Epidemiology, presentation and aspirations in Pectus carinatum: a retrospective cohort study.

PURPOSE: To describe the epidemiology and clinical presentation of children with pectus carinatum; and the outcome of their first hospital clinic appointment. METHODS: A single surgeon, retrospective cohort study of children presenting between 1998 and 2022. Demographic and clinical data were collected from the medical record. RESULTS: We present 430 children with pectus carinatum; 364 boys and 66 girls. Their asymmetry was identified at a median age of 11 years (range 0-16 years). Girls developed carinae earlier than boys (10 vs 14 years, p = 0.003), were more likely to present with pain (4

Anaesthetic management of a patient with tumour-induced osteomalacia for an orthopaedic procedure.

Tumour-induced osteomalacia is a metabolic disorder that causes hypophosphataemia, resulting in bone mineralisation defects. If not treated definitively with tumour resection, this produces life-threatening anatomical changes. The anaesthetic management is tailored to address tumour location, perioperative positioning, as well as airway and pulmonary concerns. We report the anaesthesia delivery for the surgical excision of a tibial mass in a man in his late 30s with oncogenic osteomalacia. He had cervical spine deformity, pectus carinatum, severe kyphosis, lower limb atrophy with flexion contra

Differences in chest wall between Dutch adolescents with pectus carinatum and the general pediatric population: A three-dimensional scanning analysis.

INTRODUCTION: This study compares the External Haller Index (EHI) in pectus carinatum patients to healthy adolescents, using 3D body imaging. METHODS: We conducted a cross-sectional study including pectus carinatum patients (aged 12-18) from Amsterdam UMC, and healthy adolescents from Sports Centre: Hoppers and the Jacobus Fruytier Scholengemeenschap, between December 2024 and May 2025. The External Haller Index was calculated using 3D scans (VECTRA®H2). Primary outcome was the difference in External Haller Index between pectus carinatum patients and healthy adolescents. Secondary outcomes in

Superior mesenteric artery syndrome and nutcracker syndrome in a patient with pectus carinatum and spine scoliosis.

A male patient with pectus carinatum and spine scoliosis deformity presented with a 7-day history of recurrent episodes of nausea, vomiting and abdominal distention. He reported a weight loss of about 10 kg in the last 6 months and an altered eating habit. The contrast-enhanced CT of the abdomen revealed a significant compression of the third part of the duodenum, reduced the aortomesenteric distance and left renal vein compression between the abdominal aorta and superior mesenteric artery. A diagnosis of superior mesenteric artery syndrome with nutcracker syndrome was made. The patient's symp

A pictorial essay of thoracic wall diseases: multiple pathologies in the same anatomical site.

OBJECTIVES: To describe diagnostic and radiological features of the main pathologies affecting the thoracic wall, providing a pictorial atlas based on several clinical cases extracted from our archive. MATERIALS AND METHODS: A wide variety of pathologies affect the tissues of the thoracic cage; these conditions are often encountered by radiologists during examinations performed for unrelated clinical questions. Modern imaging techniques enable the detection of these pathologies and allow definitive diagnoses to be achieved. RESULTS: Pathological processes that involve the chest wall may be c

Diminished GALNS activity in induced pluripotent stem cells of mucopolysaccharidosis IVA caused by compound p.S162Y and p.C165F mutation.

BACKGROUND: Mucopolysaccharidosis (MPS) IVA is a lysosomal storage disorder caused by mutations in the gene encoding the galactosamine (N-acetyl)-6-sulfatase (GALNS) enzyme. Children with MPS IVA usually develop pectus carinatum, genu valgum and multiple skeletal abnormalities. AIM: To establish a patient-derived induced pluripotent stem cell (iPSC) disease model to investigate the effects of two GALNS missense mutations. METHODS: The medical history and clinical manifestations of a patient with MPS IVA were first inspected. The effects of the identified GALNS mutations were predicted throug

Modified Nuss procedure versus Ravitch in concurrent repair of pectus deformity and open-heart surgery.

ObjectivePectus excavatum and, less commonly, pectus carinatum are congenital chest wall deformities. These may be associated with kyphoscoliosis, pulmonary, and cardiac diseases. However, the incidence of concomitant cardiac disease in patients with pectus deformities is not well-documented. There is no consensus on the optimal age for repair, the most effective technique, or whether a simultaneous or staged approach is preferable. This study presents our experience with combined pectus and cardiac surgery.MethodsThirteen patients (aged 6-32 years) with pectus deformities and concurrent cardi

Beyond the usual spectrum: Atrial septal defect in a Marfan syndrome patient with severe aortic pathologies.

Marfan's syndrome is a rare connective tissue disorder which mainly affects cardiovascular, skeletal, and ocular systems. Common cardiovascular manifestation of Marfan's includes aortic root dilatation, aortic regurgitation, and mitral valve prolapse. However, presence of congenital heart defects like atrial septal defect (ASD) is very rare. Here, we report the case of a 22-year-old male with Marfan's syndrome who had an uncommon coexistence of ASD and severe aortic pathologies. The patient had clinical features like tall stature, long arm span, and musculoskeletal abnormalities such as pectus

Use of a novel maximum protrusion index to describe severe pectus carinatum.

BACKGROUND: The absence of standardized criteria for grading pectus carinatum (PC) severity hampers clinical decision-making and insurance approvals. Although the Haller Index (HI) is frequently used, it underestimates deformity by measuring the distance to the posterior sternum rather than the point of maximal protrusion. This study introduces the Maximum Protrusion Index (MPI), a novel metric designed to more accurately reflect PC severity. METHODS: MPI is defined as the transverse chest diameter divided by the anterior-posterior distance from the vertebral column to the point of maximal ch

Targeted Axillary Dissection for Clinically Node-Positive Breast Cancer Cases that Turn Negative After Neoadjuvant Systemic Therapy Using Charcoal Tattooing.

The magnitude of the problem of sickle cell disease (SCD) in India is very large and diverse. For people living with SCD there are several unmet health care needs making it pertinent to focus on recognizing the important public health burden of SCD in the country. In a bid to achieve this goal, the Indian College of Hematology (ICH) of the Indian Society of Hematology and Blood Transfusion (ISHBT) in collaboration with Indian Council of Medical Research (ICMR) has formulated a Guideline on Management and Control of Sickle Cell Disease in India. This was achieved by a national taskforce compris

Pectus excavatum and carinatum: a narrative review of epidemiology, etiopathogenesis, clinical features, and classification.

BACKGROUND AND OBJECTIVE: A wide variety of congenital chest wall deformities that manifest in infants, children and adolescents exists, among which are pectus excavatum and pectus carinatum. Numerous studies have been conducted over the years aiming to better understand these deformities. This report provides a brief overview of what is currently known about the epidemiology, etiopathogenesis, clinical presentation, and classification of these deformities, and highlights the gaps in knowledge. METHODS: A search was conducted for all the above-described domains in the PubMed and Embase databa

VAMP1-Related Congenital Myasthenic Syndrome: A Case Report and Literature Review.

Congenital myasthenic syndrome-25 (CMS-25) is an autosomal recessive neuromuscular disorder caused by a homozygous mutation in VAMP1 gene. To date, only eight types of allelic variants in VAMP1 gene have been reported in 12 cases of CMS-25. Here, we report on an 8-year-old boy with motor developmental delay, axial hypotonia, myopathic face, muscle weakness, strabismus, ptosis, pectus carinatum, kyphoscoliosis, joint contractures, joint laxity, seizures, and recurrent nephrolithiasis. He also had feeding difficulties and recurrent aspiration pneumonia. Brain magnetic resonance imaging at 20 mon

Overview of chest wall deformities.

Chest wall deformities are congenital disorders characterized by abnormal development and appearance of the thoracic wall. The most common form is the pectus excavatum deformity, known as shoemaker's chest. Pectus carinatum, known as pigeon chest, is the second most common deformity. In general, most patients are asymptomatic, but cardiopulmonary problems may accompany the disease. The indication for treatment is mostly cosmetic. Treating patients before they reach adulthood increases the chance of success. Surgical treatment can be open or minimally invasive.

A new custom-made bivalve brace for pectus carinatum in children and adolescents: preliminary promising experience of 140 patients from a tertiary center.

INTRODUCTION: International research suggests that poor patient compliance is the main cause of tutor failures in the context of potential novel orthopedic bivalve braces for conservative treatment of pectus carinatum. Our entire experimental study is based on the hypothesis that a rigid bivalve brace that patients can accept could solve the main problem associated with the conservative approach-poor compliance. The hypothesis was to reduce the thickness and weight of the classic bivalve brace to ensure concealment and make it sustainable enough to be worn several hours a day without compromis

Bracing of Pectus Carinatum in Children: Current Practices.

BACKGROUND: Although effective, compressive orthotic bracing (COB) in children with pectus carinatum is still not standardized. This study has aimed to analyze current practices amongst members of the Chest Wall International Group (CWIG). METHODS: A web-based questionnaire was mailed to all CWIG members at 208 departments. It included 30 questions regarding diagnostic work-up, age for COB indication, type of COB used, daily wearing time, treatment duration, complications, and recurrence rate. RESULTS: Members from 44 departments have responded (institutional response rate 21.2%). A total of

A minimally invasive hybrid procedure to correct pectus arcuatum.

BACKGROUND: Pectus arcuatum, also known as horns of steer anomaly or Currarino-Silverman Syndrome, is a distinct chest wall anomaly characterized by severe manubriosternal angulation, a shortened sternum, and mild pectus excavatum. The anomaly is typically repaired using open techniques, employing orthopedic fixation devices. Here, we report the results of a minimally invasive hybrid procedure to repair pectus arcuatum. METHODS: The procedure combines a standard Nuss procedure to correct the depressed sternum with a short upper chest (in boys) or inter-mammary (in girls) incision for bilatera

The role of 3D printing in chest wall reconstruction.

Technology is advancing fast, and chest wall surgery finds particular benefit in the broader availability of three-dimensional (3D) reconstruction and printing. An increasing number of reports are being published on the use of these resources in virtual 3D reconstructions of chest walls in computed tomography (CT) scans, virtual surgeries, 3D printing of real-size models for surgical planning, practice, and education, and of note, the manufacture of customized 3D printed implants, changing the fundamental conception from a surgery that fits all, to a surgery for each patient. In this review, w

Correction of the Manubriosternal Angle: The Missing Link in the Diagnosis and Management of Anterior Chest Wall "Pectus" Deformities.

A normal manubriosternal angle (MSA) varies between 157 and 161 degrees, and it is either increased (in pectus carinatum) or decreased (in pectus excavatum). Measurements of the MSA can accurately define the severity of both deformities and gauge the degree of response to therapeutic intervention(s). Correction of the MSA during repair of pectus deformity by complete upper transverse sternotomy seems necessary. We found no report of complete upper transverse sternotomy in repairing pectus deformities. Similarly, the MSA was never used in defining the severity of pectus deformity. We describe a
