evaluating the degree of hip instability. Unlike other causes of leg length discre

The aetiology of Perthes' disease. Genetic, epidemiological and growth factors in 310 Edinburgh and Glasgow patients.

The aims of this survey were to establish the familial incidence of Perthes' disease, to note any associated developmental anomalies and to collect information on preceding trauma or synovitis, on the pregnancy and birth, and on various sociological factors. Height and weight measurements were obtained for 217 patients, and comparisons made with those of their parents, unaffected sibs and (local) controls. Results showed an extremely low frequency of Perthes' disease among relatives, with no obvious pattern of inheritance. As genetic factors were not apparent, environmental and sociological ca

[Juvenile hip pain. 2. Femur head epiphysiolysis, hip dysplasia, tumors].

The early symptom in hip joint diseases in children is pain. Pain is localized in the groin and thight, but mostly in the knee. Other important signs are limping and reduced internal rotation. If a hip disease is suspected it is necessary to take X-rays in two planes. If diagnosis is early and special therapy started immediately, the results are usually excellent without deformation of the hip. Otherwise early osteoarthritis can develop. This is important because osteoarthritis in the hip joint is in 75% of the cases due to hip joint diseases in childhood. The problems of diagnosis and treatme

THARIES surface replacements: a review of the first 100 cases.

The first 100 cases of the 135 THARIES surface replacement procedures with 4--32 months follow-up, are evaluated in terms of clinical results, radiographic information and complications. The short but detailed follow-up suggests that this procedure was an excellent alternative for the younger and more active population. The overall hip ratings (pain, walking and function) and range of motion are comparable to that of stem-type total hip replacements. When the THARIES and conventional hip results are further compared in 34 patients matched by sex and age for 3 major etiological groups (osteoart

Menarcheal age in girls with congenital dislocation of the hip.

The age at menarche was recorded in a series of girls with congenital dislocation of the hip and compared with a control sample drawn from the population at risk. The menarcheal age of the CDH patients was decreased by, on average, six months, the decrease being caused by the presence among the CDH girls of a few individuals with a very early menarche. These girls were heavier and taller than other CDH girls, who in their turn, are taller and heavier than other girls. The findings support the concept of CDH as a manifestation of some hormonal deviation, which also causes early physical maturat

Paralytic instability of the hip in myelomengoceles.

The results of a Colonna type arthroplasty with several modifications were evaluated in 29 hips of 22 myelomeningocele children with subluxations or dislocations. The operations were performed only in children with high neurosegmental levels where no muscle transfers were possible or when previous surgical procedures failed to maintain hip stability. Improvement in radiographic and clinical stability was demonstrated in 79 per cent of the hips. Eighty-three per cent of the hips lost motion in the flexion-extension arch with a mean loss of 30 degrees. The arthroplasty improved hip stability but

[Treatment, at birth, of luxating deformities of the hip. Difficulties, failures and complications. Discussion apropos of coxa vara deformities].

The author has studied 400 newborn babies thought to have unstable hip. The importance of careful clinical and radiological examination is emphasized. An abduction splint must be applied with caution and serial radiographs taken until the hip is normal. In spite of this, one instance of coxa magna, three of osteochondritis and five of coxa vara were found. A classification of coxa vara is suggested to distinguish concordant types compatible with normal hip function from other types.
