Positive effects of strength training on dynamic muscle function in adults with Fontan circulation: a pilot study.

BACKGROUND: Impaired muscle function, aerobic capacity, and fatigue are common in individuals with Fontan circulation. Knowledge regarding the effects of strength training in this population is limited. Therefore, the study aimed to investigate the effects of strength training on dynamic muscle function, aerobic capacity, and fatigue in adults with Fontan circulation compared to matched controls. METHODS: In this pilot non-randomised controlled trial, nine patients with Fontan circulation (median age 28.9 years [IQR: 23.4-35.0], 44.4% women) and nine age- and sex-matched controls completed a

The Effect of Flexible Flatfoot on the Running Function in School-Age Children.

Flexible flatfoot is common among school-age children and significantly affects walking efficiency, balance stability, and joint-movement coordination in children. The demands on the skeletal structure and muscle function are increased during running; however, the impact of a flexible flatfoot on children's running capabilities is unclear. In this study, we aimed to investigate the effects of flexible flatfoot on the running function of school-age children. Participants with flat feet (n = 28) and typical feet (n = 27) ran on a flat surface at their chosen maximum pace. At the same time, the k

Aberrant Insertion of the Anterior Cruciate Ligament on the Lateral Meniscus: A Case Report.

CASE: A 16-year-old woman presented with acute on chronic knee pain and instability following a twisting injury. The tibial insertion of the anterior cruciate ligament (ACL) was nonvisualized on magnetic resonance imaging. A cord-like ACL, originating from the lateral intercondylar notch and inserting smoothly into the anterior horn of the intact lateral meniscus, was found on arthroscopy. Her ACL was reconstructed with quadriceps tendon autograft. CONCLUSION: We report a rare case of dysplastic ACL inserting into the lateral meniscus in a patient without obvious skeletal deformities. This pa

Maintained gait in persons with arthrogryposis from childhood to adulthood.

BACKGROUND: Individuals with arthrogryposis multiplex congenita (AMC) exhibit a range of modes of ambulation, from walking independently to requiring a wheelchair. Presence of joint contractures and muscle strength plays a crucial role, and, in some patients, orthoses are necessary to facilitate or enable walking. METHODS: Gait was assessed with a three-dimensional (3D) gait analysis, calculated as a gait deviation index (GDI) of nine kinematic variables, and compared between childhood and adulthood. RESULTS: A total of 12 persons, 8 with community and 4 with household ambulation, who had un

Investigation of Gait Characteristics and Kinematic Deviations in Rare Genetic Disorders with Instrumented Gait Analysis.

BACKGROUND: Dravet Syndrome (DS), Helsmoortel-Van Der Aa Syndrome (HVDAS) and Tuberous Sclerosis Complex (TSC) are rare genetic syndromes, sharing intellectual disability (ID) and motor delay. In DS, two distinct gait patterns, crouch and non-crouch, have been described using instrumented 3D gait analysis (i3DGA). This cross-sectional study measures gait in participants with TSC and HVDAS. The findings are compared to the known crouch and non-crouch gait patterns observed in DS and to typical gait. METHODS: Participants (6-22 years) with DS (n = 37; 19 crouch and 18 non-crouch), HVDAS (n = 12

The impact of wide step width on lower limb coordination and its variability in individuals with flat feet.

Flat foot is a common condition marked by the collapse of the medial longitudinal arch, leading to altered lower limb biomechanics and increased risk of musculoskeletal injuries. We aimed to investigate if wide step width changes the lower limb inter-joint coordination and its variability in flat feet individuals. Twenty flat-footed individuals participated in this cross-sectional study. Lower limb kinematics were assessed by 3-dimensional motion analysis during walking and running on a treadmill with preferred and wide step widths while receiving visual feedback. Inter-joint coordination was

Association between orthopedic manifestations and tethered cord release in patients with spina bifida: a survival analysis.

PURPOSE: Patients with spina bifida (SB) are at risk for symptomatic tethered cord syndrome (TCS). Orthopedic decline, a common manifestation of TCS, is an indication for tethered cord release (TCR). Our objective is to determine if patients with SB who have undergone specific orthopedic operations (release of hip or knee contracture and correction of cavus foot) require TCR at a higher rate than those not undergoing these operations. METHODS: An institutional database was queried to identify all children with SB from 2009 to 2022. Data included functional level of lesion (FLOL), ambulatory s

Proximal femoral focal deficiency/congenital femoral deficiency: a proposal for a new classification.

Congenital femoral deficiency (CFD) or proximal femoral focal deficiency (PFFD) has been a controversial topic to present, describe, classify, and treat. Since 1939, many classifications have been proposed to describe the disease and plan possible treatment - however, no single widely accepted classification. The currently available classification could not fill the gap between description and surgical planning. Some provided a graphical description of the disease with possible treatment plans, yet there is minimal evidence of its reliability and reproducibility. We propose to develop a new cl

Predicting joint loading in Asian overweight and obese females with flexible flatfoot: a regression analysis of anthropometric parameters and gait dynamics.

Current methods for obtaining accurate joint loading data lack simplicity, efficiency, and cost-effectiveness. This study aims to generate joint loading prediction models using anthropometric parameters and walking speed in overweight or obese females with flexible flatfoot. Sixteen participants' motion capture data from walking trails and anthropometric parameters were collected. The lower limb joint contact forces and the walking speed were calculated via a musculoskeletal model. Regression analysis was used to generate the prediction model. The second peak of knee joint contact force reveal

Children with and without relapsed clubfoot show task-specific deviations in lower limb kinematics during several dynamic activities compared to typically developing children.

BACKGROUND: The occurrence and the treatment of a relapsed clubfoot is a challenge in clubfoot care. Early recognition of relapse is important to minimize the invasiveness of treatment and its impact on foot functionality later in life. Gaining insight into a child's functional performance during various challenging activities will provide a comprehensive understanding of treatment outcomes which might result in crucial information for identifying relapse indicators. RESEARCH QUESTION: This study aims to identify kinematic differences during dynamic activities between relapsed and non-relapse

Correlation Between Socioeconomic Status and Brace Compliance in Idiopathic Clubfoot Deformities.

BACKGROUND: Congenital talipes equinovarus (CTEV), or clubfoot, is one of the most common foot deformities seen at birth. The Ponseti technique is the most common method to treat clubfoot and consists of gentle manipulation with serial casting, a percutaneous Achilles tendon tenotomy, and bracing for the first few years of life. The purpose of this study was to determine whether socioeconomic factors influence compliance with clubfoot bracing for families with infants who have idiopathic clubfoot treated by the Ponseti method. METHODOLOGY: All patients with clubfoot deformity who began primar
