Development of a structured Goal Attainment Scaling framework for spastic foot treated with selective tibial neurotomy: an approach using a drop-down menu.

BACKGROUND: Goal Attainment Scaling (GAS) is increasingly used to assess individualized outcomes in spasticity management, yet its clinical adoption remains limited due to challenges in defining and standardizing goals. OBJECTIVES: This study aimed to facilitate the clinical use of GAS in adults undergoing selective tibial neurotomy for spastic foot management by developing a comprehensive, structured drop-down menu of goal areas to enhance its accessibility, efficiency, and usability in daily practice. METHODS: This cohort study included 59 adults with spastic equinovarus foot of various et

Treatment of Bilateral Severe Equinus Deformities Secondary to Neurosarcoidosis in a Pediatric Patient: A Case Report.

CASE: A 4-year-old boy with neurosarcoidosis developed bilateral equinocavovalgus deformities that significantly altered his gait. Our management included serial short leg stretching casts which corrected the neuromuscular deformity and led to a normal, reciprocal heel-toe gait. CONCLUSION: To the best of our knowledge, this is the first reported case of severe bilateral equinocavovalgus deformity in a pediatric patient with neurosarcoidosis, successfully treated with serial casting without the need for surgical correction. Correction has remained durable at the 2-year clinic follow-up.

Flatfoot and artificial intelligence: Can language models be trusted for patient education?

Flatfoot (pes planus) is common in children and adults. Although often asymptomatic, it may alter lower-limb biomechanics and contribute to pain or injury. Patients and caregivers increasingly use artificial intelligence chatbots such as chat generative pretrained transformer (ChatGPT) and Google Gemini for medical information, yet their responses on flatfoot remain insufficiently studied. This study compared the factual accuracy, added value, omissions, and readability of responses generated by ChatGPT and Google Gemini to standardized patient-oriented questions. In this cross-sectional compa

Clinical characterization of tethered cord syndrome due to lipomatous filum and filum terminale lipoma in symptomatic children.

OBJECTIVE: Tethered cord syndrome (TCS) is heterogeneous, and a filum with lipomatous tissue can present in multiple patterns. The aim of this study was to determine whether filum characteristics are associated with particular symptomatology. METHODS: A descriptive cross-sectional retrospective study was conducted over 2 years (2023-2025) at a single institution. Patients with TCS and filum with lipomatous tissue were evaluated using 24 TCS-related variables and 10 filum-related variables. Statistical methods included using the chi-square test and ANOVA for frequency differences, multivariabl

Compensatory proximal malalignment despite preserved macroscopic gait in pediatric flexible flatfoot: A dual-level analysis of static alignment and dynamic kinematics.

BACKGROUND & OBJECTIVE: Previous biomechanical studies on pediatric flexible flatfoot have treated the subject as a single analytical unit, thus failing to elucidate mechanical transmission within the lower extremity closed kinetic chain. This study investigates the impact of distal arch collapse on macroscopic gait parameters, static joint alignment, and dynamic kinematic deviation by integrating a dual-level analytical strategy at both the subject and individual limb levels. METHODS: 153 children with pediatric flexible flatfoot (25 unilateral, 128 bilateral) were included, and 3D gait data

Evaluation of foot biomechanics following surgical interventions, including SPOTT and SPLATT procedures in the management of equinovarus deformity in cerebral palsy hemiplegia.

BACKGROUND: Cerebral Palsy (CP) is a group of disorders affecting movement and posture and is the most common physical disability in children. Surgical interventions, such as split anterior and posterior tibial tendon transfers (SPLATT and SPOTT), can be necessary to correct foot and ankle deformities in CP. This is a retrospective pre-/post-operative cohort study that aims to specifically investigate effects on lower limb and foot biomechanics in children with CP. METHODS: Fifteen children aged 8-16 years at initial assessment (10 girls, 5 boys) underwent SPLATT and SPOTT procedures as part

Congenital Clubfoot as an Early Manifestation of Duchenne Muscular Dystrophy?

BACKGROUND: Duchenne muscular dystrophy (DMD) is the most common X-linked progressive neuromuscular disorder of childhood. The classic course of DMD is characterized by onset after a period of normal motor development. However, in recent years, evidence has been accumulating of prodromal features of the disease, including speech delays and neurocognitive impairments. METHODS: This study investigates whether congenital bilateral clubfoot may represent the first clinical manifestation of DMD. RESULTS: We present two cases of male patients with congenital bilateral clubfoot who were subsequentl

Ponseti-Based Hybrid Approach for Bilateral Teratologic Clubfoot With Congenital Peroneal Nerve Dysfunction: A Case Report.

CASE: We report the case of a newborn boy with bilateral clubfoot associated with congenital peroneal nerve dysfunction who responded poorly to the standard Ponseti method. Successful initial correction was achieved through multiple tenotomies of the Achilles, posterior tibialis, flexor digitorum longus, and flexor hallucis longus tendons. Recurrence of equinus deformity without hindfoot varus was treated with posterior soft tissue release, during which all previously transected tendons were found to have completely regenerated. CONCLUSION: This case highlights the value of a Ponseti-based tr

Tibialis anterior muscle function in ankle-foot deformities as derived from intraoperative force measurements.

Ankle-foot deformities, such as clubfoot, pose treatment challenges due to complex and patient-specific biomechanics. Accurate assessment of muscle force behavior is essential for tailoring interventions, yet conventional gait analysis is limited in isolating muscle-specific function. Intraoperative force measurements via the muscle's tendon offer a unique opportunity to directly quantify muscle mechanics. This study aimed to characterize the passive and active force-angle relationship of the tibialis anterior (TA) muscle, intraoperatively. Ten patients (ages 3-62) undergoing corrective foot s

Age-Dependent Progression of Neurological Involvement in PRPP Deficiency: Insights from a Four-Generation Family and Systematic Review.

Loss-of-function (LoF) variants in the PRPS1 gene, encoding the phosphoribosyl pyrophosphate (PRPP) synthetase 1 enzyme, cause rare neurometabolic disorders historically viewed as discrete entities: nonsyndromic deafness (DFNX1), Charcot-Marie-Tooth disease type X5 (CMTX5), and Arts syndrome. A major clinical challenge is the temporal dissociation between early auditory failure and subsequent neurodegeneration, causing fragmented diagnostics. We systematically quantified this diagnostic latency and reconceptualized the disease spectrum through a molecular lens. A PRISMA-compliant systematic re

Position, size and orientation of the peroneus longus tubercle in patients with cavovarus feet.

BACKGROUND: Plantarflexion of the first ray is an important component of cavovarus foot deformities, which is thought to be due to the pull of the peroneus longus muscle. We inferred its action by studying its insertion into the peroneus longus tubercle (PLT). Our aim was to determine how PLT orientation and morphology differs in cavovarus feet compared to controls on weightbearing computed tomography (WBCT) imaging. METHODS: We retrospectively analysed 30 WBCT scans from patients with cavovarus feet and Charcot-Marie Tooth disease. We compared these to 30 normal feet using previously describ

STAC3D Against the Odds: Characterizing the Musculoskeletal Phenotype of STAC3 Gene Disorder.

INTRODUCTION: STAC3 Disorder (STAC3D) is a rare genetic condition characterized by musculoskeletal abnormalities, myopathic facies, palatal anomalies, micrognathia, short stature, and an increased susceptibility to malignant hyperthermia. It was initially described in the Lumbee Native Americans of North Carolina, though it is now found globally. This study aims to characterize the musculoskeletal phenotype of STAC3D to aid in its diagnosis and clinical management. METHODS: A retrospective cohort study of STAC3D patients from 2014 to 2025 at a single institution was conducted. Patients diagno

Impact of Non-Tobacco Nicotine Dependence on Fusion Success and Complications in Midfoot Arthrodesis.

BackgroundNon-tobacco nicotine dependence (NTND) products, such as vaping, nicotine patches/pouches, gum, and lozenges, have become increasingly prevalent. While the negative effects of cigarette smoking on bone healing are well established, the impact of NTND on surgical outcomes remain unclear, particularly in foot and ankle surgery. This study aimed to evaluate the effect of NTND on short- and long-term postoperative complications following midfoot arthrodesis, a procedure commonly performed for arthritis, trauma, and congenital deformities.MethodsThis retrospective cohort study was conduct

Phenotypic and Molecular Features of a Large ODDD Family: Expanding the Spectrum of CX43-Related Disorder.

We present a family of five siblings who came to our attention with a clinical and radiological diagnosis of familial hypomyelinating leukodystrophy. Despite brain white matter abnormalities being present in all siblings, the clinical phenotype was variable: the three brothers presented with a clear-cut late-onset spastic paraplegia, whereas the two sisters displayed only mild pyramidal signs. Molecular analysis revealed a single relevant variant shared by all affected siblings, namely the likely pathogenic variant c.659C>T (p.Ser220Phe) in the GJA1 gene. Variants in this gene are generally as

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Structural fragility in digital public health communication.

Inherited genetic disorders are progressive in nature and lead to organ dysfunction or death in severe cases. At present, there are no permanent treatment options for >95% of inherited disorders. Different modes of inheritance, type of gene(s) involved, and population-based variations add further complexity to finding suitable cures for approximately 400 million patients worldwide. Gene therapy is a very promising molecular technique for the treatment of rare genetic disorders. Gene therapy functions on the basis of restoration, replacement, inhibition, and, most recently, editing of gene(s) t

Erbin deficiency exacerbates sepsis‑induced liver injury through mitochondrial ROS/NLRP3‑mediated inflammatory responses.

BACKGROUND: Idiopathic pulmonary fibrosis (IPF) is a rare interstitial lung disease with a poor prognosis that currently lacks effective treatment methods. Preventing the acute exacerbation of IPF, identifying the molecular subtypes of patients, providing personalized treatment, and developing individualized drugs are guidelines for predictive, preventive, and personalized medicine (PPPM / 3PM) to promote the development of IPF. Oxidative stress (OS) is an important pathological process of IPF. However, the relationship between the expression levels of oxidative stress-related genes (OSRGs) an

Lysosomal metabolism in pancreatic cancer and its crosstalk with host physiology.

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine

PRMT7 at the crossroads of cancer progression and therapeutic targeting (Review).

Acute lung injury (ALI) affects over 10% of patients hospitalised in critical care, with acute respiratory distress syndrome (ARDS) being the most severe form of ALI and having a mortality rate in the region of 40%. There has been slow but incremental progress in identification of biomarkers that contribute to the pathophysiology of ARDS, have utility in diagnosis and monitoring, and that are potential therapeutic targets (Calfee CS, Delucchi K, Parsons PE, Thompson BT, Ware LB, Matthay MA, Thompson T, Ware LB, Matthay MA, Lancet Respir Med 2014, 2:611--620). However, a major issue is that ARD
