Myocardial bridges in man: clinical correlations and angiographic accentuation with nitroglycerin.

Little is known of the clinical significance of myocardial bridges, which may be recognized angiographically as systolic coronary artery narrowing (SCAN). A retrospective review of a 1 year's experience (313 consecutive coronary arteriograms) revealed 5 patients with SCAN, an incidence of 1.6%. SCAN involved the proximal and/or middle segments of the left anterior descending coronary artery in all patients. It is of particular note that the administration of nitroglycerin noticeably accentuated the SCAN phenomenon in each of 3 patients to whom it was administered. Four of the 5 patients had le

[Arterial fontanel pulsation].

Infants with bounding arterial pulses frequently show a visible or palpable arterial pulsation of the fontanel. This simple clinical sign is neither described in the textbooks nor mentioned in connection with congenital heart diseases (CHD). To investigate its significance 501 infants were examined 178(35,5%) showed an arterial pulse of the fontanel. Out of 421 with no sign of cardiovascular malformation or disease on physical examination, 111(26,4%) had a fontanal pulse (f.p.). Out of 80 infants with CHD, 67(83,8%) exhibited a fontanel pulse (f.p.), while only 13(16,2%) did not. All infants w

Syndrome of congenital ventricular diverticulum and midline thoraco-abdominal defects.

The long-term follow-up of a 30-year-old patient who had a correction of the syndrome of left ventricular diverticulum and thoraco-abdominal defects is presented. The main features of the syndrome include a diverticulum of the left ventricle, a ventricular septal defect, and sometimes other cardiac anomalies. The thoraco-abdominal defects consist of foreshortened sternum, pericardial and diaphragmatic defects, and umbilical hernia. The association of these anomalies is thought to be due to a developmental failure of the primitive paramidline mesoderm. The diagnosis can be made clinically by th

[Determination of cardiac output by the method of tetrapolar chest rheography and evaluation of its metrological possibilities].

Tetrapolar chest rheography is an up-to-day, convenient and bloodless method of determining the cardiac stroke volume (CSV) and cardiac output (CO) in dynamic observations. The paper presents and analysis of the method and considers the possibilities and limits of its application in determining the stroke volume and cardiac output. Advantages of the Kubicek procedure as modified by the authors by comparison with other rheographic methods are shown, along with methodological procedures employed for dtermining the CSV and CO and a nomogram that significantly facilitates the calculation of the va

Quadricuspid semilunar valves.

Forty-seven cases of quadricuspid semilunar valves which were autopsied were reviewed. The ratio of quadricuspid pulmonic valve to quadricuspid aortic valve was 5:1. Among the 35 patients with quadricuspid pulmonic valves, there were ten patients with clinical and pathologic evidence of coexisting congenital cardiac defects, eight of which resulted in severe cyanotic heart disease in infancy. In the remaining 25 patients the quadricuspid pulmonic valve was an incidental finding at autopsy. Three of the seven patients with quadricuspid aortic valves had aortic insufficiency, while the remaining

d-Transposition of the great vessels in the neonate. A clinical diagnosis.

We reviewed the records of 66 neonatal patients with d-transposition of the great vessels (d-TGV) admitted to our institution. The initial history, physical examination by the cardiologist, chest rotengenogram, ECG, hemoglobin level, and hematocrit value were analyzed. Although all patients all patients were cyanotic and 65% were tachypneic, other signs of a major congenital cardiac anomaly were usually absent. The number of clinical signs (poor general status, cyanosis, respiratory distress, S2 increased and single, murmur, classical chest roentgenogram, abnormal ECG) present in each patient

[Solitary azygos prolongation (author's transl)].

Azygos prolongation of the inferior vena cava is a congenital abnormality which is normally associated with a congenital cardiac lesion, abnormalities of the inferior caval system and, in some instances, abdominal heterotaxia. The authors report here a case which is exceptional in the sense that the azygos prolongation was isolated. Further radiological studies are essential in order to make the diagnosis following the discovery of a right-sided laterotracheal opacity on a plain chest film. Only five other cases were found in the literature.

Traumatic arteriovenous fistula of the internal mammary artery.

A rare case of arteriovenous fistula of the left internal mammary artery after blunt chest trauma is described. The patient was seen 3 weeks later because of a pulsating mass which varied in size, over the left midsternal area. Aortography and arteriography showed an aneurysmal sac draining into a vein in the left chest wall and then into the left axillary and neck veins. The fistula was ligated and excised followed by complete recovery. The literature is reviewed and differential diagnosis from congenital heart conditions, A-V shunts of other thoracic wall vessels, and vascular tumors is disc

Conjoined thoracopagus twins.

A case of vaginally delivered stillborn female thoracopagus conjoined twins is presented. Each twin had a cleft lip and cleft palate. They shared a common pericardial sac and a common abdominal cavity. The liver was conjoined, and the small bowel was common to both twins. One twin was asplenic. Although the twins were monozygotic, their viscera were neither identical nor mirror-image to each other, and the visceral anomalies were unique to each twin. The hearts were fused, and communications existed between the circulations at the atrial and ventricular levels. One twin had single atrium, soli

Cardiac disease in the wheezing child.

Cardiac and pulmonary disease are so closely interrelated that it is often difficult to determine in young infants which is the primary offender. As illustrated in these case reports, failure to recognize the true nature of the disease process may lead to unnecessary procedures and delays which can be life-threatening. Statistically, the wheezing infant very likely is suffering from primary pulmonary disease; however, congenital cardiac abnormalities can cause pulmonary symptoms which completely dominate the clinical picture and lead to erroneous diagnoses. Although rare, the basis of cough an

Aberrant left pulmonary artery. Clinical and embryologic factors.

The case of an infant with a pulmonary arterial sling (or aberrant left pulmonary artery) is presented. Associated congenital anomalies found in the original patient and in each of four other patients with this lesion, identified from postmortem files, are noted. The importance of associated anomalies of the respiratory tract is emphasized, since if present, they may affect the outcome after sucessful surgical correction of the vascular lesion. Previous theories of the embryologic origin of the anomalous artery are discussed, and a new theory is presented.

Agenesis of the lung.

Agenesis of the lung is rare. The cases of ten patients with this malformation are recorded and illustrated. Most had congenital malformation of other organs, especially the heart, as well. All had hypoplasia of the pulmonary artery or its branches. A pantaloon-like appearance of the trachea and the main bronchi on the bronchograms is described. Exercise tolerance was impaired in all patients.

CORRECTION OF ANOMALOUS VENOUS RETURN FROM THE RIGHT LUNG TO THE INFERIOR VENA CAVA (SCIMITAR DEFORMITY).

Anomalous pulmonary venous drainage to the inferior vena cava is a rare congenital cardiac defect. Oxygenated blood from the right lung enters the right atrium resulting in a left-to-right shunt. Because the radiographic shadow of the anomalous vein resembles a curved saber, this defect has been called the "scimitar" deformity. From 1958 through June 30, 1975, 11 patients underwent surgical correction of this anomaly at our institution with 10 survivors. Diagnosis was made by routine roentgenography of the chest in all but one patient. Eight patients had total correction which consisted of imp

[Two cases of craniolacunia associated with meningocele and meningoencephalocele (author's transl)-a1].

Craniolacunia (lacunar skull, Luckenschadel) is characterized by multiple, round or oval, radiolucent defects, sharply separated by dense strip of bone (honey comb like configuration) which tend to cluster in the cranial vault on plain skull film. Craniolacunia is present at birth and frequently associated with myelomeningocele, encephalocele or other congenital abnormalities of the central nervous system. Patients with carniolacunia have high mortality due to these associated lesions, and to the secondary effects of these neurological lesions. Recently, it is interested that the presence of c

Pulmonary regurgitation in large atrial shunts without pulmonary hypertension.

Seven patients with pulmonary regurgitation (PR), normal pulmonary artery (PA) pressures and large left-to-right atrial shunts are reported. Six had secundum atrial septal defects (ASD) and one had anomalous pulmonary venous drainage. These comprised 4% of 180 patients with atrial shunts and normal PA pressures. Pulmonary regurgitation was diagnosed clinically by mid-frequency diastolic decrescendo murmurs beginning after the pulmonic component of the second heart sound, and diagnoses were confirmed by catheterization. In two patients who had serial preoperative catheterizations over 8 and 16

Studies of malformation syndromes of man XXXXI B: nosologic studies in the Hanhart and the Möbius syndrome.

We reviewed etiologic and phenotypic aspects of those orofacial and limb anomalies usually diagnosed as Hanhart syndrome and Mobius syndrome but also those described, among others, under names such as aglossia-adactylia syndrome, gloss-palatine ankylosis, ankyloglossia superior, peromelia and micrognathia, cleft palate/lateral synechiae syndrome, and the Charlie M. syndrome. By coding the degree of severity of the limb defects it was possible to compare these cases quantitatively and to determine the nosologic significance of associated cranial nerve palsies and chest abnormalities. We analyze

Surgical treatment of congenital valvular aortic stenosis.

Twenty-two patients with congenital valvular aortic stenosis were surgically treated between 1967 and July 1975. Five (23%) were under 1 year of age (group I) and 17 (77%) were between 2 and 24 years (group II). All infants exhibited severe congestive heart failure and electrocardiographi (ECG) evidence of left ventricular hypertrophy (LVH) with strain pattern. In group II, angina was present in three cases, syncope and fatigue in two; the ECG indicated LVH in 10 cases (59%) with strain pattern in five (29%). A bicuspid aortic valve was present in 77% (17/22) of the cases; 32% had other cardia

Stenotic semilunar valve in persistent truncus arteriosus.

The clinical, hemodynamic, and pathologic findings in two newborn infants with persistent truncus arteriosus and stenosis of the truncal valve are described. In one case the anatomic features of the basic condition were classic, with a dysplastic semilunar valve which was mainly stenotic and also incompetent, while in the other the truncus arteriosus arose exclusively from the right ventricle and was almost exclusively stenotic. A ventricular septal defect was the only outlet for the left ventricle. In this case, mitral stenosis was also present and associated with a left-to-right shunt at the

Interpreting cardiac catheterization data. Part 2.

Data obtained by cardiac catheterization, properly interpreted, are an extremely useful supplement to information obtained from the medical history, physical examination, electrocardiography, chest x-ray examination, and other noninvasive diagnostic techniques. Elevations of pressure in the cardiac chambers and great vessels and corresponding patterns of pressure curves are discerned in the presence of various pathologic conditions affecting the heart. Blood samples are taken during cardiac catheterization for measurement of cardiac output and to detect intracardiac shunting. Attempts have bee
