Megalencephaly-polymicrogyria-polydactyly-hydrocephalus (MPPH) syndrome with aplasia cutis congenita due to PIK3R2 mutations: case report.

BACKGROUND: Megalencephaly-polymicrogyria-polydactyly-hydrocephalus (MPPH) syndrome is a rare congenital developmental disorder characterized by megalencephaly, polymicrogyria, posterior polydactyly, and hydrocephalus and driven by PI3K-AKT-mTOR pathway dysregulation. This report presents a case of genetically confirmed MPPH in which the patient also exhibited aplasia cutis congenita (ACC), a cutaneous phenotype rarely described in this syndrome. CASE DESCRIPTION: We describe a full-term female infant with macrocephaly [head circumference > +2 standard deviations (SDs)] and distinctive cranio

Hyperphagia severity is underestimated in adults with Bardet-Biedl syndrome - a mixed-method cross-sectional study in the United Kingdom.

INTRODUCTION: Bardet-Biedl syndrome (BBS) is a rare genetic disease caused by primary ciliary dysfunction leading to retinal degeneration, rod-cone dystrophy, polydactyly, hyperphagia, early-onset obesity, renal dysfunction, hypogonadism, and learning difficulties. Hyperphagia, a pathological insatiable hunger leading to excessive and unusual food intake and persistent, obsessive food-seeking behaviours, is a frequent symptom associated with BBS. This study assessed hyperphagia severity in adults with BBS living in the UK, using a standard structured questionnaire accompanied by an in-depth se

Long-read sequencing reveals a hidden Alu-mediated splice defect in CPLANE1, causing orofaciodigital syndrome type VI.

Orofaciodigital syndrome type VI (OFD VI) is a recessive ciliopathy characterized by excessive polydactyly, molar tooth sign, cleft lip, and developmental delay, caused by pathogenic variants in CPLANE1. Here, we present a patient with OFD VI that remained genetically unexplained after routine genetic testing, including short-read whole genome sequencing (WGS). Using long-read sequencing, we found two biallelic splice-site variants in CPLANE1, c.8633-4_8633-3del, and an Alu element insertion close to an exon-intron boundary. Transcript analysis showed that each variant independently resulted i

Experience with patients presenting with the clinical features of Holt-Oram syndrome: a single center retrospective study.

BACKGROUND: Holt-Oram syndrome, also known as atrio-digital syndrome, is a rare autosomal dominant genetic disorder primarily characterized by upper limb malformations and congenital heart defects, associated with mutations in the TBX5 gene. Diagnosis is typically based on clinical presentation, and the prognosis for affected individuals is closely related to the severity of cardiac involvement. Our study aims to highlight some key features of Holt-Oram syndrome to improve the level of clinical diagnosis. METHODS: We collected 11 patients with clinical features strongly suggestive of Holt-Ora

Pain catastrophizing and its associated factors in parents of children after polydactyly or syndactyly surgery: a cross-sectional survey.

BACKGROUND: Polydactyly and syndactyly are common congenital hand malformations in children, with surgical correction as the primary treatment. Postoperative pain in children can trigger pain catastrophizing in parents, which may impair family care quality and children's rehabilitation. This study aimed to evaluate the current status of pain catastrophizing and its associated factors in parents of children after polydactyly or syndactyly surgery. METHOD: A cross-sectional survey was conducted from February 2024 to October 2025, enrolling parents of children who underwent polydactyly or syndac

Bi-allelic variants in CDK20 cause a severe ciliopathy with midline brain and facial anomalies.

CDK20, also known as CCRK, is a cyclin-dependent kinase that regulates cell growth, primary cilium structure, and Sonic Hedgehog signaling. CDK20 has not yet been associated with a known human disorder. In this report, we present a cohort of seven individuals from five unrelated families with bi-allelic variants in CDK20 and an overlapping phenotype of ventriculomegaly or hydrocephalus, midline brain anomalies, abnormal nose, midline cleft lip and/or palate, cryptophthalmos or anophthalmia, postaxial polydactyly, and a sandal toe gap. Immunoblot analysis of fibroblasts derived from two affecte

Radial polydactyly: towards a unifying approach for surgical management.

INTRODUCTION: Radial polydactyly is often underestimated in its complexity. While many cases can be managed by excision and reconstruction of a dominant thumb, suboptimal outcomes persist when soft-tissue imbalance or bony deviation is not fully addressed. Expectations for aesthetic symmetry have also increased, placing greater demands on surgical precision and judgement.Limitations of current approaches and classifications:In cases where both duplicates are small (typically <70% of the contralateral thumb), achieving adequate thumb girth and a satisfactory nail complex frequently requires aug

Case series: Joubert syndrome and eosinophilic esophagitis.

Joubert syndrome (JS) is a rare genetic disorder characterized by developmental abnormalities, particularly in the brainstem and cerebellar vermis, alongside multisystem manifestations such as kidney and liver anomalies, polydactyly, cleft lip or palate, and tongue defects. The underlying ciliopathy causing JS may also contribute to gastrointestinal symptoms and immune dysregulation via Wnt signaling and impaired epithelial maintenance. Dysmotility including Hirschsprung disease has been documented at increased rates in JS and in other ciliopathies. Our case series highlights how JS patients f

Motor Nerve Transfers in Complete and Incomplete Brachial Plexus Injuries: A State-of-the-Art Review.

Brachial plexus injuries are challenging conditions. Over the past decades, nerve transfer surgery has progressively evolved from proximal nerve reconstruction toward selective distal neurotization strategies, considerably expanding the possibilities for functional restoration. As the number of described donor-recipient combinations has increased, the literature has become increasingly fragmented, often focusing on isolated techniques or specific functional targets. The aim of the present study was to provide a comprehensive state-of-the-art overview of currently available motor nerve transfer

Ocular and Systemic Phenotyping of Bardet-Biedel Syndrome Type 7 (BBS7) in a Palestinian Male: Case Report and Literature Review.

INTRODUCTION: Bardet-Biedl syndrome (BBS) is a rare autosomal recessive ciliopathy with at least 26 identified causative genes. BBS7 accounts for approximately 1.5% of cases. The syndrome is characterized by retinal degeneration, obesity, polydactyly, intellectual disability, and renal anomalies. BBS7 encodes a core subunit of the BBSome complex, which is essential for ciliary function and intracellular signaling. CASE PRESENTATION: We report a 9-year-old Palestinian boy with BBS7, confirmed by whole exome sequencing identifying a homozygous pathogenic variant in the BBS7 gene [c.712_715del];

Diagnosing the canvas: challenging the diagnosis of Prader-Willi syndrome in Eugenia Martínez Vallejo (1674-1699).

Eugenia Martínez Vallejo (1674-1699), famously portrayed by Juan Carreño de Miranda at the court of King Carlos II of Spain, has long been considered a historical example of Prader-Willi syndrome (PWS). More than 20 publications have supported this diagnosis based on her severe obesity, facial appearance, and presumed small extremities. We revisited this diagnosis using historical medical documentation, auxological analysis, and next-generation phenotyping. Historical documents preserved from that period describe hyperphagia beginning immediately after birth, with a weight of 25 kg at 1 year o

The real-world safety profile of amitriptyline: A comprehensive pharmacovigilance analysis based on the large-scale FDA adverse event reporting system database.

Despite its established efficacy, the long-term safety profile of amitriptyline among broad patient populations remains insufficiently characterized. We thoroughly characterized the real-world safety profile of amitriptyline through a systematic analysis of the FDA Adverse Event Reporting System (FAERS). Adverse events were collected from Q1 2004 to Q1 2026, and four disproportionality algorithms-reporting odds ratio (ROR), proportional reporting ratio (PRR), Bayesian confidence propagation neural network (BCPNN), and multi-item gamma Poisson shrinker (MGPS)-were employed to identify safety si
