ATP1A3-related syndromes: our case-series unveiling a dynamic, fever-triggered and overlapping array of neurological phenotypes.

INTRODUCTION: ATP1A3-related neurological disorders show a broad spectrum of manifestations, usually with autosomal dominant transmission. Classical phenotypes include alternating hemiplegia of childhood (AHC), rapid-onset dystonia-parkinsonism (RDP), and syndrome characterized by cerebellar ataxia, areflexia, pes cavus, optic atrophy and sensorineural hearing loss (CAPOS). Additional rarer forms include childhood-onset-schizophrenia (COS), encephalopathy with MRI abnormalities without hemiplegia (D-DEMØ), fever-induced paroxysmal weakness and encephalopathy (FIPWE), and relapsing encephalopat

Clinical and pathological findings in two Italian siblings of Romani ancestry with charcot-marie-tooth type 4D and review of the current literature.

Charcot-Marie-tooth disease type 4D (CMT4D) is an early-onset, severe autosomal recessive demyelinating neuropathy, caused by mutations in the N-myc downstream-regulated gene 1 (NDRG1). Because of its rarity and predominance among specific ethnic groups, clinical knowledge remains limited. We report the case of two siblings of Romani ancestry, a 38-year-old man and a 40-year-old woman, with homozygous NM_006096.4:c.442C > T, p.(Arg148*) variant, and provide a review of the current literature. Both patients presented with severe distal-proximal sensorimotor neuropathy, muscle weakness and atroph

Expanding the Phenotypic Spectrum of NDUFS6-Related Disease: From Neonatal Mitochondrial Encephalopathy to Childhood-Onset Axonal Neuropathy.

Biallelic variants in NDUFS6, encoding an accessory subunit of mitochondrial complex I, were initially associated with lethal neonatal mitochondrial encephalopathy and Leigh syndrome. Recent studies have demonstrated that NDUFS6 variants can also cause childhood- or adolescent-onset axonal neuropathy and Charcot-Marie-Tooth (CMT)-like phenotypes, indicating marked clinical heterogeneity. Here, we report a patient with a novel homozygous truncating NDUFS6 variant presenting with a neuropathy-predominant phenotype accompanied by epilepsy, in the absence of neonatal metabolic decompensation. Phenotypic Heterogeneity in Titinopathies with Peripheral Nerve Involvement in Pediatric Age: Two Case Reports.

Background/Objectives: Titin (TTN; OMIM 188840) is the largest known human sarcomeric protein, and pathogenic variants in the TTN gene cause a broad spectrum of inherited myopathies and cardiomyopathies. The extent to which TTN variants may also involve the peripheral nervous system remains poorly defined. We aimed to describe two pediatric patients carrying heterozygous truncating TTN variants with neurophysiological evidence of peripheral nerve involvement, and to review the existing literature on this underrecognized association. Pathogenic variants in the TTN gene are associated with a wid

Novel Heterozygous Variant in a Child with Axonal Charcot-Marie-Tooth disease.

Charcot-Marie-Tooth (CMT) disease comprises a genetically diverse range of disorders affecting the peripheral nervous system. We report an axonal CMT (CMT 2) case from India with a novel heterozygous variant of uncertain significance detected in the ganglioside-induced differentiation-associated protein 1 (GDAP1) gene. A 12-year-old male patient presented with severe distal lower limb weakness (bilateral foot drop) progressing proximally. Bilateral pes-cavus, high-steppage gait, reduced power in bilateral lower limbs, bilaterally absent ankle jerks, and no cerebellar or pyramidal signs/vocal c

[Clinical pattern and evolution of Charcot-Marie-Tooth disease in pediatrics].

Pediatric Charcot-Marie-Tooth (CMT) disease comprises a heterogeneous group of inherited peripheral nervous system neuropathies that manifest during childhood or adolescence. Clinically, it is characterized by progressive distal muscle weakness, atrophy, areflexia, sensory impairment, and orthopedic deformities such as pes cavus or scoliosis. In early-onset cases, hypotonia, motor delay, and limited independent ambulation may be observed. From a genetic perspective, the most frequent subtypes in the pediatric population include PMP22 duplication (CMT1A), mutations in MPZ (CMT1B), mutations in

Clinical and Genetic Spectrum of ATP1A3-Related Disorders: A Multicenter Cross-Sectional Study.

BACKGROUND AND OBJECTIVES: ATP1A3-related disorders comprise an expanding group of ultra-rare neurologic conditions, classically including rapid-onset dystonia-parkinsonism (RDP), alternating hemiplegia of childhood (AHC), and cerebellar ataxia, areflexia, pes cavus, optic atrophy, and sensorineural hearing loss (CAPOS) syndrome. However, accumulating reports suggest a broader and overlapping phenotypic spectrum. In this context, we established the ATP1A3 Study Group to comprehensively characterize the phenotypic and genotypic spectrum of ATP1A3-related disorders in a Brazilian cohort. METHOD

A Case of Advanced Charcot-Marie-Tooth Disease Showing Extreme Lumbosacral Nerve Root Hypertrophy.

Charcot-Marie-Tooth (CMT) disease is the most common hereditary motor and sensory neuropathy. While diagnosis is typically established through clinical findings, nerve conduction studies (NCS), and genetic testing, imaging manifestations such as nerve root hypertrophy are increasingly recognized. This case report presents a 43-year-old man with lifelong progressive distal weakness who presented with severe lower extremity atrophy, pes cavus deformities, and wheelchair dependence. Neurologic examination revealed marked distal weakness and hyporeflexia, while nerve conduction studies demonstrate

Genetic Variants of Na+,K+-ATPase Associated With Neurological Disorders: A Systematic Review.

Neurological disorders encompass a wide range of severe symptoms and manifestations, many of which are associated with genetic variants that affect ionic homeostasis. Na+,K+-ATPase, a transmembrane enzyme responsible for maintaining electrochemical gradients in cells, plays a crucial role in neuronal excitability and brain function. The present review aimed to systematically identify and organize published data on Na+,K+-ATPase variants reported in human studies with neurological outcomes, compiling this information into a structured format. Variants in the ATP1A1 (α1) gene are associated with

Clinical and Electrodiagnostic Characterization of Pediatric Triple A Syndrome: A Cohort of 17 Patients.

INTRODUCTION/AIMS: Triple A syndrome is an autosomal recessive disorder characterized by alacrima, achalasia, and adrenal insufficiency. Neurological involvement is common, but its electrodiagnostic (EDX) patterns are varied. The resulting phenotype, often marked by spasticity, can lead to diagnostic confusion with other motor neuron disorders. We aimed to define the neurological and EDX spectrum in a pediatric cohort and clarify their diagnostic relevance. METHODS: This was a cross-sectional study of 17 pediatric patients with genetically confirmed triple A syndrome. All patients underwent n

Unraveling the Genetic Landscape of Foot Arch Morphology: A Systematic Review of Single Nucleotide Polymorphisms.

Variations in foot arch morphology, including flat feet (pes planus) and high arches (pes cavus), range from asymptomatic to debilitating. Limited research exists on the genetics of foot arch geometry. This systematic review aims to identify single nucleotide polymorphisms (SNPs) linked to foot arch morphology. The review protocol was registered in PROSPERO (CRD42024537877). PubMed, The Cochrane Library, Embase, and Web of Science were searched for studies on SNPs related to foot arch morphology published up to December 2023. Nineteen eligible studies (2006-2020) identified 137 SNPs across con

First Iranian Family with a Novel Missense Variant in MYO9B Gene Causing Charcot-Marie-Tooth Disease.

A heterogeneous clinical and genetic Charcot-Marie-Tooth (CMT) disease, with peripheral nerve damage resulting in chronic motor and sensory polyneuropathy, has been linked to the mutation in over a hundred genes. We report the adult onset of CMT in three siblings of an Iranian family manifesting with muscle weakness and wasting, foot drop, and pes cavus. Whole-exome sequencing (WES) identified a novel homozygous missense mutation in the MYO9B gene, inherited from obligatory carrier parents. This likely pathogenic variant contributes to chronic demyelinating sensorimotor polyneuropathy and cond

A Novel Homozygous Frameshift Variant of SACS Gene in the Turkish Siblings With Autosomal Recessive Spastic Ataxia of Charlevoix-Saguenay (ARSACS).

Pathogenic variants of sacsin (SACS) gene cause autosomal recessive Spastic Ataxia of Charlevoix-Saguenay (ARSACS). It is a hereditary neurological disorder manifested with gait ataxia, intentional tremor, lower limb pyramidal signs and pes cavus. It was originally described in the late 1970s and has a high prevalence in northeastern Quebec, Canada. Here, we present for the first time a new SACS frameshift variant in two Turkish siblings. We detected a new homozygous frameshift variant of the SACS gene in the Turkish siblings diagnosed with ARSACS for the first time.

Arch support insoles reduce fatigue of the lower extremity muscles in people with flatfoot during a walking task.

People with flatfoot encounter challenges while walking. This study investigated whether arch support insoles (ASIs) can reduce fatigue of the lower extremity muscles during a walking task in people with flatfoot compared with flat insoles (FIs). A total of fifteen female college students with flatfoot were recruited and considered eligible for the study, with each subject fulfilling the inclusion criteria of Chippaux-Smirak index > 45%. A paired sample t test was used to compare the difference in power spectral area (PSA) of the subdivided frequency band interval of 20-140 Hz between ASIs and

A potential relationship between soccer shoes and pes cavus: a pilot study.

BACKGROUND: Preventive team testing showed an above-average number of pes cavus among soccer players. This raised the question of whether wearing relatively small soccer shoes - as it is often the case for soccer players - shoes might be responsible and cause a kind of foot compression and a muscular-induced pes cavus. METHODS: This cross-sectional study included 198 male youth and adult soccer players (age: 18.6±5.8 years, height: 176.3±14.8 cm, body mass: 69.9±11.5 kg). The delta between shoe and foot length was compared with arch index (AI) and normalised truncated navicular height (NTNH)
