The external Haller index as a new, reliable, and accurate evaluation of pectus excavatum associating clinical assessment, 3-D surface scanning measurement, and MRI.

INTRODUCTION: Pectus excavatum (PE) is the most common chest wall deformity. The traditional Haller index (HI), as measured using computed tomography (CT), guides surgical decision-making. Radiation-free magnetic resonance imaging (MRI) and less expensive alternatives, such as clinical thorax calipers or 3-D imaging, can measure the external Haller index (EHI). MATERIALS AND METHODS: PE patients and controls with normal thoracic anatomy (other diagnoses) were enrolled prospectively (2015-2021). All participants underwent cardiac MRI as part of routine clinical practice. PE patients also under

Experiences and health-related quality of life following minimally invasive surgical repair for pectus excavatum - a mixed methods study.

BACKGROUND: Pectus excavatum is the most common congenital chest wall deformity. Minimally invasive repair for pectus excavatum (MIRPE) has demonstrated positive and psychsocial outcomes. The aim of this study was to gain a comprehensive understanding of health-related quality of life (HRQoL) in patients undergoing MIRPE. METHODS: This prospective cohort study applied a modified mixed methods convergent design and included 19 individuals scheduled for MIRPE with follow-up 18 months postoperatively. The Nuss Questionnaire modified for Adults 10-item version (NQ-mA-10) and RAND-36 were administ

Complete sternal cleft with pectus excavatum associated with tetralogy of Fallot.

Complete sternal cleft with pectus excavatum (PE) is a very rare congenital midline defect of the sternum. Most of the published reports describe this entity in isolation without associated intracardiac defects. We report a case of an 11-year-old child with a complete cleft sternum with PE and tetralogy of Fallot (TOF), who presented with cyanotic spells. The child underwent total correction for TOF, followed by chest wall reconstruction in the same setting.

A randomized control trial for assessing virtual reality for perioperative anxiolysis and postoperative pain modulation in adolescents undergoing MIRPE.

BACKGROUND: Pectus deformities, including pectus excavatum, are associated with psychosocial burden and in severe cases impaired cardiorespiratory function. Minimally invasive repair of pectus excavatum (MIRPE) is the standard corrective surgery; however, the postoperative pain can be significant. Patients' perioperative anxiety can be severe as they are cognitively aware of the awaiting surgical challenges but often lack effective coping strategies. Virtual reality (VR) is a non-invasive, non-pharmacological modality that has shown promise in reducing anxiety and modulate pain in the pediatri

Myotonometry Assessment in Children and Adolescents with Pectus Excavatum Included in a Physical Exercise Program.

Context/Objectives: Pectus excavatum (PE), the most common anterior chest wall deformity in children and adolescents, impacts posture and is frequently associated with axial deviations due to biomechanical alterations of the spine and the properties of the involved musculature. Methods: We assessed 35 patients with PE with a Haller index below 3.25, aged between 5 and 17 years, who completed a three months specialized physical exercise program after proper training and instruction by a specialist. All patients were assessed before starting the exercise program and at the end of the treatment.

Arteriopathies: Too big to be true.

Arteriopathies in early childhood present significant diagnostic challenges due to phenotypic overlap between Marfan syndrome, Loeys-Dietz syndrome, Ehlers-Danlos syndrome, and hereditary cutis laxa. We report a 2-year-old male, born of consanguineous parentage, evaluated for persistent cough and hypertension. Clinical features included coarse facies, pectus excavatum, and mild hepatomegaly. Echocardiography revealed a massively dilated ascending aorta (56 mm), aortic annular dilatation, moderate aortic insufficiency, and severe post-subclavian coarctation with marked post-stenotic dilatation.

The Seesaw technique: A novel hybrid single-bar approach for asymmetric pectus excavatum correction.

Asymmetric pectus excavatum (PE) is surgically challenging due to combined depressive and protrusive components. We describe a novel hybrid "Seesaw" technique that integrates the Nuss and Abramson principles using a single pectus bar. In six patients with asymmetric PE, the bar followed an extra-thoracic path on the protruded side to provide compression and an intra-thoracic path on the depressed side to provide elevation through a single-incision access. Early outcomes were favorable, with excellent cosmetic correction, high patient satisfaction, and one case of bar rotation. The Seesaw techn

Design of a Patient-Specific 3D-Printed Splint for Surgical Correction of Pectus Excavatum in a Kitten.

Pectus excavatum is a rare congenital deformation of the chest wall. Associated clinical signs depend on the severity of the deformity of the sternum and costochondral cartilages. After thoracic radiographic studies, a patient-specific plastic external splint was designed based on a previously made plaster cast of the chest of the kitten. The splint was designed using 3D scan equipment and data processing to obtain the computer-aided model that was then used to manufacture the splint using 3D printing technology. The surgical application of the customized 3D-printed external splint for the cor

Efficacy and Safety of Vacuum Bell Therapy in Pectus Excavatum: A Systematic Review of Clinical Outcomes.

INTRODUCTION: Pectus excavatum (PE) is the most common congenital chest wall deformity. The vacuum bell (VB) is a noninvasive alternative to surgical correction, increasingly used in pediatric and adolescent patients. However, evidence on its efficacy, safety, and predictors of success remains fragmented. OBJECTIVE: To systematically review and synthesize available evidence on the efficacy, safety, and clinical outcomes of VB therapy in patients with PE. METHODS: A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 g

Cryoanalgesia in Thoracic Surgery.

Cryoanalgesia is a technique for reducing postoperative pain that involves freezing peripheral nerves while preserving their ability to regain function over time. The technique has been used for decades, and randomized trials have shown a reduction in pain and opioid use in some patients undergoing thoracotomy and pectus excavatum repair. In these trials, outcomes were highly dependent on the control groups. Randomized studies of patients undergoing minimally invasive thoracic surgery have failed to show the same results. Furthermore, several studies have reported chronic pain and neuropathic-

Clinical outcomes of sub-millisievert CT evaluation of congenital chest wall deformity using photon counting CT: a new standard of care?

BACKGROUND: Pectus excavatum is the most common congenital chest wall deformity and is typically treated surgically. Disease severity is commonly assessed using the CT-derived Haller Index (HI), creating opportunities to reduce radiation exposure in pediatric patients. Photon-counting detector CT (PCD-CT) enables substantial dose reduction; however, the clinical impact of sub-millisievert (sub-mSv) imaging has not been well established. OBJECTIVE: To evaluate the impact of sub-millisievert (sub-mSv) PCD-CT scans on clinical outcomes in the preoperative assessment of congenital pectus deformit

Cannula-assisted sternal displacement for VATS correction of pectus excavatum in three cats.

CASE SERIES SUMMARY: Three skeletally immature cats were treated for pectus excavatum using a modified video-assisted thoracoscopic surgery (VATS) technique that involved using a thoracoscopic cannula to intraoperatively ventrally displace affected sternebrae, thereby assisting the placement of circumsternal sutures while simultaneously protecting underlying viscera. All cases recovered well from their procedure, and the described technique was successful in allowing good radiographic improvement of pectus excavatum in all cases. No intraoperative complications were encountered and all cases w
