Congenital hip disease in a Cree-Ojibwa population: a retrospective study.

Retrospective study of data from annual surveys and hospital records over 23 years confirmed the 1950 report of a high prevalence of congenital hip disease (CHD) in the Cree-Ojibwa population of Island Lake, Man. Annual ascertainment rates ranged from 35 to 600 cases per 1000 live births; 5-year rates for dislocation or subluxation were the highest reported for any population. The minimally declining rates of CHD may reflect upgrading in criteria for hip abnormality as well as decreasing isolation and increasing outbreeding of the population. The preponderance of females (female:male ratio, 1.

Dosimetric studies in the radiological examination of the hips in young infants with a special fenestration method of gonad protection.

A special lead shield of 1,5 mm Pb, with a fenestration was used for gonad protection during X-Ray of the hips for detection of dysplasia. The window allows the delineation of all parts of the skeleton required to make a correct diagnosis and check the standardized position of the pelvis, while all other parts, especially the gonads are safely shielded. The scattered radiation for the testes was further reduced by a trough-shaped lead shield behind the scrotum. The effectiveness of this gonad protection was measured by direct dosimetric studies on the infant with LiF and CaF2: Dy dosimeters at

[Indication and efficiency of different operations in congenital hip dislocation (author's transl)].

The efficiency of different operations for congenital hip dysplasia is discussed here. Up to the age of 6 years Salters osteotomy and the acetabuloplasty of Lance in a modified form will achieve similar results. In cases with an acetabular index higher than 35 degrees acetabuloplasty is more effective. After our own experience acetabuloplasty can be used in all age groups where there is no congruency between acetabulum and femoral head. In congruent cases Salters osteotomy can be used up to 14 years combined with varus-osteotomy if dysplasia is of moderate degree. Chiaris osteotomy should be r

Triple osteotomy of the innominate bone. A procedure to accomplish coverage of the dislocated or subluxated femoral head in the older patient.

The triple osteotomy of the innominate bone is a total redirectional reconstruction and is obtained by circum-acetabular osteotomies through the ilium, ischium and pubis. The method achieves coverage of a dislocated or subluxated femoral head where other iliac osteotomies are ineffective or incomplete. Triple osteotomy was performed on 175 hips including 121 congenital dislocation-subluxations. The patients' ages ranged from 6 to 35, with 70 per cent falling between 9 and 12. Eighty-six per cent had a satisfactory result. Most of the unsatisfactory results were in cerebral palsy, myelodysplasi

Congenital dislocation of the hip. The relationship of premanipulation traction and age to avascular necrosis of the femoral head.

A retrospective study involving 276 patients with 319 congenitally dislocated hips was undertaken to assess whether premanipulation fraction prevents avascular necrosis. Roentgenographic criteria were utilized and only cases of frank dislocation unassociated with other disorders were included. It was found that in a child less than one year old, premanipulation traction of twenty-one days or more substantially reduces the incidence of avascular necrosis. For the child more than a year old, however, the prognosis is not as good, even if premanipulation traction is prescribed for twenty-one days

[Low back pain from the orthopedic point of view. Diagnosis and differential diagnostic. (author's transl)].

The diagnostic clarification of low back pain from the orthopedic point of view requires a specific exploration of the clinical history and an exact elicitation of findings not only of the vertebral column but also of the hip joints and the lower extremities. In the differential diagnosis of the disease processes which must be considered, congenital deformities, disturbances of statics and functional muscular states, inflammatory, degenerative, metabolic, tumorous and post-traumatic changes of the vertebral column must be demarcated from diseases of other etiology.

Arthrogryposis multiplex congenita. Search for prenatal factors in 66 sporadic cases.

In a family and epidemiological survey of 66 cases of arthrogryposis multiplex congenita all cases were found to be sporadic and no family association with clubfoot, congenital dislocation of the hip, or hereditary neuromuscular disease was found. The mothers were significantly older than average. Oligohydramnios was noted in only one-third of cases but many other complications of pregnancy, including probable attempts at abortion, had occurred. It is likely that most cases of arthrogryposis are nongenetic and result from a defective intrauterine environment, whether hormonal, vascular, mechan

Congenital dislocation of the hip in the newborn. A correlation of clinical, roentgenogrhic and anatomical findings.

This study correlates roentgenography, arthrography, morbid anatomy and histology in four newborn infants with congenital dislocation of the hip. Three died shortly after birth. In two of these, manipulative reduction was impossible. Both demonstrated secondary adaptive changes at autopsy, indicating that the dislocation had been present for some time in utero. The first had a paralytic dislocation (meningomyelocele), and the second showed no pathologic findings other than the dislocated hip. The third infant had a dislocation that was reduced at birth. The child died of pneumonia on the 15th

The anatomy and pathology of congenital dislocation of the hip.

At least 1 per cent of all newborn infants in the United Kingdom have CDH (Fig 10). Among these infants, approximately 85 per cent are normally formed and 15 per cent malformed. Among the normally formed infants, 90 per cent of the CDH are of the Grade 1 type, and 10 per cent of Grades 2 and 3. Such infants have a perinatal mortality of about 5 per cent. In contrast, among the malformed infants, about 50 per cent are CDH Grade 1, and 50 per cent Grades 2 and 3; their perinatal mortality is around 70 per cent. In other words, a much higher proportion than usual of the severe grades of CDH are f

Intertrochanteric femoral osteotomy with concentric reduction of the femoral head in treatment of residual congenital acetabular dysplasia.

A method of classification based upon increase of the anteversion and valgus deformity of the upper femur associated with a dysplastic acetabulum and subluxation of the femoral head demonstrates 4 main groups: I or II in patients 1 to 3 years of age; III, patients approximately 2 years of age; IV, patients with absolute contraindication for surgical intervention. The Monticelli technique employs a Kirschner wire for the internal fixation and incorporates it in a hip spica. The results of the surgical treatment are very gratifying when classification and specified indications are correct. In fo

An evaluation of conservative and operative methods in the treatment of congenital hip dislocation.

An evaluation of different methods of closed reduction of dislocated hips during different periods of time shows that manual reduction and fixation in Lorenz position account for a high rate of avascular necrosis. Even with functional and slow reduction by Pavlik harness and overhead extension, there is still a certain percentage of femoral capital necrosis. The fetal position recommended by Fettweis and Salter seems to reduce the incidence of necrosis. Acetabuloplasty combined with detorsion-varus-ostcotomy provides the means of normalizing a hip joint with a greater degree of success than mo

The hip in myelomeningocele. Management directed towards a minimum number of operations and a minimum period of immobilisation.

The hip problems in 116 children with myelomeningocele are discussed. The management described stresses the importance of selection of the type of operation; major surgery is appropriate only for those children who benefit significantly, and this generally means only those with strong quadriceps muscles on both sides. Some children with acetabular dysplasia gain immediate stability if acetabuloplasty is combined with other hip operations. Children who lack strong quadriceps muscles are best served by simpler procedures, such as tendon excision, designed to rid them of fixed deformity and to pr

Congenital dislocation of the hip associated with congenital muscular torticollis.

Four recently identified cases of congenital dislocation of the hip associated with congenital muscular torticollis out of 49 consecutive cases of frank dislocation of the hip suggest that: the coexistence of congenital muscular torticollis and congenital dislocation of the hip occurs more commonly than is generally appreciated; the presence of either abnormality should initiate a careful appraisal for the presence of the other; although both conditions are seen more commonly in children delivered by breech extraction, a direct causal relationship has not yet been established although the abno

Diastrophic dwarfism.

Diastrophic dwarfism is an unusual variety of short-limb dwarfism characterized by the pathognomic triad of ear calcifications, hitch-hiker thumbs, and severe clubfoot. Other clinical features include joint contractures and subluxations, symphalagism of the fingers, kyphoscoliosis, hip dysplasia, and occasional cleft palate. Radiographic findings include flaring of the metaphyses of tubular bones and flattened epiphyses which appear late. The major orthopedic problem in diastrophic dwarfism is correction of the recalcitrant clubfoot deformity; cervical spine subluxation or kyphoscoliosis may a

Congenital deformities about the knee.

Congenital deformities about the knee are the result of abnormalities of all the anatomical structures that make up this joint. In some cases the structural changes observed are due to primary disorders of formation and growth of one or more of the anatomical components of the knee. Disorders are known to occur in the integumentary, muscular, ligamentous supporting, and skeletal systems. Examples of these disorders are seen in the bone dysplasias and joint laxity syndromes. In other cases exaggerated intrauterine positions of the limb seem to bring about certain adaptive changes in the embryol

[Treatment of undesirable results following operations on the proximal end of the femur in children (author's transl)].

The initial results following operations on the proximal end of the femur in children may not be permanent. They are frequently altered by further growth of the bone. This influence of growth on the operative result cannot always be predicted. The author reports 6 cases with poor results following operations on the proximal end of the femus. In 3 children varus-rotation osteotomies resulted in pseudarthrosis and/or severe angulation. Resection of a solitary bone cyst and operative treatment of fractures of the proximal femur in 2 other cases produced similar complications. Treatment of these u

Hip dysplasia in the African Bantu.

Dysplasia of the hip is almost unknown in the Bantu peoples of Africa, except in cases of arthrogryposis and in those with partly European ancestry. Evidence is produced to show that the importance of intrinsic genetic factors in hip dysplasia far outweighs that of the extrinsic factors. These are of great importance only if allowed to act on a hip in which genetic factors have already determined dysplasia. Evidence is also produced to contradict the theory that the "piggyback" carrying method used by Bantu mothers is the reason for the immunity from dysplasia of the hip. The absence of "prima

[Coxa valga in childhood. Does it need treatment? (author's transl)].

Coxa valga in a child is not necessarily a pathological condition which needs treatment as long as the acetabulum shows adequate development for the patients age and constitution. Varus osteotomy is not indicated in these cases and may even result in poor development of the hip following operation. The author demonstrates a case in which fracture of the femoral neck on one side healed in varus position. Varus osteotomy was subsequently done on the other side to equalize both hips. This resulted in extreme valgus position and narrowing of the joint space of both hips at puberty. The neck-shaft

Metabolism of estradiol in greyhounds and German shepherd dogs. An investigation with special reference to hip dysplasia.

Metabolism of estradiol was investigated in 5 dogs, 3 female Greyhounds with radiographically perfect hip joints and 2 female German Shepherd dogs with hip dysplasia (one pregnant and the other non-pregnant). One of the Greyhounds was studied both when pregnant and non-pregnant. The non-pregnant dogs were injected with C14-labelled estradiol-17beta i.v. and 5 mg estradiol-17beta benzoate i.m. The pregnant dogs were given only radiolabelled estradiol-17beta. Twenty-four-hour-specimens of urine were collected from the dogs for 6--8 days.
