CASE: We describe lower extremity management of a 14-year-old adolescent boy and 13-year-old adolescent girl with amyoplasia type of arthrogryposis who underwent bilateral knee disarticulation for recurrent knee flexion contracture and persistent pain. Both patients reported improvements in gross motor mobility and demonstrated enhanced scores across all Patient-Reported Outcomes Measurement Information System (PROMIS) domains. They both recommend this procedure to other similarly affected patients and affirm they would undergo the procedure again.
CONCLUSION: Amputation in the amyoplasia pop

This study evaluated if a lower-extremity exoskeleton affects error between marker-based ($M^{+}$) and markerless ($\mathbf{M}^{\boldsymbol{-}}$) motion capture in children with crouch gait. Two participants (P1: female, P2: male, ages 6 and 11 years old, cerebral palsy and spina bifida, both with assistive devices) walked with (Exo) and without (NoExo) an exoskeleton spanning the knee and ankle. Mean absolute difference (MAD) between $\mathrm{M}^{+}$ and $\mathrm{M}^{-}$ of gait cycle normalized hip, knee, and ankle angles were analyzed. Key outcomes of an ongoing clinical trial including mea

Pseudoarthroses (false joints) occur as a complication of fracture healing in the normotopic skeleton but have not been reported at ectopic sites. Fibrodysplasia ossificans progressiva (FOP) is an ultra-rare genetic disorder characterized by progressive heterotopic ossification (HO) and congenital skeletal abnormalities including developmental arthropathy in the normotopic skeleton. Here, we report ectopic pseudojoint formation in 6 patients with FOP, all occurring after painful flare-ups and HO in soft connective tissues of the knee, hip, and forearm. To our knowledge, FOP is the only human c

We describe the clinical presentation and evaluation of an 11-year-old girl with no reported past medical history, seen by her primary care physician for intermittent knee pain. Outpatient X-rays revealed findings concerning for rickets, prompting further evaluation with blood work. The patient was urgently referred to the emergency department due to abnormal laboratory results and was subsequently found to be in end-stage kidney disease with severe anemia, metabolic acidosis, and significant electrolyte abnormalities. Despite these abnormalities, she was hemodynamically stable with appropriat

PURPOSE: Different cast configurations are available in the literature for the application of the Ponseti cast. To dispel the ambiguity and generate better evidence, we tested 2 cast configurations (standard vs. reinforced) for their effectiveness in terms of retained ankle dorsiflexion and overall cast integrity.
METHODS: We randomized the 2 limbs of a child with bilateral idiopathic clubfeet (n=30 children) to receive a weight-matched standard cast (circular rolls only) or a reinforced cast (additional knee and plantar slabs). The post tenotomy cast was chosen as the index cast for the rese

This study investigated the effects of excessive body weight and flexible flatfoot (FFF) on gait patterns in females. Sixty participants were divided into four groups (overweight/obese or normal weight, each with either normal feet or FFF; 15 per group). Walking data were analyzed using two-way ANOVA and post-hoc pairwise comparisons with statistical non-parametric mapping to assess the main and interaction effects of obesity and flatfoot on lower-limb biomechanics. Results indicated that overweight/obesity was the primary factor influencing lower-limb biomechanics (p = 0.049), with no signifi

Protein S gene abnormalities are the most common congenital predisposition to thrombophilia in the Japanese population, but not in the Caucasian population. It is important to measure protein S activity, specific activity, and antigen levels in patients with thrombophilia. A 52-year-old man presented with suspected deep vein thrombosis after previously visiting an orthopedic clinic with right lower extremity swelling after his long-distance walk 5 days prior. His D-dimer level was elevated, and a thrombus was found from the femoral to the below-the-knee veins on ultrasonographic echography. An

Congenital knee dislocation (CKD) is a rare condition that can present as an isolated finding or in association with genetic syndromes. It is most commonly diagnosed postnatally and classified into three grades based on severity. While generally having a favorable prognosis, particularly for milder cases, prompt treatment is crucial. We present a case of CKD successfully treated with conservative measures, with serial casting and Pavlik harness, in an otherwise healthy infant. This report aims to increase awareness of the clinical presentation and management of CKD in general practitioners giv

Congenital knee dislocation (CKD) is a rare musculoskeletal condition that may occur in isolation or be associated with other congenital abnormalities. Prognosis depends on the severity of the deformity and the timing of diagnosis and treatment. Comprehensive assessment and close follow‑up are essential to optimize recovery and to exclude associated anomalies. We report the case of a female newborn with CKD, diagnosed postnatally, associated with developmental dysplasia of the hip (DDH), generalized ligamentous laxity, congenital heart disease, and a chromosomal abnormality. Early conservative

Gait disturbance is a common motor symptom in Angelman syndrome (AS), but its characteristics have been poorly studied quantitatively. This study aimed to analyze gait characteristics in school-age children with AS using three-dimensional gait analysis (3DGA). Patients with clinically and genetically confirmed AS and healthy children aged 6-15 years were included. For gait assessments, 3DGA was performed using an eight-camera motion analysis system and eight force plates. Gait metrics, including gait speed, step length, step width, gait variability, gait deviation index, and kinematic and kine

This study aimed to investigate the impact of corrective-developmental games on proprioception, coordination, balance and motor proficiency in individuals with Down syndrome. The current quasi-experimental study with pre-test and post-test design explores the impact of corrective-developmental games (fine-gross motor skill games and univariate-multivariate games) on the proprioception, coordination, balance and motor proficiency of individuals with Down syndrome. The research sample comprises 50 individuals with Down syndrome, with an average age of 17.38 years, divided into two groups: interv

OBJECTIVES: Open spina bifida (OSB) is associated with lower limb and bladder dysfunction. We documented bladder function and lower limb motor function on ultrasound throughout gestation in a cohort of fetuses with OSB. The association of the prenatal findings with postnatal dysfunction was investigated, along with the impact of talipes on pre- and postnatal motor function.
METHODS: A prospective cross-sectional cohort study was performed including all fetuses with isolated OSB, which were assessed at the University Hospitals Leuven between July 2015 and December 2019. The anatomical level of

BACKGROUND: Foot orthosis hardness governed the trade-off between medial arch correction, plantar pressure redistribution, and load transfer along the lower limb, yet the relationship between hardness and biomechanical improvement in flexible flatfoot remains unclear.
AIM: To investigate how different arch support hardness levels affect biomechanics and muscle forces in adults with flexible flatfoot, and to determine how these effects vary with hardness.
METHODS: Twenty women aged 18-40 years with flexible flatfoot (navicular drop > 10 mm, arch height index < 0.31), all asymptomatic, with no

Myopathic Ehlers-Danlos syndrome (RmEDS) is an emerging hybrid phenotype that combines connective and muscle tissue abnormalities. It has been associated with variants of the COL12A1 gene, which are known as Ullrich congenital muscular dystrophy-2 (UCMD2; 616470) and Bethlem myopathy-2 (BTHLM2; 616471). Here, we report two splicing mutations of COL12A1 identified in three patients from two unrelated families who present a combination of joint hypermobility and axial, distal, and proximal weakness. The muscular strength of their neck and limb muscles was assessed at 4/5 (MRC); however, when mea

Cartilage microfracturation is a surgical technique specifically designed to address chondral defects, which are injuries to the cartilage that covers the ends of bones in joints. These defects can result from traumatic injuries, degenerative conditions such as osteoarthritis, or congenital abnormalities. The primary objective of microfracture surgery is to promote the regeneration of functional cartilage tissue, thereby restoring joint function, alleviating pain, and enhancing mobility. The procedure involves creating small, controlled perforations, or microfractures, in the subchondral bone
