Unusual Revelation of an Anomalous Right Coronary Artery: Non-ST Segment Elevation Myocardial Infarction (NSTEMI) in the Context of Drug Abuse.

Coronary artery anomalies (CAAs) are rare congenital abnormalities. Certain variants, such as those with an intramural course, are associated with increased risk of ischemia and sudden cardiac death, particularly under conditions of increased cardiac demand. We report the case of a 36-year-old male with polysubstance abuse who presented with chest pain after cocaine intake. Initial labs, testing, and imaging revealed respiratory failure, hypoxia, elevated cardiac biomarkers, and new-onset reduced ejection fraction. After the patient was medically stabilized, coronary computed tomography angiog

Multifocal Fibromuscular Dysplasia in a Horseshoe Kidney: Coincidence or Consequence?

Spontaneous coronary artery dissection (SCAD) is increasingly identified as a significant cause of acute coronary syndrome and is often associated with fibromuscular dysplasia (FMD), a non-atherosclerotic, non-inflammatory vascular disease affecting medium-sized arteries. SCAD and FMD frequently co-occur, warranting extracoronary vascular imaging. Horseshoe kidney, a rare congenital anomaly, has been linked to various genitourinary and vascular abnormalities but has not previously been associated with FMD. A man in his early 50s presented with acute chest pain and was diagnosed with SCAD of th

The Role of Stress Echocardiography in Patients With Anomalous Aortic Origin of Coronary Arteries: Two Tertiary Cardiac Centers' Experience.

AIMS: The objective of our study was to establish the prevalence of ischemia during exercise stress echocardiography (ESE) in patients with anomalous aortic origin of coronary arteries (AAOCA). METHODS AND RESULTS: A cohort of 46 patients with AAOCA was retrospectively included in this study. Treadmill/bike exercise stress echo tests for ischemia assessment were performed and analyzed according to ESC guidelines. Computed tomography coronary angiography (CTCA), cardiac magnetic resonance, myocardial perfusion scintigraphy and invasive angiography with intravascular ultrasound if needed were u

Anesthetic management for open reduction and internal fixation of the mandibular fracture in a patient with congenital pulmonary hypoplasia: a case report.

BACKGROUND: Congenital pulmonary hypoplasia is an exceptionally rare developmental disorder that is usually diagnosed in neonates and seldom encountered in adult.Consequently, evidence-based perioperative strategies for adult CPH patients undergoing non-thoracic surgery remain virtually absent. We present a case of an adult with CPH who required general anesthesia for open reduction and internal fixation of the mandibular fracture, and we describe a comprehensive perioperative management plan centered on lung-protective ventilation strategy and multimodal analgesia. CASE PRESENTATION: This re

Esophageal magnetic anastomosis for long gap congenital esophageal atresia: A case report.

RATIONALE: Long gap esophageal atresia (LGEA) is a rare congenital malformation. Magnamosis represents a minimally invasive technique for LGEA to achieve esophageal recanalization for feeding. Few cases had been reported worldwide. PATIENT CONCERNS: An infant was diagnosed as congenital esophageal atresia by using prenatal ultrasound imaging including the absence of gastric bubble and polyhydramnios. DIAGNOSES: LGEA was confirmed after birth by esophagography through the gastrostomy and esophagus simultaneously. INTERVENTIONS: Laparoscopic gastrostomy was performed on the 3rd day after birt

Delayed thoracoscopic lobectomy in symptomatic newborns with congenital pulmonary airway malformation.

OBJECTIVE: Surgery for symptomatic congenital pulmonary airway malformation (CPAM) is uncontroversial. However emergency surgery for neonates with CPAM presented as progressive respiratory distress still carries high risks. This article introduces a new strategy aimed at improving the safety and effectiveness of surgery. METHOD: 4 neonates were diagnosed with CPAM with mediastinal shift by prenatal diagnosis and experienced progressive respiratory distress after birth. Insertion of a chest drain in the main cyst of the CPAM was performed as effective interim management to relieve respiratory d

Analysis of pulmonary sequestration cases, based on the 11-year birth defects surveillance data of Changsha hospitals.

Pulmonary sequestration (PS) is a rare congenital malformation that is characterized by the absence of a connection between a portion of the lung tissue and the tracheobronchial tree, with blood supply from arteries throughout the body. Abnormal lung tissue cannot perform the normal gas exchange function. In the absence of timely diagnosis and early intervention, some cases may need labor induction, and some of the infants who survive may develop symptoms in childhood. The manifestations are repeated lung infections, dry cough, chest pain, chest tightness, and hemoptysis. Some cases have no sy

Cardiothoracic surgical incision type and later scoliosis development in trisomy 21 patients following infant cardiothoracic surgical procedures.

PURPOSE: One factor that may lead to the development of early-onset scoliosis (EOS) is chest wall procedures during infancy, which are common for patients with Trisomy 21 requiring cardiothoracic intervention involving thoracotomies. Studies examining scoliosis incidence in patients with prior thoracotomy demonstrate a wide incidence range. Our study aimed to assess if scoliosis prevalence is higher in patients with Trisomy 21 who underwent prior thoracotomy compared to patients who underwent sternotomy for congenital heart disease repair. METHODS: An IRB-approved, single-center retrospective

Cardiac involvement of Gorlin-Goltz syndrome: new light among the shadows of an old congenital disorder.

Mutations in the PTCH1, PTCH2, or SUFU genes cause the hereditary, autosomal dominant Gorlin-Goltz syndrome (GGS), which is characterized by high penetrance and variable expressivity. Although its clinical manifestations are primarily marked by multiple basal cell carcinomas, other endocrine, neurological, ophthalmologic, genital, and respiratory alterations have been reported in the literature. Despite the association with cardiac fibromas, cardiovascular involvement is rarely reported. Here, we present a case of a patient with myocarditis of unknown origin, later diagnosed with GGS. We discu

Early onset scoliosis: Can Best Practice Guidelines Be Provided in Europe?

BACKGROUND: The goal of treatment in early onset scoliosis patients is to control the deformity while allowing chest and spinal growth. However, management, either conservative or surgical, remains controversial and require further clinical evidence. The aim of this study was to determine best practice guidelines for the management of early onset scoliosis patients in Europe. METHODS: A Delphi approach was used among European experts to determine consensus in: (1) identification of progression risk factors, (2) clinical and radiographic evaluation, (3) conservative treatment, and (4) surgical

Polythelia (Supernumerary Nipple): Clinicopathological Characterization of an Atavistic Lesion.

Polythelia refers to the presence of additional nipples typically located along the "milk line." While its atavistic nature and associations with congenital anomalies are well documented, its clinicopathological features are less defined. This retrospective study reviewed 131 lesions diagnosed as polythelia in 112 patients from 1999 to 2024. The median age of patients at the time of excision was 28 years (range: 6 months to 72 years). Most (101, 90.2%) patients were female. Lesions were located around the breast (42%), chest (28%), axilla (15%), inframammary fold (8%), and abdomen (6%). Histop

Automated, Standardized, Quantitative Analysis of Cardiovascular Borders on Chest X-Rays Using Deep Learning.

BACKGROUND: The analysis of cardiovascular borders (CVBs) in chest x-rays (CXRs) traditionally relied on subjective assessment and does not have established normal ranges. OBJECTIVES: The authors aimed to develop a deep learning-based method for quantifying CVBs on CXRs and to explore its clinical utility. METHODS: This study used a prevalidated deep learning to analyze CVBs. A total of 96,129 normal CXRs from 4 sites were used to establish age- and sex-specific normal ranges of CVBs. The quantified CVBs were standardized into z-scores for newly inputted CXRs. The clinical utility of the z-s

Wolff-Parkinson-White syndrome: a masquerading clinical condition in an 8-year-old Nigerian girl.

BACKGROUND: Wolff-Parkinson-White (WPW) syndrome is a congenital abnormality of the cardiac conduction system characterized by the presence of an accessory pathway, which can predispose affected individuals to supraventricular tachycardia (SVT), atrial fibrillation, ventricular fibrillation, and sudden cardiac death. Despite its clinical significance, WPW syndrome is often underdiagnosed, particularly in resource-limited settings where cardiac arrhythmias may be misattributed to other conditions. CASE PRESENTATION: We report an eight-year-old Nigerian girl with WPW syndrome who was repeatedly

Immediate chest tube removal at the completion of anterior vertebral tethering is well-tolerated without an increased risk of pulmonary complication.

INTRODUCTION: Though chest tube removal at the completion of an endoscopic thoracic procedure is well accepted in the pediatric and adult general surgery literature, this practice has never been studied in pediatric patients treated with anterior vertebral tethering (AVT) for AIS. This study retrospectively analyzed pulmonary complications in a large series of AIS patients consecutively treated with chest tube removal at the completion of AVT. The rate of pulmonary complication in this series was then compared with the published rate of pulmonary complication in patients managed with chest tub

Don't Miss the Warning Signs: Syncope Masquerading as a Major Congenital Heart Defect in a 42-Year-Old Patient.

Congenital heart defects (CHDs) are typically diagnosed in infancy, yet some may go unidentified until adulthood and pose significant clinical challenges. We present the case of a 42-year-old female patient with multiple CHDs, including sinus venosus atrial septal defect (ASD), partial anomalous pulmonary venous return (PAPVR), and moderate mitral regurgitation discovered incidentally after presenting to the emergency department with atypical chest pain. Initial warning signs were overlooked, and asymptomatic mitral valve prolapse (MVP) diagnosed in childhood had progressed into moderate mitra

Clear Cell Papulosis Associated With Multiple Developmental Abnormalities of the Skin.

Clear cell papulosis of the skin is a rare condition in children characterized by multiple small hypopigmented macules or flat papules, predominantly distributed in the pubic area and over the abdomen, chest, and axillae, along the milk lines, and by mucin- and cytokeratin 7-positive large clear cells within the epidermis. Associations of clear cell papulosis with other diseases or disorders have been rarely described. We present a case of clear cell papulosis associated with a congenital sacral dermoid cyst, nevus of Ota, presumed accessory breast tissue, thin and lightly pigmented hair, cong

Anomalous Left Circumflex Artery With ST Depressions and T-wave Inversions in a Healthy Athlete: A Case Report.

An anomalous left circumflex artery is a congenital anatomic variant of typical coronary circulation that can potentially contribute to cardiac ischemia or altered blood flow. These variants can cause changes to be seen on electrocardiograms, particularly to the T wave and ST segments, including depressions, inversions, or elevations. We describe the case of a healthy 35-year-old man with previously undiagnosed anomalous origin of the left circumflex artery from the right coronary artery ostium with evidence of ST-segment changes and T-wave inversions on electrocardiogram during exercise stres

Bilateral Pneumothorax After Minimally Invasive Repair of Pectus Excavatum: Report of a Rare Life-Threatening Complication.

Minimally invasive repair of pectus excavatum (MIRPE) creates an iatrogenic communication between the pleural cavities, known as a "buffalo chest." Patients with pectus excavatum are also at increased risk of spontaneous pneumothorax due to congenital apical blebs. When these two conditions coexist, the risk of bilateral spontaneous pneumothorax becomes potentially life-threatening. This study aims to evaluate the incidence and characteristics of spontaneous pneumothorax following MIRPE, with particular attention to the presence and role of congenital blebs. We retrospectively reviewed patients

Cardiovascular Collapse During Scoliosis Surgery in a Patient With Coffin-Lowry Syndrome and Mesocardia.

Coffin-Lowry syndrome (CLS) is a rare X-linked disorder marked by intellectual disability, craniofacial and skeletal anomalies, and progressive spinal deformities like scoliosis and hyperlordosis. Corrective posterior spinal fusion (PSF) surgery is often required. Mesocardia, a congenital anomaly where the heart is positioned centrally in the thorax, may affect perioperative hemodynamics, particularly in the prone surgical position. This report presents a case illustrating the perioperative challenges of PSF in a patient with both CLS and mesocardia. A 17-year-old male with CLS, mesocardia, an
