Differences in Plantar Pressure Distribution Between Adults with Asymptomatic and Symptomatic Flexible Flatfeet During Walking and Heel-Strike Running.

Flatfeet involve a collapse of the medial longitudinal arch, hindfoot valgus, and forefoot abduction. Flexible flatfoot is the most common type and can often be corrected with physiotherapy or orthotics. While some individuals remain asymptomatic, others develop symptoms for reasons that are not fully understood. This cross-sectional study compared plantar pressure distributions in 16 adults with asymptomatic and 16 with symptomatic flexible flatfeet (FPI-6 > 6; navicular drop > 5 mm), using a resistive-sensor-equipped pressure plate during walking and heel-strike running. During walking, symp

Sequential correction of clubfoot deformities with Ponseti casting and tenotomy: Insights from the Dimeglio scoring framework.

BACKGROUND: This study evaluated the point during Ponseti treatment when the foot reaches a neutral position and quantified how percutaneous Achilles tenotomy influences each component of clubfoot deformity. An additional aim was to provide a visual representation of correction trends using the Dimeglio scoring framework. METHODS: Infants under six months of age with unilateral idiopathic clubfoot were assessed at each visit with the Dimeglio score. Changes in total and individual deformity parameters were analyzed throughout serial casting and following tenotomy. RESULTS: The total number o

Calcaneal shortening for talonavicular medial subluxation in pediatric equinovarus deformity.

INTRODUCTION: Medial subluxation of the talonavicular joint is a common component of equinovarus deformity. Traditional cuboid or cuneiform osteotomies may not adequately correct this deformity. This study evaluated the radiographic correction and prognostic factors of calcaneal shortening for midfoot deformity. METHODS: We retrospectively reviewed 28 patients (32 feet) with equinovarus deformity treated by calcaneal shortening between 2011 and 2021. The shortening osteotomy was performed between the middle and posterior facets of the calcaneus to facilitate lateral translation of the navicul

Fetal study of soft tissue anomalies and osteocartilage anlagen defects in congenital club foot.

Congenital talipes equinovarus (CTEV) is a common musculoskeletal entity that affects 1-2 per 1000 of live births worldwide. Most are idiopathic with a typical deformity of midfoot cavus, forefoot adductus, hindfoot varus, and equinus. Controversy exists whether CTEV results primarily from in-utero soft-tissue abnormality or the defect is in the osteocartilage anlagen. This fetus study aimed to elucidate the gross anatomical morphology and morphometry of bones, joints, and soft tissue in CTEV compared with the normal foot. This comparative cross-sectional study done on 12 normal feet and four

Efficacy of Total Tibialis Anterior Tendon Transfer using Button Anchor in Management of Residual Dynamic Supination in Congenital Talipes Equino Varus.

INTRODUCTION: Congenital talipes equino-varus (CTEV) is amongst one of the most common paediatric foot deformities. Ponseti's method is the standard way of treatment, however, some patients are left with residual or partially corrected deformities. Dynamic supination is one amongst them, where the foot supinates in swing phase of the gait cycle. It is due to a strong tibialis anterior and its weak antagonist. MATERIALS AND METHODS: We undertook a prospective interventional study in thirty patients of CTEV with residual dynamic supination deformity and treated them with tibialis anterior tendo

Horns, nails, and leaky kidneys: A rare case of congenital nephrotic syndrome.

Nail-patella syndrome (NPS) is an uncommon autosomal dominant condition marked by nail dysplasia, skeletal abnormalities, and variable renal manifestations, resulting from mutations in the LMX1B gene. We report a rare case of a 23-year-old male presenting with nephrotic-range proteinuria, characteristic skeletal manifestations of NPS, and a family history of renal failure. Genetic testing identified a previously unreported heterozygous missense variant in the homeodomain of LMX1B (c.791A>C; p.Gln264Pro), supporting its pathogenicity. The absence of patellar hypoplasia in our patient highlights

Functional, Radiological, and Biomechanical Outcomes Following Deformity-Driven Multilevel Surgical Correction for Fixed Cavovarus Foot Deformity.

BACKGROUND: Fixed cavovarus foot deformity is characterised by a plantar-flexed first ray, hindfoot varus, and abnormal plantar loading, often resulting in pain, instability, and gait dysfunction. The surgical correction we performed involved a deformity-driven approach to address the structural apex and associated components. Evidence linking this multilevel correction strategy to objective biomechanical improvement remains limited. METHODS: This prospective case series evaluated outcomes following a uniform deformity-driven multilevel surgical correction protocol for fixed cavovarus deformi

Flexible flatfoot in children: update on surgical treatment and comparison of interposition materials. Porous titanium wedges versus calcaneal autograft and hydroxyapatite.

Paediatric flexible flatfoot is the most common orthopaedic deformity encountered in paediatric practice. Although most cases are asymptomatic and resolve spontaneously, a subgroup of patients develops pain, functional impairment, and progressive deformity, requiring surgical treatment after failure of conservative management. Evans and Cotton osteotomies remain the standard procedures for correcting hindfoot valgus and restoring the medial longitudinal arch, both requiring a structural interposition material to maintain correction. Traditionally, autografts, allografts, and xenografts have be

The First Documented Case of Ectrodactyly in Ghana: A Case Report.

Ectrodactyly, also called split-hand/foot malformation, is a rare congenital limb defect. We report what seems to be the first documented case of ectrodactyly in Ghana. A term female neonate, delivered by elective cesarean section at 38 weeks and 1 day to a 45-year-old multiparous mother with poorly controlled chronic hypertension and well‑controlled gestational diabetes, was found to have bilateral cleft‑type deformities of both hands and feet. Both the index and middle fingers were absent from the right hand, while the middle finger was absent from the left hand. The second and third toes of

Defining Radiographic Threshold Values for the Progressive Collapsing Foot Deformity Classification System: A Large-Cohort Diagnostic Study.

BACKGROUND: The Progressive Collapsing Foot Deformity (PCFD) classification categorizes deformity patterns into 5 classes (A, hindfoot valgus; B, midfoot/forefoot abduction; C, medial column collapse; D, peritalar subluxation; E, ankle valgus). Currently, the classifying process relies on surgeons' experience and interpretation of clinical and radiographic measurements. The goal of the present study was to establish cut-off threshold values for commonly used PCFD measurements assessing class A to D, using 2 large cohorts of PCFD patients and controls. METHODS: This prospective comparative dia

Weightbearing CT Evaluation of Contact and Distance Mapping of the First Tarsometatarsal and Metatarsophalangeal Joints Following MIS Hallux Valgus Correction.

BackgroundThree-dimensional distance and coverage mapping (DM and CM) generated through weightbearing CT (WBCT) can aid in understanding the complex articular relationship in hallux valgus (HV). Minimally invasive (MIS) hallux valgus correction has become popular in correcting this deformity. Prior biomechanical studies have demonstrated that alterations occur at the first metatarsophalangeal (MTP) and tarsometatarsal (TMT) articulations following correction. The purpose of this study is to evaluate DM and CM characteristics at the first MTP and TMT joint as well as the medial metatarsosesamoi

A Case Report of a Novel Myelin Protein Zero (MPZ) Pathogenic Variant in Charcot-Marie-Tooth Disease Combined With Type 2 Diabetes.

Charcot-Marie-Tooth (CMT) disease is the collective term for the most common inherited peripheral neuropathies, affecting both motor and sensory nerves. A typical CMT patient presents with slowly progressive distal muscle weakness and atrophy that primarily involves the small foot muscles, peroneal muscles, and, often later, the muscles of the hands and forearms. Foot deformities, most commonly pes cavus and claw toes, are common and can lead to gait impairments. In this paper, we report a patient with an intermediate CMT (CMT-Int) type carrying a novel pathogenic variant in the MPZ gene combi

Multilevel Laminectomy for Lumbar Spinal Stenosis With Low Back Pain in Achondroplasia: A Case Report.

BACKGROUND Achondroplasia is an autosomal dominant skeletal dysplasia and the most common genetic cause of dwarfism, characterized by shortened pedicles, thickened laminae, and reduced interpedicular distances that lead to congenital narrowing of the spinal canal. These anatomical abnormalities frequently result in lumbar spinal stenosis (LSS) requiring surgical management. Surgical treatment typically involves decompression of neural elements, with or without fusion, to restore canal patency and prevent postoperative deformity. However, adjusting the surgical approach to the unique anatomical

Management of Lower-Extremity Deformity in Arthrogryposis Multiplex Congentia: A Narrative Review.

Arthrogryposis multiplex congenita (AMC) is a highly heterogeneous constellation of disorders defined by non-progressive congenital multiple joint contractures, typically manifesting across multiple limbs. The pathology results in significant functional impairment, including restricted range of motion, chronic arthralgia, and secondary musculoskeletal deformities like scoliosis. Given that AMC is an umbrella designation encompassing over 300 distinct etiologies, the profound clinical variability poses substantial diagnostic and therapeutic challenges. The relative rarity of AMC and the absence
