Further Exploring the TRRAP Genotype-Phenotype Correlations: Report of Three New Patients With A Focus on Skeletal Anomalies.

TRRAP encodes a multidomain pseudokinase involved in histone acetyltransferase complexes. TRRAP pathogenic variants were linked to neurodevelopmental disorders, intellectual disability, congenital anomalies, and hearing loss. We report on three unrelated patients with TRRAP missense variants. Patient #1, a girl with severe intellectual disability, autism features, and preaxial polydactyly, displays the c.5575C>T, p.(Arg1859Cys) variant. Patient #2, a boy with developmental delay and facial anomalies, harbors the c.5647G>A, p.(Gly1883Arg) variant. Patient #3, a girl with developmental delay, ep

The human ciliopathy protein RSG1 links the CPLANE complex to transition zone architecture.

Cilia are essential organelles, and variants in genes governing ciliary function result in ciliopathic diseases. The Ciliogenesis and PLANar polarity Effectors (CPLANE) protein complex is essential for ciliogenesis, and all but one subunit of the CPLANE complex have been implicated in human ciliopathy. Here, we identify three families in which variants in the remaining CPLANE subunit CPLANE2/RSG1 also cause ciliopathy. These patients display cleft palate, tongue lobulations and polydactyly, phenotypes characteristic of Oral-Facial-Digital Syndrome. We further show that these alleles disrupt tw

A homozygous frameshift variant in the CILK1 gene causes cranioectodermal dysplasia.

Cranioectodermal dysplasia (CED) is a ciliopathy characterized by skeletal and ectodermal abnormalities, renal failure, and liver fibrosis. Pathogenic variants in genes that encode the intraflagellar transport (IFT) complex components, particularly IFT-A, are responsible for approximately two-thirds of the CED cases. However, the cause of the remaining cases remains unknown. Ciliogenesis-associated kinase 1 (CILK1) is a highly conserved ciliary serine/threonine kinase with an N-terminal catalytic domain responsible for kinase activity and a C-terminal non-catalytic domain that interacts with t

Uncovering a Diagnosis Through Reanalysis of UBA2 Variants in a Patient with Syndactyly, Polydactyly, and Aplasia Cutis Congenita: A Short Report and a Review of the Literature.

Background: Aplasia cutis congenita and ectrodactyly skeletal syndrome (ACCES) is caused by heterozygous variants in the UBA2 gene, with phenotypic heterogeneity encompassing a range of diverse skeletal, dermatological, and neurological features. Aims: The goal of our research was to suggest that pathogenic frameshift variant c.52_58dupGGCCGGG p.(Val20Gfs*31) could lead to the development of ACCES and also to review the literature to document phenotypic variability among individuals with UBA2 variants, providing further insights into this ultrarare syndrome. Methods and Result: We report a cas

Evidence of the correlation between air pollution and different types of birth defects: based on a distribution-lag non-linear model.

INTRODUCTION: As global fertility rates decline, exploring the root causes of birth defects (BDs) becomes urgent. Air pollution, with its ability to penetrate the placental barrier as exogenous toxins, has garnered notable attention in this regard. METHODS: BD data was collected from five hospitals in Changzhi City birth from 2019 to 2021, air quality data originated from hourly observations at five monitoring stations within the city. Using the distributed lag non-linear model (DLNM), the study aimed to determine the non-linear exposure-lag-effect relationship, evaluating the delayed impact

Enhancer Poising Enables Pathogenic Gene Activation by Noncoding Variants.

Single nucleotide variants within enhancers-non-coding DNA elements that regulate transcription-often lead to aberrant gene activation and contribute to a wide range of genetic disorders (Claussnitzer et al. 2015; Doan et al. 2016; Turner et al. 2017; Yanchus et al. 2022; Lettice et al. 2008). The mechanism by which ectopic gene activation occurs through these gain-of-function enhancer mutations remains poorly understood. Using the ZRS, a benchmark disease-associated enhancer of Sonic hedgehog ( Shh ), as a model, we demonstrate that poised (i.e., accessible but inactive) chromatin sensitizes

[Two cases of skeletal ciliopathies in one family].

Cilia are thin extensions on the cells of eukaryotic organisms. They are formed by a special protein transport mechanism - the intraflagellar transporter (IFT). The IFT consists of two proteins: complex A and complex B. Mutations in the genes of the IFT-A complex (IFT43, IFT121, IFT122, IFT139, IFT140, and IFT144) lead to the development of skeletal ciliopathies. These include Sensenbrenner, Jeune, and short-rib polydactyly syndrome [1,2]. We report two cases of different ciliopathies in a non-related family; both parents are heterozygous carriers of a pathogenic mutation in the IFT122 gene.

Does the JK Classification Have Good Reliability for Making Treatment Choices and Prognosticating Outcomes in Radial Polydactyly?

BACKGROUND: The Wassel-Flatt classification has been widely used to categorize radial polydactyly, but it has limitations in describing variations such as the hypoplastic and triphalangeal types. To address these limitations, the modified Wassel-Flatt classification was introduced. However, this classification also has drawbacks in categorizing of Type VII and fails to include angular-type hypertrophied epiphysis of the distal phalanx. Therefore, the JK classification, a revised version of the modified Wassel-Flatt classification, was recently proposed. However, its reliability and usefulness

Molecular Landscape in Limb Anomalies: Diagnostic Yield and New Candidate Genes.

Congenital limb anomalies remain without an etiological diagnosis in up to 65% of patients. To help close this gap, we describe the genetic diagnostic outcomes of a large cohort. Patients whose primary indication for genetic consultation was a limb anomaly were included from 2014 to 2024. Demographic, investigation, and diagnostic information were extracted, described, and compared. One hundred and thirty-two patients were included in the final cohort, with an average molecular diagnostic yield of 36%. The most common conditions were polydactyly (24%) and radial anomalies (19%). Fifty percent

A population description of CULAs using combined documentation of compound phenotypes according to the Oberg-Manske-Tonkin classification.

BACKGROUND/PURPOSE: Congenital upper limb anomalies (CULAs) exhibit a wide range of manifestations. For this reason, the Oberg-Manske-Tonkin (OMT) classification was introduced to achieve etiologically correct and universal classification. Combined use of codes has been advocated to prevent the loss of important phenotypic information in compound phenotypes. Therefore, the aim of our study was to present a description of our population over the last 10 years, using the most recent (2020) version of the OMT and combined documentation when necessary. METHODS: All patients who visited our tertia

Bardet-Biedl syndrome: a rare cause of end-stage kidney disease. Case report.

INTRODUCTION: Bardet-Biedl syndrome (BBS) is a rare autosomal recessive disorder. Diagnosis can be established with specific symptom criteria, confirmed by genetic testing. Early detection and monitoring are crucial due to the high risk of serious organ damage in affected children. We report a case of BBS at younger age admitted to our training program for severe renal failure. Our case is the first reported observation in our country, which will contribute to scientific research in Morocco and in Africa. CASE PRESENTATION: An 18-year old female born to consanguineous parents, with a history

Compound heterozygous missense and intronic variants in B9D1 contribute to a recurrent Meckel syndrome pedigree.

BACKGROUND: Meckel syndrome (MKS) is an embryonically lethal ciliopathy with severe clinical manifestations, including defects of the central nervous system, bilateral renal cystic dysplasia, and postaxial polydactyly. B9 domain-containing 1 (B9D1, NP_056496.1) is a member of a small family of proteins associated with basal bodies and primary cilia in mammalian cells. B9D1 variants are associated with MKS and Joubert syndrome. However, to date, only a few cases have been reported. METHODS: In this study, we investigated a prenatally diagnosed recurrent MKS pedigree. Two fetuses of different s

Bi-allelic INTU variants define a ciliopathy disorder characterized by orofacial, digital, and cardiac anomalies.

The primary cilium is a small organelle that plays key roles in cellular signaling. Defects in primary cilia formation, morphology, and function cause a heterogeneous group of developmental syndromes termed ciliopathies. The inturned planar cell polarity protein (INTU) gene acts in the CPLANE complex to facilitate ciliogenesis and support cilia signaling. Bi-allelic genetic variants in INTU have previously been reported in seven patients with pleiotropic disorders, but a core set of phenotypes from these patients has not been codified and functional studies into these variants have failed to f
