Treatment of plagiocephaly in infancy.

Plagiocephaly is commonly attributed to the synostosis of a single coronal suture. Esthetic deformities associated with plagiocephaly range from mild to severe. Because the deformities are progressive, and more difficult to correct in older children and adults, early diagnosis and correction are important. Linear craniectomies, previously widely practiced, have not yielded good results. Current craniofacial techniques allow extensive orbitocranial reconstruction, which can be done safely in infants and in older children as well. These techniques have produced excellent cosmetic results and are

Genetic aspects of early childhood scoliosis.

Eighty-seven families with early onset scoliosis were evaluated. These were divided into 3 groups: resolving infantile idiopathic scoliosis (15 families), progressive infantile idiopathic scoliosis (21 families), and congenital scoliosis due to vertebral malformations (51 families). The children with congenital scoliosis were subdivided into those who had closed neural arch defects (19 families) and those who did not (32 families). Resolving infantile idiopathic scoliosis was usually associated with plagiocephaly, and both deformations tended to show spontaneous recovery. These children were o

[Ophthalmologic complications of craniofacial stenoses. Apropos of 244 cases].

The authors present a review of the most commonly observed ophthalmological troubles in 244 cases of children affected by cranio-facio stenoses and hospitalized for a first examination. The fundus examination shows that nearly all types of cranio-facio stenoses can be complicated by intracranial hypertension with potential damage on the optic nerve. The hyper and hypotelorisms, particularly frequent, carry along aesthetic prejudice and effect on the ocular motility. In the most severe forms of the diseases (Alport), pseudo-exophthalmy by retrusion of the face can justify preventive surgical pr

Computed axial tomographic assessment of cranioorbital anatomy in unilateral clefts.

Anatomy of the cranioorbital region in the unoperated infant with cleft lip/palate, is not well known. In this study, computed axial tomography was performed in 7 infants with unoperated unilateral cleft lip combined with cleft palate but no recognized craniofacial syndromes, and on 5 age-matched control infants. Significant differences were found between the infants with cleft lip/palate and the normal infants. There was more plagiocephaly in the cleft group, and the flattened forehead on the involved side was associated with less facial projection. The angles of the petrous portions of the t

Unilateral epicanthal fold: diagnostic significance.

The dysmorphologic evaluations of 56 children with torticollis and plagiocephaly were reviewed. Half of the children were referred because of the cranial asymmetry, and the remainder because of other structural defects or developmental delay. A unilateral epicanthal fold was present in 22 of 54 fully documented cases (41%). In 78% of these, the fold was ipsilateral to the area of occipital flatness. Because most infants are examined in an en face position, the presence of a unilateral epicanthal fold should prompt further investigation for a readily treatable torticollis.

Skeletal and functional craniofacial adaptations in plagiocephaly.

The present report aims to contribute to our understanding of craniofacial development in plagiocephaly. A previously unreported dry skull with plagiocephaly and two clinical cases with unoperated plagiocephaly are presented. The clinical cases were followed longitudinally with roentgencephalometry in lateral, frontal, and axial projections. In addition, in one of the cases, electromyographic analysis of the temporal, masseter, sternomastoid, and trapezius muscles was carried out. The dry skull revealed premature closure of the sphenofrontal suture in addition to the coronal suture. Furthermor

[Treatment of fronto-orbital malformations associated with craniostenosis].

From 1977 to 1982, 13 children affected by craniostenosis involving the supraorbital ridge were treated by surgery and subsequently followed up. Six cases of plagiocephaly, one of brachycephaly, three of Apert's syndrome, and three cases of Crouzon's disease were operated on, employing in seven cases classical lineal craniectomies, an in six other, craniotomies with advancement and remodelling. The results obtained are discussed in terms of functional and cosmetic improvement, the therapeutic criterion being in terms corresponding with the gravity of the malformation and the age at which the s

The hip in the moulded baby syndrome.

The moulded baby syndrome comprises: head moulding (plagiocephaly); pelvic obliquity with unilateral loss of hip abduction in flexion; and occasionally scoliosis, torticollis and bat ears. The hips, however, are radiologically normal and do not require the treatment used in the management of congenital dislocation or dysplasia. A review of 67 hips confirms this finding.

Cephalometric radiography and computed tomography in infants undergoing major craniofacial surgery--a comparison.

Craniofacial growth after surgery in children is not completely understood. We have therefore formed a programme for pre- and postoperative studies with both roentgencephalometric- and computed tomographic investigations. This regimen results in overlapping information. Thus, an assessment of the advantages and disadvantages of the two methods was made and exemplified in four patients. In our experience both cephalometry and CT are essential for the diagnosis, surgical planning and follow-up of surgically treated children with craniofacial anomalies. To minimize the radiation doses and the dia

[Early treatment of orbital asymmetry in plagiocephaly].

Plagiocephaly results from premature unilateral closure of the coronal suture with subsequent frontal and orbital asymmetry and inequality of ocular levels requiring complex osteotomies if treatment is delayed. From 1976, early treatment from the age of approximately 6 months has been instituted by means of bilateral frontal remodelling. Results in 25 neonates in whom the operation was performed demonstrated that not only was the frontal correction maintained with growth, but that principally the facial asymmetry was reduced and later orbital surgery was never necessary. More complicated probl

Cranial defects in the Goldenhar syndrome.

Four patients are presented with the Goldenhar syndrome (GS) and cranial defects consisting of plagiocephaly, microcephaly, skull defects, or intracranial dermoid cysts. Twelve cases from the literature add hydrocephalus, encephalocele, and arhinencephaly to a growing list of brain anomalies in GS. As a group, these patients emphasize the variability of GS and the increased risk for developmental retardation with multiple, severe, or unusual manifestations. The temporal relation of proposed teratogenic events in GS provides an opportunity to reconstruct biological relationships within the 3-5-

Plagiocephaly causing superior oblique deficiency and ocular torticollis. A new clinical entity.

Unilateral coronal suture stenosis provokes a shortening of the orbital roof of one side. It is argued that the effectiveness of the superior oblique muscle is reduced because of reduced length of its unreflected part and because of the increased angle between the reflected part of the tendon and ocular axis. This condition causes a clinical picture of superior oblique palsy, with ocular torticollis. We report four cases of this newly described clinical entity with various degrees of involvement of ocular motility.

[Compatibility between primary cranial surgery and secondary cranio-facial surgery (author's transl)].

Craniofacial surgery can radically correct the most severe dysmorphias of fronto nasal encephaloceles, trigonocephaly, plagiocephaly, telorbitism, Crouzon, Apert, Chotzen... Its procedures can make to normal their general morphology, the projection of the mid face and maxilla, centering and inter relationship of the orbital cavities. It can also correct or improve many dysfunctions such as : naso-pharyngeal atresia, dental malocclusion, exorbitism, exotropia and, consequently, strabismus. According to syndromes and degree of severity, craniofacial surgery is undertaken between 2 to 10 years of

Craniofacial growth in premature craniofacial synostosis.

Serial cephalometric radiographs of 55 patients with different types of premature craniofacial synostoses were analysed. The aim of the study was to answer the following four questions in an effort to provide a better rational for classification, treatment, and possibly add insight into the mechanisms controlling craniofacial growth. (1) Is the shape of the skull syndrome specific. (2) How does the craniofacial complex grow. (3) What is the effect of craniectomy on the growth of the cranium. (4) Can roentgencephalometry provide a rational basis for planning reconstructive surgery. The results

Craniofacial growth in plagiocephaly and Crouzon syndrome.

Very little information is available in the literature regarding the morphology and growth of the cranial base, the orbits and the jaws, in patients with premature craniosynostosis. The present report deals with the craniofacial morphology and growth in plagiocephaly and Crouzon syndrome. For each anomaly a dry skull was compared with a clinical case followed longitudinally with roentgencephalometry. Metallic implants in the jaws were employed in one case. Cranial base involvement was demonstrated for both types of premature craniosynostosis. In Crouzon syndrome the cartilagenous cranial base
