Molecular Dynamics Simulations of 4 GALC Variants Causing Krabbe Disease.

Krabbe disease, also known as globoid cell leukodystrophy, is a rare lysosomal disorder affecting the white matter of the central and peripheral nervous systems. It is characterized by neurodegeneration, with infantile-onset Krabbe disease as the most common form, typically diagnosed within the first year of life and associated with high morbidity and mortality. This autosomal recessive disease is caused by mutations in the GALC gene, which encodes the lysosomal enzyme β-galactocerebrosidase (GALC). This study focuses on examining the structural effects of 4 GALC variants (Gly59Arg, Ser68Phe,

Impaired IFN-γ-mediated innate and adaptive immunity in Coffin-Siris syndrome type 2: immunological insights from a patient with a recurrent ARID1A mutation.

BACKGROUND: Coffin-Siris syndrome type 2 (CSS2), caused by ARID1A mutations, is characterized by neurodevelopmental delay and recurrent infections. The ARID1A p.Ala1077Glu variant has been linked to CSS2, but the immune mechanisms underlying infection susceptibility remain poorly understood. METHODS: The variant was identified by trio-based whole-exome sequencing and confirmed by Sanger sequencing. Immune status was assessed via serum immunoglobulins, complement measurements, and flow cytometric immunophenotyping. RNA sequencing was performed on PMA-stimulated PBMCs from the patient and healt

Madelung Deformity: A Current Concepts Review.

Madelung deformity is a rare congenital wrist deformity characterized by growth disturbance of the volar-ulnar aspect of the distal radial physis, leading to progressive wrist deformity, pain, and stiffness. Madelung deformity is frequently associated with genetic conditions such as Leri-Weill dyschondrosteosis and Turner syndrome, and more recently, mutations in the GNAS gene have been implicated. Diagnosis is based on clinical findings and radiographic criteria, with plain radiographs demonstrating characteristic volar-ulnar radial curvature, positive ulnar variance, and carpal subluxation.

Expanding the Coffin-Siris syndrome spectrum: genetic, dysmorphic, and endocrine findings in eight cases.

UNLABELLED: This study aims to expand the spectrum of Coffin-Siris syndrome (CSS), a rare and heterogeneous disorder, by thoroughly discussing its genetic, dysmorphic, and endocrine features through new cases and contributing to the literature. Eight patients who were referred to the genetics clinic with various complaints and subsequently diagnosed with CSS through microarray or clinical exome sequencing analyses were included in the study. The dysmorphic, genetic, and endocrine characteristics of eight genetically confirmed patients were evaluated. The patients, aged between 5 months and 6 y

Review of the Molecular and Developmental Basis of Myhre Syndrome, Bench Research.

Myhre syndrome (MS) is a connective-tissue disorder within the acromelic dysplasia spectrum. It is characterized by congenital craniofacial, skeletal, cutaneous anomalies, respiratory, cardiovascular along with intellectual disability, deafness, and progressive fibrosis. MS is caused by germline variants in the transcriptional co-regulator SMAD4 (mothers against DPP homolog 4). SMAD4 is a key intracellular mediator of signal transduction and participates in several pathways, notably canonical TGFβ and BMP signaling. These pathways play central roles in developmental and homeostatic processes.

Clinical presentations of macrodactyly in adults vs. children: a descriptive analysis.

INTRODUCTION: Macrodactyly of the hand is an uncommon anomaly. There are few studies looking at late presentation or adults with macrodactyly. This study specifically aims to describe the presentation and surgical management of adult macrodactyly and to compare these findings with those in children, highlighting the distinct challenges associated with late presentation. METHODS: This is a retrospective study of 13 adults (18 years or older) and 19 children with macrodactyly of the upper limb who underwent surgery in a 13 year period. Clinical features including radiographic findings, number a

ARID1B regulates sphingolipid metabolism and myelin development via STAG2: Mechanistic insights into ARID1B-related coffin-siris syndrome.

OBJECTIVES: Coffin-Siris syndrome (CSS) is predominantly attributed to variants in ARID1B gene, however, the molecular pathways connecting ARID1B to myelination and neural development are not well elucidated. METHODS: We employed ARID1B knockdown in SK-N-SH cells coupled with transcriptomic analysis to identify downstream targets. We conducted quantitative Real-Time PCR, Western blotting, chromatin immunoprecipitation (ChIP) and co-immunoprecipitation (Co-IP) assays to investigate the regulatory mechanisms of ARID1B. The clinical phenotypes of three CSS patients with pathogenic ARID1B variant

Posthumously Diagnosed Myhre Syndrome Presenting With Pleural Remodeling and Endometrial Cancer.

Myhre syndrome (OMIM 139210) is a genetic condition defined by neurodevelopmental disability, characteristic facial features, and multisystem proliferative fibrosis. While various types of lung disease have been reported, pleural remodeling leading to restrictive lung disease has not yet been described. There is also a possible increased risk of malignancy, although this is difficult to confirm given the rarity of the condition. A 43-year-old woman with short stature, intellectual disability, recent unintentional weight loss, and a uterine mass was admitted to the intensive care unit for respi

Staged Centralisation and Pollicisation for Radial Longitudinal Deficiency: A Case Series Emphasising Pre-pollicisation Index-Middle Finger Pinch Training.

Background: Radial longitudinal deficiency (RLD) is a rare congenital malformation that severely impairs grasp and pinching due to radial ray and thumb hypoplasia or aplasia. Standard management typically involves early centralisation to align and stabilise the wrist, followed by pollicisation when the thumb cannot be reconstructed. However, little attention has been paid to pre-pollicisation-rehabilitation strategies that might facilitate the acquisition of postoperative pinch function. This study aimed to report early functional outcomes after staged reconstruction for severe RLD, highlighti

A de novo SOX11 mutation causing hypogonadotropic hypogonadism: a case report and literature review.

BACKGROUND: SOX11 variants have been associated with Coffin-Siris syndrome and a broader neurodevelopmental spectrum (IDDMOH), but their role in isolated or syndromic hypogonadotropic hypogonadism (HH) remains under-recognized. CASE PRESENTATION: We report a 13-year-old Chinese girl who presented with delayed puberty (Tanner B1, PH1), infantile uterus and low basal gonadotropins. Trio whole-exome sequencing identified a de-novo heterozygous SOX11 c.346_348del (p.Tyr116del) variant. Olfactory and pituitary MRI revealed bilateral olfactory-bulb/nerve hypoplasia and a pituitary height of 3.4 mm,

Camptodactyly-arthropathy-coxa vara-pericarditis (CACP) syndrome masquerading as juvenile idiopathic arthritis: a case series with a novel PRG4 variant and emphasis on bone health.

BACKGROUND: Camptodactyly-arthropathy-coxa vara-pericarditis syndrome is a rare autosomal recessive disorder caused by loss-of-function variants in the proteoglycan 4 gene, which encodes lubricin. It is frequently misdiagnosed as juvenile idiopathic arthritis, leading to prolonged immunosuppressive therapy. CASE PRESENTATION: We report two unrelated boys born to consanguineous parents who presented with progressive large-joint swelling and were initially diagnosed with juvenile idiopathic arthritis. Both received multiple disease-modifying and biologic agents without clinical response. Inflam

PRG4-Related Camptodactyly-Arthropathy-Coxa Vara-Pericarditis Syndrome Mimicking Juvenile Idiopathic Arthritis: A Case-Based Review.

Juvenile idiopathic arthritis (JIA) represents the most common cause of chronic arthritis in childhood; however, not all early-onset arthropathies are inflammatory in origin. We report the case of a 4-year-old girl initially diagnosed with oligoarticular JIA and treated with methotrexate followed by a tumor necrosis factor inhibitor, without significant clinical improvement and despite persistently normal inflammatory markers. Clinical reassessment raised suspicion of a non-inflammatory arthropathy, supported by characteristic radiographic findings including metaphyseal flaring of the distal f

Perioperative management in primary palatoplasty-an international survey.

Orofacial clefts are common congenital defects. Primary palatoplasty aims to address the functional problems associated with the underlying deformity and is necessary for normal speech acquisition. International consensus on the perioperative management of primary cleft palate surgery is limited. A 34-question online survey was disseminated via email between June and September 2022, to identify differences and similarities in the perioperative management of children undergoing primary palatoplasty. Overall, 297 cleft surgeons were invited to take part, of whom 72 (24.2%) responded to the surve

SMAD4-BPIFA1 Axis Governs Airway Antiviral Defense in Myhre Syndrome.

Myhre syndrome is a rare autosomal-dominant disorder caused by gain-of-function (GOF) variants in SMAD4. Although it is primarily recognized as a fibrotic disease, we and others have identified respiratory infection-associated morbidity as a major clinical burden. Regardless, the epithelial-intrinsic mechanisms underlying increased viral susceptibility in Myhre syndrome remain poorly understood. Here, using patient-derived nasal airway epithelial cells (NECs) and respiratory syncytial virus (RSV) infection as a model, we demonstrate that infection is markedly exacerbated in Myhre syndrome epit
