BACKGROUND: Split-hand/foot malformation (SHFM) is a serious congenital anomaly. A multitude of pathogenic genes associated with SHFM have been identified; TP63, DLX5, FGFR1, and WNT10B are currently recognized. As a transcription factor, TP63 plays an important role in the development of the ectoderm.
METHODS: We identified a pathogenic variant by extracting DNA from the peripheral blood of a patient with SHFM and performing whole-exome sequencing (WES). After verifying the variant through Sanger sequencing, a series of analytical procedures were conducted to determine its pathogenicity. The

Pediatric hand surgery frequently requires precise surgical reconstruction of the upper limb to prevent worsening deformity. However, there is a balance of achieving a perfect radiologic film of the planned surgery and causing inadvertent harm from performing overly aggressive surgery. Precise K-wire placement and immobilization are important for optimal results; however, surgery that is too invasive results in several downsides, including loss of reduction, poor bony healing, loss of blood supply, growth plate failure, and scar contractures from poor wound healing. Here, we review this import

Pectus excavatum (PE) is the most common congenital anterior chest wall anomaly, often requiring surgical correction due to both functional and aesthetic concerns. Gender-affirming surgery has seen a significant increase in recent decades, with mastectomy being the primary procedure for transgender men. For transmasculine patients with PE seeking chest masculinization, the possibility of addressing both conditions in a single surgery has not been well explored. We report the first known case of a 33-year-old transgender patient with PE undergoing a combined procedure of masculinization top sur

BACKGROUND: Sprengel deformity is a rare congenital malformation of the scapula. To date, there have been limited reports analyzing the clinical outcomes of the modified Woodward procedure and conservative management in the treatment of Sprengel deformity. This study aims to further investigate the clinical efficacy of these 2 treatment strategies.
METHODS: A retrospective analysis was conducted on 28 patients (30 shoulders) with Sprengel deformity treated at our institution between 2013 and 2024, of whom 15 underwent the modified Woodward procedure. Shoulder function, appearance, and scapula

Sprengel's deformity is a rare congenital musculoskeletal condition characterized by elevation and malposition of the scapula, leading to cosmetic deformity and functional limitations of the shoulder complex. The condition is commonly associated with restricted scapulothoracic mobility, reduced glenohumeral range of motion, muscle weakness, and impaired upper-limb function, which may affect activities of daily living and quality of life. This case report describes the clinical presentation, radiological findings, and physiotherapy management of a patient with Sprengel's shoulder. A structured

The intersection of art and orthopaedic surgery represents a profound synthesis of technical precision, aesthetic sensibility, and humanistic care. This narrative review examines the artistic dimensions of orthopaedic surgery, with particular emphasis on hand and upper extremity procedures in which form and function are inextricably linked. We trace the historical evolution of surgical artistry from Renaissance anatomists and craftsmen, including Ambroise Pare, whose articulated prosthetic hands united the ingenuity of locksmiths with the vision of surgeons, through nineteenth-century operativ

Metacarpal synostosis is a rare congenital hand anomaly characterized by abnormal fusion between adjacent metacarpals. Although most commonly involving the fourth and fifth metacarpals, atypical fusion patterns may occur. We report a rare case of Y-shaped synostosis between the third and fourth metacarpals in a 17-year-old man and describe surgical correction using 3 triangular osteotomies. Realignment and stabilization were achieved with mini plates and screws. At 1-year follow-up, correction of angular deformity and digital shortening was maintained, with satisfactory functional and cosmetic

INTRODUCTION: Macrodactyly is a rare congenital malformation characterized by the overgrowth of all mesenchymal elements in one or more digits of the hands or feet. Although diagnosis typically occurs at birth, prenatal detection via ultrasound has been reported. The condition imposes a significant psychological burden on children and their families. Pediatricians are often the first clinicians to inform and counsel anxious parents; accurate recognition of this malformation and clear communication regarding the multidisciplinary approach, clinical evolution, and overall prognosis are therefore

EHMT1 and EHMT2 genes encode human euchromatin histone lysine methyltransferase 1 and 2 (EHMT1 alias GLP; EHMT2 alias G9a) that form heteromeric GLP/G9a complexes with essential roles in epigenetic regulation of gene expression. While EHMT1 haploinsufficiency has been established as the cause of Kleefstra syndrome 1, the pathogenesis of G9a dysfunction in human disease remains largely unknown. We identified seven de novo EHMT2 variants in patients with clinical presentation, episignatures, histone modifications and transcriptomic profiles similar to those of Kleefstra syndrome 1. In vitro stud

Congenital deformities of the digits can greatly impact hand function, aesthetics, and psychosocial well-being. Given the hand's crucial role in fine motor activity and social interaction, even subtle abnormalities can prompt clinical evaluation. This chapter provides a comprehensive overview of five congenital digit deformities (camptodactyly, clinodactyly, brachydactyly, macrodactyly, and symphalangism), with emphasis on their clinical presentation, underlying etiologies, diagnostic evaluation, and management strategies.

The thumb accounts for approximately 40% of overall hand function, and deformities carry significant implications for pediatric hand development and quality of life. This article reviews three distinct congenital and acquired thumb deformities: pediatric trigger thumb, triphalangeal thumb, and congenital clasped thumb. Pediatric trigger thumb, an acquired condition resulting from a size mismatch between the flexor pollicis longus tendon and the A1 pulley, is staged using the Sugimoto classification and can resolve spontaneously, though surgical release of the A1 pulley reliably restores full r

BACKGROUND: Preaxial polydactyly is a common congenital hand anomaly with substantial anatomic variability. Although several classification systems exist, the relationship between anatomic subclassification and postoperative outcomes remains incompletely defined. This study evaluated clinical and radiographic outcomes after surgical reconstruction, compared associations of the Wassel-Flatt (WF) and JK classifications with outcomes, and assessed the Japanese Society for Surgery of the Hand (JSSH) and Rotterdam scoring systems.
METHODS: We retrospectively reviewed 58 patients (59 thumbs) treate

BACKGROUND: Congenital split foot/hand is a rare limb anomaly. Although various surgical techniques have been described, detailed gross anatomical studies of soft tissue adaptation, particularly in the foot, are extremely rare. This study presents a detailed anatomical description of a case of severe bilateral split foot.
METHODS: A comprehensive dissection was performed on the lower limb of a 64-year-old male donor with bilateral split foot/hand.
RESULTS: Radiographic evaluation classified the deformity as Blauth type IV, characterized by the absence of the lateral cuneiform bone and severe

Myhre syndrome is a rare genetic disorder characterized by progressive multisystem involvement. Gain-of-function missense heterozygous variants affecting the Ile500 residue and Arg496 residue of the SMAD4 gene are implicated in this condition. In this article, we aim to understand the spectrum of congenital anomalies in Myhre syndrome by studying a cohort of previously unreported patients alongside published literature. Our analysis revealed that the musculoskeletal system was the most common system to be affected, followed by the cardiovascular system. Intrauterine growth restriction was the

Poland syndrome (PS) is a rare, non-hereditary congenital malformation characterized primarily by hypoplasia or absence of the pectoralis major muscle on one side. Key clinical manifestations include hypoplasia of the breast and nipple on the same side, which may be accompanied by hand deformities on the same side. In extremely rare cases, the syndrome affects the ribs, leading to rib hypoplasia and resulting in ipsilateral diaphragmatic displacement. The pathogenesis is likely related to disruption of the subclavian artery blood supply during embryonic development. Prenatal diagnosis of this

INTRODUCTION: The human forearm is a finely tuned system in which the radius and ulna act as an interdependent pair to enable stability, mobility and efficient load transfer. Disruption through trauma, malunion, congenital deformity or failed surgical intervention produces imbalance, presenting with restricted motion, instability, pain, and functional loss. Advances in three-dimensional (3D) imaging, planning, and printing now allow for more accurate analysis and more precise correction of forearm deformities.Reconstruction:Restoration of bone morphology alone does not ensure functional recove

Myhre syndrome is a rare multisystem disorder characterized by distinctive facial features, hearing loss, and progressive fibrosis affecting the skin, joints, lungs, and cardiovascular system. It is caused by heterozygous pathogenic variants in the SMAD4 gene. In this review, we detail the cutaneous manifestations observed in two previously reported patients with genetically confirmed Myhre syndrome. While skin biopsies demonstrated pan-dermal thickening of collagen bundles, the immunohistochemical staining patterns were distinct from those seen in other inflammatory sclerosing disorders. Addi

Aseptic nonunion is prevalent in orthopedic practice, causing persistent pain and functional impairment. Humeral shaft fractures, accounting for 3-5% of all fractures, have nonunion rates of 2-33% in nonoperative and 5-10% in surgical management. This study, the largest case series on operative management of humeral shaft nonunion, examines surgical techniques and outcomes. This retrospective study, conducted at a referral center in eastern India, included 132 patients with aseptic humeral shaft nonunion from 2002 to 2020. Cases were from May 2002 to April 2012 and May 2015 to December 2020. P
