Poland's syndrome: improved surgical management.

Single-stage reconstruction of the chest wall combined with simultaneous augmentation mammoplasty and transfer of an island pedicle myocutaneous flap of latissimus dorsi muscle are major improvements over previous multiple-stage procedures that provide less satisfactory cosmetic results in management of patients with Poland's syndrome. Utilization of the single-stage technique in 2 patients demonstrated its efficacy as proven by excellent cosmetic results. In 1 patient with absent second, third, and fourth costal cartilages and ribs, Marlex mesh covered with a synthetic dura mater graft was em

Conductive deafness, symphalangism, and facial abnormalities: the WL syndrome in a Japanese family.

We report on a family in which four relatives were affected with an autosomal dominant syndrome of unusual facial appearance, nasal abnormality, conductive deafness, pectus carinatum, and symphalangism. This appears to be only the second report of the WL symphalangism syndrome of Herrmann and the first studied in a Japanese family.

[Funnel chest and pigeon chest today].

The authors report on the results of sternochondroplasty according to Ravitch in a group of 31 patients with funnellike or carinated chest. The results appear to be conditioned to the type of anomaly, to the kind of surgery performed, to the age at which the patients undergo surgery. From a functional viewpoint results appear to be markedly better in pectus carinatum as compared to those in pectus excavatum. The latter anomaly requires a prolonged traction of the anterior thoracic "volet": this excludes any possibility of using external traction techniques (poor results in 25-30% of cases). Th

The King syndrome: malignant hyperthermia, myopathy, and multiple anomalies.

Heterogeneity in malignant hyperthermia is being recognized with increasing frequency. In 1973, King described a subgroup of malignant hyperthermia patients with slowly progressive myopathy, short stature, kyphoscoliosis, pectus carinatum, cryptorchidism, and a characteristic facial appearance. Seven cases have been reported, all in males. We present the case of a girl with similar clinical findings who died in a malignant hyperthermia crisis and review the previously reported cases of King syndrome. The delineation of King syndrome provides further evidence for the heterogeneity of malignant

Pectus carinatum and pectus excavatum: are they the same disease?

Eighty cases of anterior chest wall deformities were reviewed. The group consisted of 47 patients with pectus carinatum and 33 patients with pectus excavatum. This 1.4:1 distribution has not been reported in the literature, where a 1:10 distribution is regularly cited. Important bronchopulmonary changes were found by chest x-rays in both groups. Formerly, alterations in pectus excavatum were attributed to the mechanical effect of the depressed sternum. The presence of the same symptoms and bronchopulmonary radiographic changes in patients with pectus carinatum suggests a primary pulmonary defe

Surgical correction of pectus excavatum and carinatum.

This paper contains an analysis of the long-term results in 85 patients who had pectus excavatum or carinatum deformities repaired at the North Middlesex Hospital between 1951 and 1977. Seventy-seven patients had operations for correction of pectus excavatum and eight for pectus carinatum. A variety of surgical techniques was used. In the excavatum deformities the best results were obtained by the extensive resection of all deformed cartilages, the correction of the sternal deformity by a simple transverse wedge osteotomy, and by stabilising the chest with a stainless steel plate. For pectus c

Skin and bone lesions (dermato-osteolathyrism), possible side effects of D-penicillamine treatment, in a boy with cystinuria.

A 2 1/4 year-old boy was treated for cystinuria and urolithiasis with high fluid intake, sodium bicarbonate, and D-penicillamine, over a period of 5 3/4 years, unauthorized interruptions and prescribed pauses included. Therapy was partially sucessful but regrowth of calculi coincided with interruptions of D-penicillamine administration and also with the institution of a low-dose D-penicillamine regime. Flat feet, scoliosis, pectus carinatum, hypermobility of joints, molluscoid pseudotumors and atrophic scars were alarming side effects of D-penicillamine. However, the possibility was not exclud

[Corrective surgery of congenital chest malformations of the type Pectus excavatum and Pectus carinatum (author's transl)].

The authors presents an original process for surgical treatment of deformations of chest anterolateral wall, insisting on the procedures and techniques of the recommended process. The compulsory operative steps are: wide chondrectomy, retrosternal loosening, transversal osteotomy of the sternum and contention with retrosternal metallic plates. The author considers surgery to be necessary for all these anterolateral chest deformations and the best age for their correction to be after 10 to 12 years. The original technique for loosening and preserving the perichondrium in its wholeness is descri

Premature cataracts associated with generalized lentigo.

Generalized lentigo (leopard syndrome) is an autosomal dominant trait characterized by lentigo, sensorineural deafness, retarded growth (below 25%), ocular hypertelorism, mandibular prognathism, pectus carinatum or excavatum, dorsal kyphosis, winging of the scapulae, valvular pulmonary artery stenosis, electrocardiographic conduction defects, and genitourinary defects. Ocular evaluations of patients with generalized lentigo have revealed the appearance of multiple small white punctate and comma-shaped opacities in the cortex and nuclci of the lenses of affected patients. On the basis of age of

Pectus carinatum: results of surgical therapy.

There are a variety of protrusion defects of the anterior chest which are collectively referred to as pectus carinatum. Over a 14-yr period, 13 patients have had surgical correction of pectus carinatum at The Johns Hopkins Hospital. Technique includes costal cartilage resection and a sternal osteotomy. There was no mortality, complications were few and results are good. Repair is recommended for patients in late adolescence due to the cosmetic nature of the procedure, the apparent lack of an associated postural deformity and the good results of operation in this age group.

Pectus carinatum.

In the mirror of the pertinent literature, we present our experiences gained in 161 operations for pectus carinatum. The Type I (keel chest) deformity is corrected by bilateral resection of the costal cartilages, transverse osteotomy of the sternum, detachment of the xiphoid process, and resection of the lower end of the body of the sternum. The sternum is maintained in its corrected position by utilizing the pulling force of the rectus muscles through the reattached xiphoid and by tacking the pectoralis muscles together in front of the breast bone. Type II (pouter pigeon breast) is handled by

Partial duplication of the long arm of chromosome 5: a case due to balanced paternal translocation and review of the literature.

A partial duplication of the distal segment of the long arm of chromosome 5 (q31 leads to qter) was observed in an infant with congenital malformations and dysmorphic features. The phenotypically normal father had a balanced translocation between the long arm of chromosome 5 and the short arm of chromosome 9: 46,XY,t(5;9)(q31;p24). The clinical and cytogenetic data obtained from six patients with partial duplications of two different long arm segments of chromosome 5 suggest that partial duplication of the distal long arm of chromosome 5 is associated with microcephaly, hypertelorism, epicanth

A distinct skeletal dysplasia in an infant from consanguineous parents.

An infant with a distinct set of skeletal anomalies was studied. The patient's main phenotypic features were short-limb dwarfism, craniofacial disproportion with prominent forehead, short neck and trunk with pectus carinatum, and platyspondyly, protuberant abdomen, acromesomelic shortness of limbs, bilateral palm simian crease, short feet with brachydactyly of the 2nd toe, and prominent heels. Differential diagnosis suggests that the case described had a previously unrecognized skeletal dysplasia. The fact that the parents were first cousins suggests a genetic, probably autosomal recessive eti

Surgical management of pectus deformities.

We analyzed the long-term results in 87 patients who had pectus deformities repaired at Fitzsimons Army Medical Center during the period 1953 to 1975. Seventy-one patients had operations for correction of pectus excavotum; 16 had pectus carinatum deformities repaired. A variety of surgical techniques was used in these patients. Our findings show that in excavatum deformities the best results were obtained by removing all deformed cartilage, mobilizing the sternum, using an anterior osteotomy, and stabilizing the chest with a metal strut (which was removed after one year). [Conservative treatment of deformities of the anterior chest wall].

The authors have treated 55 deformities of the anterior chest wall in children. There were 37 cases of symmetrical pectus carinatum, 15 cases of asymmetrical pectus carinatum and 3 cases of pectus arcuatum. Associated lesions of the ribs, scapulae and spine are described. Treatment in every case was by plaster casts followed by a plaster jacket and exercises. The results were much more satisfactory in deformities associated with prominence than with depression of the sternum. In pectus carinatum, the results were better when the apex of the deformity was more distal. The best age for treatment
