Isolated anomalous right ventricular muscle bundle in the asymptomatic adult.

Muscular obstruction to right ventricular outflow is usually at the infundibular level, but, less commonly, it can occur within the body of the ventricle due to an anomalous muscle bundle. This lesion is almost always associated with a ventricular septal defect or other cardiac lesions, and is usually discovered in infancy or childhood. Two asymptomatic adults with severe obstruction due to right ventricular muscular band as an isolated lesion are presented. The lesion can be tentatively diagnosed on clinical grounds and confirmed by angiocardiography. The advisability of surgical correction i

Infarcted extralobar pulmonary sequestration.

A 32-year-old man who presented with symptoms suggesting empyema was found at thoracotomy to have infarction of an extralobar pulmonary sequestration. He was completely relieved by excision of the sequestration after securing the systemic vascular pedicle which had undergone torsion. Recognition of the anomalous bloody supply and other congenital anomalies in such cases is emphasized.

A family of Holt-Oram syndrome.

A family of Holt-Oram syndrome is reported. Twenty of 41 living members in the last 3 generations were examined. The propositus expressed typically the cardinal features of this syndrome: atrial septal defect, patent ductus arteriosus, cardiac arrhythmias and conduction block of various types, and hypoplasia of the left thumb. Two individuals were clinically diagnosed to have atrial septal defect without any upper limb anomaly, and 3 showed upper limb anomaly without any sign of congenital heart disease. Other 5 members showed only minor abnormalities, such as funnel chest or mild right ventri

Adult deficiencies and duplications of head and thoracic structures resulting from microcautery of blastoderm stage Drosophila embryos.

Drosophila embryos were damaged by microcautery at the cellular blastoderm stage at the sites of presumptive imaginal cells, identified from fate maps. The resulting adults were analyzed for abnormal structures. Cautery of any of the presumptive imaginal regions can lead to defects in the adult cuticle, though the majority of adults which hatch are morphologically normal. The abnormal adults had one or more discs either deleted, incompletely formed, or misarranged. Several of the structures which were incomplete had duplicated regions. The results suggest that, from the time fo their initial f

The hyperperfused lung. Detection in congenital heart disease.

Thirty-nine children with various cardiac abnormalities were examined with perfusion lung scintigraphy with technetium Tc 99m macroaggregated albumin. Nuclide accumulation within each lung, which is proportional to the division of pulmonary arterial flow, was determined. Twenty-six patients had hyperperfusion of one lung, with a particularly high incidence occurring with the tetraoloy of Fallot, pulmonary artery stenosis or atresia, and following surgical systemic-pulmonic anastomoses. Consistent detection of imbalance in pulmonary flow by inspection of chest x-ray films was possible when one

The lungs in lymphangiomyomatosis and in tuberous sclerosis.

Two cases of pulmonary lymphangiomyomatosis (PL) are described and 33 other cases from the literature are reviewed. These are compared with one case of tuberous sclerosis with pulmonary involvement (PTS) and 32 other cases from the literature. There are no differences in lung function between these two conditions, both of which show airways obstruction associated with diffuse radiological lung changes. There are, however, both clinical and radiological differences and also differences in the distribution of the lesions and the histological location of the excessive smooth muscle; these indicat

Congenital localized stenosis of the pulmonary veins.

One case of localized stenosis of the extrapulmonary portion of pulmonary veins is reported in a 3 years old child. The chest X-Ray showed a rounded and pulsatile opacification over the right hilum and on angiocardiography the right superior pulmonary vein looked very dilated. 20 other cases of segmental stenoses of pulmonary veins were collected from the literature.

[Congenital heart diseases in newborns and infants; early detection, differentiation and accuracy of clinical diagnoses (author's transl)].

In order to facilitate for the general physicians the making of a suitable selection of babies who are in the most urgent need of specialized treatment at cardiac centres, simple methods for diagnosing and qualifying congenital cardiovascular diseases were elaborated. The following "minor" criteria were taken for suspecting a CHD: 1) cardiorespiratory distress following birth, 2) sequentially repeated Apgar score below normal, 3) "pneumonia" symptoms with respiratory distress, dyspnoea and cyanosis, attacks of unconsciousness, 4) feeding difficulties, failure to thrive, inexplicable irritabili

Congenital posterolateral diaphragmatic hernia in the adult.

Five adult patients with the Bochdalek type of congenital diaphragmatic hernia are presented. Four of the hernias were on the left side and one on the right. One of the patients was asymptomatic, while 3 of them had gastrointestinal symptoms. In 2 of these 3 cases, the symptoms became worse when the patients were lying down. One patient complained mostly of dyspnoea, and also had a patent ductus arteriosus. The other anomaly, in addition to hernia, was diaphragmatic pulmonary sequestration. All the patients were operated on transthoracally. In one case, laparotomy was also performed. There was

Unilateral congenital dysplasia of lung associated with vascular anomalies.

Hislop, A., Sanderson, M., and Reid, L. (1973).Thorax, 28, 435-441. Unilateral congenital dysplasia of lung associated with vascular anomalies. In three cases of unilateral lung dysplasia the structural changes have been analysed quantitatively to throw light on the nature of the anomaly and stage of its appearance. In one case the pulmonary artery was hypoplastic and the systemic supply normal but increased, suggesting onset in late intrauterine life or early childhood: in the other two cases the blood supply was only systemic, from both normal and abnormal arteries, suggesting onset in the e

Partial left pericardial defect with herniation of the left atrial appendage.

A case is reported of herniation of the left atrial appendage through a partial pericardial defect, probably congenital. The diagnosis was suggested by the history of chest pain and bulging of the middle segment of the left heart border on the plain chest film, without other signs. Angiography revealed a dilated left atrial appendage. An artificial left pneumothorax confirmed the presence of a pleuropericardial defect. The surgical procedure included excision of the appendage and closure of the defect.

Serum aspartate transaminase changes following open-heart surgery with ischaemic arrest (with and without coronary artery perfusion).

Patients with aortic disease irrespective of whether they had coronary perfusion or not showed a significant difference in the level of postoperative SGOT levels compared with other groups. Aortics with coronary perfusion did not show a correlation between SGOT levels and total perfusion time while the other groups did. The levels were not of prognostic significance.

Congenital mitral incompetence and coarctation of aorta. Report of two cases treated surgically.

Two patients with congenital mitral incompetence and coarctation of the aorta are presented. One patient had associated patent ductus arteriosus, bicuspid aortic valve, and endocardial fibroelastosis. The diagnosis in the two patients presented is well established by clinical, laboratory, and surgical findings and also by necropsy examination in one case. It is proposed that the rarity of reported cases in the literature may have resulted from the frequent diagnosis of left ventricular failure in infancy secondary to coarctation, leading to the assumption that a mitral insufficiency murmur, wh

Spina bifida cystica. Results of treatment of 270 consecutive cases with criteria for selection for the future.

The disappointing results of treatment in 270 consecutive unselected cases of spina bifida admitted during a 27-month period are detailed. Massive effort has led to much avoidable suffering at an exorbitant cost in manpower and money. This study confirms the validity of those adverse prognostic criteria defined in an earlier study and which form a basis for selection. These are (1) thoracolumbar lesions, (2) severe paraplegia, (3) gross enlargement of head, (4) kyphosis, and (5) other severe congenital defects, or birth injuries. It is shown again that selection is possible on the first day of

Patent ductus arteriosus in infants and children. A review of 936 operations (1946-69).

Nine hundred and thirty-six consecutive cases of closure of a patent ductus arteriosus in infants and children are reported. Among 789 without any other cardiac anomalies there were 11 deaths, seven of these with severe congenital anomalies other than cardiovascular. There was one incomplete closure. The deaths in this series were mostly associated with additional congenital cardiac anomalies. Forty-eight of the 59 who died had additional cardiac anomalies. Forty-seven of the deaths were in infants. For 691 patients over the age of 1 year the hospital mortality rate was less than 0·5%. Ligatio

Congenital complete absence of left pericardium. Report of two cases.

Two cases of congenital complete absence of the left pericardium are presented. One, in a youth aged 17 years, had an associated patent ductus arteriosus; the other occurred in a woman aged 65 years with carcinoma of the lower third of the oesophagus. In both, the pericardial lesion was discovered as an incidental finding—at thoracotomy. The anatomical and radiographic features are described. Complete absence of the left side of the pericardium needs no surgical treatment and is compatible with normal life expectancy. On the other hand, because of the dangers of herniation of the heart with st

Extralobar pulmonary sequestration.

Extralobar sequestration of pulmonary tissues is relatively rare and seldom produces symptoms. An incidental finding is described of extralobar sequestration of the lung in a Nigerian child, presenting as an anterior mediastinal polycystic mass. The mass was attached to the right pulmonary artery by a fibrovascular pedicle which contained a small elastic artery, veins, and nerve bundles. Sequestrated lobes are usually situated in the posterior basal lobes of the lungs and may be associated with other congenital abnormalities, but the case reported here differs in these respects. Death was due
