Genetic Syndromes Associated With Congenital Upper Limb Differences.

Congenital upper limb differences occur in approximately 20-30 per 10,000 live births. These differences frequently prompt reconstructive surgery and may present in isolation or as markers of systemic syndromes. Embryologically, limb development depends on coordinated signaling along the proximal-distal (apical ectodermal ridge/fibroblast growth factor [FGF]), radial-ulnar (zone of polarizing activity/sonic hedgehog), and dorsal-ventral (wingless-related integration site 7) axes, whose disruptions lead to specific phenotypic patterns. Among upper limb differences, those with a well-established

Dual molecular diagnosis of CEP290 and GLI3 mutations identified in an infant with leber congenital amaurosis and postaxial polydactyly, a Bardet-Biedl syndrome phenocopy.

BACKGROUND: Leber congenital amaurosis (LCA) is one of the earliest-onset and most severe forms of inherited retinal disease, and Bardet-Biedl syndrome (BBS) is a rare non-motile primary ciliopathy with a diverse multi-organ phenotype. To our knowledge, we present the first reported genetically confirmed case of LCA10 co-occurring with isolated postaxial polydactyly type A1 harboring CEP290 and GLI3 mutations, which was initially misdiagnosed as BBS. CASE PRESENTATION: A 6-month-old female infant presented with no visual tracking, nystagmus, and postaxial polydactyly. Targeted gene panel test

HOXD12 a candidate gene for a novel form of synpolydactyly.

Synpolydactyly is a rare limb deformity characterized by a unique combination of syndactyly and polydactyly. Synpolydactyly has an autosomal dominant mode of inheritance often with reduced penetrance. Variants in HOXD13, FBLN1, GLI3, and TTC30B have been reported to underlie the etiology of synpolydactyly. Here, we describe a three-generational Pakistani family segregating a form of synpolydactyly which has not previously been reported that includes, severe hand involvement characterized by bilateral syndactyly of the third, fourth, and fifth fingers, accompanied by preaxial polydactyly in all

Reconstruction of Wassel-Flatt type IV-VII radial polydactyly with a modified Bilhaut-Cloquet procedure: a consecutive series of 22 cases.

INTRODUCTION: Radial polydactyly reconstruction requires procedures that achieve both functional and aesthetic outcomes. When both duplicates are of similarly small diameters, a combination procedure may be necessary. Furthermore, primary surgical alignment is crucial to ensure a straight thumb and avoid future deviations. This study evaluates the modified Bilhaut-Cloquet procedure for the reconstruction of Wassel-Flatt types IV-VII radial polydactyly in paediatric patients. METHOD: The study included 22 patients aged 6-60 months. Inclusion criteria include similar-sized duplicated thumbs tha

Association Between Thumb Hypoplasia, Radial Longitudinal Deficiency, and Radial Polydactyly.

PURPOSE: The expression of the sonic hedgehog protein has been associated with induction of both radial longitudinal deficiency (RLD) and radial polydactyly in animal studies. We sought to investigate the co-occurrence of thumb hypoplasia/RLD and radial polydactyly in a large registry patient cohort and to investigate the clinical presentations of such patients. METHODS: The Congenital Upper Limb Differences (CoULD) registry was queried for all patients with contralateral diagnoses of thumb hypoplasia/RLD and radial polydactyly, as defined by the Oberg-Manske-Tonkin classification system, bet

Autologous Periosteal-Cartilaginous Flap Reconstruction of the Radial Collateral Ligament in Thumb Polydactyly: A Case Series With Functional and Cosmetic Outcomes.

INTRODUCTION: Thumb polydactyly is a common congenital anomaly that requires reconstruction to restore joint stability, motion, and cosmetic symmetry. Conventional techniques often fail to adequately reestablish radial collateral ligament (RCL) support, contributing to postoperative deviation or instability. This study describes an autologous periosteal-cartilaginous flap technique for RCL reconstruction after an excision of the radial duplicate digit. METHODS: A retrospective case series was conducted at Prince Sultan Military Medical City, Riyadh. Patients who underwent thumb polydactyly co

Trifid epiglottis in a neonate with choanal atresia and hydrometrocolpos: the third reported case and the first symptomatic neonatal presentation.

BACKGROUND: Trifid epiglottis (TE) is an extremely rare congenital laryngeal anomaly. To the best of our knowledge, only two cases have previously been reported in the literature. Both of them have been incidentally detected and associated with polydactyly. CASE PRESENTATION: We report the third known case of TE in a two-day-old female neonate who was admitted to the emergency department due to abdominal distension, oliguria, and poor feeding within the first 24 h of life. Examination revealed stridor during crying, left postaxial polydactyly, right syndactyly, and an anteriorly displaced anu

Wedging osteotomy to correct metacarpal adduction for the treatment of Wassel types V and VI duplicated thumb: surgical strategy, outcomes, and anatomical findings.

This study presents a wedge osteotomy technique for correcting metacarpal adduction in the treatment of Wassel types V and VI thumb polydactyly, evaluating its clinical efficacy and reporting intraoperative anatomical abnormalities. A retrospective analysis of 37 thumbs treated between January 2016 and June 2023 was performed. Preoperatively, the median inter-metacarpal angle (IMA) was 0°, and the median radial deviation angle of the metacarpophalangeal (MCP) joint of the ulnar thumb was 30°. All cases underwent excision of the hypoplastic radial thumb, thenar muscle relocation, and a radial c

Congenital hallux varus with ventricular septal defect: a unique case report.

INTRODUCTION AND IMPORTANCE: Congenital hallux varus (CHV) is a very rare foot deformity, defined by the medial deviation of the great toe at the metatarsophalangeal joint. It can appear in isolation or with other anomalies, such as polydactyly, metatarsus adductus, or a longitudinal epiphyseal bracket. However, an association with a ventricular septal defect (VSD) has not been described before. Early surgical correction is essential to restore function, improve cosmetic appearance, and support normal motor development. PRESENTATION OF CASE: A 13-month-old male presented with congenital right

Ultrasound improves preoperative Wassel classification of thumb polydactyly in children.

BACKGROUND: The Wassel classification for thumb polydactyly relies on radiographs; however, invisible unossified cartilage in young children can lead to misclassification and suboptimal surgical planning. This study evaluated whether ultrasound improves the accuracy of polydactyly classification compared with radiographs. MATERIALS AND METHODS: This prospective study included 31 cases of radial polydactyly initially classified as Wassel type IV or type II based on radiographs. All patients underwent preoperative ultrasound to identify potential cartilaginous connections between the proximal a

A Rare Morphological Variant of Postaxial Foot Polydactyly With an Accessory Articulating Osseous Structure of the Fifth Ray.

Postaxial polydactyly of the foot is a common form of digital duplication that often causes difficulty with footwear, metatarsalgia, and cosmetic concerns. Although it may present as an isolated anomaly, it can also be associated with genetic conditions. Surgical excision is the primary intervention, with generally excellent outcomes. A 5-year-old male presented with postaxial polydactyly of the left foot, characterized by an accessory articulating osseous structure associated with the fifth ray. The main complaints were difficulty with footwear and cosmetic appearance. Radiographic evaluation

Polydactyly and syndactyly in a Chinese family with Floating-Harbor syndrome: an expansion of the clinical phenotype.

Floating-Harbor syndrome (FLHS) is a rare neurodevelopmental and skeletal disorder caused by truncating variants in exons 33 and 34 of the SRCAP gene. It is characterized by distinctive facial features, delayed bone age, short stature, and moderate intellectual disability. While digital anomalies have been reported in approximately half of the more than 100 known cases, the phenotypic spectrum continues to expand. Here, we describe a family in which two individuals were identified with FLHS. Both the proband and her mother presented with typical manifestations, including classic facial dysmorp
