Foot Conditions in Adults: Common Foot Deformities.

Common foot deformities can impact mobility, quality of life, and overall foot health. Great toe deformities (eg, hallux valgus, hallux rigidus/limitus), lesser toe deformities, and arch deformities (eg, pes planus, pes cavus) are structural abnormalities that can arise from hereditary factors, trauma, biomechanical dysfunctions, or systemic conditions. These deformities can cause hyperkeratotic lesions that may contribute to pain and even lead to ulcerations. A standardized comprehensive foot examination, proper diagnosis, and early intervention are important to avoid complications. Conservat

Anti-Signal Recognition Particle Antibody-Positive Immune-Mediated Necrotizing Myopathy Diagnosed by Re-Evaluation 40 Years After Symptom Onset.

Immune-mediated necrotizing myopathy (IMNM) is a frequent subtype of idiopathic inflammatory myopathies. Distinguishing juvenile-onset IMNM from muscular dystrophy is often difficult, particularly when the disorder takes a chronic course. We describe a 45-year-old Japanese woman with progressive generalized weakness. Her early motor milestones were normal until age 4, but by 5 years she developed Gowers' sign. At that time, she was noted to have proximal weakness, scapular winging, gait impairment, and markedly elevated serum creatine kinase (6069 IU/L). A biopsy of the left biceps brachii sug

Clinical and Radiological Heterogeneity in Anti-Neurofascin-155 Autoimmune Nodopathy: A Case Series Analysis.

BACKGROUND AND AIMS: Autoimmune nodopathy with anti-neurofascin-155 (NF155) antibodies is increasingly recognized as a distinct disease entity. Although these patients have been described, not all aspects have been fully elucidated. We investigated clinical phenotypes, biomarker profiles, and treatment outcomes in patients with anti-NF155 antibody-positive autoimmune nodopathy. METHODS: We retrospectively analyzed seven Japanese patients (aged 13-27 years) with anti-NF155 antibody-positive autoimmune nodopathy treated at Kumamoto University Hospital between 2017 and 2020. Clinical, electrophy

Mitochondrial Trifunctional Protein Deficiency due to HADHA Variants Masquerading as Charcot-Marie-Tooth Disease.

BACKGROUND AND AIMS: Mitochondrial trifunctional protein deficiency (MTPD) is an inherited disorder of fatty acid β-oxidation caused by mutations in HADHA or HADHB genes. It typically presents with cardiomyopathy or hepatic failure in early childhood; however, it may rarely present in adulthood with the neuromyopathic form. METHODS: We describe a patient with MTPD with isolated neuropathy mimicking Charcot-Marie-Tooth disease (CMT) as the first and only presenting symptom. Clinical and electrophysiological examinations were conducted, including nerve conduction studies, needle electromyograph

Beyond the usual spectrum: Atrial septal defect in a Marfan syndrome patient with severe aortic pathologies.

Marfan's syndrome is a rare connective tissue disorder which mainly affects cardiovascular, skeletal, and ocular systems. Common cardiovascular manifestation of Marfan's includes aortic root dilatation, aortic regurgitation, and mitral valve prolapse. However, presence of congenital heart defects like atrial septal defect (ASD) is very rare. Here, we report the case of a 22-year-old male with Marfan's syndrome who had an uncommon coexistence of ASD and severe aortic pathologies. The patient had clinical features like tall stature, long arm span, and musculoskeletal abnormalities such as pectus

Multidimensional evaluation of clinical and functional impairment in a large population of patients with Charcot-Marie-Tooth.

Charcot-Marie-Tooth (CMT) disease is one of the most common inherited neuropathies, leading progressive muscular weakness, pes cavus, loss of deep tendon reflexes, distal sensory loss, and gait impairment. Charcot Marie Tooth Neuropathy Scale (CMTNS) represents the gold standard to assess and grade the impairment in CMT patients. The aim of this retrospective study was to assess the correlation between CMTNS and functional status in CMT patients. We analyzed data of patients with diagnosis of CMT, collecting the following data: age; gender; BMI; CMT type (Demyelinating- Axonal- Other or mixed

The fourth family in the world with a novel variant in the ATP5MK gene: four siblings with complex V (ATP synthase) deficiency.

Mitochondrial Complex V (ATP synthase) deficiency nuclear type 6 (MC5DN6) is a progressive neurodegenerative disorder characterized by autosomal recessive inheritance and developmental regression, particularly in gross motor skills, which manifests in early childhood. This study aims to present the discovery of a novel variant in four male siblings aged 13 years 9 months to 25 years, making this the fourth family reported globally, while also raising awareness of rare mitochondrial diseases. Four individuals from the same family were retrospectively evaluated based on their demographic, clinic

Clinical Characteristics of Gait Disturbance in Charcot-Marie-Tooth Disease and Future Directions in Physical Therapy.

Charcot-Marie-Tooth disease (CMT) is a hereditary and progressive peripheral neuropathy affecting both motor and sensory nerves. It is among the most common inherited neuropathies and is primarily classified into demyelinating (type 1) and axonal (type 2) forms based on motor nerve conduction velocity, with over 100 genetic subtypes identified. Due to this genetic and clinical heterogeneity, the onset, severity, and degree of motor and sensory impairments vary widely among individuals.  One of the hallmark manifestations of CMT is gait disturbance. As the disease progresses, individuals often

The importance of foot posture when recording lower leg electromyography when walking in non-textured and textured foot orthoses.

Foot posture describes the anatomical variance in an individual's overall foot shape, an important consideration in the provision of foot orthoses. Current orthoses designs could be optimized by considering the topographical organization of cutaneous mechanoreceptors. Currently, the effect of foot orthoses designs to enhance skin stimulation across different anatomical foot posture remains unknown. Thus, the purpose of this study was to investigate how foot posture variance modulates lower leg muscle activity when walking in non-textured orthoses and in textured orthoses which facilitates cuta

Case report: A novel variant (H49N) in Myelin Protein Zero gene is responsible for a patient with Charcot-Marie-Tooth disease.

This report presents a case of Charcot-Marie-Tooth dominant intermediate D (CMTDID), a rare subtype of Charcot-Marie-Tooth disease, in a 52 years-old male patient. The patient exhibited mobility impairment, foot abnormalities (pes cavus), and calf muscle atrophy. Whole exome sequencing and Sanger sequencing suggested that a novel variant (NM_000530.8, c.145C>A/p.His49Asn) of MPZ may be the genetic lesion in the patient. The bioinformatic program predicted that the new variant (p.His49Asn), located at an evolutionarily conserved site of MPZ, was neutral. Our study expands the variant spectrum o
