Robotic Mitral Valve Repair in Patients With Thoracic Deformity.

BACKGROUND: Robotic mitral valve repair (MVr) offers superior visualization and dexterity, potentially overcoming challenges in severe thoracic deformity such as pectus excavatum. This study compared surgical outcomes after MVr in patients with and without thoracic deformity and assessed the impact of thoracic deformity on operative times in robotic MVr. METHODS AND RESULTS: We reviewed 359 patients undergoing robotic MVr for degenerative mitral regurgitation (2018-2022). Patients with a Haller index ≥3.25 categorized as the deformity group (Group D; n=35), and those with a Haller index <3.25

The pugilist and the price of controlled violence.

The Hellenistic bronze known as The Boxer at Rest presents one of the most striking representations of trauma in ancient art. Unlike the idealized athletic bodies of Classical Greece, the sculpture depicts a fighter marked by swelling, facial asymmetry, auricular deformity, scars, and signs of repeated injury. Viewed through a contemporary medical lens, these features correspond closely to the cumulative consequences of chronic exposure to blunt-force trauma. This article examines the sculpture at the intersection of medicine, neuroscience, and anthropology. The visible injuries represented by

A 20-year bibliometric analysis on the development trends and hotspots of pectus excavatum research: 2005-2025.

BACKGROUND: Despite significant advances in surgical techniques for pectus excavatum (PE), the evolution of research priorities and clinical foci in this field remains poorly defined. This study aims to map the thematic evolution of PE research using bibliometric methods, thereby guiding future clinical research directions. METHODS: Publications on PE from 2005 to 2025 were retrieved from the Web of Science Core Collection (WoSCC). Bibliometric analyses, including co-authorship, co-citation, keyword co-occurrence, burst detection, and trend analyses, were conducted to evaluate research produc

Third Patient With Biallelic Variants in SMAD6 With an Overlapping Phenotype: Developmental Delays, Dysmorphic Features, and Cardiovascular Abnormalities.

SMAD6 encodes an inhibitory SMAD protein that modulates BMP and TGF-β signaling. Heterozygous pathogenic variants in SMAD6 have been primarily associated with aortic valve disease, radioulnar synostosis, and nonsyndromic sagittal and metopic synostosis. However, only two syndromic patients with biallelic variants have been reported in the literature. We report a 4-year-old girl with neurodevelopmental delays, dysmorphic features, complex congenital heart disease, renal asymmetry, and arterial tortuosity. Whole exome sequencing showed two homozygous SMAD6 variants of uncertain significance: c.1

Three-dimensional analysis of condylar displacement after orthognathic surgery in patients with facial asymmetry.

PURPOSE: This study aimed to evaluate condylar positional changes in patients with facial asymmetry undergoing orthodontic treatment and orthognathic surgery by comparing the affected side (the side toward which the chin deviates) and the non-affected side (the contralateral side) across different treatment stages. MATERIALS AND METHODS: A retrospective cone-beam computed tomography (CBCT) analysis was conducted on 25 patients with facial asymmetry (menton deviation >4 mm) treated at Taipei Veterans General Hospital. CBCT scans were obtained at 4 timepoints: pre-treatment (T1), post-orthodont

Pectus Excavatum-Induced Preload Insufficiency Presenting as Long-Standing Sinus Tachycardia.

BACKGROUND: Severe pectus excavatum is an uncommon cause of hemodynamic compromise in adults and may present with unexplained tachycardia when preload filling is impaired by cardiac compression. CASE SUMMARY: A 65-year-old woman with long-standing palpitations presented with sinus tachycardia despite high-dose beta-blockade. Electrocardiogram and ambulatory monitoring showed sinus rhythm with marked heart rate variability. Right heart catheterization revealed low filling pressures, with a mean right atrial pressure of 1 mm Hg and preserved cardiac output. Cardiovascular magnetic resonance sho

Advancing Preliminary Findings: A Follow-up Study on Parental Education to Prevent Plagiocephaly.

INTRODUCTION: Positional plagiocephaly is characterized by cranial asymmetry and can be the result of prolonged uneven pressure on the developing skull. Extrauterine causes include extended time in the supine position, one-sided feeding, or insufficient tummy time. Beyond cosmetic concerns, it can affect visual, auditory, dentofacial, and speech development. Following the "Back-to-Sleep Campaign," prevalence rose to 16% to 22.1% in 6-week-old infants ( Iran J Child Neurol. 2022;16(2):85-92). Despite its extensive impact on development and finances, many caregivers and parents lack awareness, u

Is cryoanalgesia superior to thoracic epidural analgesia for postoperative recovery after the nuss procedure? An updated meta-analysis.

BACKGROUND: The Nuss procedure is commonly used to correct pectus excavatum, with cryoanalgesia (CA) and thoracic epidural analgesia (TEA) as pain management options. While a prior meta-analysis showed shorter hospital stays with CA, publication of recent studies warrants a more comprehensive evaluation of outcomes not previously reported. METHODS: A literature search identified studies comparing CA and TEA in Nuss procedures. Pooled odds ratios, mean differences (MDs), and standardized mean differences (SMDs) with 95% confidence intervals (CIs) were calculated using RevMan 8.13.0, with a sub

The Bilateral Inferior Pedicled Anterior Intercostal Artery Perforator-Sparing Flap for Correcting Funnel Chest Deformity in Gender-affirming Masculinizing Chest Surgery.

Pectus excavatum (PE) is the most common congenital anterior chest wall anomaly, often requiring surgical correction due to both functional and aesthetic concerns. Gender-affirming surgery has seen a significant increase in recent decades, with mastectomy being the primary procedure for transgender men. For transmasculine patients with PE seeking chest masculinization, the possibility of addressing both conditions in a single surgery has not been well explored. We report the first known case of a 33-year-old transgender patient with PE undergoing a combined procedure of masculinization top sur

Factors Associated with Helmet Therapy Outcomes in Positional Plagiocephaly.

Background: Helmet therapy is considered to be a treatment for infants with positional plagiocephaly. Although some studies suggest that anterior fontanelle (AF) size may also affect treatment outcomes, evidence and influence remain unclear. The aim of this study is to assess the impact of anterior fontanelle size on the effectiveness of helmet therapy, with the goal of determining the optimal timing and patient criteria for treatment. Methods: We conducted a retrospective study of 94 infants treated with helmet therapy for positional plagiocephaly at Kwangju Christian Hospital between January

Anaesthetic Management of CHARGE Syndrome with Anorectal Malformation in an Infant: A Case Report.

CHARGE syndrome is a rare congenital disorder with multisystem involvement, posing significant anaesthetic challenges. We report the perioperative management of an 11-month-old male who was diagnosed with anorectal malformation and CHARGE syndrome and who underwent posterior sagittal anorectoplasty. Key anaesthetic considerations included craniofacial asymmetry, facial nerve palsy, and anticipated airway difficulties. Inhalational induction with careful preoxygenation, video laryngoscopy, and caudal analgesia facilitated safe anaesthesia. Post-operative airway compromise was managed with conti

SMILE: neural signal acquisition and intra-body transmission for facial nerve bypass-An acute feasibility study and proof-of-concept in a rat model.

Facial paralysis is a disabling condition with severe functional and aesthetic consequences. Facial paralysis affects approximately 1.8% of individuals over their lifetime, with approximately 30% of affected patients developing persistent deficits; among these, patients with permanent flaccid paralysis and severe facial asymmetry do not resolve with pharmacological treatment and require surgical intervention. It is specifically this surgically relevant subgroup that represents the target population of the SMILE framework. The SMILE framework (bypaSs of a facial nerve lesion through intra-body

Warm acupuncture combined with precise meridian sinew needling for refractory peripheral facial paralysis: a propensity score-matched retrospective study on clinical efficacy and infrared thermal imaging.

BACKGROUND: Refractory peripheral facial paralysis (RPFP) affects 15%-20% of patients with facial palsy, leading to poor recovery and reduced quality of life. Current treatments yield limited efficacy, and objective evidence for acupuncture-based therapies remains scarce. METHODS: We addressed this gap by conducting a propensity score-matched retrospective study in 166 RPFP patients (H-B grade ≥ III, duration ≥ 3 months). Patients were divided into an observation group (warm acupuncture plus precise meridian sinew needling, n = 83) and a control group (conventional acupuncture, n = 83). Outco

Percutaneous closure of an ultra-long-tunnel-type patent foramen ovale: a rare case with multimodal imaging guidance.

Patent foramen ovale (PFO) is a common congenital cardiac anomaly, and ultra-long tunnel PFO (> 20 mm) presents a significant interventional challenge despite routine standard closure. A 68-year-old female with recurrent headaches, chest pain history, and prior right coronary artery stenting was confirmed to have a 37-mm ultra-long tunnel PFO via transesophageal echocardiography (TEE) and digital subtraction angiography (DSA). After initial percutaneous closure failure, the patient was successfully treated with a modified trans-tunnel puncture technique and deployment of an Abbott 18/25 mm Amp

Postoperative outcomes after off-label cryoanalgesia during minimally invasive repair of pectus excavatum in children younger than 12 years.

BACKGROUND: Intraoperative cryoanalgesia has been widely adopted to reduce postoperative pain, opioid use, and length of stay after minimally invasive repair of pectus excavatum (MIRPE). However, its use in children younger than 12 years remains off-label, and outcomes in this age group are poorly characterized. This study evaluated postoperative outcomes after cryoanalgesia during MIRPE in carefully selected children younger than 12 years. METHODS: We performed a single-center retrospective chart review of patients younger than 12 years who underwent MIRPE with thoracoscopic transthoracic cr

Giant Lipoma of the Temporal Region Extending Into the Buccal Space: Radiologic, Surgical, and Molecular Correlation.

Lipomas are common benign tumors of adipose tissue; however, their occurrence in the head and neck region, particularly within deep facial compartments, is relatively uncommon. Lesions arising in the buccal space may pose diagnostic and surgical challenges due to their proximity to critical anatomic structures. A 69-year-old female presented with a gradually enlarging, painless swelling of the right midface, resulting in facial asymmetry. Clinical examination identified a soft, mobile mass without neurological deficit. Magnetic resonance imaging revealed a well-circumscribed lesion within the

Artificial intelligence -based modeling and comparative evaluation of craniofacial soft tissue and subcutaneous fat thickness using ct imaging for forensic identification in a northwestern indian population.

BACKGROUND: Craniofacial soft tissue thickness (CFSTT) is a critical parameter in forensic facial reconstruction, serving as a link between skeletal morphology and facial appearance. However, most existing studies rely on mean CFSTT values without considering the contribution of subcutaneous fat layer thickness, limiting the accuracy of reconstruction models. OBJECTIVE: This study aims to develop an artificial intelligence (AI)-assisted, CT-based framework for the comparative evaluation of craniofacial soft tissue thickness and subcutaneous FLT in a Northwestern Indian population, with emphas
