Explainable deep learning-based multiclass classification of foot radiographs into normal, plantar fasciitis, and flatfoot.

BACKGROUND: The medial longitudinal arch plays a critical role in foot biomechanics, and its abnormalities are associated with conditions such as flatfoot and plantar fasciitis. Early and accurate diagnosis of these disorders is clinically important, but plain radiographs have limitations in visualizing soft tissue pathology. PURPOSE: This study aimed to develop and interpret a deep learning model capable of classifying foot radiographs into normal, plantar fasciitis, and flatfoot categories. METHODS: A DenseNet-121 architecture was trained on 9500 synthetic lateral foot X-ray images from th

Clinical and radiographic efficacy of subtalar screw arthroereisis in the treatment of pediatric flexible flatfoot.

BACKGROUND: Subtalar arthroereisis has been reported to be an effective treatment technique for flexible flatfoot (FF) in children. Although many devices for this procedure exist, arthroereisis using screws is still globally used. Therefore, this study aims to revisit subtalar screw arthroereisis (SSA) and investigate its outcomes. METHODS: We retrospectively reviewed 353 flexible flatfeet in 178 pediatric patients who underwent SSA between 2007 and 2020. Clinical and radiological assessments were conducted pre-implantation and pre-explantation. Radiographic angles were measured to quantify c

Virtual 3D landmark palpation in clubfoot: feasibility of an innovative standardized morphometric protocol. A preliminary study.

BACKGROUND: Reliable morphological assessment remains a clinical challenge in congenital clubfoot management, as current clinical scoring systems fail to capture the complex three-dimensional anatomy required for precise evaluation and treatment planning. Virtual palpation of anatomical landmarks offers a standardized, objective approach to define bony axes, but requires rigorous validation to become clinically applicable. METHODS: 3D models (dried clubfoot and normal foot) were reconstructed from segmented CT scan datasets using image segmentation. For reproducibility purpose, thirty-two ana

Outcomes of subtalar arthroereisis for pediatric and adolescent flexible flatfoot: A systematic review comparing metallic versus bioabsorbable implants.

BACKGROUND: Subtalar arthroereisis is widely used to treat flexible flatfoot in growing patients. Metallic implants have historically been standard, whereas bioabsorbable devices have emerged as an alternative that may reduce implant-related symptoms and avoid elective hardware removal. We systematically compared clinical and radiographic outcomes, complication rates, and patient-reported satisfaction between metallic and bioabsorbable implants. METHODS: We performed a PRISMA-compliant systematic review and meta-analysis. PubMed, Embase, the Cochrane Library, and Web of Science were searched

The Way of the Recession: Surgical Off-loading via Intramuscular Fascial Lengthening in the Diabetic Foot.

Diabetic foot ulcers are a common and devastating complication of diabetes, often linked to equinus deformity, which raises plantar forefoot pressures and impairs healing. Conservative strategies like casting and orthoses are first-line but limited by high recurrence and compliance challenges. Surgical off-loading procedures-Achilles tendon lengthening, gastrocnemius recession, and lesser tendon recessions-target the root biomechanical cause and demonstrate superior durability in ulcer prevention. Evidence, including research by Laborde and others, supports their role in healing neuropathic an

Talar Index: A New Radiologic Marker for Clubfoot Equinus.

BACKGROUND: Residual equinus is an important indicator for deciding the need for tenotomy and checking for recurrence of clubfoot deformity. This study aims to validate a novel, radiologic marker-the Talar Index-for objective assessment of residual equinus in clubfoot. METHODS: A cross-sectional observational study was conducted on infants with idiopathic clubfoot treated using the Ponseti protocol, assessed just before percutaneous Achilles tenotomy. The validity of the talar index was tested against the unaffected feet. A positive talar index was defined as the presence of residual equinus.

Muscle synergy asymmetry between the affected and unaffected limbs during the stance phase in unilateral flatfoot.

BACKGROUND: Musculoskeletal conditions can disrupt neuromuscular coordination during gait, leading to asymmetry and altered muscle synergy patterns. Understanding how the nervous system reorganizes synergies in individuals with unilateral flatfoot (UF) is essential for improving clinical assessment and rehabilitation strategies. METHODS: Surface electromyography (sEMG) of major lower-limb muscles during the stance phase was collected from 24 individuals with UF. Synergists were extracted by non-negative matrix factorization (NMF) and analyzed with vector coding (VC) to examine bilateral phase

Radiographic Assessment and Measurement Reliability in Defining Flat-Top Talus in Ponseti-Treated Clubfeet.

INTRODUCTION: Flat-top talus (FTT) deformity in clubfoot may lead to ankle impingement and early degenerative changes. Several quantitative radiographic parameters have been proposed to define FTT, but their reliability in children remains unclear. We aimed to compare quantitative radiographic measures and evaluate their inter- and intrarater reliability relative to qualitative assessment of talar dome morphology. METHODS: This retrospective dual-center cohort included Ponseti-treated clubfeet (2005 to 2023) with at least one adequate lateral radiograph. Two independent raters per site measur
