BACKGROUND: Bladder exstrophy is a congenital malformation occurring in fetal development characterized by an open pelvic ring and inferiorly rotated pelvis, impacting typical gait and development. Little is known about the gait patterns in children with bladder exstrophy, especially if they undergo an osteotomy surgery for pelvic reconstruction.
OBJECTIVE: This study aimed to investigate gait patterns of children with and without bladder exstrophy, as well as children with bladder exstrophy with and without osteotomies.
METHODS: Fifteen participants with bladder exstrophy (mean age 10.6 y;

UNLABELLED: Coffin-Siris Syndrome (CSS) is a neurodevelopmental disorder caused by variants in genes encoding BRG1- and BRM-associated factor (BAF) chromatin-remodeling complex. ARID1B gene variants are the most common cause of CSS. This study aimed to identify novel pathogenic ARID1B variants in patients clinically diagnosed with CSS and to explore their pathogenic role. In this study, eight patients clinically diagnosed with CSS were enrolled, and whole exome sequencing (WES) was performed to identify potential pathogenic variants. Heterozygous variants in the ARID1B gene were identified in

The SMARCA4 gene encodes a catalytic subunit of the BRG1/BRM-associated factor complex, which regulates gene expression through chromatin remodeling. Heterozygous missense variants in this gene have been linked to Coffin-Siris syndrome, characterized by intellectual development disorder and various congenital anomalies (distinctive facial features, hypoplastic fifth digits, and malformations of the heart and central nervous system), but it is not typically associated with structural eye anomalies. Truncating variants in SMARCA4 have been associated with rhabdoid tumors predisposition syndrome,

SMARCB1 encodes a core component of the BAF chromatin remodelling complex and pathogenic variants in this gene are associated with neurodevelopmental disorders such as Coffin-Siris syndrome and intellectual disability with choroid plexus hyperplasia. The relationship between reduced or dysfunctional SMARCB1 protein products and severe functional brain changes associated with Coffin-Siris syndrome, including intellectual disability and severely delayed language and motor development, remains largely unknown. We performed cellular, molecular, and behavioural analyses of a Coffin-Siris syndrome m

Coffin-Siris syndrome 6 is a rare autosomal dominant genetic disorder characterized by short stature, intellectual disability, and distinctive facial features. In this study, we reported a child with Coffin-Siris syndrome 6 accompanied with liver cirrhosis, which was caused by a novel nonsense variant c.4771C > T in the ARID2 gene. This variant leads to the premature generation of a stop codon. In vivo experiment has confirmed that this variant leads to downregulation of ARID2 protein expression. The potential association between liver phenotype and ARID2 gene variant in the patient requires f

BACKGROUND: Existing classifications for forearm deformity in multiple hereditary exostoses (MHE) have demonstrated low interrater reliability and limited clinical applicability. Our primary research question was whether a Delphi consensus process could generate a more reliable and clinically useful classification for longitudinal surveillance of MHE-related forearm deformities.
METHODS: Fifteen pediatric hand surgeons from the Congenital Upper Limb Difference (CoULD) Study Group participated in 7 rounds of online Delphi surveys (June 2022 to September 2023). An investigator team facilitated

Meningomyelocele is a congenital defect resulting from the incomplete closure of the neural tube during embryonic development, making it the most common structural birth anomaly of the central nervous system. Over time, these deformities can progress to the knee and ankle joints due to the development of contractures. In addition, rotational deformities of the lower extremities are also seen. This study outlines a mid-childhood female with a complaint of fixed knee flexion and a spastic equinovarus deformity that has been affecting her walking and performing activities of daily living for the

A male infant was admitted to the neonatal intensive care unit for respiratory distress 12 minutes after preterm birth. Prenatal fetal echocardiography suggested transposition of the great arteries with an intact ventricular septum. Amniocentesis and family-based whole-exome sequencing identified a heterozygous missense variant in SOX11 (c.152G>C, p.Arg51Pro), leading to a diagnosis of Coffin-Siris syndrome type 9. Postnatal echocardiography and computed tomography angiography confirmed Berry syndrome. Single-stage corrective surgery was performed with good recovery. This report presents the f

Geleophysic dysplasia type 1 is a rare skeletal dysplasia caused by biallelic pathogenic variants in ADAMTSL2. Affected children typically show a "happy-natured facial appearance," postnatal short stature with acromelic limb shortening, progressive joint contracture, and skin tightness. Most patients are diagnosed in childhood, and very little is known about the neonatal manifestation. We report a Japanese girl with Geleophysic dysplasia type 1, whose neonatal manifestations were enough to raise suspicion of the disorder on retrospective clinical and radiological review. At birth, she presente

UNLABELLED: The purpose of this study is to compare the natural history of clinical and radiologic features in patients with acromelic dysplasias. Twelve patients from nine families with genetically confirmed dysplasia types with acromelia were included in the study, and eight of them were followed-up for a median of 8.1 years. Monoallelic disease-causing variants were identified in FBN1 (acromicric dysplasia, n = 3) and GNAS (Albright hereditary osteodystrophy (AHO), n = 1). Biallelic disease-causing variants in ADAMTSL2 (geleophysic dysplasia type 1, n = 4), ADAMTS10 (Weill-Marchesani syndro

Pathogenic variants in BAF complex genes are linked to neurodevelopmental disorders, termed BAFopathies. Coffin-Siris syndrome (CSS) is the most well-defined BAFopathy caused by variants in several BAF subunit genes. ARID1A variants are responsible for 6-8% of CSS cases. We report a female individual with clinical and phenotypic manifestations suggestive of CSS. Trio exome sequencing failed to identify a causative variant. Short-read genome sequencing revealed a de novo 24 kb intronic deletion in ARID1A. Methylation analysis showed a BAFopathy-like profile supporting pathogenicity. Transcripto

BACKGROUND AND PURPOSE: Several European countries offer nationwide prenatal ultrasound screening for fetal anomalies. Few studies have examined prenatal detection of upper limb anomalies, and existing reports originate exclusively from high‑volume centers outside Europe. We aimed to assess the timing of diagnosis in patients with all Oberg-Manske-Tonkin diagnoses except tumorous dysplasias referred to the largest specialized unit in Norway, investigate prenatal detection rates, and explore associations between patient‑ and hospital‑related factors and prenatal detection.
METHODS: We extracte

Myhre syndrome is a rare autosomal-dominant disorder caused by gain-of-function pathogenic variants in SMAD4 and is now recognized as a progressive multisystem fibrotic disease. Although transforming growth factor-β (TGF-β) signaling plays a central role in hepatic fibrogenesis, hepatobiliary involvement in Myhre syndrome has not been systematically evaluated. We report the first case of Myhre syndrome complicated by rapidly progressive biliary cirrhosis requiring liver transplantation in a 15-year-old male with a confirmed SMAD4 p.Ile500Val variant. Following an infectious episode, the patien

BACKGROUND: Total hip arthroplasty (THA) for secondary osteoarthritis related to developmental dysplasia of the hip (DDH) restores hip mechanics. We aimed to quantify pre-to-postoperative changes in ipsilateral knee and global lower-limb alignment after unilateral THA to determine whether alignment changes differ by Crowe classification and femoral stem geometry.
METHODS: This retrospective cohort study included 74 patients who underwent unilateral primary THA for DDH-related secondary osteoarthritis, with a mean follow-up of 45.4 months. The predefined primary radiographic outcome set compri

Cardiotrophin-like cytokine factor 1 (CLCF1) is an essential gene that shows exceptional sequence conservation among vertebrates. CLCF1 loss-of-function mutations cause Crisponi/cold-induced sweating syndrome-2, a disorder with severe and complex phenotypes, underscoring the non-redundant roles of CLCF1. Initially identified as a member of the interleukin-6 (IL-6) cytokine family with activity in neurons and immune cells, CLCF1 is now recognized for broader roles in thermoregulation, glomerular function, myelopoiesis, oncogenesis, and muscle fitness. Despite growing interest in the multifacete

BACKGROUND: Supination posture is a common secondary deformity in brachial plexus birth injury (BPBI) that results in functional upper limb impairment. Radioulnar synostosis using a vascularized ulnar periosteal flap has recently been reported, with dorsal forearm scarring as the main reported disadvantage. This report evaluates outcomes in a series of children with forearm supination deformity and complete pronation palsy secondary to severe BPBI undergoing radioulnar synostosis in slight pronation using a novel proximal pedicled vascularized radial periosteal flap (VRPF).

Van Maldergem Syndrome (VMS) is a rare autosomal recessive disorder caused by pathogenic variants in the atypical cadherin genes DCHS1 or FAT4 and is marked by craniofacial, skeletal, and neurodevelopmental abnormalities. Although DCHS1-FAT4 binding is mediated by their respective extracellular domains, the in vivo function of the DCHS1 intracellular domain (ICD) is poorly defined. To test its function, we generated mice in which the DCHS1 ICD was deleted and replaced with a V5 epitope tag (Dchs1ΔICD-V5). Homozygous Dchs1ΔICD-V5/ΔICD-V5 mice are viable but exhibit VMS-like craniofacial flatten
