The femur length/head circumference relation in obstetric sonography.

The relationship of sonographic femur length to biparietal diameter (BPD) in utero may be useful in detecting short-limb dwarfism, hydrocephaly, microcephaly, and measurement errors. Because BPD can be adversely affected by head shape changes (e.g., dolichocephaly), falsely high or low values of femur length/BPD could result. Because head circumference is less affected by head shape changes than is BPD, it should be a better standard of head size against which to judge limb length. The authors report normal values for the relationship between femur length and head circumference, based on a cro

FG syndrome in a premature male.

A 30-week premature male infant is presented with dolichocephaly, frontal bossing, down-slanting palpebral fissures, hypertelorism, long philtrum, micrognathia, cleft palate, and imperforate anus. He is the fifth patient to be presented with FG syndrome and sensorineural deafness. The patient's syndromic manifestations became more obvious during an inpatient observation period of 3 months.

Duplication 10p in a girl due to a maternal translocation t(10;14) (p11:p12).

We present a dup (10p) due to a t(10;14) (p11;p12)mat with a malformation syndrome in a girl. The analysis of 37 published cases shows that 31 patients (16 males; 15 females) had either a mother or a father carrying a balanced translocation; one case was due to a paternal and another due to a maternal pericentric inversion; two cases were due to de novo translocations; one case had a partial duplication of 10p; and one case had a supernumerary ring chromosome composed of 10p material. The phenotypic spectrum of the condition was analyzed. It is a specific multiple congenital anomalies/mental r

Estimation of fetal weight by ultrasound.

Birth weight (BW) and log10 birth weight (LBW) are expressed as linear, multilinear, parabolic, and polynomial regression functions of the abdominal circumference (AC), and combinations of fetal index measurements, namely, AC and head circumference (HC); biparietal diameter (BPD) and AC; and BPD, HC, and AC. The relationship of somatic weight and log10 somatic weight to AC was similarly determined. The analysis generated 20 equations whose accuracy in providing BW estimates is compared. Several equations provide BW estimates whose mean percentage deviations from measured BWs do not differ sign

An oculocerebral hypopigmentation syndrome.

An oculocerebral hypopigmentation syndrome consisting of growth retardation, dolichocephaly, cataracts, high arched palate, small, widely spaced teeth, generalized hypopigmentation, psychomotor retardation, progressive neurological manifestations and hypochromic anemia is described in sibs. The finding of parental consanguinity supports autosomal recessive inheritance. The syndrome resembles the Cross syndrome (1,2).

Duplication 9q34 syndrome.

Phenotypic, karyotypic, and developmental homology between affected children of carriers of an inverted insertion (9) (q22.1q34.3q34.1) led to recognition of a new chromosome syndrome: dup 9q34. Individuals with dup 9q34 have slight psychomotor retardation, understand simple directions, and acquire a limited vocabulary. In childhood, many are hyperactive. Clinical features include low birth weight, normal birth length, and initial poor feeding and thriving. Musculo-skeletal systems are affected: there are joint contractures, long thin limbs, and striking arachnodactyly. There is abnormal impla

The breech head and its ultrasound significance.

Breech babies were shown to have a smaller mean biparietal diameter (BPD) neonatally compared with that of a matched group of vertex babies. This was due to a mild skull deformation which occurred in at least one-third of 100 consecutive term breech babies examined. Features of this skull deformation (dolichocephaly, a prominent occiput with a suboccipital shelf, an elongated face and a parallel-sided head) constitute the 'breech head'. The caliper-determined occipitofrontal/biparietal diameter ratio (OFD/BPD) in these newborn infants was consistently above 1 . 30. This ratio, when calculated

Two chromosomal syndromes in the same family: monosomy and trisomy for part of the short arm of chromosome 10.

A grandfather with balanced translocation t(1:10) gave rise to three possible combinations involving chromosome 10: balanced translocation and trisomy for part of the short arm 10p13 leads to pter in the second generation, and mono- and trisomy 10p13 leads to pter in the third generation. The clinical signs and symptoms of the present case with monosomy 10p13 leads to pter are compared with those of 9 earlier reported cases with a deleted 10p. Together they represent a clinically recognizable syndrome with antimongoloid eye slant, ptosis, epicanthus, high arched or cleft palate, flat nasal bri

Estimating fetal age: effect of head shape on BPD.

Several recent obstetrical sonographic examinations in this department demonstrated that variations in the shape of the fetal skull (e.g., dolichocephaly, brachycephaly) may adversely affect the accuracy of the biparietal diameter (BPD) measurement in estimating fetal age. In each case the cephalic index of the fetal skull (short axis/long axis x 100) was in either the dolichocephalic or brachycephalic range based on established postnatal criteria. Consequently, normal values were determined (mean, 78.3) for the cephalic index in utero based on 316 obstetrical sonographic studies performed at

Partial trisomy 9q due to maternal 9/17 translocation.

A patient with partial trisomy 9q due to material 9/17 translocation was studied and compared with four previously reported cases. The similarity of their clinical features allowed us to delineate a distinct clinical syndrome, which is characterized by psychomotor retardation, dolichocephaly, beaked nose, deep-seated eyes, and long fingers and toes. There is an overlap between some of the features of this syndrome and those of trisomy 9.

Hajdu-Cheney syndrome.

Hadjdu-Cheney syndrome is a rare type of syndrome characterized by acro-osteolysis, dolichocephaly with multiple Wormian bones, absence of frontal sinuses and joint laxity. A case of this syndrome is presented. A histological study of the osteolytic lesion revealed destruction characterized by microfractures with a poor reparative process. It is postulated that an abnormality of osteoblast or osteoid function is the pathogenesis of this syndrome.

Partial trisomy 10p in two generations.

Two cases of partial 10p trisomy due to a t(10;20)(p12;p12) in two generations of a family are presented. Analysis of 20 known cases of such aberrations confirmed the opinion of Schleiermacher et al. (1974) that partial trisomy 10p is a distinct clinically recognizable entity. The most important diagnostic features of this syndrome are dolichocephaly, prominent forehead, wide open sutures and fontanelles, broad root of the nose, cleft lip and palate, clubfoot, and cystic changes in kidneys.

Partial trisomy-5p.

Two sibs with partial trisomy-5p are reported. Their father is the carrier of a balanced translocation 46,XY,t(4q+;5p-). Twelve cases of partial trisomy-5p--including our two patients--have been reported. The most common abnormalities found were mental retardation, short stature, dolichocephaly, prominent nasal bridge, prognathism, seizures, hypotonia, ear abnormalities, increased ulnar loops on the fingertips, and cryptorchidism in affected males.

[Pathological anatomy of Edwards' syndrome (trisomy 18)].

Data of the literature (200 section observations) and those of 10 authors' own observations on the syndrome of trisomy with relation to chromosome 18 are presented. The pathologoanatomic diagnosis of Edwards' syndrome can be established without an investigation of chromosomes on the basis of a complex of congenital defects present in the newborn. The main of them are: prenatal hypoplasia, dolichocephaly, microgenia, concha auriculae drawn in the horizontal plane, flexor position of bones, shortness and thickness of the great toe and protruding heel, thickness and distortion of convolution of t

Dolichocephaly--a source of error in serial cephalometry.

Two patients are presented in whom ultrasonic cephalometry suggested intrauterine fetal growth retardation. The respective newborn infants were normally grown and had dolichocephaly with a normal head circumference. Dolichocephaly should be considered when cephalometry alone suggests a diagnosis of placental insufficiency.

The acro-osteolysis syndrome: Morphologic and biochemical studies.

The acro-osteolysis syndrome consists of dissolution of terminal phalanges of the hands and feet, dolichocephaly with multiple wormian bones, delayed closure of cranial sutures, absence of frontal sinuses, a prominent occipital ridge, skeletal demineralization, vertebral and extremity fractures, joint laxity, and coarse hair. Studies of bone morphology reveal diminished bone density and bone formation. Osteoblasts have widely dilated smooth endoplasmic reticulum. It is postulated that an abnormality of a structural protein is the pathogenic basis of this disease.
