A patient suffering from an ichthyosiform dermatosis, partial deafness, and pes cavus is described. Most of the body surface was covered by fine white scales; red scaling plaques were present on the arms. There were slight hyperkeratoses of the palms and soles, keratosis pilarislike lesions with perifollicular redness on the trunk, and multifocal alopecia. A biopsy specimen of the scaly erythematous plaque demonstrated hyperkeratosis with follicular plugging, papillomatosis, and acanthosis. Direct immunofluorescence studies of lesional skin using monoclonal antibodies to epidermal prekeratin,

The topic of pes cavus has, for many years, been the object of much discussion, with numerous literary works dedicated to its etiology and treatment. Anterior tarsal resection was presented in the literature over 50 years ago. The authors describe a rationale for performing this procedure as well as other adjunct soft tissue procedures for the correction of a rigid anterior global cavus with its apex at the navicular cuneiform joint.

This study identifies correlates of low back pain in a general population sample and defines a profile of subjects with low back pain. A multidisciplinary approach was employed that required surveying and physically assessing 674 subjects on 105 variables in biographical, anatomical, strength and flexibility measurement categories. No attempt was made to select subjects from specific occupational, age, athletic, psychological and anatomical groups or subjects with specific biographical features, which may have resulted in a sample that was atypical of the general population. The results of thi

We report a case of bilateral optic nerve hypoplasia in a patient with pupillary light-near dissociation, spastic paraparesis with deep hyperreflexia, mild distal hypotrophy especially of lower limbs, bilateral pes cavus and signs of motor peripheral neuropathy. Other associated signs were rhinolalia, hypotrophy and fasciculations of the tongue and cardiomyopathy. We differentiate the condition from a number of hereditary ataxias and paraplegias. The clinical features of Strümpell-Lorrain spastic paraplegia and amyotrophic lateral sclerosis seem to resemble most closely that of our patient.

Treatment of varus deformity of the rearfoot is satisfactorily corrected with a lateral closing wedge osteotomy of the calcaneus. Whether the condition being treated is club foot, pes cavus deformity, or calcaneal varus alone, statistics show the varus component is consistently resolved with this osteotomy approach. A case report of calcaneal varus is presented, including a postoperative complication with resolution.

Multiple endocrine neoplasia type 2b (MEN 2b) is a rare genetic disorder. Affected individuals have malignant thyroid tumors, pheochromocytoma, and ganglioneuromatosis. The musculoskeletal manifestations of MEN 2b include a Marfanoid habitus, pes cavus, scoliosis, slipped capital femoral epiphysis, joint laxity, poor muscle development, and delayed maturation. The initial clinical presentation of MEN 2b frequently involves the musculoskeletal system. The characteristics ganglioneuromatosis of the lips and tongue, however, should alert the orthopedic surgeon to the underlying disorder. Effectiv

Charcot-Marie-Tooth disease results in pes cavus or equinocavovarus deformity. An accepted surgical correction is triple arthrodesis of the foot. In a retrospective study of 45 Charcot-Marie-Tooth patients, two cases developed neuropathic ankle joint after triple arthrodesis. This procedure prevents mobility of the tarsal joint and consequently increases stress on the ankle joint. If coupled with decreased joint sensation, it may predispose the ankle to neuroarthropathy. This paper emphasizes joint position sense as an important consideration before triple arthrodesis or any other surgical pro

From the tomodensitometric examination of 9 cases of hollow foot, the authors present their methodology, reminding the difficulties encountered, and they analyze the results. After a first tomodensitometric study including 16 cases of normal feet, confirming the existence of a suro-achilleo-plantar system by studying correlations between plantar intrinsic muscles and muscles of the posterior compartment of the leg, they establish: the advantage of this method of evaluation of section areas and evaluation of the maximum traction force of the intrinsic muscles of the foot (reproducibility), the

The arch of the foot serves as an adaptable, supportive base for the entire body. This article discusses how the arch of the foot affects the normal biomechanics of the lower limb. An anatomical overview of the three components of the arch of the foot is presented, identifying the medial longitudinal arch as the arch mainly responsible for related structural problems throughout the lower limb. Deviations in the normal structure of the medial longitudinal arch produce unbalanced, functionally unstable conditions of the foot such as pes cavus or pes planus. Specific evaluation criteria for both

The clinical features of 31 cases of syringomyelic syndrome, confirmed by metrizamide CT myelography (CTM) or magnetic resonance imaging (MRI), were analysed. Bilateral suspended sensory disturbance, previously regarded as a characteristic feature, was encountered in only 2 cases (6.5%). Thirteen patients (41.9%) showed no motor deficits of the upper limbs and 7 patients (22.6%) did not have weakness or atrophy in any of the four extremities. Cervical segmental signs were absent in 7 cases (22.6%) and in 2 (6.5%) signs were only present in the lower limbs. Scoliosis and foot deformity, such as

A 14 year old boy with scapuloperoneal muscular atrophy, pes cavus, areflexia and distal sensory loss (Davidenkow syndrome) is described. Nerve conduction velocities were diminished. Sural nerve biopsy demonstrated a reduction in the number of myelinated fibers and early "onion-bulb" formation. These observations support the hypothesis that the scapuloperoneal amyotrophy associated with distal sensory loss may represent a variant of type I hereditary motor sensory neuropathy.

The treatment of pes cavus in the adult by the V osteotomy of Japas has several advantages if performed before the onset of secondary arthritic degeneration. The correction is carried out at the apex of the cavus and is technically simple. Consolidation is rapid, and correction is achieved with no significant sacrifice of movement. The authors describe a modification of the medial part of the osteotomy line which was devised because of the difficulty of controlling the various fragments of navicular and cuboid during reduction in the original technique, as well as a significant incidence of os

The Roussy-Lévy-Syndrome ist generally considered a pheno-typic variant of the hereditary motor and sensory neuropathy type I (HMSN I), i.e. the classic Charcot-Marie-Tooth-neuropathy, and is thus nosographically distinguishable from Friedreich's ataxia. While posterior column symptomatology, dementia and rapid progression are recognized as diagnostic criteria for Friedreich's ataxia and HMSN I is clinically defined by the typical features of a progressive polyneuropathy, the symptom triad "gait disturbance, areflexia and pes cavus" of the Roussy-Lévy-Syndrome has comparatively little diagnost

This study reviews the preliminary results of a previously unreported midtarsal "dome" osteotomy, all operations being performed by a single surgeon. The procedure is centered at the apex of the deformity and allows for extensive angular and rotational correction of the midfoot. The initial review has demonstrated 67% satisfactory results in 22 patients representing 35 feet. Satisfactory results were obtained in 15 of 16 feet (94%) in patients greater than 8 years of age. Careful short-term analysis suggests that the midtarsal "dome" osteotomy is best indicated for the child greater than 8 yea

Twenty-six patients with paralytic pes cavus were managed by early soft tissue correction and tendon transfer. Eighteen had spina bifida, 6 had peroneal muscular atrophy and two had cerebral palsy. The most frequent operations were flexor hallucis longus tenodesis, Girdlestone's flexor to extensor tendon transfer and plantar release. The indications for these procedures are discussed and the results presented, with particular reference to static and dynamic foot pressure studies performed at review. Follow-up averaged 5.2 years. Toe correction was found to be successful in most cases but plant

The authors have performed 34 Dwyer's calcaneal osteotomies in children with pes cavus confined to the medial arch in non-paralytic lesions (poliomyelitis and spina bifida were excluded). In 12 cases it was associated with osteotomy of the 1st metatarsal or with plantar release. No arthrodeses were performed in these 34 cases. After an average follow-up of five years the results were doubtful: in 24 instances the deformity was the same or worse. In 21 cases a secondary operation was necessary. The authors considered that the Dwyer's procedure corrects only the varus of the heel which is a seco
