Neurotization in microsurgical repair of obstetric brachial plexus palsy: Surgical outcome of different techniques.

BACKGROUND: In the treatment of obstetric brachial plexus palsy (OBPP), neurotization, or nerve transfer, has become a crucial microsurgical procedure, especially when nerve roots are avulsed or non-functional. To reinnervate a denervated target and restore motor function, a functional donor neuron must be redirected. The objective of the study is to assess the outcomes of various neurotization approaches in individuals with brachial plexus injuries sustained during pregnancy. METHODS: Thirty patients with OBPP who received microsurgical treatment using various brachial plexus neurotization p

A Case of Short-Rib Thoracic Dysplasia: Diagnostic Clues in Early Pregnancy and Genetic Analysis.

To investigate the diagnostic clues and genetic etiology of short-rib thoracic dysplasia (SRTD) in early pregnancy, we report a case of a fetus suspected of having SRTD based on ultrasound findings at 13 weeks of gestation. Prenatal genetic testing revealed a pathogenic variant in the DYNC2H1 gene, and post-abortion examination of the specimen along with X-ray findings confirmed the diagnosis as short-rib thoracic dysplasia 3 (SRTD3; OMIM #613091). Such cases are rarely reported in the early gestational period. The detection and subsequent confirmation in this case at 13 weeks demonstrate that

Anomalous Extensor Indicis With an Intermediate Tendon: Surgical and Functional Considerations.

The extensor indicis proprius (EIP) is regarded as one of the most consistent muscles of the dorsal forearm, originating from the distal third of the posterior ulnar surface and passing through the fourth extensor compartment to insert into the dorsal digital extensor expansion (DDEE) of the index finger. Despite its relative constancy, the EIP exhibits a notable range of morphological variations, including supernumerary tendons, anomalous insertions, and accessory muscle bellies. A double-bellied configuration of the EIP, particularly with an intermediate tendon bearing a capsular attachment,

Biallelic variants in DNAH10 are associated with skeletal developmental abnormalities and ciliary dysfunction.

BACKGROUND: Primary cilia are essential for skeletal development by coordinating key signalling pathways in osteoblasts and chondrocytes. While pathogenic variants in approximately 40 genes have been linked to skeletal ciliopathies, additional causative genes remain to be identified. Although DNAH10 is well characterised in motile cilia, its role in primary cilia remains unclear. METHODS: Whole-exome sequencing was performed in two individuals with skeletal developmental abnormalities and identified biallelic DNAH10 variants. In vitro assays were used to assess the effects of these variants o

Delayed diagnosis of Townes-Brocks syndrome accompanied with kidney failure.

Townes-Brocks syndrome (TBS) is a rare autosomal dominant disorder typically characterized by the triad of anorectal malformations, external ear anomalies and hand malformations such as preaxial polydactyly, caused by mutations in the SALL1 gene. Kidney involvement, although less common, can progress to end-stage kidney failure. Early recognition of the characteristic features, particularly those related to SALL1, is crucial for accurate diagnosis, management, and genetic counseling. Here, we present a Turkish family with TBS in which the diagnosis was delayed due to the absence of the classic

Genetic Syndromes Associated With Congenital Upper Limb Differences.

Congenital upper limb differences occur in approximately 20-30 per 10,000 live births. These differences frequently prompt reconstructive surgery and may present in isolation or as markers of systemic syndromes. Embryologically, limb development depends on coordinated signaling along the proximal-distal (apical ectodermal ridge/fibroblast growth factor [FGF]), radial-ulnar (zone of polarizing activity/sonic hedgehog), and dorsal-ventral (wingless-related integration site 7) axes, whose disruptions lead to specific phenotypic patterns. Among upper limb differences, those with a well-established

Dual molecular diagnosis of CEP290 and GLI3 mutations identified in an infant with leber congenital amaurosis and postaxial polydactyly, a Bardet-Biedl syndrome phenocopy.

BACKGROUND: Leber congenital amaurosis (LCA) is one of the earliest-onset and most severe forms of inherited retinal disease, and Bardet-Biedl syndrome (BBS) is a rare non-motile primary ciliopathy with a diverse multi-organ phenotype. To our knowledge, we present the first reported genetically confirmed case of LCA10 co-occurring with isolated postaxial polydactyly type A1 harboring CEP290 and GLI3 mutations, which was initially misdiagnosed as BBS. CASE PRESENTATION: A 6-month-old female infant presented with no visual tracking, nystagmus, and postaxial polydactyly. Targeted gene panel test

A unilateral accessory belly of the extensor indicis muscle: morphological presentation and its clinical implications.

PURPOSE: The extensor indicis muscle is an important muscle of the deep layer of the posterior compartment of the forearm, as it allows independent extension of the index finger. Numerous forms of variations of the extensor indicis muscle have been reported. This case report aims to describe a rare unilateral presence of an accessory muscle belly of the extensor indicis. CASE REPORT: An incidental finding during the routine dissections of the posterior compartments of the forearm and the dorsum of the hand is presented. The spindle-shaped accessory muscle of the extensor indicis originated fro

HOXD12 a candidate gene for a novel form of synpolydactyly.

Synpolydactyly is a rare limb deformity characterized by a unique combination of syndactyly and polydactyly. Synpolydactyly has an autosomal dominant mode of inheritance often with reduced penetrance. Variants in HOXD13, FBLN1, GLI3, and TTC30B have been reported to underlie the etiology of synpolydactyly. Here, we describe a three-generational Pakistani family segregating a form of synpolydactyly which has not previously been reported that includes, severe hand involvement characterized by bilateral syndactyly of the third, fourth, and fifth fingers, accompanied by preaxial polydactyly in all

Reconstruction of Wassel-Flatt type IV-VII radial polydactyly with a modified Bilhaut-Cloquet procedure: a consecutive series of 22 cases.

INTRODUCTION: Radial polydactyly reconstruction requires procedures that achieve both functional and aesthetic outcomes. When both duplicates are of similarly small diameters, a combination procedure may be necessary. Furthermore, primary surgical alignment is crucial to ensure a straight thumb and avoid future deviations. This study evaluates the modified Bilhaut-Cloquet procedure for the reconstruction of Wassel-Flatt types IV-VII radial polydactyly in paediatric patients. METHOD: The study included 22 patients aged 6-60 months. Inclusion criteria include similar-sized duplicated thumbs tha

Bilateral extensor pollicis et indicis accessorius: clinical and anatomical perspectives.

PURPOSE: The extensor muscles of the hand play a crucial role in thumb and finger movement. Thumb extension is primarily mediated by the extensor pollicis longus muscle and extensor pollicis brevis muscle, and extension of the index finger is facilitated by the extensor indicis muscle and extensor digitorum muscle. This report describes a rare anatomical variation, the extensor pollicis et indicis accessorius, in which a single tendon bifurcated to insert on both the index finger and thumb. METHODS: This variation was identified bilaterally during routine dissection of a 92-year-old female do

Association Between Thumb Hypoplasia, Radial Longitudinal Deficiency, and Radial Polydactyly.

PURPOSE: The expression of the sonic hedgehog protein has been associated with induction of both radial longitudinal deficiency (RLD) and radial polydactyly in animal studies. We sought to investigate the co-occurrence of thumb hypoplasia/RLD and radial polydactyly in a large registry patient cohort and to investigate the clinical presentations of such patients. METHODS: The Congenital Upper Limb Differences (CoULD) registry was queried for all patients with contralateral diagnoses of thumb hypoplasia/RLD and radial polydactyly, as defined by the Oberg-Manske-Tonkin classification system, bet

Autologous Periosteal-Cartilaginous Flap Reconstruction of the Radial Collateral Ligament in Thumb Polydactyly: A Case Series With Functional and Cosmetic Outcomes.

INTRODUCTION: Thumb polydactyly is a common congenital anomaly that requires reconstruction to restore joint stability, motion, and cosmetic symmetry. Conventional techniques often fail to adequately reestablish radial collateral ligament (RCL) support, contributing to postoperative deviation or instability. This study describes an autologous periosteal-cartilaginous flap technique for RCL reconstruction after an excision of the radial duplicate digit. METHODS: A retrospective case series was conducted at Prince Sultan Military Medical City, Riyadh. Patients who underwent thumb polydactyly co

Free functional muscle transfer in children: A long-term case series from a single microsurgical unit.

BACKGROUND: Restoration of active motion in children with upper-limb paralysis or muscle loss remains challenging. Free functional muscle transfer (FFMT) is a reconstructive option when local muscle donors are unavailable, but uncertainties persist regarding optimal timing, donor nerve selection, and long-term effects of growth and secondary deformities. This study reports a single-institution experience addressing these issues. METHODS: A retrospective review was conducted of 35 children (<18 years) who underwent upper-limb FFMT with a minimum follow-up of 24 months. All reconstructions used

Trifid epiglottis in a neonate with choanal atresia and hydrometrocolpos: the third reported case and the first symptomatic neonatal presentation.

BACKGROUND: Trifid epiglottis (TE) is an extremely rare congenital laryngeal anomaly. To the best of our knowledge, only two cases have previously been reported in the literature. Both of them have been incidentally detected and associated with polydactyly. CASE PRESENTATION: We report the third known case of TE in a two-day-old female neonate who was admitted to the emergency department due to abdominal distension, oliguria, and poor feeding within the first 24 h of life. Examination revealed stridor during crying, left postaxial polydactyly, right syndactyly, and an anteriorly displaced anu

Proprioceptive role of the abductor pollicis longus in thumb carpometacarpal osteoarthritis: An anatomical and immunohistochemical study.

OBJECTIVE: Thumb carpometacarpal (CMC) osteoarthritis (OA) is multifactorial and may involve anatomical variability, altered biomechanics, joint laxity and impaired proprioception. We hypothesized that variability of the abductor pollicis longus (APL) tendon and proprioceptive innervation of its insertions are associated with structural degeneration in thumb CMC OA, including cartilage degeneration and osteophyte area. DESIGN: In this cross-sectional study, twenty fresh-frozen cadaver hands were dissected (mean age 68 years, range 53-95) between June 2022 and December 2023. Anatomical and imm
