Pectus deformities in children: a pictorial review.

Pediatric chest wall deformities, such as pectus excavatum (PE), carinatum (PC), and arcuatum (PA), may result in psychosocial and cardiorespiratory morbidity. Pediatric radiologists play a key role in imaging-based characterization, preoperative assessment, and the evaluation of postoperative complications. This pictorial essay highlights key imaging characteristics, indices for surgical planning, and common postoperative findings for pectus deformities.

Determinants of Successful Pectus Carinatum Bracing: A Narrative Review.

Pectus carinatum (PC) is a chest wall deformity characterized by the anterior projection of the sternum that typically manifests in adolescence and may result in notable psychosocial morbidity despite insubstantial physiological signs. Historically considered less common than pectus excavatum, recent trends suggest that PC may be underdiagnosed, with increasing recognition and referrals to specialized pectus clinics. The main objective of this review is to provide a comprehensive understanding of the role of bracing, particularly the dynamic compression system, in the non-surgical management o

Wang technique: a novel minimally invasive surgical fixation technique for the treatment of pectus excavatum.

BACKGROUND: Pectus excavatum (PE) is the most common thoracic deformity. Currently, the modified Nuss procedure with steel bar implantation is the primary surgical treatment for PE. Traditionally, the bar is fixed using a stabilizer; however, this method is relatively complex and associated with complications. We designed a novel minimally invasive surgical fixation technique (the Wang technique) for the treatment of PE. This study assessed its efficacy and technical characteristics to provide valuable insights into the surgical treatment of PE. METHODS: The clinical data of patients with mod

Multicenter Evaluation of Opioid Prescribing After Minimally Invasive Repair of Pectus Excavatum Before The 2024 American Academy of Pediatrics Guideline: Room to Improve.

INTRODUCTION: To better understand baseline prescribing patterns before the release of the American Academy of Pediatrics (AAP)'s 2024 clinical guidelines on opioid prescribing, we evaluated the degree to which discharge pain management practices following minimally invasive repair of pectus excavatum (MIRPE) aligned with AAP's recommendations across 10 children's hospitals. METHODS: Patients aged ≤21 y who underwent MIRPE (January 2022 to October 2023) were retrospectively reviewed. Those who underwent MIRPE for recurrent disease or underwent concurrent procedure(s) at the time of repair wer

Physiological Factors of Exercise Intolerance in Patients With Pectus Excavatum: A Retrospective, Single-Center Study.

INTRODUCTION: Pectus excavatum (PE) causes a sunken thoracic wall, which is associated with adolescent exercise intolerance. Since the underlying physiological factors remain uncertain, this retrospective chart review addressed this knowledge gap by reviewing cardiopulmonary exercise tests (CPETs) and identifying physiological factors of exercise intolerance. METHODS: With the Nemours Institutional Review Board's approval (IRB approval number: STUDY00000806), this study enrolled patients with PE who underwent CPET and chest magnetic resonance imaging (MRI) for Haller index at the Nemours Chil

Virtual Reality for the Management of Postoperative Pain and Anxiety in Children and Adolescents Undergoing Nuss Repair of Pectus Excavatum: Randomized Controlled Trial.

BACKGROUND: Virtual reality (VR) is a novel technology with implications for pain and sensory processing. VR may serve as a novel, scalable method to deliver clinically validated therapy for pain management as an alternative or adjunct to opioids for acute pain. Given that psychological factors and pain perception are both components of postoperative pain, it may also be beneficial to incorporate modalities that decrease anxiety, such as active relaxation and guided meditation with VR. Unfortunately, these therapies are not widely available due to multiple barriers. VR has the potential to del

Noninferiority of Ultrasound-Guided Modified Intercostal Block to Traditional Approach for Analgesia After Minimally Invasive Repair of Pectus Excavatum in Children: A Randomized Trial.

BACKGROUND: For pediatric pectus excavatum, the standard treatment is minimally invasive repair of pectus excavatum (MIRPE). A major challenge, however, is the severe postoperative pain. Although ultrasound-guided intercostal nerve block (UINB) offers effective analgesia, the technique's complexity and associated safety concerns are significant barriers, deterring its routine use. Modified intercostal nerve block (MINB) is effective in adult thoracic surgery but unvalidated in pediatric MIRPE. OBJECTIVE: To evaluate MINB's noninferiority to UINB for postoperative analgesia and safety in child

High-Resolution Quantification of Two-Way Nanobody Synergy Using Automated Liquid Handling and Computational Modeling.

Evaluating single-domain antibody cooperativity is essential for developing potent, escape-resistant antiviral biologics. Here, we present a protocol that reproducibly quantifies functional synergy between neutralizing nanobody pairs in standardized viral infectivity assays. Controlled automated liquid handling prepares two-dimensional concentration matrices, minimizing pipetting variance and systematic error. Neutralization data are fitted using quantitative models that independently estimate potency, cooperativity, and efficacy to distinguish additive, synergistic, and antagonistic effects b

Lung transplant feasibility after lobar resection in a case of pectus excavatum and severe unilateral space loss.

An optimized approach to donor-recipient size matching is essential to expand transplant opportunities and improve post-operative outcomes. A commonly used and pragmatic metric is the donor-to-recipient predicted total lung capacity ratio. However, thoracic deformities or unilateral lung parenchymal disease may significantly distort true intrathoracic volume, leading to misleading predicted total lung capacity estimates and inappropriate donor lung selection. Clinical experience has shown increased morbidity and mortality in cases of severe size mismatch. To address such discrepancies, several

PurePectus bars: Initial surgical experience and early outcomes in 111 patients.

BACKGROUND: Bar rotation and migration remain common challenges in pectus excavatum repair using traditional Nuss bars. The PurePectus system (KLS Martin), a titanium-based construct, is designed to mitigate these issues by enhancing bar stability with bilateral connector plates. We report early outcomes following its implementation. METHODS: We conducted a retrospective review of patients who underwent pectus excavatum repair using the PurePectus system between September 2023 and January 2025. Demographic, operative, radiographic, hospitalization, and postoperative data were collected and an

Impact of Severe Uncorrected Pectus Excavatum on Outcomes After Aortic Surgery in Marfan Syndrome.

BACKGROUND: The prognostic impact of uncorrected severe pectus excavatum (PE) in patients with Marfan syndrome undergoing aortic surgery remains unclear. This study evaluated whether severe PE influences clinical outcomes after aortic surgery. METHODS: We retrospectively analyzed 253 patients with Marfan syndrome who underwent aortic surgery between 2005 and 2020. Patients were stratified into a nonsevere PE group (n = 191; including no PE and nonsevere PE) and a severe PE group (n = 62; Haller index >3.5). The primary end point was 10-year mortality. Secondary outcomes included in-hospital m

Effect of vacuum bell therapy on subcutaneous fat of the chest wall in patients with pectus excavatum.

BACKGROUND: Vacuum bell therapy is increasingly used as a nonoperative treatment for pectus excavatum (PE). However, whether this intervention induces subcutaneous fat thickening and the mechanisms underlying such changes remain uncertain. This study aimed to quantify vacuum bell-associated adipose tissue changes and clarify their clinical implications. METHODS: Patients with PE treated with vacuum bell therapy at our center between July 2022 and January 2024 underwent low-dose chest computed tomography at treatment initiation and after one year of therapy. Measured parameters included the Ha

Exploring the Use of Instant Messaging Groups in the Postoperative Period for Pectus Excavatum Patients.

As a National Centre for Pectus Surgery, our department receives referrals from across England. To strengthen perioperative support for patients undergoing the Nuss procedure in the RESTORE Trial (August 2024-May 2025), individual WhatsApp groups were introduced, reflecting growing evidence of the value of text-based communication in surgical care. Fifty-two groups were established, each averaging 6.7 members, including patients, relatives, secretarial, and research staff. These groups facilitated direct communication with the clinical team. All conversations were anonymized prior to analysis.

It's not about the dent: Anteroposterior insufficiency in pectus excavatum.

BACKGROUND: Pectus Excavatum (PEx) is an anterior chest wall deformity associated with sternal depression. Minimal invasive repair of PEx (MIRPE) and open pectus repair depend on anterior ribs to elevate the sternal defect. Many PEx patients, however, have inadequate anteroposterior (AP) distance to support a complete repair. The quantity and frequency of AP insufficiency (API) in a PEx surgical population is reported. METHODS: A retrospective analysis of 100 pectus excavatum (PEx) patients with a CT or MRI Haller Index >3.2 was performed. Demographics, Haller Index (HI), Correction Index (CI

Lung volume evaluation of modified Nuss procedure for pectus excavatum using three-dimensional computed tomography: A single-center retrospective analysis.

BACKGROUND: The Nuss procedure is widely used for Pectus excavatum (PE) repair. This study aimed to evaluate the efficacy of the modified Nuss procedure and identify prognostic factors by analyzing lung volume (LV) changes via three-dimensional computed tomography (3D-CT). METHODS: PE patients receiving the modified Nuss procedure between 2019 and 2022 were retrospectively reviewed. 3D-CT was performed preoperatively, postoperatively, and at 2-year postoperatively to measure the volume of each lung, lobe and segment. Chi-square test or Fisher's exact test was used for intergroup comparisons.

Diagnosis and Management of Pectus Excavatum in Primary Care.

Pectus is the most common congenital chest wall deformity, with pectus excavatum being the most prevalent type characterized by a sunken sternum. It has been proven that in severe cases this anomaly can result in cardiopulmonary compromise affecting patients' daily activity and ability to exercise. Despite the latest research, the deformity may still be viewed by some as a cosmetic issue that does not necessitate treatment. Since primary care physicians are often the first to evaluate these patients, a thorough understanding and comprehensive treatment approach for pectus is needed. Patients w

Comprehensive assessment of pectus excavatum: analyzing chest wall anatomy, cardiac rotation, and severity classification for enhanced diagnosis and treatment.

Pectus excavatum (PE) is the most common congenital chest wall deformity, characterized by a depression of the anterior chest wall which may compromise cardiac function and cause symptoms like exercise intolerance, chest pain, and shortness of breath. While diagnosis is often based on appearance, imaging-based metrics provide objective severity assessment. This study evaluated associations between PE severity indices and cardiac rotation angle in 37 adolescents generated two sex-specific anatomical models of severe cases for future diagnostic and treatment planning. Chest computed tomography (

Clinical evaluation of accelerated breath-held and free-breathing cine cardiac MRI using model-based deep learning reconstruction (SonicDL) in children and young adults.

BACKGROUND: Balanced steady-state free-precession (bSSFP) cine imaging is the clinical standard for ventricular function assessment but requires multiple breath-holds, which can be challenging for pediatric and young adult patients. Deep learning (DL)-accelerated cine imaging offers the potential to reduce breath-hold burden, shorten scan time, and enable free-breathing acquisitions. OBJECTIVE: To clinically evaluate a commercially available DL-accelerated cine bSSFP sequence (SonicDL) across multiple acceleration levels and breath-hold/free-breathing configurations, and to assess diagnostic
