Background: The PITX1 gene encodes a transcription factor that plays a crucial role in the development of the lower limbs, pelvis, and structures derived from the first branchial arch. Pathogenic variants in PITX1 are associated with a limited spectrum of rare disorders, including congenital talipes equinovarus with or without long bone anomalies and/or mirror-image polydactyly, and Liebenberg syndrome. In 2020, a novel clinical phenotype, Mandibular-Pelvic-Patellar (MPP) syndrome, resulting PITX1 missense variants, was proposed. Case presentation: We report the fourth documented case of MPP s

CASE: A 30-month-old boy with arthrogryposis multiplex congenita presented with severe, recurrent teratological clubfoot refractory to serial casting, Achilles tenotomy, and plantar fascia release. Because of repeated cast slippage and patient distress, comprehensive posterior, plantar, and medial releases were performed, with cast exchange 2 weeks thereafter. A robust, taut tibialis anterior prompted addition of tibialis anterior tendon transfer to the lateral cuneiform. Postoperatively, plantigrade alignment was achieved both clinically and radiographically and maintained at the 2-year follo

BACKGROUND Achilles tenotomy is the final and most critical step of the Ponseti method for correcting equinus deformity in idiopathic congenital talipes equinovarus (CTEV). The procedure can be performed using either a needle or a scalpel; however, it remains uncertain whether these 2 percutaneous techniques provide different levels of tendon relaxation and radiological correction. MATERIAL AND METHODS This retrospective study included 130 feet of 90 patients with CTEV treated by the Ponseti method between January 2022 and December 2024. Patients underwent percutaneous Achilles tenotomy either

Background: Plantar fasciitis is a common condition likely caused by abnormal foot alignment, such as flatfoot. Insoles are commonly used to treat flatfoot, and systematic reviews have shown that insoles improve pain; however, the underlying mechanism is unclear. This study aimed to investigate the effects of insoles on plantar fascial tension during running in individuals with flatfoot. Methods: The participants were 14 individuals with flatfoot. The task involved running under two conditions: with and without insoles. Insoles that absorbed the shock and supported the medial longitudinal arch

INTRODUCTION: Nail-patella syndrome (NPS) is a rare autosomal dominant disorder characterized by nail dysplasia, hypoplastic or absent patellae, elbow anomalies, and iliac horns. Its association with congenital talipes equinovarus, or clubfoot, is extremely uncommon and rarely documented.
CASE REPORT: We describe a 1.5-year-old male child, born of a 2° consanguineous marriage, who presented with bilateral clubfoot and complete absence of fingernails. Great toenails were absent bilaterally, whereas the remaining toenails were preserved. The patellae were non-palpable clinically, and ultrasound

BACKGROUND: To evaluate the efficacy of the Ponseti method in the treatment of idiopathic congenital clubfoot (CCF) in a public health service, during 2 distinct periods, focusing on the impact of orthosis type (ankle-foot orthosis and abduction orthosis).
METHODS: This retrospective longitudinal cohort study analyzed medical records of 552 patients (842 feet) with CCF treated with the Ponseti method at a single hospital. Individuals were categorized into 2 groups: group 1 (2009 to 2017), which used an ankle-foot orthosis (AFO) after casts; and group 2 (2017 to 2021), which used an abduction

BACKGROUND: Polydactyly is a common congenital limb anomaly characterized by supernumerary digits, most frequently involving the hands or feet. Foot polydactyly is traditionally classified into preaxial, postaxial, and central types, with postaxial forms being the most common and central forms the rarest. Heptadactyly, defined as the presence of seven digits in a single limb, represents an exceptionally rare and severe expression within this spectrum, most often described as central duplication. Isolated unilateral heptadactyly of the foot with combined preaxial and postaxial duplication is ex

BACKGROUND: Understanding dimensions of both sinus tarsi (ST) and tarsal canal (TC) in normal and flatfeet may influence the design, usage and possible control of postoperative complications of arthroereisis. This study was to assess 3D features of the tarsal sinus and canal complex (TSCC) in both normal control and flatfeet in children under non-weightbearing and weightbearing conditions.
METHODS: Three-dimensional (3D) computer-aided design modeling from WBCT scans of 22 children with flexible flatfeet (age 9-14) and 14 with normal control feet (age 9-15) were used to evaluate volume and di

BACKGROUND: Talipes equinovarus (TEV), commonly known as clubfoot, is a congenital skeletal deformity characterized by abnormal three-dimensional positioning of the foot, ankle, and lower limb. This condition arises from developmental anomalies affecting multiple tissues in the lower limb, leading to disrupted alignment of foot and ankle joints. Without timely and appropriate intervention, TEV can result in persistent pain, structural deformities, and long-term functional impairments.
SUMMARY: TEV is a congenital skeletal (musculoskeletal) disorder with unresolved molecular etiology. However,

The Ponseti method is the gold standard for managing congenital talipes equinovarus (CTEV); however, relapses leading to surgical intervention remain a significant challenge. We investigated the factors associated with a higher risk of relapse. A retrospective study of 31 children (≤4 years) with CTEV treated with the Ponseti method between January 2014 and December 2023 was conducted. Demographic and clinical data-age at treatment, aetiology, Pirani score, number of casts, percutaneous Achilles tenotomy (PAT), surgery, bracing, relapses, and follow-up-were collected. BACKGROUND AND AIMS: The etiology of Idiopathic Congenital Talipes Equinovarus (ICTEV) remains poorly understood. Several theories have been proposed, including genetic factors. Unlike genes, which are essentially static in composition and size, transcriptome or microRNA (miRNA) varies greatly and might be influenced by multiple variables. This research aims to determine the expression of MiRNA that influences the occurrence of ICTEV in the Indonesian population.
METHODS: A cross-sectional study was conducted using samples obtained from patients with ICTEV under the age of three years. Transc

Lateral column lengthening (LCL) calcaneal osteotomy is a well-established surgical procedure used in flexible flatfoot reconstructive surgery. The effects of LCL on subtalar joint (STJ) range of motion (ROM) have not been thoroughly explored in the literature. Our study aimed to objectively measure the average decrease in the total ROM of the STJ following LCL. A retrospective cohort study was conducted on 15 patients (20 feet) who underwent LCL for various indications. Data for STJ ROM was collected from both the preoperative and postoperative periods. Mean follow-up for the cohort was 12 we

OBJECTIVE: Talipes equinovarus (TEV) is a congenital foot deformity that can present either in an isolated form or as part of a syndromic condition. In syndromic TEV, dysmorphic features, systemic anomalies, and developmental delay are more frequently observed. This study aims to evaluate the differences in clinical characteristics, dysmorphic findings, and quality of life between isolated and syndromic forms of TEV in children.
MATERIALS AND METHODS: Ninety TEV patients (37 syndromic, 53 isolated) diagnosed between 2010 and 2022 were included. Demographic and clinical data were retrospective

BACKGROUND: Idiopathic congenital talipes equinovarus (CTEV) is commonly treated with the Ponseti method, including foot abduction boots and bar (FABB) bracing to maintain correction. Complications such as pressure sores may disrupt bracing and increase recurrence risk, yet published data on brace-related complications are limited.
OBJECTIVE: To determine the frequency, type, and timing of FABB-related complications in idiopathic clubfoot and compare pressure sore rates between Markell and Mitchell FABB.
STUDY DESIGN: A retrospective review was conducted of 247 children (374 feet) with idiop

BACKGROUND: Flatfoot alters lower limb biomechanics and can negatively affect interjoint coordination during gait. Antipronation insoles are commonly prescribed to correct excessive foot pronation, yet their long-term effects on interjoint coordination remain unclear. This study aimed to examine whether prolonged use of antipronation insoles improves interjoint coordination in individuals with flat feet.
METHODS: Twenty-four participants (12 with flat feet and 12 with normal arches) were included. Spatiotemporal and interjoint coordination parameters were evaluated under four gait conditions:

Clubfoot is one of the most common congenital musculoskeletal deformities and remains a major cause of preventable childhood disability in low- and middle-income countries. In Pakistan, access to timely, standardized clubfoot care is limited, particularly outside major urban centers, resulting in delayed treatment and preventable lifelong disability. This technical report describes the development, evolution, and operational components of a sustainable clubfoot care and rehabilitation model established in Gujrat, Pakistan, beginning in 2010. The program transitioned from a single-room outpatie

BACKGROUND: The Ponseti method is the current global standard for the management of idiopathic clubfoot and achieves high initial correction rates. However, relapse during childhood remains a clinically important challenge, with widely varying estimates reported across studies. This systematic review and meta-analysis aimed to estimate the pooled prevalence of relapse following Ponseti treatment and synthesize associated risk factors.
METHODS: A systematic search of PubMed/MEDLINE, Scopus, and Web of Science was conducted for studies published between 1 January 2015 and 30 January 2026. Obser
