Acute Pericarditis in a Patient With Poland Syndrome.

Poland syndrome is a rare congenital anomaly characterized by unilateral absence or hypoplasia of the pectoralis major muscle. We report the case of a 32-year-old man with previously undiagnosed Poland syndrome who presented with acute, severe, tearing chest pain radiating to the back, which worsened with sitting up and improved with lying down. The cardiac physical exam was negative for murmur and friction rub. Electrocardiograms showed focal ST-segment elevations in precordial leads V2-V4, and serial troponin levels were negative. Life-threatening cardiac causes of chest pain, such as acute

MUSCULOSKELETAL ALTERATIONS OF ORTHOPEDIC INTEREST IN MUCOPOLYSACCHARIDOSES.

Mucopolysaccharidoses (MPS) are lysosomal storage diseases characterized by the improper accumulation of glycosaminoglycans due to genetic deficiencies in specific enzymes. This accumulation leads to progressive cellular and tissue dysfunction, affecting multiple systems, particularly the musculoskeletal system, resulting in multiple dysostoses with deformities in the spine, thorax, and limbs. Clinically, MPS manifests as recurrent respiratory infections, sleep apnea, severe cardiac lesions, and hydrocephalus, among other symptoms. Bone and joint alterations markedly impair motor function and

A Rare Congenital Triad in an Adult With Coronary Artery Disease: A Case Report From Georgia.

A 47-year-old man with a history of New York Heart Association (NYHA) class III heart failure and paroxysmal atrial fibrillation presented with recurrent chest pain, exertional dyspnea, and exercise intolerance. He was diagnosed with an acute myocardial infarction and underwent stenting of the left anterior descending artery. During the same hospitalization, transthoracic echocardiography revealed a sinus venosus atrial septal defect (ASD), moderate tricuspid regurgitation, and right heart dilation, raising suspicion for partial anomalous pulmonary venous drainage (PAPVD). Further imaging conf

Endoscopic Mitral Surgery in Noonan Syndrome-Case Report and Considerations.

Background: Totally endoscopic techniques have become increasingly popular in cardiac surgery, with minimally invasive mitral valve repair emerging as an effective alternative to median sternotomy. This approach could be particularly advantageous for patients with Noonan syndrome, who often present with structural thoracic anomalies and other comorbidities like bleeding disorders. Endoscopic mitral valve surgery is rapidly establishing itself as the new standard of care for mitral valve operations, demonstrating both safety and efficacy. Noonan syndrome is an autosomal-dominant multisystem dis

Anomalous left coronary artery from the pulmonary artery: A case report of ALCAPA syndrome with bicuspid aortic valve.

RATIONALE: Anomalous left coronary artery from the pulmonary artery (ALCAPA) syndrome, also known as Bland-White-Garland syndrome, is a rare congenital heart defect predominantly diagnosed in infants and associated with high early mortality rates without surgical intervention. While ALCAPA generally presents alone, it can sometimes occur alongside other congenital anomalies, such as a bicuspid aortic valve (BAV), a combination scarcely documented in the literature. This case report explores a rare presentation of ALCAPA syndrome in a 27-year-old adult with concurrent BAV. PATIENT CONCERNS: Th

Cavitary Lung Abscess Secondary to a Tracheal Bronchus: A Pediatric Patient With Noonan Syndrome.

A 13-year-old male with a past medical history of Noonan syndrome, pulmonary valve stenosis status post balloon valvuloplasty and subsequent surgical valvotomy with main pulmonary artery augmentation, splenomegaly, and von Willebrand disease type 1 presents with chest pain, fever, and chronic, recurrent vomiting. At the outlying facility, computed tomography of the chest revealed a right upper lobe pulmonary cavitation. After an extensive workup, he was found to have a right-sided tracheal bronchus via bronchoscopy. Broad-range polymerase chain reaction analysis of the biopsied specimen demons

Multi-Organ Transplantation in Adult Congenital Heart Disease: Navigating the Unique Challenges of a Distinct Patient Population.

The prevalence of adult congenital heart disease (ACHD) is increasing, with heart failure being the leading cause of death. For many ACHD patients, heart transplantation is the only treatment option for advanced heart failure, though significant extracardiac involvement may require multi-organ transplantation. Despite the rising number of ACHD transplants, multi-organ transplants in this population remain challenging, and a substantial gap remains between those in need and those who receive a transplant. While short-term outcomes may be worse for ACHD patients, long-term outcomes are comparabl

Congenital pulmonary airway malformation type 2 associated with intralobar pulmonary sequestration in an adult patient: a case report.

Congenital pulmonary airway malformation (CPAM) and pulmonary sequestration (PS) are among the most common congenital lung malformations and their association is extremely rare. In exceptional cases, patients with CPAM remain asymptomatic until adulthood. We report the case of a 30-year-old man who was hospitalized in our pneumology department for fever, cough and chest pain. Radiological and histological findings confirmed the coexistence of CPAM and PS. The patient underwent surgical resection and recovered well. This is one of the few reported cases in the literature that describe the uncom

Bilateral Costal Cartilage Harvest for Auricle Reconstruction: A New Technique to Prevent Postoperative Thoracic Deformity.

HYPOTHESIS: Unilateral costal cartilage harvesting (UCCH) for auricle reconstruction in children tends to cause thoracic deformities. Therefore, our study aimed to develop a novel bilateral costal cartilage harvesting (BCCH) method to prevent and reduce thoracic deformities. METHODS: Patients with unilateral microtia who underwent either UCCH (n = 50) or BCCH (n = 46) were enrolled in this study. The grafts for the BCCH group were harvested from the 6th costal cartilage of the ipsilateral hemithorax and the 7th and 8th cartilage from the other hemithorax. Separating Out Pulmonary Sequestration.

Pulmonary sequestration (PS) is a rare congenital anomaly that accounts for 1% to 6% of all pulmonary malformations at birth. It is characterized by a focal area of pulmonary tissue that does not have direct communication with the tracheobronchial tree and does not get blood supply from the pulmonary circulation. We present the case of a 28-year-old female with a history of recurrent pulmonary infections who was found to have intralobar sequestration and underwent curative surgical excision. Because pulmonary sequestration is commonly misdiagnosed, as it can mimic other conditions on chest X-r

Clinical characteristics, imaging findings, management, and outcomes of patients with scimitar syndrome at a tertiary referral healthcare center in Colombia.

Scimitar Syndrome is part of a complex spectrum of congenital cardiovascular anomalies related to anomalous pulmonary venous return. Depending on the extent of involvement, treatment can be either expectant or surgical. Prognosis and survival have been controversial, with some results supporting early surgical management. This research aims to disclose the outcomes and describe the management, clinical and imaging characteristics of patients diagnosed with Scimitar Syndrome treated in a tertiary referral healthcare center. Longitudinal descriptive observational study. The study included all pa

Multidisciplinary follow-up in a patient with Morgagni hernia leads to diagnosis of Marfan syndrome.

BACKGROUND: congenital diaphragmatic hernia (CDH) is a birth defect occurring in isolated or syndromic (chromosomal or monogenic) conditions. The diaphragmatic defect can be the most common one: left-sided posterolateral, named Bochdalek hernia; or it can be an anterior-retrosternal defect, named Morgagni hernia. Marfan syndrome (MFS) is a rare autosomal dominant inherited condition that affects connective tissue, caused by mutations in fibrillin-1 gene on chromosome 15. To date various types of diaphragmatic defects (about 30 types) have been reported in association with MFS, but they are het

Malposition of Central Venous Catheter into Coronary Sinus throughout the Persistent Left Superior Vena Cava and Other Complications Related to Catheterization.

This case concentrates on the persistent left superior vena cava (PLSVC), a rare vascular anomaly which contributes to central venous catheter (CVC) misplacement. A 72-year-old woman with renal insufficiency presented to the hospital with recurrent bleeding from her permanent CVC device placed in the right common jugular vein. An initial attempt to replace the device was unsuccessful, necessitating the placement of a secondary catheter in the left jugular vein. Shortly after the procedure, the patient developed swelling of the face and neck. Further diagnostic imaging, including a chest radiog

Pediatric vascular ring outcomes for surgically repaired vs. unoperated children: a single-center experience.

BACKGROUND: Vascular rings represent 1% of congenital cardiovascular abnormalities. Phenotypic expression varies from asymptomatic to severe forms related to either oesophageal or tracheal compression. While refinement in prenatal screening led to an increase in fetal diagnosis, optimal management in asymptomatic neonates and infants is currently a matter of debate. We report our center experience of vascular ring management over three decades. METHODS: In this single-center retrospective study, data were extracted from patient medical records. [Surgery for complicated accessory lobe of the azygos vein].

Congenital anomalies of respiratory system are quite diverse and not all of them are subject to surgical treatment. One example is accessory lobe of the azygos vein. This anomaly usually has no clinical manifestations and requires only follow-up, as well as attention in surgery on the right half of the chest for some other disease. This situation changes when complications occur, for example, purulent-inflammatory process. Therapy is not always effective, and lung tissue destruction requires surgical treatment. Progressive destruction complicates diagnosis and choosing surgical tactics. We pre

Right-sided Morgagni diaphragmatic hernia presenting as isolated left shoulder pain.

A woman in her 60s presented to the emergency department with excruciating, deep left shoulder pain and was found to have a right-sided Morgagni hernia, a rare type of congenital diaphragmatic hernia (CDH). She did not have chest pain, palpitations, shortness of breath, cough, abdominal pain, constipation, diarrhoea, nausea, vomiting or other symptoms classically associated with CDHs in adults. Laparoscopic robotic-assisted repair with mesh placement was performed, and the patient's recovery was uncomplicated, with no recurrence of shoulder pain. Our patient's presentation was unusual due to t
