Graft Position and Short-term Radiographic Outcomes After Pediatric Calcaneal Lengthening for Symptomatic Flexible Flatfoot: A Retrospective Comparative Study.

BACKGROUND: Symptomatic flexible flatfoot in children often warrants calcaneal lengthening osteotomy (CLO) once conservative measures fail. Although the procedure restores the medial longitudinal arch, the optimal positioning of the trapezoid allograft along the osteotomy site remains unclear. Thus, we aim to test whether superior (≤2 mm) vs inferior (>2 mm) graft position is associated with short-term radiographic changes after pediatric CLO. METHODS: We retrospectively reviewed 27 children (36 feet) who underwent CLO between 2015 and 2024. Feet were stratified by graft superior border posit

A Rare Case of Dual De Novo Mutations Presenting With Infantile Spasms, Congenital Contractures, and Axial Hypotonia.

This report presents the case of a male infant with a complex neonatal course marked by congenital hand and foot deformities, axial hypotonia, and subsequent development of treatment-resistant infantile spasms. Despite extensive initial investigations yielding normal results, including a normal brain magnetic resonance imaging (MRI), whole exome sequencing (WES) revealed two distinct, likely de novo, autosomal dominant mutations: a pathogenic variant in the CACNA1E gene, consistent with Developmental and Epileptic Encephalopathy-69 (DEE69), and a likely pathogenic variant in the FBN1 gene, an

Surgical Correction of a Severe Bilateral Cleft Foot Using a Diamond Incision and Cannulated Screw Fixation: A Case Report and Literature Review.

Cleft foot is a rare congenital deformity, characterized by the absence of one or more central rays of the foot and presenting with a wide spectrum of clinical and anatomical variations. Surgical intervention aims to improve this deformity on three levels: functional rehabilitation, cosmetic appearance, and the ability to wear standard footwear. This case report describes the successful surgical management of a severe bilateral cleft foot (Blauth and Borisch Type III and Abraham Type II). The technique involved a diamond-shaped incision for cleft closure, cannulated screw fixation to narrow th

External Fixation for the Management of Clubfoot Sequelae in Children and Adolescents.

The Ponseti method has become the first line treatment for all cases of clubfoot including both idiopathic and syndromic. Despite the Ponseti methods simplicity and reproducibility, a large population of patients still lack access to optimal initial care and either present late with severe deformity or have had multiple previous surgical interventions. For both cases, Circular External Fixation can be considered for progressive correction. This article covers the indications for each of these external fixation constructs, as well as the different progressive correction techniques.

Clinical and genetic characterization of Weaver syndrome: A case report of an EZH2 mutation and review of the literature.

RATIONALE: Weaver syndrome is a rare congenital overgrowth disorder characterized by a wide spectrum of clinical manifestations that often overlap with other overgrowth syndromes. It is primarily caused by pathogenic variants in the Enhancer of Zeste Homolog 2 (EZH2) gene on chromosome 7q36.1. Globally, fewer than 70 cases have been reported, with only a few documented in the Chinese population. PATIENT CONCERNS: We report a 13-day-old Chinese male infant, born with macrosomia (birth weight: 5.04 kg), who was admitted for persistent neonatal jaundice. Physical examination and subsequent follo

Effects of ankle joint biomechanics and foot postures on Achilles tendon force during running through a simulation study.

Achilles tendinopathy is a prevalent injury observed among runners. Achilles tendon force (ATF) and differences in foot posture are considered factors in the occurrence of Achilles tendinopathy. This study was conducted using computer simulations. The simulation ranges were determined for the ankle joint angle (-25° to + 25°) and plantarflexion moment (0-160 Nm) at variable running speeds using a public dataset. The ankle angle was used to compute the Achilles tendon moment arm. Moment arms specific to the normal foot, pes cavus, and pes planus were calculated using radiographs from a previous

Biomechanical effects of varying arch support hardness in foot orthosis for adults with flexible flatfoot: A comprehensive Bayesian statistical analysis.

BACKGROUND: Foot orthosis hardness governed the trade-off between medial arch correction, plantar pressure redistribution, and load transfer along the lower limb, yet the relationship between hardness and biomechanical improvement in flexible flatfoot remains unclear. AIM: To investigate how different arch support hardness levels affect biomechanics and muscle forces in adults with flexible flatfoot, and to determine how these effects vary with hardness. METHODS: Twenty women aged 18-40 years with flexible flatfoot (navicular drop > 10 mm, arch height index < 0.31), all asymptomatic, with no

The association between foot morphological development and bone maturation in children: a cross-sectional study.

PURPOSE: Accurate assessment of pediatric foot development is essential for guiding clinical interventions. While chronological age (CA) is commonly used, it may not account for individual variations influenced by skeletal maturity. This study hypothesizes that bone age (BA) correlates more strongly with foot morphology than CA, particularly in predicting pediatric flatfoot progression. METHODS: A retrospective cross-sectional study was conducted with 141 healthy children aged 9-13 years. BA was assessed via hand-wrist X-rays (China 05 RUS-CHN method), and 3D foot parameters were measured usi

Muscle force imbalances predict clubfoot recurrence risk: A musculoskeletal modelling approach.

BACKGROUND AND OBJECTIVES: Idiopathic clubfoot, primarily treated with the Ponseti method, recurs in 20-40 % of cases, often within the first two years post-treatment. This recurrence is commonly linked to imbalances in the inverting and everting muscle and ligament forces surrounding the foot, disrupting ankle-foot joint alignment during gait. This study aimed to evaluate the biomechanical factors contributing to clubfoot recurrence using advanced musculoskeletal modeling. METHODS: Experimental data, including 3D segment positions, lower limb muscle activity, and ground reaction forces, were

Intrafamilial Phenotypic Variability of the FGFR1 p.Cys277Tyr Variant: A Case Report and Review of the Literature.

BACKGROUND: Split-hand/foot malformation (SHFM) is a rare congenital limb anomaly defined by the absence or hypoplasia of the central rays of the autopod. SHFM occurs as an isolated entity or part of genetic syndromes with several causative copy-number variations or monogenic alterations known to be involved in the disease pathomechanism. On the other hand, cleft lip/palate (CL/P) usually results from polygenic and environmental factors, with the complex interplay of both leading to this malformation. Pathogenic variants in FGFR1 have been linked to phenotypically distinct disorders, including
