Cephalic Alar Base Malposition: Recognition and Correction with an Advancement Flap.

BACKGROUND: Cephalic alar base malposition (CABM) represents an underrecognized cause of nasal base disharmony and is frequently misdiagnosed as alar rim retraction. In this condition, the alar base is vertically displaced in a cephalic direction, creating imbalance in the alar-columellar relationship and often exaggerating the appearance of nasal tip ptosis. This study describes a reproducible surgical technique using a full-thickness advancement flap to reposition the alar base inferiorly while preserving the natural contour of the alar-facial groove. METHODS: A retrospective study was perf

Quantitative Assessment of Facial Expression Asymmetry in Parkinson's Disease.

OBJECTIVE: Hypomimia, or reduced facial expressiveness, is a cardinal motor feature of Parkinson's disease (PD). Although limb motor symptoms in PD are characteristically asymmetric, whether facial hypomimia exhibits comparable asymmetry remains poorly understood. We investigated facial expression asymmetry in PD using AI-based computer vision and machine learning. METHODS: Videos of instructed emotional facial expressions were collected from 101 individuals with PD and 94 healthy controls (HCs) across two independent cohorts. Facial landmarks were extracted to compute region-specific facial

Evaluating ChatGPT's Reliability in Analyzing Facial Asymmetry: A Comparative Study With Human Evaluators Using Cross Sectional Static Images.

OBJECTIVE: Facial asymmetry assessment is often subjective and time intensive, thus we aim to evaluate reliability of ChatGPT in analyzing facial asymmetry compared to human raters. METHODS: Thirty patients with unilateral facial paralysis who underwent facial reanimation surgery were included in this study. Sixty static 2D frontal images (pre and postoperative) were obtained from our database. Facial asymmetry was assessed using the Sunnybrook resting symmetry scale and a 0-4 global asymmetry rating scale. Two human raters evaluated all images independently. Facial asymmetry in unicoronal craniosynostosis: automated three-dimensional quantification of surgical outcomes.

Craniosynostosis is the premature closure of one or more cranial sutures which may result in functional and aesthetic complications. Non-syndromic unicoronal craniosynostosis commonly leads to facial asymmetry. This study aimed to quantify changes in facial asymmetry before and after surgical treatment using an automated 3D technique. A retrospective study was performed including 32 patients treated at Radboud University Medical Center Nijmegen between 2011 and 2024. Pre- and postoperative three-dimensional facial photographs were analysed, and asymmetry was quantified using regional root-mean

Early findings of morphological changes in the temporomandibular joint after orthognathic surgery in mandibular prognathism with facial asymmetry.

This study was performed to investigate early morphological changes in the temporomandibular joint (TMJ), after patients with mandibular prognathism with facial asymmetry (MPA) received orthognathic surgery. The morphological changes in the TMJ were analyzed on computed tomography multi-sectional reconstructed images first taken before preoperative orthodontic treatment and then again 6 months after orthognathic surgery, in 39 patients with MPA. A combination of Le Fort I osteotomy and bilateral sagittal split osteotomy (BSSO) was performed in 22 patients and BSSO alone was performed in 17 pat

Accuracy of fiducial-based augmented reality in auricular reconstruction.

OBJECTIVES: Reconstruction with autologous costal cartilage remains the gold standard surgical treatment for total or partial ear reconstruction. The different auricular subunits are sculpted from costal cartilage and assembled. The assembly requires a particular skillset and training, including a thorough three-dimensional (3D) understanding of auricular anatomy. The correct positioning of the ear framework can be particularly difficult in patients with hemifacial microsomia because of facial asymmetry. We developed a digital workflow that can help the surgeon in the correct positioning of th

Optimising Fronto-Orbital Distraction for Management of Unicoronal Craniosynostosis.

PURPOSE: Unicoronal craniosynostosis (UCS) involves premature fusion of the left or right coronal suture, resulting in skull deformities and facial asymmetry. Fronto-orbital distraction (FOD) has proven effective for surgically correcting UCS; however, determining optimal surgical parameters (e.g. distraction angle, length, and distractor number) remains challenging. This study aimed to optimise FOD for UCS using finite element (FE) analysis and simulated surgical outcomes. METHODS: We developed a patient-specific FE model using pre- and postoperative computed tomography (CT) scans from a 23-

Intraosseous Non-Hodgkin Lymphoma Mimicking a Periapical Lesion.

INTRODUCTION: Periapical radiolucent lesions are commonly of endodontic origin; however, some persistent cases may indicate non-endodontic pathologies, including malignancies. Diffuse large B-cell lymphoma (DLBCL) is the most prevalent subtype of lymphoma affecting the oral and maxillofacial region, yet its primary occurrence in the jaws is rare. This case report highlights the diagnostic challenges in differentiating endodontic lesions from periapical lymphomas and includes a literature review of documented cases. CASE REPORT: A 44-year-old male presented with a persistent periapical lesion

Association between idiopathic scoliosis and facial asymmetry: a gender-balanced case-control study.

This study aimed to investigate the association between idiopathic scoliosis (IS) and facial asymmetry in a gender-balanced case-control design. Based on power analysis, 100 participants were recruited (50 IS patients: 25 male, 25 female; 50 controls: 25 male, 25 female). Facial asymmetry was evaluated through clinical examination and frontal cephalometric analysis. Statistical analysis was performed using chi-square tests and logistic regression, with post-hoc power calculation. Facial asymmetry was present in 41 (82%) of IS patients compared to 18 (36%) of controls (p < 0.001). The achieved

Simultaneous Orthognathic Surgery and Costal Bone Grafting in Hemifacial Microsomia: A Rare Case Report and Literature Review.

Hemifacial microsomia (HFM) is a congenital craniofacial anomaly frequently associated with mandibular hypoplasia. Achieving stable correction in severe cases remains challenging due to abnormal mandibular anatomy and deficient temporomandibular joint structures. We report a rare revision case of a 20-year-old female with type IIA HFM who developed progressive postoperative resorption following two-jaw surgery, resulting in a "floating" ramus-condyle segment and subsequent recurrent facial asymmetry. Revision surgery consisted of unilateral sagittal split ramus osteotomy on the unaffected side

Minimally Invasive Protocol for the Management of Unilateral Condylar Hyperplasia: Case Series on Seven Patients.

Background/Objectives: Unilateral condylar hyperplasia is an idiopathic condition that causes facial asymmetry and occlusal problems. Currently, traditional treatment protocol is the combination of orthognathic and extra-oral condylectomy surgery via pre-auricular incision, which can create aesthetic problems with additional risks of facial nerve damage. The purpose of this study was to report management of condylar hyperplasia patients through minimally invasive condylectomy that was planned via virtual methods. Methods: The custom-made cutting guides were produced, and unilateral condylectom

Left-Sided Pure Trigeminal Motor Neuropathy With Radiologically Isolated Contralateral Subclinical Involvement: A Case Report.

A 54-year-old woman presented with slowly progressive left facial asymmetry and jaw weakness over three years, without sensory disturbances or other cranial nerve deficits. Clinical examination revealed wasting of the left-sided muscles of mastication with preserved facial sensation. Magnetic resonance imaging demonstrated marked fatty atrophy involving the left temporalis, masseter, medial, and lateral pterygoid muscles, along with subtle contralateral fatty infiltration of the right medial pterygoid muscle, suggesting possible early subclinical involvement. Fat-suppressed and high-resolution

Retromandibular Anteroparotid Versus Transparotid Approach for Subcondylar Mandibular Fractures: A Retrospective Comparative Study of 80 Cases.

Background/Objectives: Subcondylar mandibular fractures represent a challenging subset of maxillofacial trauma due to their proximity to the temporomandibular joint and the facial nerve. The retromandibular approach can be performed through either an anteroparotid or a transparotid route, but comparative clinical data remain limited. This study aimed to evaluate clinical outcomes, complication profiles, and operative parameters associated with the retromandibular anteroparotid versus transparotid approach for open reduction and internal fixation (ORIF) of subcondylar fractures. Methods: A retr

Surgical treatment of displaced intraarticular pediatric condyle fractures.

AIM: Conservative treatment of pediatric condyle fractures remains the generally accepted norm even today. However, longterm results of conservatively treated displaced pediatric condyle fractures often display adverse effects on the affected temporomandibular joint (TMJ), and also on the growing facial skeleton. We analyzed results of surgical treatment in pediatric intraarticular condyle fractures. PATIENTS AND METHODS: Children with displaced intraarticular condyle fractures were treated surgically, with open reduction and internal fixation (ORIF). The articular disc was addressed in each

Congenital Temporomandibular Joint Ankylosis: Investigating Potential Genetic Etiologies with Whole Exome Sequencing.

Background: Ankylosis of the temporomandibular joint (TMJ) is a rare developmental disorder that involves fibrous or bony fusion within the joint. It is a severe structural and functional disorder. Typically, the phenotype manifests as joint immobilization and results in facial deformity and trismus. To date, ankylosis is rarely diagnosed as congenital and its occurrence mechanism has not been thoroughly understood. We observed a female patient who as a newborn showed slight facial asymmetry and impaired mandibular retraction. In addition, non-uniform occlusal fissures were noted; the lower pa

Skeletal correction in craniofacial microsomia: a case series and algorithm for management.

Craniofacial microsomia is the second most common congenital anomaly of the head and neck region. The associated facial asymmetry due to mandibular hypoplasia, whether unilateral or bilateral, is a prominent feature of the disorder. Surgical management of the skeletal deformity is dependent on the grade of severity of the affected sides. Prepubertal interventions in patients with Kaban type IIB or III include condyle-ramus reconstructions as a prelude to final orthognathic correction of the facial asymmetry and occlusion. This retrospective study of the skeletal management of 29 patients with

Significance of medio-lateral depth (MLD) in TMJ ankylosis surgery-A newer severity index (VSI) based surgical outcomes study.

INTRODUCTION: Temporomandibular joint (TMJ) ankylosis is a debilitating disorder caused by fibrous or osseous fusion of the joint components, significantly restricting mandibular mobility. As TMJ ankylosis disrupts normal facial growth, resulting in persistent functional and aesthetic challenges. The Medio-Lateral Depth (MLD) of the bony ankylotic mass is a vital yet underexplored factor influencing the complexity of surgery. Increased MLD and density of the ankylotic mass significantly elevate surgical complexity and the risk of postoperative complications. This study aims to assess the role
