BACKGROUND: Several radiographic and cadaveric studies have suggested that specific anatomical features of the Lisfranc joint complex, including a shallower medial mortise of the second metatarsal (M2), shorter M2 length, or pes cavus alignment, may be associated with ligamentous Lisfranc injuries. These associations have largely been derived from 2-dimensional imaging or post-injury assessments. Weightbearing computed tomography (WBCT) enables 3-dimensional evaluation of midfoot anatomy under physiologic load and allows a more robust reassessment of these proposed anatomical associations.

Background: The superficial plantar-medial release (S-PMR) refers to a group of surgical procedures involving the release of the plantar aponeurosis and adjacent medial plantar soft tissues that are used in selected cases of plantar fasciitis and cavovarus foot deformity. The procedure aims to address pain and contracture of the plantar aponeurosis and intrinsic foot muscles, which may contribute to pathological foot alignment and gait instability. Due to the close proximity of highly variable neurovascular structures in the plantar region, precise anatomical knowledge and a patient-specific,

OBJECTIVE: This study assesses whether an association exists between radiographic measurements of progressive collapsing foot deformity and MRI assessment of posterior tibial tendon (PTT) dysfunction.
MATERIALS AND METHODS: After institutional approval, a retrospective search was conducted for patients who presented between 2017 and 2023 with foot and ankle pain and for whom 3D ankle MRIs and weight-bearing ankle radiographs were performed within 3 months of each other. One hundred fifty-seven feet that met the criteria were identified. The talonavicular uncoverage angle (TU), Meary's angle,

BACKGROUND: X-linked Charcot-Marie-Tooth disease type 1 (CMTX1), caused by pathogenic variants in GJB1 encoding connexin 32, is primarily an inherited peripheral neuropathy. A minority of patients develop transient central nervous system (CNS) dysfunction with reversible white matter lesions, which may mimic acute paediatric stroke. Triggers such as fever, infection and exercise have been reported, but allergy-related exposure has rarely been discussed and causal mechanisms remain uncertain.
CASE PRESENTATION: A 9-year-old boy developed acute left-sided weakness, dysarthria and central facial

BACKGROUND: National conversation has justifiably been concerned with firearm-related deaths and much less attention has been paid to the consequences of surviving a firearm injury. We assessed the risk of hospital readmission, length of stay (LOS) during hospitalization, and costs within 90-days after surviving an index firearm injury and compared these data with pedestrians and occupants involved in motor vehicle crash (MVC).
METHODS: Nationwide Readmission Database, a nationally representative readmission database from 2013 and 2014 was used to create a retrospective cohort study. The prim

High-arched feet affect approximately 10%-15% of the general population. Although the relationship between plantar pressure and bilateral symmetry is well studied, there is limited evidence regarding the use of Symmetry Index and pressure platforms. The objective of the study is to compare dynamic foot pressures and Symmetry Index during gait between subjects with Pes Cavus and subjects with normal feet. The analysis of asymmetric pressure patterns could benefit from studies that compare dynamic foot pressures and Symmetry Index values in subjects with high-arched feet and normal feet. This an

BACKGROUND: There has been increasing interest in the percutaneous dorsal closing-wedge Zadek calcaneal osteotomy (PDCWZO) for the treatment of insertional Achilles tendinopathy (IAT). This study aims to investigate the clinical efficacy and radiographic outcomes of the percutaneous dorsal closing-wedge Zadek calcaneal osteotomy in patients with and without pes cavus foot morphology.
METHODS: A multicentre observational study of patients undergoing PDCWZO for IAT. Patients were stratified into cavus (calcaneal pitch angle > 20 degrees) and non-cavus (calcaneal pitch angle ≤ 20 degrees) groups

Chronic ankle instability (CAI) can develop in up to 40% of patients after a first-time ankle sprain. Advanced imaging and statistical shape modeling (SSM) provide the opportunity to evaluate if subtle differences in foot and ankle morphology, such as hindfoot alignment or cavus features, may contribute to structural predispositions for recurrent instability. In this study, a 14-bone SSM was created from weight-bearing CT data (n = 80) from patients with CAI (n = 23), cavus foot type (n = 29), and rectus (n = 28) foot type. Scans for the CAI group were from pre-operative imaging of patients un

BACKGROUND: Germline pathogenic variants in the Additional Sex Combs-Like 1 (ASXL1) gene are associated with the neurodevelopmental Bohring-Opitz syndrome and cancers like Wilms' tumours. Bohring-Opitz syndrome is a phenotypically heterogeneous condition characterised by feeding difficulties, syndromic facial abnormalities, abnormal posturing and developmental delays, and it has only been reported in infant/adolescent patients. There are no reported neuromuscular phenotypes associated with ASXL1.
CASE PRESENTATION: A 71-year-old male proband exhibited congenital facial, extraocular, proximal

BACKGROUND: Subtle cavus foot (SCF), identified by the "peek-a-boo" sign and a positive Coleman block-test, is frequently associated with mechanical foot pain. While prefabricated insoles are commonly used in clinical practice, evidence supporting their effectiveness in SCF - particularly when modified according to SCF-specific design principles - is limited.
PURPOSE: This prospective randomized controlled trial compared Specifically Optimized Prefabricated Insoles (SOPI) with unmodified Plain Prefabricated Insoles (PPI) in individuals with clinically identified SCF and mechanical foot pain.

BACKGROUND: Fixed cavovarus foot deformity is characterised by a plantar-flexed first ray, hindfoot varus, and abnormal plantar loading, often resulting in pain, instability, and gait dysfunction. The surgical correction we performed involved a deformity-driven approach to address the structural apex and associated components. Evidence linking this multilevel correction strategy to objective biomechanical improvement remains limited.
METHODS: This prospective case series evaluated outcomes following a uniform deformity-driven multilevel surgical correction protocol for fixed cavovarus deformi

In order to maintain lateral ankle stability when performing a total ankle arthroplasty (TAA), there must be a lateral strut from an intact and competent distal fibula. The aim of the present case report is to describe an alternative approach to failed ankle arthrodesis with a staged custom fibular implant and subsequent TAA. At final follow-up of 36 months from index procedure, there was no reported nonunion about the hindfoot complex with adequate seating of the total ankle arthroplasty components and pain free patient ambulation. We hope this novel approach will add to the current body of l

Charcot-Marie-Tooth (CMT) disease is the collective term for the most common inherited peripheral neuropathies, affecting both motor and sensory nerves. A typical CMT patient presents with slowly progressive distal muscle weakness and atrophy that primarily involves the small foot muscles, peroneal muscles, and, often later, the muscles of the hands and forearms. Foot deformities, most commonly pes cavus and claw toes, are common and can lead to gait impairments. In this paper, we report a patient with an intermediate CMT (CMT-Int) type carrying a novel pathogenic variant in the MPZ gene combi

Multicentric carpotarsal osteolysis (MCTO) is a rare skeletal dysplasia classically associated with carpal and tarsal osteolysis. Its presentation is highly variable, ranging from subtle musculoskeletal deformities to progressive bone loss and renal involvement. We report a nine-year-old female who presented with progressive ankle pain, cavovarus foot deformity, and gait disturbance. Initial radiographs demonstrated osteopenia and bilateral pes cavus without frank osteolysis. Genetic testing identified a pathogenic MAFB variant, confirming the diagnosis of MCTO. This case highlights the broad

Background/Objectives: This study aimed to clinically investigate how variations in foot morphology influence spinal and lower limb alignment, based on the concept of an ascending kinetic chain. Methods: We analyzed the medical records of 100 patients who met the inclusion criteria. The X-ray image data used in the analysis included weight-bearing lateral views of both feet, whole-spine anteroposterior (AP) and lateral views, and full-length standing AP scanograms of the lower legs. In the obtained X-ray images, Calcaneal Inclination Angle (CIA), Tibiotalar Tilt Angle (TTA), Tibiotalar Angle (

RATIONALE: Friedreich ataxia (FRDA) is a rare inherited autosomal recessive neurological disorder, characterised initially by unsteadiness in standing and walking, and slowly progressing to wheelchair dependency, usually in the late teens or early twenties. Scoliosis, pes cavus and cardiomyopathy are often present at diagnosis. As the disease progresses, individuals usually develop slurred speech, auditory impairment (especially in a noisy environment), urinary tract morbidity, diabetes, anxiety, depression, muscle spasticity and visual disturbances. Cardiac abnormalities cause premature death

BACKGROUND AND OBJECTIVE: Charcot-Marie-Tooth disease type 4 C (CMT4C) is a hereditary neuropathy caused by recessive mutations in the SH3TC2 gene. Due to overlapping symptoms with acquired conditions like chronic inflammatory demyelinating polyneuropathy (CIDP), misdiagnosis is common. This study examines the clinical and electrophysiological profiles of five patients initially misdiagnosed with CIDP but later confirmed to have CMT4C.
METHODS: Data were collected on demographics, clinical features, nerve conduction studies, and ultrasonographic findings. Patients underwent genetic testing to

BACKGROUND AND AIMS: CMT1H is a rare, autosomal dominant, demyelinating subtype of CMT caused by variants in FBLN5. Symptomatic cranial nerve involvement has never been reported in patients with CMT1H.
CASE REPORT: We report a 45-year-old woman with a history of long-standing diplopia. On examination, she had bilateral limitation of eye abduction, with double vision in all directions of gaze, particularly horizontally. She was unable to heel-walk, had mild lower limb distal weakness, decreased tendon reflexes with absent Achilles reflexes, reduced distal vibration sense, pes cavus, and hammer

BACKGROUND: Biallelic variants in VPS41, encoding a subunit of the HOPS complex, cause autosomal recessive spinocerebellar ataxia 29 (SCAR29), a rare neurodevelopmental disorder with an incompletely defined phenotypic and molecular spectrum.
METHODS: We investigated a 24-year-old man with cerebellar ataxia, hypotonia, and intellectual disability. Exome sequencing identified four candidate VPS41 variants. Because maternal DNA was unavailable, long-read genome sequencing was performed to determine allelic configuration, followed by RNA and protein analyses.
RESULTS: In addition to typical SCAR

OBJECTIVES: The AFG3L2 gene encodes a mitochondrial AAA-protease involved in inner mitochondrial membrane (IMM) proteostasis. Heterozygous variants in the AFG3L2 proteolytic domain cause Spinocerebellar Ataxia type 28, heterozygous variants in the AFG3L2 ATPase domain cause Optic Atrophy type 12, while biallelic variants lead to recessive Spastic Ataxia type 5. In this study, we aimed to investigate the link between AFG3L2 haploinsufficiency and Charcot-Marie-Tooth (CMT) phenotypes.
METHODS: We performed clinical evaluation, genetic analyses, electrophysiology, and functional studies in 1 pat
