BACKGROUND: Lesser toe deformities such as hammer, mallet, claw, or cross over toe deformities are common in the elderly. They are attributed to mechanical, neuromuscular, congenital, inflammatory arthritis, or other disorders. They become clinically relevant when they produce pressure and friction at the distal ends of the digits leading to preulcerative callus or ulcer formation. These deformities can be difficult to offload with custom orthotics, accommodative footwear, and off-the-shelf devices such as crest pads and toe crutches.
CASE: Mr. D was an 82-year-old man with bilateral lesser t

OBJECTIVE: To investigate the correlation between fourth brachymetatarsia and hallux valgus, and to provide evidence-based support for clinical decision-making.
METHODS: A retrospective analysis was conducted on 68 patients (89 feet) with congenital fourth brachymetatarsia treated between August 2021 and November 2025. All patients were female, aged 18-44 years (mean, 28.1 years). The hallux valgus angle (HVA), intermetatarsal angle (IMA), and shortening length of the fourth metatarsal were measured on standardized weight-bearing anteroposterior foot X-ray films. Hallux valgus-related complic

Neonatal diabetes mellitus (NDM) is a rare genetic condition characterized by persistent hyperglycemia presenting within the first six months of life. It is most commonly caused by mutations affecting pancreatic development or β-cell function. ZNF808 is a recently identified gene that plays an essential role in human pancreatic development, with biallelic pathogenic variants causing autosomal recessive pancreatic agenesis. We report the case of a seven-month-old Saudi male infant with a history of intrauterine growth restriction who presented at 2.5 months of age with severe hyperglycemia and

Flatfoot, a disorder defined by the flattening or loss of the medial longitudinal arch (MLA), affects approximately 15.6% of the child population. It may result from congenital or acquired dysfunction of muscles, tendons, or ligaments in the foot and ankle, as well as skeletal abnormalities. Clinically, this disorder manifests as foot pain, gait abnormalities, and compensatory injuries in lower limb joints (e.g., knee and hip), thereby affecting motor function and quality of life. Short foot exercises (SFE) as a noninvasive rehabilitation strategy enhances arch height and improves dynamic foot

BACKGROUND: Flatfoot deformities are characterized by collapse of the medial arch due to tendinous and ligamentous dysfunction. The degree of deformity can be evaluated radiographically by measuring the talo-first metatarsal angle (Meary angle). These deformities have traditionally been corrected using a medial cuneiform dorsal opening wedge (Cotton) osteotomy; however, this procedure requires bone grafting to fill the osteotomy site and results in lengthening of the medial column. To address these concerns, the plantar closing wedge of the medial cuneiform bone, known as the reverse Cotton os

BACKGROUND AND PURPOSE: Surgical outcomes in patients with flexible adult-acquired flatfoot deformity (AAFD) have not been evaluated in large prospective register studies. Calcaneal osteotomy and hindfoot arthrodesis are commonly used. We aimed to compare the results of these 2 procedures using preoperative and 1-year postoperative patient-reported outcome data from the Swedish Quality Register for Foot and Ankle Surgery (Swefoot).
METHODS: We analyzed data regarding feet that had primary surgery for flexible (grade II) AAFD between 2017 and 2022 in Sweden. The primary outcome was the Self-Re

Progressive collapsing foot deformity (PCFD) is an acquired condition characterised by collapse of the medial longitudinal arch, hindfoot valgus and forefoot abduction. This condition predominantly affects middle-aged and older patients and can lead to significant pain and functional impairment. Clinical assessment is key to identifying a flexible versus rigid foot deformity. The Johnson and Strom classification provides a framework for staging the disease process and guides management. Treatment can involve non-operative measures such as orthoses, or operative measures tailored to the compone

Posterior tibial tendon dysfunction (PTTD) now renamed progressive collapsing foot deformity, is the most common cause of adult acquired flatfoot deformity. When conservative approaches are unsuccessful, a medial displacement calcaneal osteotomy (MDCO) is commonly paired with tendon repair or reinforcement. This study aimed to elucidate the mechanical function of the foot and ankle joints in 15 patients undergoing combined MDCO, for which the restoration of joint mechanics and energetics remains poorly understood. A control group of 32 healthy volunteers was included for comparison. Foot joint

BACKGROUND: Rigid adult-acquired flatfoot deformity (AAFD) often requires arthrodesis. While triple arthrodesis is standard, double arthrodesis has been proposed as an alternative. However, comparative evidence remains limited. This study compared their clinical efficacy, radiographic correction, and plantar pressure distribution.
METHODS: Patients who underwent double or triple arthrodesis for rigid AAFD in our department from April 2013 to December 2019 were retrospectively reviewed. Outcomes included AOFAS scores, Foot Function Index (FFI), radiographic parameters (Meary's angle [MA], talo

Adult acquired flatfoot deformity (AAFD) is a progressive condition characterized by collapse of the medial longitudinal arch, pain, and functional impairment. It predominantly affects middle-aged women and is associated with obesity, diabetes, hypertension, and anatomic variants. MR imaging is the gold standard for evaluating osseous, tendinous, and ligamentous structures, enabling accurate diagnosis and staging. This review summarizes the role of MR imaging in AAFD, detailing normal anatomy, pathophysiological mechanisms, classification systems, and advanced imaging protocols. Key MR imaging

Background: Adult acquired flat foot deformity (AAFD) is a triplanar deformity characterised by collapse of the medial longitudinal arch. The most frequently used surgical method is the Evans calcaneal osteotomy. It is usually used in the paediatric age group and there are very few studies related to the use of the modified calcaneal osteotomy procedure in adults. The aim of this study was to evaluate the radiographic improvements after this procedure in the adult age group and to investigate whether or not there was real probability of increased arthritis in the STJ and CCJ. Methods: The stud

Although Dupuytren's disease (DD) and relapsed Clubfoot (RC) are clinically distinct conditions, both exhibit fibrotic tissue remodeling and contracture. This exploratory study investigated whether DD and RC share molecular features associated with fibroproliferative contracture. Pathological tissues from DD nodules and contracted tissues from RC together with their respective control tissues (n = 6/group), were analyzed using label-free quantitative proteomics. The analysis identified 12 significantly upregulated proteins shared between both pathological conditions relative to their controls

Meningomyelocele is a congenital defect resulting from the incomplete closure of the neural tube during embryonic development, making it the most common structural birth anomaly of the central nervous system. Over time, these deformities can progress to the knee and ankle joints due to the development of contractures. In addition, rotational deformities of the lower extremities are also seen. This study outlines a mid-childhood female with a complaint of fixed knee flexion and a spastic equinovarus deformity that has been affecting her walking and performing activities of daily living for the

CASE: A 4-year-old girl with bilateral arthrogrypotic clubfoot underwent posterior release with bilateral V-Y advancement flaps. At 4 months, the patient ambulated independently over moderate distances. One-year follow-up confirmed pain-free functional ambulation, although persistent toe-walking highlighted ongoing neuromuscular challenges and minor correction loss.
CONCLUSION: V-Y advancement flaps provide effective soft tissue coverage after posterior release for arthrogrypotic clubfoot. Early multidisciplinary collaboration integrating orthopedic and plastic surgical planning is essential

Peroneal spastic flatfoot (PSFF) involves peroneal spasm and subtalar restriction, yet optimal conservative care is undefined. This pilot retrospective study explored the preliminary outcomes of botulinum toxin type A (BoNT-A) injection combined with rehabilitation in 19 PSFF patients (10 with structural abnormalities [SA], 9 without [NSA]). Pain (VAS), spasticity (MAS), and function (AOFAS-AH) were assessed at baseline, 1, 3, and 6 months. Significant improvements were observed across all metrics (P < 0.05). The median VAS decreased from 4 to 0, median AOFAS-AH improved from 67 to 100, and me

Oculodentodigital dysplasia (ODDD) is a rare genetic disorder caused by mutations in the GJA1 gene, which encodes connexin 43 (Cx43). The condition presents with a broad phenotypic spectrum including ocular, dental, craniofacial, neurological, and skeletal abnormalities. We report the case of a 24-year-old female patient with genetically confirmed ODDD, carrying the c.226C > T (p.Arg76Cys) variant in the GJA1 gene, who presented with compatible clinical signs. Ophthalmologic evaluation revealed epicanthal folds, microcorneas, nystagmus, hyperprolate corneal profiles, and corneal topography con

BACKGROUND: The Ponseti method is the gold standard treatment for idiopathic congenital talipes equinovarus (CTEV). Clinical monitoring during correction primarily relies on the Modified Pirani score, which evaluates deformity severity based on clinical findings. However, objective quantitative assessment of morphological foot changes during treatment remains limited. This study aimed to evaluate the relationship between serial foot anthropometric measurements and Modified Pirani scores during Ponseti correction of idiopathic clubfoot.
METHODS: A prospective observational cohort study was con

Van Maldergem Syndrome (VMS) is a rare autosomal recessive disorder caused by pathogenic variants in the atypical cadherin genes DCHS1 or FAT4 and is marked by craniofacial, skeletal, and neurodevelopmental abnormalities. Although DCHS1-FAT4 binding is mediated by their respective extracellular domains, the in vivo function of the DCHS1 intracellular domain (ICD) is poorly defined. To test its function, we generated mice in which the DCHS1 ICD was deleted and replaced with a V5 epitope tag (Dchs1ΔICD-V5). Homozygous Dchs1ΔICD-V5/ΔICD-V5 mice are viable but exhibit VMS-like craniofacial flatten
