Three-dimensional lower limb kinematics and kinetics in femoroacetabular impingement syndrome (FAIS) patients with and without borderline developmental dysplasia of the hip (BDDH) during level walking.

PURPOSE: The impact of femoroacetabular impingement syndrome (FAIS) on gait has been reported; however, no studies have documented the effects of Borderline Developmental Dysplasia of the Hip (BDDH) combined with FAIS on gait. This study aimed to evaluate the kinematic and kinetic abnormalities of the lower extremities in patients with combined FAIS and BDDH during level walking. METHODS: A total of 42 participants were included, consisting of 14 patients with FAIS + BDDH, 14 with isolated FAIS and 14 healthy controls. Full-cycle kinematic and kinetic data were collected via motion capture an

Percutaneous Transphyseal Screw Hemiepiphysiodesis (PETS) for Tibia Vara in Blount's Disease.

BACKGROUND: Blount's disease is a progressive developmental disorder causing pathologic genu varum due to growth disturbances in the medial proximal tibial physis. Guided growth is a primary treatment, but established methods like staples and 8-plates have limitations, including hardware failure and unpredictable correction. This study evaluates an alternative technique of hemiepiphysiodesis using percutaneous transphyseal screws (PETS) for treating juvenile and adolescent Blount's disease. METHODS: This prospective case series included 61 patients (80 knees) aged 4 to 12 years treated for Bl

Flatfoot kinematics and coordination during bilateral heel rises: Effects of knee angle variation.

Flatfoot alignment alters foot biomechanics and lower limb coordination; however, its impact on dynamic performance during bilateral heel rises under varying knee conditions remains unclear. This study compared heel-rise kinematics and intersegmental coordination in 12 flatfoot and 12 normal-foot participants performing ten bilateral heel rises at 60 BPM under randomized knee extended and knee flexed conditions. The three-dimensional kinematics were recorded using a 12-camera Vicon system. The ascending and descending phases were divided into the early, middle, and late periods. The maximum no

Survival of anterior cruciate ligament  reconstruction in patients with Ehlers-Danlos syndrome: A comparison with anatomic risk factors in existing literature.

BACKGROUND: Ehlers-Danlos Syndrome (EDS) is a connective tissue disorder characterized by joint hypermobility, ligamentous laxity, and frequent joint injuries. These features could increase the risk of anterior cruciate ligament (ACL) tears, typically managed through ACL reconstruction (ACLR). Surgical intervention in EDS is challenging due to potential complications such as poor wound healing and tissue fragility. Limited evidence exists regarding the outcomes of ACLR in EDS patients; therefore, in this study we aimed to evaluate survival rates of primary and revision ACLR and examine associa

Classification of Kinematic Coupling Patterns Between Lower Limb Joints During Running.

Abnormal foot alignment such flatfoot is involved in running injuries. The mechanism by which flatfoot is susceptible to running injuries has not yet been established. The purpose of this study was to classify kinematic coupling patterns by hierarchical clustering, adding individuals with flatfoot and motion in the transverse plane involved in running injuries, and to identify the factors that determine the clusters classified. Twenty-four young healthy adults were divided and classified into normal and flat foot groups of 12 participants each, using the foot posture index. The task involved t

A Novel Surgical Technique for the First Case of Neglected Bilateral Severe Congenital Genu Recurvatum Combined with Bilateral Talipes Equinocavovarus.

Congenital genu recurvatum (CGR) is a rare congenital knee deformity that can be diagnosed pre- or postnatally. Most CGR cases are treated during infancy by manipulation and serial casting or splinting. Only a few neglected cases of CGR in adults with the necessity of surgical treatment have been reported. This study presents the first case of neglected bilateral severe CGR combined with bilateral talipes equinocavovarus (BTE) in a 25-year-old woman. This case is unique, because BTE is secondary to CGR with persistent progression into adulthood. The patient underwent two-stage surgeries with 1

Factors Associated With Complications in 176 Crowe IV Hips Treated With Total Hip Arthroplasty.

OBJECTIVE: Total hip arthroplasty (THA) in Crowe IV hip was a challenge for surgeons, along with some complications. Currently, most evaluations focused on the surgical techniques and radiographs, which were indirectly measured parameters and easily affected by the operators. Objective factors were not considered. This study aimed to investigate objective factors to predict the complications. METHODS: We retrospectively reviewed a series of Crowe IV patients who received THA between July 2010 and December 2019 in our hospital. Demographics and surgical parameters were collected: gender, age,

Ligament reconstruction combined with osteotomy: Case report.

RATIONALE: Valgus deformity of the knee joint is caused by congenital anatomical abnormalities. The occurrence of an anterior cruciate ligament (ACL) tear in conjunction with a medial patellofemoral ligament (MPFL), alongside knee valgus deformity, is exceedingly rare in clinical practice. PATIENT CONCERNS: A 30-year-old female presented with congenital valgus knee. The left knee joint was immobile due to pain and swelling after trauma. The patient had a history of hypertension. DIAGNOSES: The patient underwent an MRI of the left knee joint at a local hospital, and was diagnosed with ACL inj

An Unnoticed Femoral Condyle Deformity in Transverse Plane Is Associated With Knee Valgus in Patients With Unilateral Developmental Dysplasia of the Hip.

OBJECTIVE: Patients with developmental dysplasia of the hip (DDH) are usually accompanied by knee valgus, and the previous studies mainly focused on the deformity of the knee in the coronal plane that led to knee valgus. In the transverse plane, the deformity of the femoral condyle had never been considered. This study aimed to analyze the transverse morphology of the femoral condyle in patients with unilateral DDH and explore the correlation between the morphology of the femoral condyle and lower limb alignment. METHODS: Sixty-seven patients (10 male and 57 female) with unilateral DDH in our

Blunted exercise hyperemic response to lower limb exercise in individuals with Down syndrome.

Autonomic dysfunction is common in individuals with Down syndrome (DS). Coordination of autonomic function is essential for appropriate peripheral blood flow delivery and blood pressure (BP) regulation during lower limb dynamic exercise. Our purpose was to examine the effects of lower limb dynamic exercise on femoral blood flow (FaBF) and BP to both active and inactive limbs at both relative and absolute intensities in individuals with and without DS. Twenty-four individuals with DS (24 ± 5 yr; 30.3 ± 6.2 kg/m2) and 24 individuals without DS (25 ± 4 yr; 26.5 ± 4.5 kg/m2) performed five knee ex

New Reconstruction Technique in Combined Tibial and Fibular Hemimelia.

Tibial hemimelia is a rare congenital deficiency that is challenging to treat. Appropriate care varies due to the many subtypes and associated knee and ankle deformities. Amputation is still the most accepted treatment, but it is not generally preferred by families. Consequently, other reconstruction options have been made available as well. In the following two cases, we performed knee joint reconstruction by fusion of the ankle joint instead of amputation. This technique has not been reported before in the literature and has been named as Cabukoglu reconstruction technique or cruris plasty.

Retrograde Femoral Lengthening Below a Total Hip Arthroplasty.

BACKGROUND: Limb length discrepancy (LLD) after total hip arthroplasty (THA) is a common occurrence and can lead to back pain, disordered gait, and decreased functional outcomes. Femoral lengthening ipsilateral to a THA using a retrograde motorized intramedullary lengthening nail (MILN) is a hip-sparing option for limb equalization. There has been little published on the technique and results of this method. METHODS: We retrospectively reviewed all patients at our institution who underwent unilateral femoral lengthening using a retrograde MILN ipsilateral and distal to a THA between April 201

Congenital Knee Dislocation: A Rare Orthopedic Enigma in Rural Nepal.

Congenital knee dislocation (CKD) (genu recurvatum congenitum) is a rare neonatal orthopedic deformity characterized by hyperextension of the knee joint and potentially associated with intrauterine mechanical factors, quadriceps contracture, or syndromic disorders. Early recognition is essential, as prompt conservative management can prevent long-term disability and reduce the need for surgical intervention. The objective of this single case report is to describe a rare presentation of bilateral congenital knee dislocation managed successfully through early conservative treatment in a rural an
