Cranial Bone Density and Scalp Perfusion in Positional Plagiocephaly: Quantitative Assessment Using Low-Dose Computed Tomography.

Positional plagiocephaly is a common cranial deformity in infancy, but quantitative evaluation of cranial bone characteristics remains limited. This study aimed to assess cranial bone density and scalp perfusion in infants with positional plagiocephaly and to evaluate changes following helmet therapy using low-dose computed tomography (CT). A total of 61 infants with moderate to severe positional plagiocephaly who underwent CT for differentiation from craniosynostosis were retrospectively analyzed. Cranial bone density was quantified using Hounsfield units at standardized points around the lam

Cranial evidence for human-like helpless infancy in Homo erectus and Homo floresiensis.

Unlike other catarrhine primates, modern humans deliver a helpless newborn who requires prolonged extensive care. Such helpless infancy is associated with fetus-like, rapid postnatal brain growth, which is essential to produce the large human brain. In the debate about its evolution, a key question has been whether it emerged in Homo erectus, whose brain sizes were approximately 20-40% smaller than Homo sapiens. We here show that a type of asymmetric cranial deformity called deformational plagiocephaly can serve as a direct indicator of helpless infancy, regardless of the developmental age of

Computed Tomography-Based Evaluation of Positional Cranial Deformities in Emergency Department Patients Aged 15 to 39 Years in Japan.

Positional cranial deformity due to sustained external pressure during infancy frequently improves in early childhood; however, its prevalence in adulthood, particularly in East Asian populations, remains unclear. The authors performed a retrospective cross-sectional analysis of thin-slice cranial computed tomography (CT) images from 242 Japanese individuals aged 15 to 39 years who underwent cranial CT at a single tertiary hospital from 2014 to 2023 and had no intracranial abnormalities. Using the cranial vault asymmetry index (CVAI) and cephalic index (CI), cranial morphology was measured on

Deep learning-based craniosynostosis classification via suture segmentation and mask-weighted classification.

INTRODUCTION: Early diagnosis of craniosynostosis (CSO) is critical to preventing neurological complications, yet skull X-ray interpretation remains subjective, and existing deep learning models often rely on secondary cranial deformations rather than the primary pathology. METHODS: To address this limitation, we propose an Integrated Suture Segmentation and Classification Pipeline that explicitly learns suture information to enhance anatomical validity and diagnostic accuracy. We constructed a balanced dataset of 1,088 skull X-ray images from 368 unique patients and developed a segmentation

Impact of Age on Success of 3D-Printed Cranial Remolding Orthoses for Deformational Head Shapes.

Background/Objectives: Previous studies regarding the efficacy of cranial remolding orthoses (CROs) have shown that earlier treatment initiation is associated with more favorable outcomes, but there are few studies utilizing 3D-printed CROs. This study evaluated the impact of treatment initiation age on the success of 3D-printed CROs in terms of morphological measurement changes and caregiver satisfaction. Methods: A total of 1054 infants who initiated treatment with a 3D-printed CRO during 2023 across six pediatric clinics in Canada were included in this retrospective cohort study. Participan

Smartphone 3D Scanning Technology and 3D Semi-Synthetic Data for Processing Infant Head Deformities Using Artificial Intelligence.

Background: Early assessment of cranial deformities in newborns, such as plagiocephaly, brachycephaly, dolichocephaly, turricephaly, and trigonocephaly, requires precise and non-invasive methods. Methodology: This study presents a methodology using a 3D scanning smartphone application to capture three-dimensional head point clouds. A total of 60 3D point cloud cases were classified according to six classes of head deformities. These 60 real 3D point clouds were expanded to 3600 semi-synthetic point clouds via controlled geometric transformations simulating realistic cranial variations. A total

Association between cranial morphology and dysgnathias in adolescents and adults: A prospective case-control study.

BACKGROUND: Cranial deformities in infancy, such as positional plagiocephaly and brachycephaly, have been suggested to influence the development of jaw malpositions. However, the long-term relationship between cranial shape and dysgnathia remains unclear. OBJECTIVE: This prospective case-control study investigates whether differences in cranial morphology, measured by the Cranial Index (CI), Cranial Vault Asymmetry Index (CVAI), Diagonal Difference (DD), and ear axis offset (EO), are associated with malocclusions classified according to the Angle system in adolescents and adults. MATERIALS A

Isolated frontosphenoidal suture synostoses: report of two cases and literature review.

INTRODUCTION: Anterior plagiocephaly is most commonly attributed to unilateral coronal synostosis. However, isolated premature fusion of the frontosphenoidal suture (FSC) represents a rare and often misdiagnosed cause due to its overlapping clinical presentation. Accurate diagnosis is essential, as treatment strategies and prognoses differ significantly. OBJECTIVES: To present a comprehensive literature review of FSC and to report two novel clinical cases-one managed with open fronto-orbital advancement and the other with endoscopic suturectomy. METHODS: A systematic review of reported FSC c

Open surgery in lambdoid-related plagiocephaly: detailed techniques, risks, and long-term results.

Lambdoid craniosynostosis is the rarest form among monosutural ones, with a prevalence ranging between 1 and 4% of all craniosynostosis. Open-strip craniectomy used to be the common treatment method; however, a better understanding of the pathology as well as a more careful analysis of outcomes and complications led to a shift towards other techniques such as open cranial vault reconstruction, device-mediated distraction, and endoscopic procedures. The present paper will deal with open surgical options, namely open cranial vault reconstruction (OCVR) and device-assisted procedures (DAP). In pa

Severe Plagiocephaly Due to Isolated Left Frontosphenoidal Craniosynostosis: An Exceptional Presentation.

Craniosynostosis is a condition characterized by premature fusion of cranial sutures, leading to abnormal head shapes and potential developmental and neurological complications. Although craniosynostosis typically involves major cranial sutures, isolated frontosphenoidal suture fusion represents an exceptionally rare entity. In this case report, we present a 2-year-old girl with a rare occurrence of early closure of the left frontosphenoidal suture, resulting in significant left-sided plagiocephaly. After frontal-orbital advancement surgery with barrel-stave osteotomies, the patient experience

Unilateral Versus Bilateral Fronto-Orbital Advancement in Non-Syndromic Unicoronal Craniosynostosis: A Systematic Review and Pooled Data Analysis.

BACKGROUND: Nonsyndromic unicoronal craniosynostosis (NS-UCS) is characterized by asymmetric craniofacial deformity and remains one of the most challenging entities in craniofacial surgery. Fronto-orbital advancement (FOAR) may be performed either unilaterally or bilaterally, yet the optimal approach remains controversial. The present study aims to compare surgical outcomes, complication profiles, and long-term stability between unilateral and bilateral FOAR to clarify selection criteria for surgical technique. METHODS: A systematic review was conducted using the PubMed, Web of Science, and C

Osteopathic Manipulative Treatment for the Breastfeeding Dyad: A Scoping Review.

BACKGROUND: Human milk is widely recognized as the optimal source of infant nutrition. While 83.9% of mothers initiate breastfeeding, only 46.3% exclusively breastfeed to 3 months. Common barriers include latch and suck difficulties, painful feeding, and breast infections or engorgement. Osteopathic manipulation, including osteopathic manipulative treatment (OMT) and therapy (OMTh), has been proposed as supportive intervention for breastfeeding challenges. RESEARCH AIM: This scoping review examines the current literature on OMT/OMTh in the breastfeeding dyad and identifies gaps to inform futu

Integrating Osteopathic Care Into Treatment of the Breastfeeding Dyad: Anatomic and Physiologic Considerations.

Care of the breastfeeding dyad involves an interprofessional and multidisciplinary team that may include lactation consultants, physicians, bodyworkers, and more. One often underutilized resource in the care of both breastfeeding parents and their infants is osteopathic physicians (Doctors of Osteopathic Medicine or DOs). DOs are full-scope physicians who have additional training in osteopathic manipulative treatment (OMT). Osteopathic manipulative treatment is a form of manual medicine that can be utilized to optimize the breastfeeding relationship. It can be useful to address latch challenge

Endoscopic surgery in lambdoid-related plagiocephaly: detailed technique, what risks, and long-term results.

Endoscopic suturectomy represents an early intervention for craniosynostosis. For unilateral lambdoid craniosynostosis, this study reviews the existing literatures and summarizes a single-center experience with endoscopic management of lambdoid-related plagiocephaly. Our findings indicate that endoscopic surgery combined with orthotic helmet therapy can effectively improve cranial deformities, torticollis, and other associated abnormalities. However, the correction of compensatory deformities and the long-term outcomes related to Chiari malformation and other neurocognitive functions still req
