Further delineation of LRSAM1-related Charcot-Marie-Tooth type 2P with parkinsonism.

LRSAM1 pathogenic variations are linked to an axonal motor and sensory polyneuropathy known as Charcot-Marie-Tooth disease type 2P, but extra peripheral neurologic impairment is suspected. We report a patient with CMT2P and parkinsonism. We describe a 66-year-old man presenting with pes cavus, gait instability, and mild distal motor weakness. Nerve conduction studies revealed sensory-motor axonal neuropathy consistent with CMT2P. After several months, he developed lower-limb and right upper-limb hypertonia, jerky eyes, and hypomimia. 123 I-FP-CIT single-photon emission computed tomography reve

Patellofemoral Biomechanics Considerations: Analysis of Factors Contributing to Patellofemoral Pain.

Patellofemoral pain syndrome is a common cause of anterior knee pain. It has a prevalence of 22.7% in the general population and tends to affect females more than males. Understanding the anatomy of the hip, knee, ankle, and foot helps to assess biomechanical risk factors that contribute to the development of patellofemoral pain syndrome. At the knee, decreased knee flexion angle, knee flexion and extension strength, and increased frontal plane projection angle have been shown to increase the risk of developing patellofemoral pain syndrome. Increased femoral internal rotation, weak hip externa

Quantifying 3D foot and ankle alignment using an AI-driven framework: a pilot study.

OBJECTIVE: Accurate assessment of foot and ankle alignment through clinical measurements is essential for diagnosing deformities, treatment planning, and monitoring outcomes. The traditional 2D radiographs fail to fully represent the 3D complexity of the foot and ankle. In contrast, weight-bearing CT provides a 3D view of bone alignment under physiological loading. Nevertheless, manual landmark identification on WBCT remains time-intensive and prone to variability. This study presents a novel AI framework automating foot and ankle alignment assessment via deep learning landmark detection. MAT

The effectiveness of radial extracorporeal shock wave therapy (rESWT) in plantar fasciitis: a 12 months randomised controlled trial in a Tunisian rehabilitation department.

BACKGROUND: Current management strategies for plantar fasciitis primarily involve conservative treatments. However, the overall effectiveness of extracorporeal shock wave therapy (ESWT) as a specific intervention for this condition has been a subject of ongoing debate in the medical community and its long term efficacy remains controversial. This study's objective is to assess the effectiveness of extracorporeal radial shock wave therapy (rESWT) for treating plantar fasciitis treatment compared to physiotherapy combined with ultrasound sessions and to screen for foot disorders implementing bar

Cole Osteotomy and Variations.

Pes cavus can be a painful and debilitating condition that is often recalcitrant to conservative treatment. Oftentimes surgical reconstruction is necessary for the patient to obtain a pain-free plantigrade foot. An intimate knowledge of pes cavus foot architecture is necessary to select appropriate surgical treatment of the symptomatic patient. Midfoot osteotomies such as the Cole osteotomy can be a powerful procedure to reduce deformities where the apex is located in the midfoot. Midfoot osteotomies are almost never performed in isolation. Midfoot osteotomies including the Cole osteotomy are

Calcaneal Osteotomies.

Calcaneal osteotomies are surgical procedures performed to correct a hindfoot deformity or realign the calcaneus in conditions that include pes planus and pes cavus. These joint-sparing procedures offer the ability to realign the calcaneus, enhancing foot biomechanics, redistributing forces, and reducing pain and dysfunction. Among the most common types of calcaneal osteotomies are the medial slide, lateral slide, and Dwyer calcaneal osteotomy, each with distinct indications, advantages, and surgical considerations.

Subtalar Joint Statistical Shape Modeling Differentiates Cavus-to-Planus Foot Types From Weightbearing CT.

BACKGROUND: Foot type significantly impacts the development and progression of foot and ankle pathologies by influencing biomechanics and force distribution. However, it is typically assessed qualitatively and with a 2D radiographic measurement called Meary's angle. This study seeks to determine the minimum number of bones required in a statistical shape model (SSM) to accurately represent the full cavus through planus spectrum, enabling future machine learning applications in clinical practice. METHODS: Our study included weightbearing computed tomography (WBCT) data from 151 patients groupe

Persistent toe walking as a prominent feature in pediatric PMP22- Related neuropathies: A retrospective cohort study.

PURPOSE: Persistent toe walking in children is often considered idiopathic; however, increasing evidence suggests that alterations in the PMP22 gene-implicated in Charcot-Marie-Tooth disease type 1 A (CMT1A) and Hereditary Neuropathy with Liability to Pressure Palsies (HNPP)-may contribute to its pathogenesis. This study investigates the association between PMP22 variants (duplications, deletions, and point mutations) and toe walking in children, aiming to delineate their clinical and genetic characteristics. METHODS: A retrospective analysis was performed on 22 pediatric patients (mean age:

A comparative phenotypic analysis of a heterogeneous PMP22 cohort presenting with persistent toe-walking versus classic PMP22-related Neuropathies.

BACKGROUND: The Peripheral Myelin Protein 22 (PMP22) gene plays a central role in peripheral nerve myelination, and dosage alterations (deletion, duplication, or point mutation) are established causes of hereditary neuropathies such as Charcot-Marie-Tooth disease type 1 A (CMT1A), CMT1E, and Hereditary Neuropathy with Liability to Pressure Palsies (HNPP). However, its potential contribution to atypical developmental motor phenotypes such as persistent toe-walking (PTW) has not been systematically explored. OBJECTIVE: To characterize the phenotypic spectrum of pediatric PMP22 variant carriers

Can anterior midfoot tarsectomy procedure be improved using patient-specific cutting guide? An experimental study.

BACKGROUND: Anterior midfoot tarsectomy remains difficult to achieve for the treatment of cavovarus foot. Computer-assisted planning and printed patient-specific cutting guides (PSCG) aim to reduce technical errors. This study investigated the accuracy and safety of PSCG-assisted anterior tarsectomy from a cadaveric analysis. HYPOTHESIS: PSCGs provide an accurate correction and safety, disregarding the experience of the operator. MATERIAL AND METHODS: Ten foot and ankle specimen underwent an anterior tarsectomy using PSCG with a dorsal closed-wedge effect of 15°. An initial CT-scan of the fo

Effects of ankle joint biomechanics and foot postures on Achilles tendon force during running through a simulation study.

Achilles tendinopathy is a prevalent injury observed among runners. Achilles tendon force (ATF) and differences in foot posture are considered factors in the occurrence of Achilles tendinopathy. This study was conducted using computer simulations. The simulation ranges were determined for the ankle joint angle (-25° to + 25°) and plantarflexion moment (0-160 Nm) at variable running speeds using a public dataset. The ankle angle was used to compute the Achilles tendon moment arm. Moment arms specific to the normal foot, pes cavus, and pes planus were calculated using radiographs from a previous

Orthopedic Manifestations of Syringomyelia: A Comprehensive Review.

Background: Syringomyelia is a complex neurological disorder characterized by a fluid-filled cavity (syrinx) within the spinal cord, frequently resulting from altered cerebrospinal fluid (CSF) dynamics. While its clinical manifestations are diverse, orthopedic complications such as scoliosis, pes cavus, and Charcot arthropathy may represent early diagnostic clues yet are often under-recognized. Methods: This comprehensive review synthesizes the current literature on the pathophysiology, clinical presentation, diagnostic strategies, and management approaches of syringomyelia, with a specific em

Rate of bony union after Jones fracture fixation in the general population.

Nonunion rates in the conservative treatment of Jones fractures can be high, therefore surgical intervention is often recommended. Most current studies in the literature look at a younger athletic population to assess bony union rates. Little has been published on healing rates in the general population. A retrospective chart review was performed of all patients undergoing surgical fixation of Jones fractures by three surgeons from 2015 to 2022. A total of 195 patients were collected. Radiographs were evaluated to determine time to fracture union, type of fixation construct, and risk factors f

PMP22-Related Neuropathies: A Systematic Review.

Background. PMP22-related neuropathies comprise a spectrum of predominantly demyelinating disorders, most commonly Charcot-Marie-Tooth type 1A (CMT1A; 17p12 duplication) and hereditary neuropathy with liability to pressure palsies (HNPP; 17p12 deletion), with rarer phenotypes due to PMP22 sequence variants (CMT1E, Dejerine-Sottas syndrome [DSS]). Methods. We conducted a PRISMA-compliant systematic review (PROSPERO ID: 1139921) of PubMed and Scopus (January 2015-August 2025). Eligible studies reported genetically confirmed PMP22-related neuropathies with clinical and/or neurophysiological data.

Spatiotemporal and foot kinematic differences during gait in individuals with cavus foot.

Cavus foot, defined by a high medial arch, alters plantar pressure distribution and gait biomechanics, often leading to increased forefoot and heel loading, joint stiffness, and musculoskeletal complications. However, specific kinematic differences compared to normal feet remain underexplored. This study aims to analyse gait kinematics differences in subjects with cavus foot versus those with normally aligned feet. A case-control study was carried out in 100 subjects (50 with bilateral cavus foot and 50 with neutral foot) using the PODOSmart® system, which consists of one pairs of smart insole
